Description
The utility model relates to medicine, namely to diagnostic surgery, and can be used in the surgery of glial tumors of the brain and spinal cord.
Glial tumors are primary brain tumors that are characterized by infiltration growth with a highly branched border.
A widely used method for detecting the boundaries (sites, fragments) of glial tumors is the excitation and observation of fluorescence of natural tissue pigments (autofluorescence) or injected drugs (photosensitizers), the concentration of which is different for normal tissue and tumor tissue.
In addition, the complex morphological structure of different sections of a glial tumor is characterized by a significant difference in their optical properties, so that measuring the optical parameters of radiation scattering by different parts of the tumor and surrounding healthy tissues provides additional information when determining the boundaries of gliomas. For glioblastomas, GRADE IV can distinguish up to three zones, the parameters of which are different from the optical characteristics of healthy tissue of the white matter of the brain: the region of active tumor growth, the perifocal zone (border of the tumor) and the region of necrosis.
A well-known device that allows simultaneous recording of spectroscopic and photometric data, and then, based on the known optical parameters of the above zones, determine the blood filling, oxygenation, concentration of protoporphyrin IX and the assessment of cellular changes in the structure of tissues, is a fiber-optic probe for carrying out operations to remove gliomas accompanied combined spectroscopy [Potapov AA, Goryainov SA, Loshchenov VB, Savelyeva TA et al. “Intraoperative combined spectroscopy (optical biopsy) of brain gliomas”, Issues of Neurosurgery 2013; 2: 3-10.]. The probe is a bundle of quartz fibers placed in a common polymer shell, two of these fibers for transmitting white radiation and recording scattered radiation (photometric measurements, a fiber for transmitting radiation to excite fluorescence, the rest to capture fluorescence radiation).
For its purpose, the probe has a number of drawbacks, namely, the restriction on repeated and especially repeated use due to the impossibility of hard disinfection of the working part of the probe after contact use during the operation. In addition, a significant drawback is the small fixed diagnostic depth with this probe, which is determined by the distance between the transmitting and diagnostic fibers. When this probe is in contact with tissue, the effective diagnostic area is enclosed in an elongated volume (“banana”), resting on the ends of the fibers and limited by the following dimensions: 0.3 mm deep into the tissue with a diameter on the surface of about 1.5 mm. In addition, when using various tools to detect a tumor site in such a small volume during the diagnosis and removal of this area, the probability of error is high, which leads to the need to remove healthy tissue with a reserve.
А., Киселев A.M., Есин И.В. A device for removing brain tumors with the allocation of tumor borders by fluorescence diagnostics with simultaneous coagulation and aspiration [RF patent 2510248 C2 "Method for removing brain tumors with the allocation of tumor borders by fluorescence diagnosis with simultaneous coagulation and aspiration and device for its implementation" Kurlov V.N., Shikunova I.A., Kiselev AM, Esin I.V. publ. 2014-03-27], which allows performing fluorescence spectroscopy of brain tissues in contact mode to clarify the boundaries of the tumor, identify visually undetectable tumor sites and simultaneously remove these sites with a single surgical tool. The system contains a source of radiation that excites fluorescence of tumor cell markers (endogenous and exogenous), a spectrometric system with a fiber output, a Y-shaped fiber optic light guide, an aspirator, and a surgical instrument - a sapphire contact neuro probe. In addition, additional laser radiation sources may be included in the system for timely coagulation of blood vessels or local laser exposure to brain tissue.
A neurosonde is a sapphire single crystal rod containing longitudinal channels. Some of the channels are closed from the side of the working end of the instrument - they are designed to accommodate quartz-polymer fiber optic fibers for transmitting radiation to excite and capture fluorescence. Also, the rod has a through longitudinal aspiration channel through which the identified tumor fragments and blood are removed from the operating area. Schemes with different shapes and placement of channels in the neuro probe both with symmetric and asymmetric arrangement of channels for fluorescence diagnostics are proposed. The device does not provide for the possibility of photometric measurements, which reduces its diagnostic effectiveness, the reliability of the determination of tumor tissue.
The technical result, which the utility model is aimed at, is to increase the efficiency (totality index) of removing glial tumors of the brain and spinal cord, increase the spatial sensitivity of the backscattering diagnosis, improve the light-energy parameters of this diagnosis, increase the service life of a sapphire neurosurgical probe to remove glial brain tumors and spinal cord.
