WITH ADJUSTABLE DOSER THE INDICATED DOSAGE " The present Utility Model concerns a fluoride pacifier with dosage adjuster adjustable to the dosage indicated by the pediatrician. The efficacy of fluoride is now accepted in prophylaxis of dental caries, which begins during pregnancy and continues until adolescence, when the formation of the new dentition is completed.
In addition to the importance of this ion, in the formation of teeth, fluoride plays an important role in increasing bone density.
In relation to the teeth, there are two ways of incorporating Fluoride: systemic via and direct contact. The mechanism of incorporation of fluoride into tooth enamel is not yet well known, but is thought to be of the ion exchange type (F "by" OH), in which hydroxyapatite is progressively replaced by fluorapatite, making crystal which is the enamel, denser and more resistant to demineralization and attack of cariogenic acids.
In the adult tooth, this substitution process is slow and time consuming, and is essentially done by airborne contact of the fluoride with the enamel, through toothpastes and fluoride elixirs.
However, during the formation and eruption of the tooth, the process is more intense, not only replacing the hydroxyapatite with the fluorapatite of the enamel, as a consequence of the direct contact with the external fluorine, but also the incorporation of this ion in the growing systemic tissue, a process favored by high blood supply and the high permeability of these tissues, at this stage.
However, in the prophylaxis of caries, oral fluoride administration with consequent systemic absorption may lead to acute toxicity (the dose lethal for children is 500 mg of fluoride) and other adverse effects.
Thus, it is of interest to decrease the fraction of fluoride, which is absorbed by the dental tissue systemically, and to increase the fraction which is incorporated by direct exposure. The present Utility Model provides a third way of absorption, called semi-systemic, that is, made through the buccal mucosa, via this, which should contribute significantly to the total fluoride supply to these tissues during odontogenesis . The device, which is the subject of this Model, was based on the existence of the innate reflex of suction in newborns that led to the widespread use of pacifiers, also taking into account that the oral administration of sodium fluoride drops is already common practice in the prophylaxis of dental caries. The aim is to incorporate in the dummies a slow and prolonged release system of sodium fluoride capable of reaching not only the objectives of the traditional means mentioned above, but also providing a reduction of the systemic fraction of fluorine administered, and an increase of the time of exposure of the teeth to fluoride, favoring its absorption at the level of the oral mucosa, besides providing a greater rigor and convenience, in compliance with the dosage indicated for each age. The pacifier with the dosing device, object of the present Model, allows to increase the time of direct exposure of the teeth to Fluoride, which is administered in the form of solution / gel, resulting in a greater enrichment in fluoride of the tooth enamel , thus rendering it resistant to cariogenic acids. The dosing device is based on the use of the suction reflex, allowing the slow and continuous release of fluoride, at the appropriate dosage for each age, and its partial absorption in the mouth (by the mucosa and teeth), significantly reducing the manifestation of adverse effects that by systemic route may result from the absorption of this element at the level of the intestines because the amount of fluorine arriving there is already lower. The fluoride pacifier to be protected consists in the adaptation of a dosing device to the teat in common pacifiers.
This dosing device is composed of a tube-shaped container (3), constituted by the same material of the teat (preferably silicone) and which longitudinally crosses the pacifier.
0 ar que ocupava este espaço é libertado através das janelas (4.1.5.1 e 4.2.2.1), agora coincidentes, do disco móvel (4.1.5) e fixo (4.2.2) do pistão retentor (4).Inside this tube 3 a retainer piston 4, with a function identical to that of the plunger of a syringe, is moved by adjusting the volume of the liquid against the pore 6 at the end of the nipple. The rear end of the teat has an aperture (2) with a cap, with a breathing portion, through which the doser is fed, with the aid of the capillary tube (8) of the applicator (1). The capillary tube (8) of the applicator engages the protrusions (4.1.1) of the movable shaft (4.1) of the retainer piston (4) of the dispenser and, when rotated clockwise, externally overlaps the movable disk window .5.1) to the window on the fixed disc (4.2.2.1) and internally, within the movable shaft of the piston (4.1), it overlaps the half-decal (4.1.3) of the movable shaft with the half-calotte (4.2.1) of the shaft fixed, allowing the feed to the tank, with the solution of sodium fluoride. The air occupying this space is released through the windows (4.1.5.1 and 4.2.2.1), now coincident, of the movable disc (4.1.5) and fixed (4.2.2) of the retainer piston (4).
