Description
Ultralvdkontrastmiddel Ultrasound contrast agent
This invention relates to new gaseous contrast agents for use in diagnostic imaging, more specifically to contrast agents containing phospholipid-stabilized gas microbubbles, and to a new method for producing gaseous contrast agents.
It is well known that ultrasound imaging is a potentially valuable diagnostic tool, for example in studies of the vascular system, especially in cardiography, and of the microvasculature in the tissue. A number of contrast agents have been proposed to improve the acoustic images obtained in this way, including suspensions of solid particles, emulsified small liquid droplets, gas bubbles and encapsulated gas or liquid. It is recognized that low-density, easily compressible contrast agents are particularly effective for acoustic backscatter, and considerable interest has therefore been shown in the production of gas-containing and gas-forming systems.
Gaseous contrast agents are also recognized as effective in magnetic resonance (MR) imaging, e.g. as susceptibility contrast agents to reduce the intensity of the MR signal. Oxygenated contrast agents can also become useful paramagnetic MR contrast agents.
It has also been observed that such gases as carbon dioxide can be used as negative oral contrast agents or intravascular contrast agents in X-ray imaging.
Use of radioactive gases, e.g. radioactive isotopes of noble gases such as xenon have also been proposed in scintigraphy, for example for blood-pool imaging.
Derfor har man vist interesse for metoder for å stabilisere gassbobler til Initial investigations involving free gas bubbles formed in vivo by injection into the heart of physiologically acceptable substances have demonstrated the potential effectiveness of such bubbles as contrast agents in echography. However, such techniques are severely limited in practice by the short lifetime of the free bubbles. Therefore, interest has been shown in methods to stabilize gas bubbles
ekkokardiografi og andre ultralydstudier, for eksempel med emulgeringsmidler, oljer, fortykningsmidler eller karbohydrater, eller ved å medføre eller innkapsle gassen eller en forløper for denne i forskjellige systemer, f.eks. porøse gassholdige mikroparukler eller innkapslede gassfylte mikrobobler. echocardiography and other ultrasound studies, for example with emulsifiers, oils, thickeners or carbohydrates, or by entraining or encapsulating the gas or a precursor thereof in various systems, e.g. porous gaseous microwigs or encapsulated gas-filled microbubbles.

There is some knowledge about the use of phospholipids as components in gaseous ultrasound contrast agents. For example, the use of ultrasound contrast agents by phospholipid liposomes where a bilayer of lipids surrounds an entrapped composition containing a gas or gas precursor is presented in US-A-4900540. The encapsulated material is typically a gas precursor such as aqueous sodium bicarbonate, which is said to generate carbon dioxide after introduction into the subject when exposed to the body's pH. The core of the resulting liposomes will therefore tend to contain liquid with extremely small microbubbles of gas which, due to their size, only show a limited echogenicity.
WO-A-9115244 presents ultrasound contrast agents with microbubbles of air or another gas formed in a suspension of liquid-filled liposomes, the liposomes apparently stabilizing the microbubbles. Such systems differ from those described in the above-mentioned US-A-4900540 where the air or the other gas is located inside the liposomes.
WO-A-9211873 describes aqueous preparations which are designed to absorb and stabilize microbubbles and thus act as ultrasound contrast agents. The compositions contain polyoxyethylene/polyoxypropylene polymers and negatively charged phospholipids. The weight ratio of polymer to phospholipid is usually about 3:1.
Ultrasound contrast agents containing gas-filled liposomes, i.e. liposomes which largely do not contain liquid, and the production of such liposomes by means of gas inoculation with vacuum drying is described in WO-A-9222247. Production of such gas-filled liposomes by means of gas inoculation with shaking in the gel stage is described in WO-A-9428780. A report on gas-filled lipid bilayers composed of dipalrnitoylphosphatidylcholine as ultrasound contrast agents is presented by Unger et al. in Investigative Radiology 29, Supplement 2, S134-S136 (1994).
Mengden av fosfolipid trenger derfor ikke å være større enn det som er nødvendig for å danne et enkelt surfaktantlag rundt 3 WO-A-9409829 presents injectable suspensions of gas microbubbles in an aqueous carrier liquid containing at least one phospholipid stabilizer, where the concentration of phospholipids in the carrier is less than 0.01% by weight, but greater than or equal to the amount where the phospholipid molecules are present only at the transition between the gas bubbles and the liquid. The amount of phospholipid therefore need not be greater than what is necessary to form a single surfactant layer around 3
gassmikroboblene, da den resulterende filmliknende strukturen hindrer at boblene kollapser eller koalescerer. Det hevdes at man ikke får mikrobobler med et liposomliknende dobbelt surfaktantlag med så lave fosfolipidkonsentrasjoner. the gas microbubbles, as the resulting film-like structure prevents the bubbles from collapsing or coalescing. It is claimed that microbubbles with a liposome-like double surfactant layer cannot be obtained with such low phospholipid concentrations.
