Przedmiotem wynalazku jest jakościowa metoda diagnostyczna oparta o technikę real-time PCR, służąca do detekcji wirusa grypy (Influenza Virus, IV) i określania jego oporności lub podatności na popularny lek przeciwwirusowy - oseltamivir, a także nukleotydy nadające się do analizy wirusa. Przedmiotem wynalazku jest zastosowanie nukleotydów (primerów i sondy) do wykrywania mutacji odpowiedzialnej za wrażliwość na oseltamivir.The subject of the invention is a qualitative diagnostic method based on the real-time PCR technique, used to detect influenza virus (Influenza Virus, IV) and determine its resistance or susceptibility to the popular antiviral drug - oseltamivir, as well as nucleotides suitable for virus analysis. The invention relates to the use of nucleotides (primers and probes) for the detection of the mutation responsible for oseltamivir sensitivity.

This feature is conducive to high genetic variability of the virus, which occurs through point mutations (antigenic drift) and reassortation (antigenic jump).

Pierwsza grupa stanowi inhibitory kanału jonowego M2. Są to przede wszystkim amantadyna, metylowa pochodna rimantydyny oraz rzadziej stosowana rybawiryna. Leki te stosowane są nie tylko w celach leczniczych, ale także w profilaktyce. Niestety wirus bardzo szybko mutuje i staje się niewrażliwy na leki tej generacji. Skuteczność tych leków została wielokrotnie potwierdzona m.in poprzez stosowanie ich, w walce z wirusem H7N7 w Chinach i Belgii oraz w walce z mutantem wirusa H5N1 zakażającym ludzi w Chinach w marcu 2003 roku [Brydak, 2004]. Jednak coraz więcej szczepów charakteryzuje się opornością na oseltamivir i przez to znacznie utrudnione jest leczenie pacjentów zainfekowanych takim szczepem. Jak wykazano oporność na lek wirus uzyskuje w wyniku pojedynczej mutacji w genie kodującym białko neuraminidazę skutkującą mutacją w pozycji 275 - zamianą Histydyny w tyrozynę (H275Y).The second group includes drugs that are competitive inhibitors of neuramimdase. Wiąże się to z koniecznością produkcji nowej szczepionki każdego roku. Czas potrzebny na wyprodukowanie działającego preparatu to około 6 miesięcy, a zdolności produkcyjne szacowane są na około 250 milionów dawek rocznie, przy zapotrzebowaniu około 2-3-krotnie większym. Obecnie stosuje się dwa rodzaje szczepionek; inaktywowane oraz atenuowane. Wyzwaniem jest stworzenie aktywnej szczepionki rekombinowanej, której produkcja będzie znacznie szybsza i mniej pracochłonna [Kamble, et al. 2003].Vaccination is the most effective way to fight the virus and its complications. Kolejnym przedmiotem wynalazku jest zastosowanie sekwencji nukleotydowej primerów oraz sond (Fig. 1A) do powielania fragmentu genu białka NA o długości około 172 nukleotydów i wykrywania mutacji odpowiedzialnej za nabycie oporności na lek przeciwwirusowy korzystnie oseltamivir. Wykrywanie mutacji prowadzi się metodą opartą o technikę real-time PCR z zastosowaniem sondy hydrolizującej (typu TaqMan, Fig. 1B), charakteryzującą się tym, że przeprowadza się reakcję real-time PCR w warunkach: denaturacja wstępna 1 x: 94°C 10 min.; 45 cykli: denaturacja 94°C 10 sek., hybrydyzacja, elongacja 72°C i analizuje się produkty reakcji poprzez analizowanie wykresu w czasie rzeczywistym i po zakończeniu reakcji, korzystając z oprogramowania dołączonego do aparatur.
