DESCRIPTION DE L'INVENTIONDESCRIPTION OF THE INVENTION
Natural medicine against HIV / AIDS and other pathologies.
The invention relates to an improved traditional medicine (MTA) obtained from the leaves and roots of the medicinal plant Alchornea cordifolia, of the family Euphorbiaceae, used to treat the Acquired Immune Deficiency Syndrome (AIDS) by the breakdown of the interaction. GP120-CD4 to prevent the virus from starting the first stage of its cycle: retrotranscriptase. This medication is indicated for the treatment of HIV-AIDS and other viral and bacterial infections. The substance is not toxic, it can accompany daily meals and be used by anyone, from pregnant women, to babies to old men.
It only reproduces by parasitism. Virus-cell contact occurs through GP 120 (Glycoprotein 120) and CD4 (Differentiation class topped with the number 4).
Les protéines régulatrices confèrent au virus une variabilité génétique dans la réplication qui fait de lui un phénomène difficile à combattre.GP 120 recognizes its host cell to which it attaches via GP41 in order to be able to spill its genetic content into it. The virus synthesizes a particular enzyme that allows it to have polymerase DNA that it did not have at the start. It is from poymerase DNA that it begins its development cycle, going successively from the viral genome, integrase, provirus, to budding. Budding releases virions. The provirus is the stage where the virus can stay asleep in the cell and it is not known when it can be activated. Regulatory proteins give the virus genetic variability in replication which makes it a difficult phenomenon to combat.
Ils existent plusieurs techniques pour lutter contre le VIH /SIDA :There are several techniques to fight against HIV / AIDS:
We can thus try to block the interaction by competition with an excess of soluble CD4 molecule. Soluble CD4 effectively blocks the entry of viruses into lymphocytes in vitro, but, despite its promising nature, the use of the soluble CD4 molecule in therapy encounters both production problems and long-term effects. term of a molecule very important for the immune system.
Other compounds, negatively charged therefore polyanionic substances (polysulfate such as heparin, dextran sulfate, pentosan polysulfate, mannan sulfate or polycarboxylates such as aurintricarboxylic acid) also act on the GP interaction 120-CD4 and therefore blocks the replication cycle of HIV in its early stages in vitro. Unfortunately their use has certain disadvantages and limitations: they are not very well absorbed orally and must therefore be injected. They can disrupt blood clotting. Finally, their anti-HIV activity varies considerably depending on the viral source. This phenomenon is undoubtedly linked to the great variability of the structures of viral glycoproteins and of GP120 in particular.
Quels sont les inhibiteurs de cette enzyme et comment agissent-ils ?Reverse transcriptase seems to be the best target for therapeutic intervention because of its role in the replication of HIV and especially because of its association with retroviruses only: no reverse transcriptase exists in humans. What are the inhibitors of this enzyme and how do they work?
Les plus connus de ces composés sont des analogues de désoxynucléotides. Les principaux de ces inhibiteurs sont les 2,3-didésoxynucléotides comme les 3-azido 2, 3-drdésoxythymidine (AZT); ou la didésoxycytidine (DDC), la didésoxyinosine (DDI). Ces analogues de type didésoxynucléotides (DDN) peuvent interférer avec la transcriptase inverse de deux manières ;The best known of these compounds are analogues of deoxynucleotides. The main of these inhibitors are 2,3-dideoxynucleotides such as 3-azido 2, 3-drdeoxoxymymidine (AZT); or dideoxycytidine (DDC), dideoxyinosine (DDI). These dideoxynucleotide (DDN) analogs can interfere with reverse transcriptase in two ways;
- Soit en inhibant la transcriptase inverse par compétition avec les substrats naturels,- Either by inhibiting reverse transcriptase by competition with natural substrates,
- Soit par incorporation dans la chaîne d’ADN proviral en formation.- Either by incorporation into the proviral DNA chain in formation.
