PL 72 504 Y1 2 Opis wzoru Przedmiotem wzoru uzytkowego jest trzpien udowy endoprotezy stawu biodrowego o konstrukcji trzpienia krótkiego, sluzacy do pierwotnej alloplastyki stawu biodrowego zachowujacej szyjke kosci udo- wej. Proteza znajdzie zastosowanie w leczeniu pacjentów aktywnych fizycznie o dobrym podlozu kost- nym u których zastosowanie tego typu trzpienia umozliwia zachowanie duzej ilosci tkanki kostnej do osadzenia przyszlych rewizji. Z US3064645A „Damped prosthesis forming a substitute for the coxo-femoral articulation"’ znane jest rozwiazanie mocowania krótkiego trzpienia w sposób zachowujacy szyjke kosci udowej. Rozwiazanie wedlug niniejszego wzoru uzytkowego dotyczy wiec krótkiego trzpienia endoprotezy stawu biodrowego. W korzystnym wariancie, szyjka 2 moze miec na powierzchniach równoleglych do plaszczyzny podzialowej trzpienia splaszczenia 5, zapobiegajace kontaktowi szyjki 2 z panewka stawu podczas wy- konywania ruchów w stawie biodrowym o wysokiej amplitudzie szczególnie w rotacji zewnetrznej i we- wnetrznej konczyny dolnej (zob. Fig. 5). Splaszczenie 5 umozliwia bardzo silna rotacje trzpienia wzgle- dem panewki w jednej plaszczyznie, bez oparcia sie szyjki o brzeg panewki powodujacego zwichniecie protezy. PL 72 504 Y1 4 Przednia i tylna powierzchnia korpusu 3 okreslone sa tu w odniesieniu do polozenia, w którym trzpien umieszczany bedzie w ciele czlowieka (a wiec w plaszczyznie strzalkowej). W górnej plaskiej czesci 8 korpusu 3, korzystnie wykonane jest lagodne wciecie 9 (Fig. 3), ko- rzystnie o póleliptycznym przekroju, korzystnie o wymiarze glebokosci h wiekszym niz wymiar srednicy d (Fig. 4). Wciecie 9 sluzy do przykladania narzedzia sluzacego do wbijania trzpienia w przygotowane loze kostne. A, do linii przekroju D. PL PL PL PL PL PL PL PL 72 504 Y1 2 Description of the design The subject of the utility design is a femoral stem of a hip joint endoprosthesis with a short stem design, used for primary hip arthroplasty preserving the femoral neck. The prosthesis will be used in the treatment of physically active patients with good bone support, for whom the use of this type of stem allows the preservation of a large amount of bone tissue for future revisions. From US3064645A "Damped prosthesis forming a substitute for the coxo-femoral articulation"' there is a known solution of fixing a short stem in a way that preserves the femoral neck. In the presented solution, the stem rests in the femur on the plane remaining after head extraction, on the inner medial side cortical layer tangentially to the retaining arch of the femur and on the lateral side it rests slightly lower on the internal layer of the cortical bone. Similarly, there is a known concept included in US7909881B2, the use of an ultra-short pin reaching only slightly beyond the neck of the femur, but its use required great precision of the instrumentation and the operator, because, according to reports, even minor inaccuracies resulted in failures in the treatment of patients. There is also a known solution from USD629903S, which describes a pin developed by the Corin company. This pin has a bent shape and short length, and due to its design it locks in the neck of the bone femur, which preserves the bone bed below the trochanter. Additionally, this implant has 4 ribs (two on each side) enabling more secure attachment in the proximal part of the bone. This has a particular impact on the rotational stability of the implant. The subject of the utility model is a femoral stem of a hip joint endoprosthesis, consisting of a cone, a neck and a body connected to each other, where the cone used to connect the head of the endoprosthesis is connected to the neck, and the neck is connected to the body used to mount the femoral stem in the bone bed, wherein the body is a body elongated in the direction of embedding the femoral stem in the bone bed, and the body includes a proximal part, from the neck side, and its opposite distal part, from the side of the free end of the body, and the cross-sections of the body have an ellipse or similar shape to an ellipse or a circle, and the area of these cross-sections decreases as the distance of the cross-section from the neck increases, and the longitudinal section of the body is limited by two arches, of which the central arch has a smaller radius than the lateral arch and the distance between arcs is the largest in the proximal part of the body and the smallest in the distal part of the body, and in the anteroposterior cross-section of the body, the angle between the lines tangent to the opposite boundary lines of the longitudinal section in the proximal part of the body is greater than the angle between the lines tangent to the opposite boundary lines of the cross-section longitudinal in the distal part of the