Opis wzoru przemys lowego Przedmiotem wzoru przemys lowego jest formularz przeznaczony do dokumentowania ca lego procesu przeprowadzanych zabiegów kosmetycznych. Istot e wzoru stanowi oryginalna forma i konstrukcja logiczno - graficzna do usystematyzowania informacji na temat zabiegu. Przedmiot wzoru jest uwidoczniony na zalaczonej ilustracji, gdzie pokazano na fig. 1 stron e pierwsz a karty zabiegu, na fig. 2 - stron e drug a karty zabiegu. Cechy istotne wzoru przemys lowego 1. Karta zabiegu w kszta lcie formularza formatu A4 z rubrykami charakteryzuje si e tym, ze na formularzu znajduje si e zbiór obwiedzionych ramkami poziomych pól ró znych rozmiarów, nad ka zdym z nich jest nadruk wyznaczaj acy temat i zakres zarejestrowanych informacji, przy czym s a to podsta- wowe informacje osobowe - imi e i nazwisko, data urodzenia oraz dane zwi azane z zabiegiem - za- równo dotycz ace zabiegu bezpo srednio jak i informacje oko lozabiegowe, jak to pokazano na za laczo- nej ilustracji fig. 1 i fig. 2. 2. The essence of the design is its original form and logical and graphical structure for systematizing information about the treatment.