The technical result is achieved due to the fact that in the sapphire neurosurgical probe, which is a sapphire rod with longitudinal channels, some of which are open, the rest are closed, they have optical fibers for transmitting radiation to excite fluorescence and fibers for transmitting fluorescence radiation, some of the fibers are used for transmitting broadband radiation in both directions, the thickness of the sapphire layer in front of the working ends of the fibers for transmitting broadband radiation along the axis does not exceed 1 m.
The technical result is also achieved due to the fact that in the sapphire neurosurgical probe, all optical fibers are collected in a bundle and placed in one closed channel of the sapphire rod. The isolation of all fibers from contact with the medium allows for mild sterilization of the quartz polymer probe, which allows it to be used repeatedly.
The technical result is also achieved due to the fact that in the sapphire neurosurgical probe, at least at some of the channels, the bottom of the channel has the shape of a hemisphere.
The component of radiation diffusely scattered by the tissue in the direction of the receiver appears as a result of a significant (close to full) deviation of the direction of motion of the photon packets from the original direction as a result of multiple scattering on the inhomogeneities of the refractive index in the volume of biological tissue. The system detects photons that fall as a result of such movement to the site of the receiving end of the diagnostic fiber (or fibers) and fit into the working aperture of this fiber. The presence of a window (a plane-parallel plate in front of the working ends of the fibers of the spectrometric system in the sapphire probe) makes it possible to increase the overlap area of the angular fields of the illuminating and receiving fibers, which leads to an increase in the light-energy efficiency of diagnostic backscattering.
Глубина диагностики при этом увеличена до нескольких миллиметров (зависит от глубины проникновения излучения), что существенно выше, чем у аналога (0,3 мм - определяется геометрией зонда).The presence of a hemispherical surface at the end of the channels containing fibers makes it possible to increase the irradiation area on the one hand (since a probe bottom having a curvature in combination with a flat contact surface acts as a negative scattering lens) and, on the other hand, allows selection of captured axial rays, since the extreme rays when they hit such a surface will experience internal reflection and are not captured by the receiving fiber. This provides an increase in the spatial accuracy of diagnostics. In this case, the diagnostic depth is increased to several millimeters (depending on the depth of radiation penetration), which is significantly higher than that of the analog (0.3 mm - determined by the geometry of the probe).
The utility model is illustrated by the drawings shown in FIG. 1-4, where in FIG. 1 shows a diagram of a sapphire neurosurgical probe for removal of brain and spinal cord tumors under the control of a combined spectroscopic diagnosis. In FIG. 2 shows a diagram of photometric measurements according to a utility model; FIG. Figures 3 and 4 show diagrams of photometric measurements in particular cases of a utility model.
1)A sapphire neurosurgical probe for removing brain and spinal tumors under the control of combined spectroscopic diagnostics contains a
6.To carry out combined spectral and photometric measurements, the working
The
With an increase in the thickness of the sapphire layer from 1 mm or more, there is a decrease in the light-energy efficiency of registration of backscattered radiation due to a decrease in the radiation density on the contact surface and an increase in radiation losses due to total internal reflection in the probe for oblique rays (illumination). In addition, a radial increase in the area of radiation scattering leads to a decrease in the spatial sensitivity of diagnostics, since photons experiencing scattering in regions substantially remote from the most probable volume of diagnostics can be captured by the diagnostic fiber.
The work of the proposed neurosurgical probe can be carried out as follows. Fibers for fluorescence diagnostics are connected to a laser source and a spectrophotometer. Fibers for transmitting broadband radiation are connected to a broadband radiation source and a spectrophotometer. The patient is prepared for fluorescence contrasting of the tumor according to the standard protocol. At various stages of the tumor removal operation, a spectrophotometric assessment of the concentration of protoporphyrin-IX or autofluorescence is performed in the vicinity of the working end of the sapphire neurosurgical probe with simultaneous registration of backscattering radiation. To distinguish between tumor sites and normal tissue, a combined spectrophotometric diagnosis is performed. To carry it out on the basis of statistical data for a given geometry of the optical system of the probe, a diagnostic diagnostic space has been preliminarily formed based on spectral information (fluorescence contrast, autofluorescence contrast) and diffuse reflectance. The data obtained during the operation is continuously placed in the sign space, which makes it possible to determine whether the diagnostic area is tumor tissue or not.
