Opis wynalazkuDescription of the invention
The subject of the invention is a new use of perampanel (general formula 1) and a pharmaceutical composition containing it, for the prevention or treatment of melanoma and its metastases.
Jednym z zaleceń profilaktycznych jest unikanie ekspozycji na światło słoneczne i stosowanie filtrów chroniących przed promieniowaniem UVA i UVB.Melanoma is a cancer originating from pigment cells, melanocytes. It is most often formed in the skin, but can occur in the mucous membrane and in the eyeball. It is a malignant tumor that metastasizes to the lymph nodes and distant metastases, most often to the lungs and brain. Treatment of melanoma at an early stage of development consists in removing the neoplastic lesion. Complementary treatment, i.e. chemotherapy, immunotherapy, radiotherapy, does not give satisfactory therapeutic results. One of the preventive recommendations is to avoid exposure to sunlight and use filters that protect against UVA and UVB radiation.

Następnego dnia dokonywano odczytu absorbancji przy użyciu czytnika do mikropłytek przy długości fali 570 nm.After incubation, the effect of the substance on cellular metabolism was determined using the MTT test. The MTT test is based on a color reaction and determines the metabolic activity of cells and, indirectly, cell proliferation. After the incubation time, 15 µl of MTT solution (5 mg/ml in PBS buffer with calcium and magnesium ions) was added to each well. A melanoma A375 suspension at a density of 2χ104 cells/ml was poured into 96-well plates. The next day, the culture medium was harvested and the cells treated with fresh culture medium (MEM with 10% FBS) as control or perampanel at 100 µM in culture medium. Cells were incubated under standard conditions (37°C in a humidified atmosphere of 95% air and 5% CO2) for 24 hours. At the same time, an experiment was performed in which, after adding fresh culture medium or medium with perampanel at a concentration of 100 μΜ, the cells were irradiated with UVB radiation at a dose of 25 mJ/cm 2 and then incubated under standard conditions for 24 hours. After this time, the effect of the substance on the induction of apoptosis and necrosis was tested using the commercially available Celli Death Detection ELISAplus ELISA kit (Roche Diagnostics). The test was performed in accordance with the manufacturer's instructions.

Naświetlanie promieniami UVB komórek melanoma osłabia działanie perampanelu przy krótszej ekspozycji na perampanel (24 i 48 godzin) natomiast nasila działanie perampanelu przy długiej ekspozycji (96 godzin) (Tabela 1 i 3).Perampanel was found to reduce the metabolism and proliferation of UVB-irradiated human melanoma cells line A375 after 24 hours incubation at 100 µM (Table 3); after 48 hours of incubation at 50 and 100 µM (Table 3); after 96 hours of incubation at concentrations of 100 nM - 100 μM (Table 3). The effect of UVB irradiation on the reduction of metabolism and proliferation of human melanoma cells by perampanel is ambiguous. Piśmiennictwo do tabeli 11: Badanie CPMS-2007-2011-002: wyniki w Clinical Pharmacology Review, NDA 202834; (https://www.fda.gov/downloads/Drugs/DevelopmentApprovalProcess/DevelopmentResources/UCM332052.pdf)References to Table 11: Study CPMS-2007-2011-002: Results in Clinical Pharmacology Review, NDA 202834; (https://www.fda.gov/downloads/Drugs/DevelopmentApprovalProcess/DevelopmentResources/UCM332052.pdf)
As indicated by the results available in the literature (Table 11), the concentration of perampanel obtained in the blood after oral administration of this substance is comparable to the lowest concentrations effectively reducing the metabolism and proliferation of human melanoma cells in vii.ro conditions (Table 1-8). This comparison indicates the possibility of using perampanel orally at epilepsy doses in the prevention and treatment of melanoma and its metastases.