There is a lot more knowledge about the choice of gases for gas microbubble-containing ultrasound contrast agents to improve such properties as the stability and duration of echogenicity. E.g. WO-A-9305819 suggests using free microbubbles of gases having a coefficient Q greater than 5 where
(where A is the density of the gas in kg/m<3>, Cs is the water solubility of the gas in mol/l and D is the diffusivity of the gas in solution in cm<3>/sec). An extensive list of gases that are said to fulfill this requirement is presented.
EP-A-0554213 suggests that gas-filled microvesicles can be made resistant to collapse under pressure by introducing into them at least one gas with a water solubility, expressed in liters of gas/liter of water under standard conditions, divided by the square root of the molecular weight, not exceeding 0.003 . Preferred gases are said to include sulfur hexafluoride, selenium hexafluoride and various Freon® gases. Such gases can i.a. are used in phospholipid-containing compositions of the type described in the above-mentioned WO-A-9215244.
Schneider et al. describes in Investigative Radiology 30(8), pp. 451-457 (1995) a new ultrasonographic contrast agent based on microbubbles which are filled with sulfur hexafluoride and apparently stabilized by a combination of polyethylene glycol 4000 and a mixture of the phospholipids distearoylphosphatidylcholine and dipalmitoylphosphatidylglycerol. The use of sulfur hexafluoride instead of air is said to provide an improved resistance to such pressure increases which occur in the left half of the heart during systole.
Representative blandinger innbefatter en gass med lavt damptrykk og begrenset løselighet i blod eller serum (f.eks. et fluorkarbon) i kombinasjon med en annen gass som raskere byttes ut med gasser i 4 WO-A-9503835 proposes the use of microbubbles containing a gas mixture of a composition which is based on considerations of gas partial pressures both inside and outside the microbubbles, to account for osmotic effects on the size of the microbubbles. Representative mixtures include a gas with a low vapor pressure and limited solubility in blood or serum (eg, a fluorocarbon) in combination with another gas that is more rapidly exchanged with gases in 4
Foretrukne stabilisatorer for slike kontrastmidler er bl.a. fosfolipider. WO-A-9516467 suggests the use of ultrasound contrast agents containing a mixture of gas A and B, where gas B is present in an amount of 0.5-41% by volume, has a molecular weight greater than 80 daltons and water solubility below 0.0283 ml /ml of water under standard conditions, while the rest of the mixture is gas A. Representative A gases are i.a. air, oxygen, nitrogen, carbon dioxide and mixtures of these. Representative B gases include fluorine-containing gases such as sulfur hexafluoride and various perfluorinated hydrocarbons. Preferred stabilizers for such contrast agents are i.a. phospholipids.
Phospholipids said to be useful in today's contrast agents may be lecithins (i.e. phosphatidylcholines), for example natural lecithin such as egg yolk lecithin or soybean lecithin and synthetic or semi-synthetic lecithins such as dimyristoylphosphatidylcholine, dipalmitoylphosphatidylcholine or distearoylphosphatidylcholine; phosphatidic acids; phosphatidylethanolamines; phosphatidylserines; phosphatidylglycerols; phosphatidylinositols; cardiolipins; sphingomyelins; mixtures of any of the foregoing and mixtures with other lipids such as cholesterol. Lecithin derivatives generally appear to be the most commonly used phospholipids, possibly because they are readily available from natural sources. The use of additives such as cholesterol in amounts of up to 50% by weight is presented in WO-A-9115244 and WO-A-9409829, while it is suggested in WO-A-9222247 to incorporate at least a small amount (e.g. about 1 mole percent) negatively charged lipid (eg, phosphatidylserine or a fatty acid) to improve stability. A preferred phospholipid composition according to WO-A-9428780 is dipalmitoylphosphatidylcholine, dipalmitoylphosphatidylethanolamine substituted with polyethylene glycol 5000 and dipalmitoylphosphatidic acid with a molar ratio of about 87:8:5. Typical mixed phospholipid compositions according to WO-A-9409829 and WO-A-9516467 are diarachidoylphosphatidylcholine and dipalmitoylphosphatidic acid with a weight ratio of approximately 100:4, although the latter reference also exemplifies the use of equal weight amounts of distearoylphosphatidylcholine and dipalmitoylphosphatidylglycerol.
It will be obvious from the foregoing that in the existing phospholipid-containing microbubble suspensions proposed for use as 5
Vanligvis er det bare en liten andel, f.eks. ca. 5 %, av ladde fosfolipider. contrast media, at least 50% of the phospholipid content is neutral phospholipids such as lecithins. Usually it is only a small proportion, e.g. about. 5%, of charged phospholipids.
The present invention is based on the discovery that there may be valuable and unexpected advantages in such parameters as product stability and acoustic properties if predominantly charged phospholipids are largely the only amphiphilic component of the microbubble-containing contrast agents. Although we do not wish to be bound by theoretical considerations, it can be assumed that electrostatic repulsion between charged phospholipid membranes stimulates the formation of stable and stabilizing monolayers at the transition surfaces between the microbubbles and the carrier liquid. The flexibility and deformability of such thin membranes will improve the echogenicity of the products of this invention compared to gas-filled liposomes with one or more lipid bilayers.
