Abstract
Description
The invention relates to medicine and relates to skin diseases.
Dermatomyositis (DM) is one of the most severe inflammatory diseases from the group of diffuse diseases of the connective tissue, characterized by a predominant damage to the muscles and skin.
Despite the successes achieved in the treatment of this disease, the prognosis for the working capacity of patients with DM is usually unfavorable, mortality remains quite high, even with timely treatment, it is 28.4%. Basically, the decrease and disability are due to the development of muscle weakness, and as a result to muscle atrophy, the appearance of contractures, which, according to histological studies, is characterized by an increase in the inflammatory process in the connective tissue and the development of fibrosis.
Currently, there are methods of treating DM, aimed mainly at eliminating the inflammatory process in muscle tissue, stopping microcirculation disorders and which do not affect the exchange of connective tissue.
[1]. As a prototype, a method for treating idiopathic DM by administering corticosteroid hormones, anticholinesterase drugs, drugs of the 4-aminoquinoline series and theonicol (compliment) 2-3 times a day every day for 5-6 months [1] was chosen.
Corticosteroid hormones are the main treatment for patients with DM.
The effectiveness of 4-aminoquinoline series drugs in DM is currently considered low.
Theonicol and its analogues stop microcirculation disorders, but do not reduce fibrosis.
In addition, the listed drugs have a number of contraindications and side effects.
Of the drugs that affect the exchange of connective tissue, hyaluronidase analogues that cause the breakdown of the interstitial substance are currently mainly used. Collagenase is the only known substance that can break down collagen molecules. Previously, collagenase and its drug, collalysin, were not used in the treatment of patients with DM.
The aim of the invention is to increase the effectiveness of treatment and the prevention of complications. To this end, according to the method of treating idiopathic DM by administering corticosteroid hormones in a generally accepted dosage according to the invention, collalysin (collagenase) is additionally administered in a daily dose of 500-750 KE once a day for 10-15 days.
A distinctive feature is that additionally administered collalysine in a daily dose of 500-750 KE intramuscularly once a day for 10-15 days.
The dosage is selected from an approximate calculation of 10 KE per 1 kg of body weight.
The method is as follows: a patient with a diagnosis of idiopathic DM is prescribed corticosteroid hormones in generally accepted doses, an additional daily dose of 500-750 KE is administered intramuscularly at a daily dose of 500-750 KE for 10-15 days.
Treatment using the method selected as a prototype was performed for 13 patients with idiopathic DM. As a result, significant improvement was achieved in 2 patients, improvement in 8 patients, and the absence of positive changes in 3 patients.
Treatment by the proposed method was carried out for 13 patients with idiopathic DM. As a result, the disappearance of clinical manifestations occurred in 3 patients, a significant improvement in 5 patients, an improvement in 3 patients. In 2 patients, no improvement was achieved.
Side effects and intolerance to collalysin were not noted.
1) повышение эффективности лечения;
2) предупреждение осложнений ДМ;
The advantages of the proposed method:
1) increasing the effectiveness of treatment;
2) prevention of complications of DM;
3) reduction of side effects and complications of therapy.
