Description

 The invention relates to printed products and can be used in conducting mandatory preventive medical examinations of people entering work or working in food enterprises, at food trade facilities, at water supply facilities, in enterprises for sanitary and hygienic services for the population, in medical and children's institutions.

 There is a well-known personal medical information card made of thin plastic with the size and shape of a credit card, on which a sheet with information about the owner, necessary in emergency circumstances and containing his address, name and medical information is pasted [1].

 The scope of the well-known medical record is limited, since the small size of the card does not allow to enter into it a sufficiently large amount of medical information about the owner, which could be used for other purposes, and not only in emergency circumstances.

 A known document containing an identity card, an insurance certificate and a medical card [2]. It consists of two sheets interconnected by a loop-shaped element. On each sheet on its two sides contains information identifying the owner, the state of health and a certificate of health insurance. This information is presented both in the form of visually readable information, and in the form of encoded information (this applies especially to the owner’s medical history), which is read using electronic equipment. In addition, part of the information is presented in the form of a microfilm, strips of films of which are fixed on the map. The volume of medical information recorded in a well-known map may be quite large, however, for playback in order to familiarize themselves with it, it is necessary to use special devices - personal computers and slide projectors, which leads to the expenditure of considerable time. Making changes to information about the state of health after a certain time is also difficult, since for this it is necessary to re-encode information each time and enter it on a new film.

 The objective of the invention is the creation of a medical document, which due to the presentation in it of specific medical information about the owner in a convenient form can be used for a long time by persons subject to mandatory preventive medical examinations by the nature of their activities.

 The problem is solved in that a medical document containing two sheets stacked on top of each other, folded along one edge, with which they are fastened together, and containing personal information about the owner and medical information about the state of health, according to the invention contains additional sheets for entering the results owner’s medical examinations, including laboratory and X-ray examinations, and additional sheets with notes on the owner’s transition from one enterprise to another, the results of completion exams at the sanitary minimum and the doctor’s conclusions on admission to work according to the results of a medical examination, while all the sheets are numbered and stitched in the form of a book with a cover, and on the main part of the sheets there are a lot of vertical and horizontal lines forming a grid on them.

 A medical document made in accordance with the invention can be used as a personal medical book, convenient for use for a long time by employees who are required to regularly undergo preventive medical examinations on time. This book not only identifies its owner, but also serves as a document binding on the administration of enterprises when hiring.

 For the convenience of using the proposed medical document, it is divided into headings, each of which consists of at least one page with the name of this heading on it, and the number of pages for each heading is selected depending on the volume and frequency of the type of examination indicated in this headings.

 In order to avoid counterfeiting a medical book, it is provided with at least one element of a protective marking. To do this, a grid of horizontal and vertical rows of protective marking elements representing a stylized image of "M" and "D" representing a "medical document" can be applied to each sheet, and the inscription "Personal medical booklet" is placed on the background of the grid.

 To use the book at the same time in at least two countries, all the textual material in it, made by the typographic method, is presented in at least two state languages.

 Each medical document has an individual serial number.

 The medical document "Personal medical book", used, for example, in the territory of the Russian Federation, contains stacked one on top of another, sequentially numbered sheets 1 (Fig. 1) in the form of a book with cover 2, on the front surface of which there is an inscription 3 on its front surface " Personal Medical Book. " The same inscription 3 is on the front surface of title page 4, on which below is an extract 5 from the Labor Code of the Russian Federation (Chapter 10, Article 154 “Medical examinations of workers of certain categories”): “Issued to persons subject to mandatory preventive medical examinations. when hiring or in the process. " On the back surface of the cover sheet 4, the inscription 3 "Personal medical book" is given on top, under which on the left is the frame 6 for a photograph of the owner of the book with the letters "MP" next to the vertical right border of the frame - the print location. To the right of the photo frame is printed individual serial number 7, for example, “In N 075850”, and under frame 6 the page field is divided into horizontal columns. One column indicates the company that issued the book, and other columns are used to sign the owner of the book, the signature of the official identifying the owner, and the signature of the head of the company with the date.

 On the last sheet of the 8th book, the rules for using a personal sanitary book are printed. All other pages 9 (between the title page and the last pages 4 and 8) relate to thirteen headings indicated by Roman numerals I to XIII. The number of pages 9 for each heading is different and depends on the amount of information entered in this heading. At the top of each page 9 is the serial number of the heading to which it relates, and the title 10 of this heading.

 Title page 4 is followed by page I of the heading "Information on the owner of the medical book" with columns to indicate personal information about the owner: last name, first name, middle name, year of birth, home address, main profession, position. The last column indicates the company where the owner works at the time of issue of the book. Starting from the pages of the II heading, a lot of vertical and horizontal lines 11, 12 forming a grid on them are plotted on each of them. One page of the II rubric “Marks on the transition to work in other enterprises” is divided into columns that indicate the date of transition, the name of the enterprise, the position of the employee and the signature of the head of the enterprise.