We have also found that the use of charged phospholipids can make it possible to create microbubble surfactants with advantageous properties when it comes to e.g. stability, dispersibility and resistance to coalescence without resorting to additives such as e.g. other surfactants and/or viscosity-increasing agents, so that the number of components introduced into the body of an individual by injection is kept to a minimum. For example, the charged surfaces of the microbubbles can minimize or prevent clumping due to the electrostatic repulsion.
Thus, according to an implementation of the present invention, a contrast medium for use in diagnostic studies is presented which includes a suspension in an injectable aqueous carrier liquid of gas microbubbles stabilized by a phospholipid-containing amphiphilic material which is characterized in that said amphiphilic material essentially consists of phospholipid and predominantly of molecules that individually have a net charge.
It is desirable that at least 75%, preferably as much as all of the phospholipid material in the contrast agents according to the invention consists of molecules with a net charge under the conditions present during their manufacture and/or use. This charge can either be positive or preferably negative. Representative positively charged phospholipids are esters of such phosphatidic acids as dipalmitoylphosphatidic acid or distearoylphosphatidic acid with amino alcohols such as hydroxyethylenediamine. Examples of negatively charged phospholipids are natural (e.g.
6 6
fra soyabønner eller eggeplomme), halvsyntetiske (f.eks. partielt eller fullstendig from soybeans or egg yolk), semi-synthetic (e.g. partially or fully
The fatty acid acyl groups of such phospholipids will typically each contain 14-22 carbon atoms, e.g. such as in the palmitoyl and stearoyl groups. Lyso forms of such charged phospholipids are also useful according to the invention, the term "lyso" denotes phospholipids containing only one fatty acyl group, which is preferably ester-linked to the 1-carbon of the glyceryl group. Such lyso-forms of charged phospholipids can advantageously be used in admixture with charged phospholipids that contain two fatty acid acyl groups.
Phosphatidylserines are particularly preferred phospholipids which can be used in contrast agents according to the invention and preferably constitute a significant proportion, e.g. at least 80% of the original phospholipid content, for example 85-92%, although this may later be somewhat reduced, e.g. to approx. 70%, by post-treatment, such as heat sterilization. It will be clear that such treatment can lead to the formation of degradation products which are not phospholipids, e.g. free fatty acids, e.g. in amounts up to 10%, references in this document to amphiphilic material consisting predominantly of phospholipid are to be understood as referring to phospholipid with a content of such free fatty acids. Although we do not wish to be bound by theoretical considerations, it may be that ion bridges between the carboxyl and amino groups of neighboring serine groups contribute to the stability of the phosphoylserine-containing systems, as for example the good pressure stability indicates. Preferred phosphatidylserines are i.a. saturated (eg hydrogenated or synthetic) natural phosphatidylserine and synthetic or semi-synthetic dialkanoylphosphatidylserines such as distearoylphosphatidylserine, dipalmitoylphosphatidylserine and diarachidoylphosphatidylserine.
An important advantage of the use of such phosphatidylserine-based contrast agents is that the body recognizes old red blood cells and platelets with a high concentration of phosphatidylserine on the surface and can thus eliminate such contrast agents from the blood stream in a manner similar to the way the red blood cells are eliminated. Moreover, since the surface of such contrast agents can be registered as endogenous by the body, it is possible to avoid that they induce negative systemic side effects such as e.g. hemodynamic effects and other anaphylactic reactions 7
WO-A-9512386). Til støtte for dette er det ikke observert akutte giftvirkninger som f.eks. endringer i blodtrykket eller hjertefrekvensen ved dyreforsøk på hunder som har fått sprøytet inn intravenøs bolus av kontrastmiddel ifølge oppfinnelsen på opptil ti ganger en normal avbildningsdose. which some liposome preparations can carry with them when introduced into the body (see e.g. WO-A-9512386). In support of this, no acute toxic effects have been observed, such as e.g. changes in blood pressure or heart rate in animal experiments on dogs that have been injected with an intravenous bolus of contrast agent according to the invention of up to ten times a normal imaging dose.