Ponadto istnieje możliwość podania perampanelu w iniekcji, np. dożylnie we wlewie 30-minutowym (https://clinicaltrials.gov/ct2/show/NCT03754582). W terapii przeciwnowotworowej podanie leku dożylnie jest jedną z podstawowych dróg podania ponieważ umożliwia uzyskanie odpowiednio dużego stężenia leku i osiągnięcie optymalnego efektu terapeutycznego. In addition, it is possible to administer perampanel by injection, e.g. intravenously as a 30-minute infusion (https://clinicaltrials.gov/ct2/show/NCT03754582). In anti-cancer therapy, intravenous administration of the drug is one of the basic routes of administration because it allows to obtain a sufficiently high concentration of the drug and achieve an optimal therapeutic effect.
A statistically significant reduction in melanoma cell proliferation is obtained in vitro at concentrations slightly higher than concentrations used in the treatment of epilepsy, but achievable after oral or other systemic administration. Comparison of the effective concentrations of perampanel with the concentration of perampanel in suspension. Since perampanel is poorly soluble in 0.0056 mg/mL water (https://www.drugbank.ca/drugs/DB08883), an appropriate vehicle must be used for topical use.
Analysis of the composition of the oral suspension (Schedule 3) indicates that it is possible to obtain a perampanel suspension by using the appropriate ingredients. The concentration of perampanel in the suspension is 0.5 mg/ml, corresponding to a concentration of 1.4 mM. The concentration of perampanel in the suspension is more than 10-fold higher than the highest used in Example 1, which resulted in a large reduction in melanoma cell metabolism and proliferation in vitro. The results indicate that although perampanel is practically insoluble in water, it is possible to obtain suspensions of perampanel at concentrations well above those effective in vitro.
In the tests performed, perampanel was suspended in DMSO and vortexed for 5 min. The highest concentration of perampanel achieved in the experiment was 50 mM. By adding the appropriate amount of the perampanel stock solution to DMSO and vortexing for 1-5 min, the following concentrations were obtained: 100 nM and 100 μM. Final dilutions (100 pM, 100 nM, 50 μΜ, 100 μΜ) were prepared in HBSS or serum culture medium depending on the experimental model used by adding the appropriate amount of perampanel stock solution to HBSS or FBS culture medium and vortexing for 1-5 min , and the total DMSO concentration in the final samples was less than 0.2%.
W farmakoterapii, zewnętrzne zastosowanie leku bezpośrednio na powierzchnię skóry pozwala na uzyskanie wysokich miejscowych stężeń leku. Można wnioskować, że zewnętrzne zastosowanie perampanelu na powierzchnię skóry pozwoli na uzyskanie wysokich miejscowych stężeń perampanelu, takich, które zmniejszają metabolizm i proliferację komórek czerniaka in vitro, co wykazano w Przykładzie 1 (Tabela 1-8).In pharmacotherapy, external application of the drug directly to the skin surface allows for high local concentrations of the drug. It can be concluded that topical application of perampanel to the skin surface will result in high local concentrations of perampanel, such as to reduce melanoma cell metabolism and proliferation in vitro, as demonstrated in Example 1 (Table 1-8).

Treatment options in melanoma in situ: topical and radiation therapy, excision and Mohs surgery. Int J Dermatol. 2010;49(5):482-91. doi: 10.1111 /j.1365-4632.2010.04423.x).
Przeprowadzona analiza pozwala wnioskować, że zewnętrzne zastosowanie perampanelu na powierzchnię skóry pozwoli na uzyskanie wysokich miejscowych stężeń perampanelu, takich, które zmniejszają metabolizm i proliferację komórek melanoma in vitro, co wykazano w badaniach Przykład 1 (Tabela 1- 8).The conducted analysis allows us to conclude that the external application of perampanel to the skin surface will allow to obtain high local concentrations of perampanel, such that they reduce the metabolism and proliferation of melanoma cells in vitro, as demonstrated in the studies Example 1 (Table 1-8).