Indeed, DDNs do not have the 3-hydroxyl group necessary for the attachment of the next deoxynucleotide in the chain in formation and the synthesis stops as soon as one of them is incorporated. One of the big problems with these inhibitors is their toxicity.
Il peut aussi inhiber des ADN polymérases cellulaires à des concentrations toutefois supérieures à celle nécessaire pour inhiber l'enzyme virale.-3The natural substrates of reverse transcriptase are also those of the polymerase enzyme which synthesizes cellular DNA. Although DDNs generally have a greater affinity for reverse transcriptase than for cellular polymerase, their specificity is not absolute. The effect of inhibitors on non-viral polymerases therefore results in side effects: toxicity to bone marrow (AZT), peripheral neuropathy (DDC, DDI) and acute pancreatitis (DDI). To this difficulty is added those which come from the virus: we have thus observed, in many patients, the appearance of resistant viral strains from the third month of treatment at ΓΑΖΤ. Other nucleoside analogues with structures quite different from the previous ones also have anti-HIV activity. These are nucleosides whose sugar is replaced by a halocarbon ring or an acyclic chain. One of the most important of these compounds is the acyclic phosphonate nucleoside (PMEA). This molecule behaves as a competitive inhibitor of DATP for reverse transcriptase. Unfortunately, this compound is not perfectly specific for reverse transcriptase. It can also inhibit cellular DNA polymerases at concentrations however higher than that necessary to inhibit the viral enzyme.
New molecules discovered in 1898: HETP and TIBO are respectively acyclouridines and benzodiazepines, better known for their tranquilizing effect.
They are truly specific for HIV1 reverse transcriptase at very low concentrations. Unfortunately, these and related molecules do not have an inhibitory effect on the replication of HIV 2. In addition, their effectiveness could be compromised by the rapid appearance of viral resistance.
It would be better to find a way to completely block the expression of viral genes. There are indeed antisense oligonucleotides whose nucleotide sequence is complementary to a part of the viral genome and which, therefore, are capable of forming a stable duplex. In principle, the antisense oligonucleotide targets could be both proviral DNA and viral messenger RNA transcribed from proviral DNA, thereby blocking either transcription or translation.
-4Les anti-sens suppriment effectivement la production de protéines virales dans les cellules chroniquement infectées par le VIH mais ils le font à des concentrations nettement supérieures à celles des inhibiteurs de la transcriptase inverse.-4 Antisense effectively suppresses the production of viral proteins in cells chronically infected with HIV, but they do so at concentrations significantly higher than those of reverse transcriptase inhibitors.
The ideal would be to eradicate the virus from the organism and thus succeed in destroying the viral DNA sequences where they are integrated. Claude Hélène’s team has shown that it is possible to manufacture an antisense oligonucleotide on which an intercalating agent such as acridine and a DNA cleavage agent, derived from a metal, have been attached. The strategy is as follows: the oligonucleotide recognizes its target, it associates stably with the aid of the intercalating agent which penetrates into the DNA of the provirus between two successive nucleotides, then the cleavage agent can section the provirus.
The HIV replication cycle offers other possibilities for pharmacological interventions. Thus, the proteins of the virus are synthesized in the form of precursors which must be cut by a particular enzyme, the protease of HIV. There are viral protease inhibitors, but the clinical potential of these substances remains to be defined. Toxic effects are not expected, but we already know that they present problems of limited strong oral absorption and very rapid elimination from the blood.
Another class of potentially promising compounds is that of inhibitors of regulatory proteins that help "wake up" and spread the virus. The provirus would thus be kept dormant. Such substances exist and show efficacy in vitro.
Other molecules theoretically likely to act on infected cells: interferons and inhibitors of a cellular protein with glycosidasic activity. The former would block the formation of viral buds. Used alone, they have actually been very disappointing. As for the second, they would have the effect of modifying the sugars present on the viral envelope. They would limit the budding of the virus or decrease the infectivity of the viral particles. Their effectiveness seems however very limited and no application concerning them is in sight in the short term.