Andre halogenerte gasser er bl.a. fluorerte, f.eks. perfluorerte 8 Any biocompatible gas can be used in the contrast agents of the invention, when it is taken into account that the term "gas" here includes all substances (including mixtures) which are substantially or completely in gas and/or vapor form at normal human body temperature of 37 °C. The gas can thus be, for example, air, nitrogen, oxygen, carbon dioxide, hydrogen, nitrous oxide, a noble gas such as helium, argon, xenon or krypton, a sulfur fluoride such as sulfur hexafluoride, disulfur decafluoride or trifluoromethylsulfur pentafluoride, selenium hexafluoride, an optional halogen silane such as tetramethylsilane, a light hydrocarbon (eg containing up to 7 carbon atoms), for example an alkane such as methane, ethane, propane, a butane or a pentane, a cycloalkane such as cyclobutane or cyclopentane, an alkene such as propene or a butene or an alkyne such as acetylene, an ether, a ketone, an ester, a halogenated light hydrocarbon (eg of up to 7 carbons), or a mixture of any of the above. It is an advantage if at least some of the halogen atoms in the halogenated gases are fluorine atoms. Thus, one can choose biocompatible halogenated hydrocarbon gases for example from among bromochlorodifluoromethane, chlorodifluoromethane, dichlorodifluoromethane, bromotrifluoromethane, chlorotrifluoromethane, chloropentafluoroethane, dichlorotetrafluoroethane and perfluorocarbons, e.g. such perfluoroalkanes as perfluoromethane, perfluoroethane, perfluoropropane, perfluorobutanes (e.g. perfluoro-n-butane, possibly in a mixture with other isomers such as e.g. perfluoroisobutane), perfluoropentanes, perfluorohexanes and perfluoroheptanes; perfluoroalkenes such as perfluoropropene, perfluorobutenes (eg perfluorobut-2-ene) and perfluorobutadiene; perfluoroalkynes such as perfluorobut-2-yne; and perfluorocycloalkanes such as perfluorocyclobutane, perfluoromethylcyclobutane, perfluorodimethylcyclobutanes, perfluorotrimethylcyclobutanes, perfluorocyclopentane, perfluoromethylcyclopentane, perfluorodimethylcyclopentanes, perfluorocyclohexane, perfluoromethylcyclohexane and perfluorocycloheptane. Other halogenated gases are i.a. fluorinated, e.g. perfluorinated 8
ketoner som perfluoraceton og fluorerte, f.eks. perfluorerte etere som perfluordietyleter. ketones such as perfluoroacetone and fluorinated, e.g. perfluorinated ethers such as perfluorodiethyl ether.
In the contrast agents of the invention, it may be advantageous to use fluorinated gases such as sulfur fluorides or fluorocarbons (e.g. perfluorocarbons) which are known to form particularly stable microbubble suspensions (see, for example, the article by Schneider et al., referred to above). Gas mixtures based on considerations of partial pressure both inside and outside the microbubbles and the resulting osmotic effects on the microbubble size, e.g. as described in WO-A-9503835, can be used if desired, for example a mixture of a relatively blood-soluble gas such as nitrogen or air and a gas which is relatively insoluble in blood, e.g. a perfluorocarbon.
However, we have found that the contrast agents of the invention, which for example contain microbubbles of a perfluoroalkane such as perfluorobutane, stabilized by phosphatidylserine, are surprisingly stable in size after being administered intravenously to a subject, and do not exhibit the previously described tendency of microbubbles of such gases to grow uncontrollably due to blood gases such as oxygen, nitrogen, and carbon dioxide diffusing into in the bubbles. Instead, they quickly reach a maximum size without any further growth being observed. The fact that one avoids an unlimited increase in the bubble diameter, which could lead to unwanted and potentially very dangerous blockage of capillary vessels, is a main advantage of the contrast agents according to this invention.
Contrast agents of the invention containing perfluoroalkanes such as perfluorobutane have also been found to be surprisingly stable under pressures similar to those typically encountered in vivo, for example, exhibiting almost complete (e.g. at least 90%) recovery of the normal size distribution and the echogenic properties after exposure to overpressure (e.g. air) of up to 300 mmHg for 90 seconds.
Det kan brukes en rekke avbildningsteknikker i ultralydapplikasjoner, for eksempel fundamental og 9 The contrast agents of the invention can be used in a number of diagnostic imaging techniques, including scintigraphy, light imaging, ultrasound, MR and X-ray imaging (also with soft X-rays). Their use in diagnostic ultrasound and MR imaging, e.g. as susceptibility contrast agents, constitute preferred aspects of this invention. A variety of imaging techniques can be used in ultrasound applications, such as fundamental and 9th
No. PCT/GB96/02413. harmonic B-mode imaging and fundamental and harmonic Doppler imaging. If desired, three-dimensional imaging techniques can be used. The contrast agent can also be used in ultrasound imaging methods based on correlation techniques, for example as described in US-A-5601085 and International Patent Application No. PCT/GB96/02413.
Ultrasound tests in vivo on dogs have shown that the contrast agents according to the invention can induce an increase in the intensity of the signal that is scattered back from the myocardium of 15-25 dB after intravenous injection of doses as low as 1-20 nine microbubbles/kg body weight. The signals can also be observed at lower doses when more sensitive techniques are used, such as color Doppler or Doppler-derived techniques, e.g. amplitude-based Doppler or such non-linear techniques as described by Tucker et al. in Lancet (1968) p. 1253, by Miller in Ultrasonics (1981) pp. 217-224 and by Newhouse et al. in J. Acoust. Soc. Am. 75, pp. 1473-1477 (1984). At such low doses, the attenuation in blood-filled rooms such as e.g. the heart chambers found to be so low that it is possible to visualize interesting regions of the myocardial vasculature. Tests have also shown that such intravenously injected contrast agents are distributed throughout the blood mass and thus improve the echogenicity of all vascularized tissues, and they will also be recycled. They have also been found to be useful as general aids for Doppler signal enhancement, and may additionally be useful in computer controlled ultrasound tomography and in physiological gating or intermittent imaging.