body, and on the front and rear surfaces of the body in the proximal part there are from one to four splines on the side of the shank, reaching at least half the length of the body, with a cross-section in the shape of a triangle with a rounded apex or similar to a triangle with a rounded top and with rounded places of contact with the cross-section of the proper cross-section of the body, with the splines running so that their lines run halfway between the medial arch and the side arch, and the cross-section of each spline has the greatest height dimension at the point closest to the cervix and this dimension narrows as the distance from the cervix increases. Preferably, in the pin according to the utility model, the neck contains flats on the surfaces parallel to the division plane of the pin, which prevent contact of the neck with the joint socket. In a preferred embodiment of the pin according to the utility model, in the distal direction in relation to at least one spline on the front and rear surfaces of the body, in the line of extension of the spline, there is at least one recess, the recess having a semi-circular cross-section and having diameters of at least one millimeter, and the beginning of the recess is at the height of the end of the spline or up to 10 mm below the end of the spline. In a preferred embodiment of the pin according to the utility model, the recess ends with a smooth exit or reaches up to the distal free end of the body. PL 72 504 Y1 3 In a preferred embodiment of the pin according to the utility model, the body is partially covered with a coating layer consisting of a porous titanium framework and an outer layer of hydroxyapatite, while the free distal end of the body is not covered with a hydroxyapatite layer and is smooth. surfaces. In a preferred embodiment of the pin according to the utility model, the body includes bevels of the front and rear surfaces so that the cross-section of the body increases in the distal direction only in the transverse axis and retains its dimensions or increases only slightly in the sagittal axis. The subject of this utility model is shown in the drawing, in which: Fig. 1 shows a front view of the femoral pin, with a visible enlargement of the detail of the cone surface. Fig. 2 shows cross-sectional views of the femoral pin of Fig. 1 along lines A, B, C and D. Fig. 3 shows a plan view of the femoral pin. Fig. 4 shows a cross-sectional view of the notch in the upper flat part of the stem body, along line E in Fig. 3. Fig. 5 shows a side view of the femoral stem. The solution according to this utility model concerns a short stem of the hip joint endoprosthesis. The solution according to the utility model is intended to meet the following requirements: to enable the greatest possible preservation of the bone bed, to ensure the greatest possible primary and secondary stability of the setting, to prevent stress shielding, and to ensure the greatest possible safety of implantation. For this purpose, a stem was designed consisting of a cone 1, a neck 2 and a part for mounting in the femur, hereinafter referred to as the body 3 (Fig. 1). The cone 1 in the shank according to the utility model shown in Fig. 1 has a shape, size and surface selected so that it can cooperate with the most popular solutions of ceramic endodenture heads. Indeed, the cone 1 is not limited to that shown in Fig. 1, and may have a different shape, size or finish without affecting the performance of the pin itself. However, the shape of the cone 1 as shown in Fig. 1 has a particular advantage of enabling cooperation with the most commonly used endoprosthesis heads. The surface roughness of the cone 4 can be determined by the Rz parameter, preferably with a value ranging between 6 and 20. The cone 1 is mounted on the body 3 via the neck 2. The neck 2 together with the cone 1 is preferably in the length range 25-40 mm, particularly preferably 29-37.5 mm. In a preferred variant, the neck 2 may have flats 5 on surfaces parallel to the division plane of the pin, preventing contact of the neck 2 with the joint socket during high-amplitude movements in the hip joint, especially in external and internal rotation of the lower limb (see Fig. .5). Flattening 5 enables very strong rotation of the stem relative to the acetabulum in one plane, without the neck resting on the edge of the acetabulum, causing dislocation of the prosthesis. The part used to embed the stem in the bone bed, hereinafter referred to as body 3, is a body elongated (extended) in the direction of embedding the stem in the bone, built on cross-sections in the shape of an ellipse or circle or close to an ellipse (Fig. 1). Cross-sections A, B, C, D have correspondingly decreasing surface areas as they move away from the neck 2, with the