Abstract
Способ катетеризации центральных вен у детей, включающий использование диализных катетеров под контролем УЗИ, отличающийся тем, что определяют наиболее безопасную для пункции центральную вену, измеряют ее диаметр, затем выбирают необходимый диализный катетер с наружным диаметром равным ½ диаметра вены, осуществляют пункцию вены иглой-катетером типа флексюли, диаметр которой на два размера меньше иглы диализного катетера, пунктируют вену под контролем УЗИ для корректировки направления и движения флексюли, после проникновения в венозное русло флексюли удаляют иглу- мандрен, при этом в просвете вены остается эластичный тонкий катетер-флексюля, далее по этому катетеру в просвет вены по Сельдингеру проводят металлический проводник меньшего размера, затем первую флексюлю удаляют, проводят дилятацию по ходу проводника и по проводнику устанавливают вторую флексюлю большего диаметра, через нее по Сельдингеру в венозное русло проводят проводник для установки диализного катетера, удаляют флексюлю большего размера, а по проводнику устанавливают диализный катетер.A method for catheterization of central veins in children, including the use of dialysis catheters under ultrasound guidance, characterized in that they determine the safest central vein for puncture, measure its diameter, then select the necessary dialysis catheter with an outer diameter equal to ½ of the diameter of the vein, puncture the vein with a catheter needle type flexula, the diameter of which is two sizes smaller than the needle of the dialysis catheter, puncture a vein under ultrasound control to adjust the direction and movement of the flexula, after penetration into the venous e channel of the flexula removes the needle-mandrel, while an elastic thin catheter-flexule remains in the lumen of the vein, then a smaller metal conductor is passed through the Seldinger catheter into the lumen of the vein, then the first flexula is removed, the dilatation is performed along the conductor, and the second a flexule of a larger diameter, a conductor is inserted through the Seldinger into the venous bed to insert a dialysis catheter, a larger flexula is removed, and a dialysis catheter is inserted through the conductor.