For ultrasound, e.g. in echocardiography, it may be useful to use microbubbles with an average size of 0.1-10 :m, e.g. 1-7 :m, to allow free passage through the pulmonary system and to achieve resonance with the preferred imaging RF frequencies of approximately 0.1-15 MHz. We have found that contrast agents according to the invention can be produced with a very narrow size distribution for the microbubble dispersion in the area that is preferred for echocardiography, so that echogenicity as well as safety in vivo is improved considerably, and the contrast agents become particularly advantageous in such areas of use as blood pressure measurement, blood flow measurement and ultrasound tomography. For example, products where over 90% (e.g. at least 95%, preferably at least 98%) of the microbubbles have diameters in the range of 1-7 nm and less than 5% (e.g. no more than 10 3%, preferably no more than 2%) of the microbubbles have diameters above 7 µm are produced without problems.
Til bruk i ultralyd kan oppfinnelsens kontrastmidler for eksempel tilføres i slike doser at mengden av tilført fosfolipid ligger i området 0,1-10 ug/kg kroppsvekt, f.eks. 1-5 ug/kg for fundamental B-mode-avbildning. For use in ultrasound, the contrast agents of the invention can, for example, be added in such doses that the amount of added phospholipid is in the range 0.1-10 ug/kg body weight, e.g. 1-5 ug/kg for fundamental B-mode imaging. It will be clear that such low phospholipid values are a significant advantage when it comes to minimizing possible toxic effects. Furthermore, the low phospholipid concentration in doses that are effective can enable dosage increases that extend the observation time without negative side effects.
The total concentration of phospholipid in injectable forms of contrast agents according to the invention can advantageously be in the range of 0.01-2 weight percent, for example 0.2-0.8 weight percent, preferably around 0.5 weight percent.
In general, we have found it unnecessary for the contrast agents according to the invention to use such additives as emulsifiers and/or viscosity-increasing agents which are common in many existing contrast agent formulations. As noted above, this is advantageous in keeping the number of components introduced into the body to a minimum and ensuring that the viscosity of the contrast agent is as low as possible. Since the preparation of the contrast agents usually involves a freeze-disturbing step as discussed in detail below, it may however prove advantageous to use one or more agents with cryoprotective and/or lyoprotective action and/or one or more fillers, for example an alcohol such as e.g. t-butanol; a polyol such as glycerol; an amino acid such as glycine; a carbohydrate, e.g. a sugar such as sucrose, mannitol, trehalose, glucose, lactose or a cyclodextrin, or a polysaccharide such as dextran; or a polyglycol such as polyethylene glycol. A comprehensive list of agents with cryoprotective and/or lyoprotective action is given in Acta Pharm. Technol. 34(3), pp. 129-139 (1988), the contents of which are incorporated herein by reference. Use of saccharides that are well tolerated physiologically, such as sucrose, e.g. in such an amount that the product becomes isotonic or slightly hypertonic, is preferred.
Dagens mikrobobleholdige kontrastmidler, for eksempel som beskrevet i WO-A-9409829, fremstilles vanligvis ved å bringe surfaktant i pulverform, f.eks. frysetørkede forhåndsrfemstilte liposomer eller frysetørkede eller spray-tørkede 11 Today's microbubble-containing contrast agents, for example as described in WO-A-9409829, are usually prepared by bringing surfactant into powder form, e.g. freeze-dried preformed liposomes or freeze-dried or spray-dried 11
Slike prosesser lider imidlertid av de ulempene at det må tilføres en høy agitasjonsenergi for at den ønskede dispersjonen skal dannes, og at størrelsen og størrelsesfordelingen til mikroboblene avhenger av den brukte energimengden og derfor ikke kan kontrolleres. phospholipid solutions, together with air or other gases and then with an aqueous carrier, and stirring to form a microbubble suspension which must then be administered to the individual shortly after preparation. However, such processes suffer from the disadvantages that a high agitation energy must be supplied for the desired dispersion to be formed, and that the size and size distribution of the microbubbles depends on the amount of energy used and therefore cannot be controlled.
We have now found that the contrast agents according to the invention can be advantageously prepared by forming a gas microbubble dispersion in a suitable phospholipid-containing aqueous medium, which, if desired, can be autoclaved or otherwise sterilized in advance, and then subjecting the dispersion to lyophilization to obtain a dried reconstitutable product. Such products, such as can be the lyophilization residue of a suspension of gas microbubbles in an aqueous medium containing amphiphilic material where the amphiphilic material predominantly consists of phospholipid where a large majority of the molecules have a net charge, is another aspect of this invention. When the dried product contains one or more cryoprotective and/or lyoprotective agents, it may for example contain a matrix of cryoprotectant and/or lyoprotectant (e.g. carbohydrate) which emits microbubbles and contains gas-filled, substantially spherical cavities or vacuoles which are surrounded by of one or more layers of the amphiphilic material.
