Abstract
Description
The invention relates to medicine, namely to dermatovenerology, and can be used in medical practice for the treatment of infectious skin diseases.
А.Л. 252). From the practice of medicine, currently known methods of treating infectious skin diseases of viral etiology, consisting in the use of interferon drugs as an etiotropic therapy (Treatment of skin diseases: a Guide for doctors. Edited by A.L. Mashkilleyson. M .: Medicine, 1990, p. . 252).
Closest to the claimed is a method comprising parenteral administration of genetically engineered α 2b -interferon, or 100,000 IU, or 1,000,000 IU, or 1,500,000 IU every other day for 3 weeks with some viral dermatoses.
- не отмечается эффективности метода при лечении инфекционных заболеваний кожи невирусной этиологии;
- необходимо сравнительно длительное употребление препарата;
The known method has several significant disadvantages:
- the effectiveness of the method is not noted in the treatment of infectious skin diseases of non-viral etiology;
- a relatively long use of the drug is necessary;
- at the recommended dosages (1,000,000 IU, 1,500,000 IU) of the drug, the development of adverse reactions is possible.
To expand the range of use while increasing the effectiveness of therapy intradermally or subcutaneously, or intramuscularly daily or every other day for 3-20 days, genetically engineered α 2 - and γ-interferon are simultaneously administered in a ratio of 10: 1 in a single dose from 500,000 to 2,500,000 IU and from 50,000 to 250,000 IU, respectively, initially at the maximum dose that does not give adverse reactions, with its change depending on the change in individual tolerance, clinical dynamics of the disease, in combination with known methods of treatment (for example, with antibiotic tics, local antiseptics).
Moreover, the total duration of treatment of infectious skin diseases of both viral and non-viral etiology is reduced by 1.2-1.5 times, the development of adverse reactions to interferon therapy is eliminated.
The proposed method was tested in the Astrakhan Regional Dermatovenerologic Dispensary, for outpatients from October to December 1991 for 11 patients.
Below are the results of testing.
П р и м е р 1. PRI me R 1. Patient D., 13 years. Clinical diagnosis: herpes zoster. Upon treatment, the patient complained of rashes on the skin of the body, accompanied by itching and burning. The disease began 5 days before treatment.
When viewed on the skin in the projection of the 5 intercostal space on the left, there were focal rashes, within which blisters with serous contents were located on an erythematous background.
The maximum dose of genetically engineered α 2 - and γ-interferons (respectively 500,000 IU and 50,000 IU) was established; within 2 days (10-12.11.1991), rashes were cut off with interferon preparations in the corresponding dosage daily. Ascorbic acid and α-tocopherol acetate inside, an external alcohol solution of brilliant green, were also prescribed. As a result of the treatment, skin rashes completely disappeared. Adverse reactions to interferon therapy were not observed. The total duration of treatment was 5 days.
П р и м е р 2. PRI me R 2. Patient K., 18 years old. Clinical diagnosis: streptostaphyloderma of bones.
Upon admission, the patient complained of painful rashes on the hands. The disease began on November 13, 1991.
When viewed on the skin of the hands there were pustules, conflicts, erosive surfaces, merging with each other with the formation of areas of continuous lesion.
Before the start of interferon therapy with the appointment of antibacterial, general strengthening agents, external use of antiseptics, pronounced dynamics of the regression of rashes was not observed.
From November 28 to December 9, 1991, the patient was daily prescribed intramuscular injections of genetically engineered α 2 - and γ-interferons in a maximum single dose that does not give adverse reactions (respectively 1,500,000 IU and 150,000 IU), which was increased to 10-11.12.91 to 2,500,000 IU of genetically engineered α 2 interferon and 250,000 IU of genetically engineered γ interferon, in combination with doxycycline, nystatin, hexavit, with external use of a solution of hydrogen peroxide, Castellani liquid.
As a result of the treatment, skin rashes completely disappeared. Adverse reactions to interferon therapy were not observed. The total duration of treatment was 14 days.
П р и м е р 3. PRI me R 3. Patient G., 32 years old. Clinical diagnosis: acne.
When handling the patient complained of rashes on the skin of the back, accompanied by soreness. The disease began in 1978. Previous treatment did not give a pronounced clinical effect. When viewed on the skin of the back and shoulders, the presence of papulopustular elements with pronounced infiltration at the base, located both focal and merging with the formation of sedentary nodes, was noted. Within 2 days (November 28-29, 1991), the patient was injected intramuscularly with the preparations of genetically engineered α 2 and γ interferons in a single maximum dose that did not give adverse reactions (respectively 2,000,000 IU and 200,000 IU). Due to the fact that after the second injection of interferons in the patient noted short (within 2 hours) fever (37.5 ° C), the dose of genetically engineered α 2 - and γ-interferon was reduced (respectively 1500000 and 150000 IU). From November 30 to December 11, 1991, interferon preparations at this dosage were administered intramuscularly daily or every other day in combination with the use of injections of benzylpenicillin - sodium salt. Inside, hexavit and ascorbic acid were prescribed. Outwardly, ichthyol ointment, an alcoholic solution of salicylic acid, and Castellani liquid were used.
As a result of the treatment, the manifestations of pyoderma were largely resolved. Adverse reactions to interferon therapy were not observed. The total duration of treatment was 21 days.
П р и м е р 4. PRI me R 4. Patient S., 54 years. Clinical diagnosis: multiple basal cell carcinomas of the face and back. When handling the patient complained of rashes on the skin of the back of the nose and back. Rashes were observed throughout the year. When viewed from the skin of the nasal and back, there were two rounded lesions not exceeding 1 cm in diameter, within which there was a narrow ridge formed from miliary dense pink nodules (“pearls”) along the edge erythematically with slight signs of skin atrophy. Histological examination revealed solid basal cell carcinoma. The maximum dose of genetically engineered α 2 - and γ-interferons (500,000 and 50,000 IU, respectively) was established, and during 12-17.02.1992, rashes were chipped off with interferon preparations in the appropriate dosage daily on 19.-28.02. In 1992, daily rashing of rashes with preparations of genetically engineered α 2 - and γ-interferons was performed in a single dose of 1,000,000 and 100,000 IU, respectively. As a result of the treatment, the tumors were resorbed. Adverse reactions to interferon therapy were not observed. The total duration of treatment was 17 days. When observing a patient during the year, relapse of the disease was not detected.
П р и м е р 5. PRI me R 5. Patient E., 73 years. Clinical diagnosis: basal cell carcinoma of the tumor. Upon treatment, the patient complained of a rash on the skin of the right infraorbital region. Rash was observed for 10 years. There was a tendency to increase the size of the neoplasm. When examined on the skin of the infraorbital region, there was a dense whitish-pink knot on the right, in the center of which was a serous-hemorrhagic crust. The dimensions of the assembly reached 1 cm in diameter. A histological examination revealed a solid basal cell carcinoma. The maximum dose of genetically engineered α 2 - and γ-interferons (500,000 and 50,000 IU, respectively) was established; during 24-28.09.1992, three-time chipping of the rash with interferon preparations was carried out in the appropriate dosage. 09/30. On October 3, 1992, three-time chipping of rashes with preparations of genetically engineered α 2 and γ-interferons in a single dose of 1,000,000 and 100,000 IU, respectively, was performed on October 4-12, 1992 - daily chipping with drugs of genetically engineered interferons in a single dose, respectively 1,500,000 and 150,000 IU.
As a result of the treatment, the tumor was resorbed.
Adverse reactions to interferon therapy were not observed.
