П р и м е р 1. Больная П. 3-х месяцев, поступила в детскую ортопедическую поликлинику с диагнозом: врожденный вывих левого бедра, подвывих правого бедра. Лечение было начато в стременах Павлика. На рентгенограмме тазобедренных суставов, сделанной в 6-месячном возрасте, отмечен подвывих левого бедра и дисплазия правого тазобедренного сустава. Дальнейшее лечение продолжено в предлагаемом устройстве. Для этого на коленные суставы ребенка надеты гильзы, изготовленные из поливика, распорка раздвинута на длину 36 см при разведении бедер до угла 90о. Заданное положение зафиксировано контргайками. Конечности удерживали в этом положении на протяжении месяца, а затем в течение следующего месяца начали уменьшать длину распорки по 2 см в неделю до требуемой длины в 28 см. При этом с помощью шарнира, заведенного в прорезь втулки, бедрам постепенно придавали положение внутренней ротации, увеличивая ее на 10о в неделю. Через месяц конечности были зафиксированы в положении сгибания и отведения в тазобедренных суставах под углом 45о и внутренней ротации под углом 40о. Заданные положения распорки фиксировали контргайками, а шарнира стопорными гайками. Фиксация конечностей была продолжена еще в течение трех месяцев, после чего устройство было снято и произведена контрольная рентгенограмма, на которой отмечено полное доразвитие компонентов тазобедренных суставов с центрацией головок бедренных костей в вертлужных впадинах. При смотре ребенка в возрасте 3-х лет жалоб не выявлено, походка обычная, движения в тазобедренных суставах в полном объеме, симптом Тренделенбурга отрицательный. На рентгенограмме патологии со стороны тазобедренных суставов не выявлено.PRI me R 1.