More specifically, we have found that dried products prepared in this way are particularly easy to reconstitute in aqueous media such as e.g. water, an aqueous solution such as a salt solution (which can be advantageously balanced so that the final product for injection is not hypotonic), or a water solution of one or more substances for adjusting the tonicity such as e.g. salts (e.g. of plasma cations with physiologically acceptable counterions), or saccharides, sugar alcohols, glycols and other non-ionic polyol materials (e.g. glucose, sucrose, sorbitol, mannitol, glycerol, polyethylene glycols, propylene glycols and the like) which do not require more than a minimal agitation that can be carried out e.g. by light shaking by hand. The size of the microbubbles that are formed in this way is always reproducible and in practice independent of how much agitation energy is used because it is determined by the size of the microbubbles that are formed in the original microbubble dispersion, this size parameter is surprisingly preserved throughout in the lyophilized and 12
rekonstituerte produktet. Therefore, since the size of the microbubbles in the initial dispersion can be easily controlled by such process parameters as the method, speed and duration of agitation, the final microbubble size can be easily controlled.
Lyophilized products according to the invention have been shown to be storage-stable for several months at room temperature. The microbubble dispersions formed by reconstitution in water or water solution can be stable for at least 12 hours, so that considerable flexibility is achieved in relation to when the dried product must be reconstituted before injection.
The process described above for the production of contrast agents according to the invention can generally be used to produce contrast agents that include suspensions in an injectable aqueous carrier liquid of gas microbubbles that are stabilized by membrane-forming lipids, which can be both neutral and charged lipids (e.g. phospholipids) as well as mixtures of these. A process for forming a contrast agent according to the invention comprises the steps: i) forming a dispersion of gas microbubbles in an aqueous medium containing an amphiphilic material which essentially consists of phospholipids and predominantly of molecules which individually have a net charge,
ii) lyofilisere den resulterende gassdispersjonen i nærvær av en kryoprotektant og/eller lyoprotektant for å oppnå et tørket produkt omfattende en kryoprotektant- og/eller lyoprotektant-matriks som inneholder biokompatible gassfylte, hovedsakelig sfæriske hulrom eller vakuoler omgitt av et eller flere lag av nevnte amfifile materiale, og ii) lyophilizing the resulting gas dispersion in the presence of a cryoprotectant and/or lyoprotectant to obtain a dried product comprising a cryoprotectant and/or lyoprotectant matrix containing biocompatible gas-filled, substantially spherical cavities or vacuoles surrounded by one or more layers of said amphiphiles material, and
iii) rekonstituere dette tørkede produktet i en injiserbar vandig bærervæske, utgjør et annet aspekt av denne oppfinnelsen, og det gjør også et rekonstituerbart tørket produkt som kan fås ifølge trinn (i) og (ii) av denne prosessen, for eksempel et produkt som inneholder en matriks som frigjør mikrobobler (f.eks. en matriks av kryoprotektant/lyoprotektant) og inneholder gassfylte, stort sett sfæriske hulrom eller vakuoler omgitt av lag av membrandannende lipidmateriale. iii) reconstituting this dried product in an injectable aqueous carrier fluid constitutes another aspect of this invention, and so does a reconstituted dried product obtainable according to steps (i) and (ii) of this process, for example a product containing a matrix that releases microbubbles (eg a matrix of cryoprotectant/lyoprotectant) and contains gas-filled, largely spherical cavities or vacuoles surrounded by layers of membrane-forming lipid material.
13 13
For example, step (i) can be realized by subjecting the lipid-containing aqueous medium to any suitable emulsifying technique, such as sonication, shaking, high-pressure homogenization, high-speed stirring or high-shear mixing, e.g. using a rotor-stator homogenizer, in the presence of the selected gas. The aqueous medium can, if desired, contain additives that increase the viscosity and/or increase the solubility of the lipid, for example alcohols or polyols, e.g. glycerol and/or propylene glycol.
The gas used in the emulsification step does not necessarily have to be the same as what you want in the final product. Most of this gas content can therefore be removed during the subsequent lyophilization step and what is left of the gas during vacuum treatment of the dried product can then be supplied with an atmosphere of the gas you want in the final product. The emulsifying gas can therefore be chosen solely to optimize the parameters of the emulsifying process, without taking the final product into account. We have found that emulsification in the presence of a sulfur fluoride such as sulfur hexafluoride or a fluorinated hydrocarbon gas such as e.g. a perfluoroalkane or perfluorocycloalkane preferably containing 4 or 5 carbon atoms is particularly advantageous for obtaining end products with a consistent and narrowly distributed microbubble size.