largest surface area in the proximal part (on the side of the connection with the neck 2, the cross-section in line A), and the smallest surface area in the distal part (cross-section in line D) (Fig. 2). The longitudinal section of the body 3 is limited by two arches 6, 7, of which the central arch 6 has a smaller radius than the side arch 7. The distance between the arches 6, 7 is greatest at the top of the body 3, i.e. on the side of the connection with the neck 2, and the smallest at the bottom (Fig. 1). Body 3 is constructed in such a way that in the proximal part the angle between the lines tangent to the opposite boundary lines of the longitudinal sections is greater than in the distal part. This provides greater resistance to the body 3 at the femoral neck. Preferably, the shank has bevels on the front and rear surfaces of the body 3, so that the cross-section of the body 3 increases only in the transverse axis and remains constant or increases only slightly in the sagittal axis. This is to prevent excessive forces splitting the bone, which will be particularly beneficial when using large pins. Alternatively, the cross-section of the body may range in shape from almost the shape of a neck 2 to the shape of a circle. PL 72 504 Y1 4 The front and rear surfaces of the body 3 are defined here in relation to the position in which the pin will be placed in the human body (i.e. in the sagittal plane). In the upper flat part 8 of the body 3, a gentle indentation 9 is preferably made (Fig. 3), preferably with a semi-elliptical cross-section, preferably with a depth dimension h greater than the diameter dimension d (Fig. 4). Notch 9 is used to place a tool for driving the pin into the prepared bone bed. On the front and rear surfaces of the body 3, in its upper (proximal) part, there is at least one spline 10, up to a maximum of four splines 10 on the side of the pin, with a cross-section in the shape of a triangle with a rounded top or close to a triangle with a rounded top. butt and with rounded contact points with the proper (elliptical or circular or approximately) cross-section of the body 3 (Figs. 1, 2 and 5). The splines 10 have the greatest height dimension (hmax) at the neck of the pin 2 and become narrower as the distance from the neck increases. The splines 10 are located halfway between the lines forming the cross-section of the stem in the anterior-posterior plane and are routed so that their line coincides with the path covered by the stem when inserting it into the medullary cavity, thus parallel to the main axis of the stem determined by a curve running approximately through the centers of gravity of successive sections, starting in the middle of the end surface of the denture cone and ending at the tip apex. The splines 10 reach at least half the length of the body 3. Lower down in relation to at least one spline 10, a maximum of four splines 10, on the front and rear surfaces of the body 3, in the line of extension of the course of a given spline 10, there is a recess 11, called a channel. - dlencze (Fig. 1). The recess 11 is semi-circular in cross-section and has a diameter of at least one millimeter. Recesses 11 are therefore also made along the path along which the pin is inserted into the bone bed. It is therefore important that the recess 11 has its beginning at a height close to the height of the end of the spline 10. The beginning of the recess 11 may be at the height of the end of the spline 10, but it may also be located at a certain distance below the end of the spline 10, for example up to 10 mm below the end of the projection 10. The recess 11 ends with a smooth exit before the distal end of the body 3 or reaches up to the distal free end of the body 3. The body 3 is partially covered with a coating layer 12 consisting of a substructure of porous titanium and an outer layer of hydroxyapatite. This layer preferably has a thickness of 70 µm to 330 µm, preferably 200 µm, and covers the side surface of the body 3 from its upper edge 13 and the section line A to the section line D. This layer is intended to ensure the greatest possible initial stability. and secondary implantation in the bone bed. The free distal end 14 of the body 3 is not covered with a hydroxyapatite layer and has a smooth, polished surface. An undercut may also be made in the tip 14 in the medial part 15 of the tip to facilitate the insertion of the implant into the bone bed. The length of the uncovered portion of the free distal end 14 may be from 1/10 to 1/3 of the length of the main axis of the stem. Whenever a description of the direction or side of an implant appears in the text of the notification, this direction and side should be understood in relation to the planned correct position of the implant in the patient's body.