The presented examples indicate that perampanel can be used in the manufacture of a drug used to prevent or treat skin diseases involving excessive exfoliation of the epidermis, because it reduces the metabolism and proliferation of melanoma cells. Reducing the proliferative capacity of melanoma cells is one of the therapeutic goals in the treatment of melanoma or the prevention of melanoma metastasis.
The presented examples show that perampanel can be formulated for administration by all available drug administration routes, including: parenteral (intravenous, subcutaneous, intramuscular, intracavitary), alimentary (oral, intragastric, enteral, rectal), inhalation, transdermal and external. The dosage form is prepared by methods known in the pharmaceutical industry. A pharmaceutically acceptable form of a composition as used herein is any form thereof which does not cause undesirable effects.
Ponadto, przykłady pokazują, że perampanel indukuje śmierć komórkową: apoptozę i nekrozę w komórkach czerniaka zarówno w warunkach podstawowych, jak również w warunkach promieniowania UVB.The presented examples indicate that perampanel can be used in the manufacture of a drug used to prevent or treat melanoma and its metastases, because it reduces the metabolism and proliferation of melanoma cells both isolated from the primary tumor and melanoma metastases. Reducing the proliferative capacity of cancer cells is one of the therapeutic goals in the treatment of cancer. Moreover, the examples show that perampanel induces cell death: apoptosis and necrosis in melanoma cells under both basal conditions as well as under UVB irradiation.
The advantage of perampanel is that it can be administered orally for a long period of time, which enables its safe use in the prevention of melanoma recurrence, e.g. after its removal, and in the prevention of metastases. Perampanel is used as an anti-epileptic drug. The essence of pharmacological treatment of epilepsy is the use of drugs continuously, sometimes throughout the life of the patient. For this reason, antiepileptic drugs must be characterized by low toxicity and only moderate side effects that are well tolerated by patients even with regular long-term use. Perampanel meets these requirements, and the examples show that, using doses recommended for the treatment of epilepsy, blood concentrations of perampanel are achieved that reduce the metabolism and proliferation of melanoma cells.
An additional advantage of perampanel is the lack of interaction with UVB light. Light is a factor in the development of melanoma, and therefore patients are advised to avoid sun exposure. The interaction of perampanel with light, demonstrated in the examples, allows its use without significantly limiting the daily activities of a melanoma patient related to staying in sunny places.
The examples presented show that perampanel can be administered by all available routes of administration, including: parenteral (intravenous, subcutaneous, intramuscular, intracavitary), maintenance (oral, intragastric, enteral, rectal), inhalation, transdermal and outwardly. The dosage form is prepared by methods known in the pharmaceutical industry. The formulation comprises perampanel and at least one pharmaceutical carrier or diluent. As used herein, a pharmaceutically acceptable form is any of its forms that do not cause adverse effects.
The proposed use of perampanel in the prevention and treatment of melanoma and its metastases is a new therapeutic option. The current treatment of melanoma consists of surgery: radical removal of the primary melanoma focus with an adequate margin of healthy tissue, and, if necessary, removal of regional lymph nodes, local recurrence and metastases. In the case of multiple, unresectable lesions, local treatment (ablative methods, radiotherapy, cryotherapy), tumor or local immunotherapy, electrochemotherapy or systemic treatment may be used. Current pharmacological treatment is based on the use of drugs affecting the immune system (immunotherapy), the use of chemotherapeutic agents, mainly cytotoxic, and the use of drugs with molecularly targeted action (Rutkowski P, Wysocki PJ, Nasierowska-Guttmejer A et al. Cutaneous melanomas. Oncol Clin Pract 2017 ; 13: 241-258. DOI: 10.5603/OCP.2017.0038). The disclosed effect of perampanel is not included in any of the mentioned categories of drugs used in the treatment of melanoma and its metastases. Moreover, currently there is no method of pharmacological prophylaxis of melanoma and its metastases. Due to the unique mechanism of action of perampanel and the acceptable potential side effects associated with its long-term use, systemic or topical use of perampanel and other AMPA antagonists may be of use in melanoma prophylaxis and melanoma metastasis.