The emulsification can advantageously be carried out at approximately room temperature, e.g. at approx. 25±10 °C. It may be necessary to heat the aqueous medium initially to facilitate the hydration and thus dispersion of the phospholipid and then allow it to cool to ambient temperature before starting the emulsification.
Visse slike dispersjoner er presentert i vår internasjonale patentsøknad Gas dispersions prepared according to step (i), especially aqueous dispersions of gas microbubbles stabilized by amphiphilic material consisting predominantly of phospholipid where a large majority of the molecules have a net charge, form an aspect of this invention. Certain such dispersions are presented in our international patent application
Patentkravet for disse intermediære dispersjonene hvor det amfifile materialet innbefatter dipalmitoylfosfatidylserin, mer spesifikt i form av natriumsaltet, enten 14 WO-A-9640275 as intermediate consumption in the production of diagnostic contrast agents containing microbubbles of gas stabilized by one or more membrane lipids which are cross-linked or polymerized in the hydrophilic part. The patent claim for these intermediate dispersions where the amphiphilic material includes dipalmitoylphosphatidylserine, more specifically in the form of the sodium salt, either 14
alene eller i kombinasjon med dipalmitoylfosfatidylcholin, og gassen er en blanding av luft og perfluorpentan, en blanding av luft og perfluorheksan eller en blanding av perfluorbutan og perfluorheksan, fravikes herved. alone or in combination with dipalmitoylphosphatidylcholine, and the gas is a mixture of air and perfluoropentane, a mixture of air and perfluorohexane or a mixture of perfluorobutane and perfluorohexane, is hereby waived.
It will be clear that since they are intermediate stages, these dispersions will not be prepared in sterile, physiologically acceptable form, while gas dispersions obtained according to step (i) according to the present invention will be prepared in sterile, physiologically acceptable form (e.g. by of sterile, pyrogen-free water or saline as the aqueous carrier fluid) if they are intended for use as contrast agents as such.
Dispersions prepared according to step (i) can advantageously be washed one or more times before the contrast medium is used or the lyophilization step (ii) begins, in order to separate and remove substances added to increase viscosity or solubility, as well as unwanted substances such as gas-free colloidal particles and too small or too large microbubbles. The washed microbubble dispersions thus obtained form an aspect of this invention. Such washing can be carried out in an otherwise known manner, where the microbubbles are separated out with such techniques as fiotation or centrifugation. The possibility of removing additives in this way and also obtaining microbubble dispersions with a particularly narrow size distribution represent important advantages of the process according to the invention, in particular because the resulting size distribution as noted above is maintained predominantly unchanged after lyophilization and reconstitution. Therefore, it is particularly preferred to use a process that includes the steps of gas dispersion, washing/separation, lyophilization and reconstitution.
Disturbance-fractionated microbubble dispersions can be produced where at least 90% of the microbubbles have a size within a range of 2 µm, where the microbubbles preferably have a volume mean diameter in the range 2-5 µm. Such dispersions and frozen and lyophilized products derived from them, e.g. as described below, are other aspects of this invention.
If one or more cryoprotective and/or lyoprotective agents are used, these can advantageously be added after the washing steps and before lyophilization.
Slike frosne gassdispersjoner, d.v.s. frosne vandige 15 Lyophilization of the gas dispersion can be carried out, for example, by first freezing it and then lyophilizing the frozen gas dispersion, for example with a generally well-known method as such. Such frozen gas dispersions, i.e. frozen watery 15
Slike produkter kan lagres frosne og tines opp hvis nødvendig, f.eks. ved enkel oppvarming og/eller ved tilsetning av en bærervæske, for å regenerere mikrobobledispersjoner som er nyttige som kontrastmidler ifølge oppfinnelsen. dispersions which release microbubbles and contain gas microbubbles stabilized by amiphilic material consisting predominantly of phospholipid where a large majority of the individual molecules have a net charge constitute another aspect of this invention. The microbubbles can advantageously be size-fractionated before they are frozen, so that the freed microbubbles preferably have a volume mean diameter within the range of 2-5 nm. Such products can be stored frozen and thawed if necessary, e.g. by simple heating and/or by adding a carrier liquid, to regenerate microbubble dispersions which are useful as contrast agents according to the invention.
Since the dried product will normally be reconstituted according to step (iii) above before use, the gas dispersion can advantageously be filled into glasses which can be sealed before lyophilization so that each of the glasses contains a suitable amount, e.g. a single dose, of lyophilized dried product for reconstitution in injectable form. By lyophilizing the gas dispersion in individual glasses instead of collectively, one avoids manipulation of the delicate honeycomb-like structure of the lyophilized product, with the risk of at least partially degrading this structure. After lyophilization and any further gas blowing as well as introduction into the air space above the product of such gas as you want to have as microbubbles in the finally formulated contrast agent, the glasses can be sealed by a suitable method. It will be clear that the ability to select gas content in the final product, together with the independent ability to control microbubble size in the final product by selecting appropriate process parameters during the first dispersing step and any subsequent washing/separation steps, allows for free choice of microbubble size and gas content, so that the products can be adapted to specific uses.
