Abstract
Description
The invention relates to urology.
According to statistics, every second man after 50 years old suffers from prostate adenoma. Usually this leads, in spite of existing treatment methods, to a surgical operation performed in one or two stages. A significant part of the operated does not withstand operations that often cause complications. Especially difficult and dangerous for the patient is an emergency operation, when the adenoma compresses the urethra and the result is its obstruction. The patient undergoes an operation that is far from unprepared for it, which increases the risk of an unsuccessful outcome of the operation and severe postoperative complications.
1-3. After surgery, the patient is in the hospital for 20-30 days and is treated for several months 1-3.
There are other obstacles that create an obstruction of the urethra, these include prostate cancer and post-traumatic structure.
A known device for the treatment of prostate adenoma by transurethral microwave therapy. The basis of the device is a microwave generator inserted into the urethra. The radiation from the generator heats the adenoma and creates an inflammatory process in it that destroys the tissues of the adenoma [1]
The disadvantages of this device include the fact that its use is limited by the size of the adenoma, the severity of detrusor changes and it cannot be used in most patients, and for emergency operations it is not applicable at all.
Known are considered as a prototype method and device, including an implant, which is a metal spiral and a grip (tweezers) located at the end of a flexible hose, which is an elastic, flexible rod. The grip is connected to the elastic rod by means of levers and hinges. A handle is attached to the rear end of the elastic rod. When installing the spiral in the urethra, the handle moves and captures the spiral, which, together with the grip, is moved with the help of a flexible hose into the urethra until it is installed in the place where the adenoma or other obstruction is located.
In this case, the channel where it is compressed by the adenoma is expanded by a spiral. Then the spiral that remains in the canal is freed from capture, and the capture with a hose is removed from the urethra [2]
The device under consideration with a spiral provides for its movement when installed in the channel along its entire length. This does not exclude injury to the inner surface of the channel. In addition, with a strong compression of the urethra adenoma or the presence of another obstruction and, as a result, its obstruction, it is impossible to open the urethra with a flexible hose and install it in the right place.
In this case, an emergency surgical operation of a patient not prepared for it is necessary, which increases the likelihood of complications and in some cases leads to death.
The objective of the invention is the elimination of trauma of the urethra and emergency surgery in patients with adenoma or prostate cancer or the presence of post-traumatic structures in acute urinary retention.
This is achieved using the method and device for its implementation.
In FIG. 1 shows a tube in a section; in FIG. 2 core, general view; in FIG. 3 holder with an implant, general view; in FIG. 4 placement of the holder with the implant in the tube; in FIG. 5 is the same.
The method consists in the preliminary expansion of the urethra by the introduction and installation of the implant.
5. The device comprises a
The method and installation of the implant (spiral) in the urethra is as follows.
In the tube 1 (Fig. 1A) insert the rod 2 (Fig. 1b) until it stops, while the front end of the rod with its rounded part is exposed from the front end of the tube. Then the
When moving along the urethra, the tube with the rod expands the narrowed part of the urethra.
2а). At the next moment, the
Next, the
Thus, the implant, when moving during its installation, does not touch the walls of the urethra and does not injure it.
The method and device can be used not only for acute urinary retention, but also to prevent it. In this case, the implant is placed before acute urinary retention occurs.