 After that, there are five sheets of the III heading "Doctor’s opinion on admission to work based on the results of a medical examination", the columns of which indicate the date, doctor’s opinion and the doctor’s signature with a seal.

 The heading IV contains the results of the examination for tuberculosis. These examinations are carried out by specialists of tuberculosis dispensaries. For this section, one sheet has been allotted, which indicates the date of the examination, the expert’s opinion and his signature, certified by a seal.

 One sheet in the book belongs to the V heading "Results of a study on the carriage of intestinal pathogens." These studies are conducted by the laboratory of a medical institution, such as a health unit or clinic. In the columns of this sheet are entered the date, conclusion of the laboratory and signature with a seal.

 The VI rubric “Results of laboratory tests and examination by a dermatovenerologist” is filled in by specialists of the Dermatovenerologic Dispensary. The three pages of this section indicate the date, conclusion of the laboratory, confirmed by a stamp, and the signature and seal of the doctor.

 The owners of this book should undergo a study on helminitis, the results of which are recorded on two pages of the VII heading. The columns of this section indicate the date, conclusion of the laboratory and signature with a seal.

 One page is reserved for the following headings: VIII "Results of a study on the carriage of diphtheria pathogens", IX "Results of a study on pathogenic staphylococcus", on which the conclusions of the laboratory of a medical institution are entered. X "Marks on the transferred infectious intestinal diseases" with the diagnosis. There is in the book XI heading "Passing the exam at the sanitary minimum", in the columns of which the names of the subjects, the duration of classes for each subject and the assessment, which is certified by the signature and seal of the teacher, are entered.

 Marks of preventive vaccinations are entered on page XII of the heading, which indicates the date of vaccination and revaccination and the doctor’s signature, certified by a seal.

 In the last XIII rubric, “Results of the examination for parenteral hepatitis”, the expert’s conclusions are given.

 Each page of the "Personal Medical Book", including the inner surface of the cover 2, has elements 13 of protective markings, which are located on horizontal and vertical lines, forming a grid. Each marking element 13 is a stylized image of the letters "M" and "D" denoting a "medical document". In addition, against the background of this grid, without breaking it, at the same place for each page is placed the inscription 14 "Personal medical book". The book can be equipped with additional degrees of protection: use paper with watermarks, fluorescent hairs, holographic signs.

 To expand the possibilities of using the book at the same time, for example, in two countries, all text material made by the printing method is presented in two languages, for example, in Russian and Kazakh.

 All entries made in the book can be made either in handwritten form or in typewritten form.

 The proposed "Personal Medical Book" should be kept by the administration of the enterprise or by the employee to whom it was issued. It must be presented at the request of employees of the state sanitary and epidemiological surveillance or other regulatory organizations. In the absence of this book, the enterprise administration and sanitary and epidemiological surveillance have the right to prevent an employee from working. Upon dismissal and transfer to work in another enterprise, the book remains with the owner and is presented at the place of new work. Workers are allowed to perform their duties only after a preliminary medical examination and the studies indicated in the book.

 In the process, employees must undergo medical examinations on time, including laboratory tests in accordance with the Instructions in force in each country.

 The volume and frequency of such studies can be changed and refined taking into account the epidemiological situation in a particular administrative region. Persons suffering from diseases listed in the relevant sections of the book are not allowed to work. Moreover, the list of diseases indicated in the book can be changed depending on the natural and climatic zone of residence of the book owner. In addition, in coordination with the Ministry of Health or the Department of State Sanitary and Epidemiological Surveillance, new rubrics may appear, clarification of existing ones or elimination of some of them.

 The proposed book is, in contrast to the well-known, printed products made by printing. It is easy to manufacture and convenient to use for a long time. This book is an extended document - a "medical passport", which in the future may become mandatory for every citizen in any country.

A medical document containing two sheets stacked on top of each other, folded along one edge, with which they are fastened together, and containing personal information about the owner and medical information about the state of health, characterized in that it contains additional sheets for entering the results of medical examinations of the owner, including laboratory and x-ray studies, and additional sheets with notes on the owner’s transition to work from one enterprise to another, the results of exams at the sanitary minimum and doctor’s conclusions on admission to work based on the results of a medical examination, while all the sheets are numbered and stitched in the form of a book with a cover, and on the main part of the sheets there are a lot of vertical and horizontal lines forming a grid on them.  2. The document according to claim 1, characterized in that it has a heading, each of which consists of at least one page with the name of this heading on it, while the number of pages for each heading is selected depending on the volume and frequency of the type of examination specified in this section.  3. The document according to claim 1 or 2, characterized in that it is equipped with at least one element of a protective marking.  4. The document according to claim 1, characterized in that all the text material in it, made by typographic method, is presented in at least two state languages.  5.
