Abstract
A device for introducing drainage through the stump of the bile duct, comprising a hollow metal tube, characterized in that the blades are mounted on the tube with the possibility of movement, and the tube is additionally placed in a movable sleeve with holders.
Description
The utility model relates to medical devices and can be used in biliary surgery.
A device is known for introducing a T-shaped drainage (RF Patent No. 61118, АВВ 17/00, BIPM No. 6, 02/27/2007), including a metal tube, inside which a drain with a metal insert is placed to push it out.
However, using the above-described device, it is difficult to establish drainage: when the metal insert is removed, the drainage leaves the common bile duct.
Closest to the attached is a device for drainage into the common bile duct (RF Patent No. 18628, А61В 17/122, BIPM No. 19, 07/10/2001), including a hollow metal tube with a diameter of 3-5 mm, the working end of which is bent under angle of 90-120ºС, and has a recess on 1/3 of the tube diameter for 4 cm on the side surface.
However, this device has the disadvantage of:
Задача полезной модели: обеспечение удержания дренажа в холедохе для быстрого оттока желчи.when it is removed, drainage is easily removed from the common bile duct, which leads to repeated attempts to drain the common bile duct and increases the operation time. The objective of the utility model: ensuring retention of drainage in the common bile duct for the rapid outflow of bile.
The task is carried out due to the fact that the blades are mounted on the tube with the possibility of movement and the tube is additionally placed in a movable sleeve with holders.
Figure 1 shows a device for introducing drainage through the stump of the bile duct, where
1 - proximal end of the device
2 - movable blades
3 - movable coupling
4 - holders for coupling movement
5 - distal end of the device
The device operates as follows:
The patient was diagnosed with obstructive jaundice upon admission. Before using the proposed device, the patient underwent laparoscopic choledocholithotomy, cholecystectomy. The proximal end of the device (1) is placed in the stump of the bile duct, the walls of the stump are clamped by moving blades (2) and fixed by a sleeve (3) using holders (4). Through the distal end (5) of the device install drainage in the common bile duct. The device is removed. The choledocholithotomy opening is sutured tightly. The operation is completed as standard.
Предлагаемое устройство позволяет установить дренаж в 4 раза быстрее по сравнению с прототипом (4 минуты и 16 - соответственно).The proposed device allows you to install drainage 4 times faster compared to the prototype (4 minutes and 16, respectively).