Depois de se atingir o nível do líquido desejado (indicado na posologia), ajusta-se o pistão à superfície do líquido, de forma a eliminar as bolhas de ar, roda-se o capilar do aplicador no sentido contrário, fechando as válvulas interna (4.1.3) e (4.2.1) e externa (4.1.5.1) e (4.2.2.1) do pistão (4), retira-se o aplicador (1) e fecha-se a tampa (2).After reaching the desired liquid level (indicated in the dosage), the piston is adjusted to the surface of the liquid in order to eliminate air bubbles, the capillary of the applicator is turned in the opposite direction, closing the internal valves ( 4.1.3) and (4.2.1) and external (4.1.5.1) and (4.2.2.1) of the piston (4), the applicator (1) is withdrawn and the cap (2) is closed.
These steps are performed with the pacifier upright, with the pore down. The pacifier is ready to be used and can have any other position.
During use, suctioning by the child causes a pressure difference between the interior of the teat and the mouth, causing the sodium fluoride solution / gel to escape through the tiny pore at the tip of the teat. The liquid outlet from the tank is accompanied by a slow descent of the retainer piston (4) towards the pore (allowed by the entry of air through the pore of the lid (2)) in order to compensate for the negative pressure shape inside the tube, in the absence of suction, avoiding the entry of saliva into the tank which would result in an undesirable contamination of the deposit by the buccal flora.
It therefore becomes necessary to balance the frictional force of the retainer piston along the tube with the negative pressure force resulting from the suction in order to maintain a slow and unidirectional flow in the system. The unidirectionality of this flow may be achieved by, for example, a ball or membrane valve (7) - FIG. 2, prior to the portion of the teat 6, as shown in the drawings. The present Utility Model is shown below in a non-limiting manner with schematic drawings of a preferred embodiment of the adjustable dosing device wherein: FIG. 1 - Represents, in longitudinal section, the fluorometric pacifier with the dosing device housed and its applicator, being: (1) - Solution applicator (2) - Tank cap with breathing pore (3) - Tubular reservoir (4) - Retainer piston (5) - Teat (6) - Teat part (7) - Unidirectional valve (8) - Applicator tube (to be attached to the retainer piston) FIG. 2 - Represents, in longitudinal section, a detail and the location of the unidirectional valve, being: (3) - Tubular reservoir (inside the teat) (5) - Teat (6) - Teat pore (7) - Unidirectional valve FIG . 3 - Represents, in lateral perspective, a detail of the retainer piston, being: (4) (4.1) (4.1.1) (4.1.2) (4.1.3) (4.1.4) (4.1.5) (4.1. 5.1) (4.1.5.2) (4.2) (4.2.1)

These drawings are intended to give an idea of the whole and how the various components of the pacifier object of this Utility Model interact and, therefore, deliberately, its proportionality has not been adequately respected.
In order not to overload the drawing, it was omitted in FIG. 5, the stabilizer disk.
In addition to the convenience provided by this dispensing device, account should be taken of the accuracy of the dosage which is administered by the fact that the level marks on the outside of the tubular container located inside the teat (3) are provided for adjusting the volume of solution to be given at the dosage indicated for the child's age. The unidirectional, ball or membrane valve (7) prevents suctioning of equal volume of saliva after suctioning and exiting a small volume of solution from the reservoir (3) via the pore (6) in to the tank, so as to match the internal and external pressures, to the tank.
Instead, the output of a small amount of solution 10 will be accompanied by advancement of the retainer piston (4), towards the outlet portion (6) of the solution, at the end of the teat (5), which advance corresponds to volume of solution left, thus equalizing the internal pressure of the tank at the external pressure.
The mechanisms that delay the solution / gel exit are based on the pore size, the viscosity of the solution and the frictional force between the ends of the fixed disc, the retainer piston and, more precisely, between the sealant / (4.2.2.2) of the fixed disc and the walls of the tubular reservoir (3), a factor which is contrary to the movement of the piston (4) as it slides along the tubular container (3) towards the pore (6), during the process of emptying, caused by suction.
LISBOA, 22 de MAIO de 1996When the Fluoride solution / gel runs out, the retainer piston (4) is directly sucked up by the suction and rests against the unidirectional valve (7), locking it, passing the pacifier to act as a vulgar pacifier. The fluoride solution to be used in this system should preferably not taste so that the child is not aware of the time at which the solution is exhausted in the pacifier. LISBON, MAY 22, 1996