In general, the frozen gas dispersion or the dried product from step (ii), e.g. after necessary and/or desired supplementation or replacement of the gas content, reconstituted by adding a suitable sterile, aqueous injectable carrier liquid, e.g. sterile pyrogen-free water for injection, an aqueous solution such as a salt solution (which can advantageously be balanced so that the final product for injection is not hypotonic), or an aqueous solution of one or more substances for adjusting the tonicity (e.g. as described above). If the dried product is in a glass, it is practical if this is sealed with a septum through which the carrier liquid can be injected with a needle that may be filled with 16
Alternatively, the dried product and the carrier liquid can be supplied simultaneously in a device with two chambers, such as e.g. a two-chamber syringe. It may be advantageous to mix or shake the product lightly after reconstitution. As mentioned above, however, the size of the gas microbubbles in the stabilized contrast agents according to the invention can essentially be independent of how much agitation energy is supplied to the reconstituted dried product. Therefore, it is unlikely that anything more than a light shake by hand will be required to achieve reproducible products with consistent microbubble size.
The following non-limiting examples serve to illustrate the invention.

Virkningen av relative meneder ladde fosfolipider The effect of relative perjury charged phospholipids
According to the general method described below and with the process parameters in table 1.1 below, dispersions of microbubbles stabilized by different phospholipids or phospholipid mixtures were made.
Solutions of the selected phospholipids or phospholipid mixtures were prepared in water containing 5.4% by weight of a mixture of propylene glycol and glycerol (weight ratio 3:10) with a phospholipid concentration of 2-5 mg/ml (for phosphatidylethanolamine, the water was adjusted to pH = 10.5 with sodium hydroxide), the phospholipid was hydrated by ultrasound treatment and/or heating to approximately 80 °C time as indicated (table 1.1) and cooled to room temperature before use. A given volume of this solution was distributed between several 2 ml chromatography glasses with 0.8-1 ml solution per. glass. The air in each jar was replaced with perfluorobutane gas, and the jars were tightly closed and shaken for 45 seconds with an Espe CapMix® (mixer for dental materials). The resulting microbubble dispersions were transferred to a larger beaker and centrifuged at 2000 rpm for 5 minutes to obtain a turbid infranatant below a floating layer of microbubbles. The infranatant was removed with a cannula and replaced with an equal volume of water of neutral pH. The washing step was repeated, but now the infranatant was replaced with a 10% by weight sucrose solution. Aliquots of 2 ml of the washed dispersion were distributed between flat-bottomed 10 ml lyophilization vials, and the vials were cooled to -47°C and lyophilized for approximately 48 hours, resulting in a white fluffy solid. The glasses were transferred to a vacuum chamber, and the air was removed with a vacuum pump and replaced with perfluorobutane gas. Before use, water was added and the jars were gently shaken by hand for several seconds to create microbubble suspensions suitable as ultrasound contrast agents.
Prøver på 20 18 The microbubbles' size distribution and volume concentration were measured with a Coulter Counter Mark E apparatus with a 50 :m aperture and measuring range 1-30 :m. Try on 20 18
:1 was diluted in 200 ml of saline saturated with air at room temperature and stood for 3 minutes to achieve equilibrium before measurement. The measurements were made on the microbubble dispersions before lyophilization (washed bubble dispersion) and after lyophilization (reconstituted with water to the same volume as before lyophilization). The data is presented in table 1.2 below.
The efficiency of the lyophilization for the different phospholipid-stabilized microbubble dispersions was calculated as survival percentage of the volume concentration after lyophilization and reconstitution. A graph (see fig. 1) shows how this parameter varies with the relative amount of charged phospholipid in the membrane. As can be seen, the efficiency of the lyophilization increases with an increased amount of charged phospholipid in the membrane, and is highest for membranes that only contain charged phospholipids.

bj Fremstilling av dispersjoner av perfluorbutanmikrobobler ved hielp av rotor-stator- blanding bj Production of dispersions of perfluorobutane microbubbles with the help of a rotor-stator mixture
500.4 mg of hydrogenated phosphatidylserine from eggs was added to 100 ml of water containing 5.4% by weight of a mixture of propylene glycol and glycerol (weight ratio 3:10). The mixture was shaken and heated to 80°C for five minutes, cooled to room temperature, shaken again and set aside overnight before use. 50 ml of the resulting solution was transferred to a round-bottom flask with a conical neck. The bottle was equipped with a glass cover with a temperature-controlled inlet and outlet connected to a water bath which maintained a stable temperature of 25 °C. A rotor-stator agitator was introduced into the solution and, to avoid gas leakage, the space between the throat wall and the agitator was sealed with a specially designed metal plug equipped with a gas inlet/outlet connection for pressure control and adjustment of the gas content. The gas outlet was connected to a vacuum pump and the solution was degassed for one minute. An atmosphere of perfluoro-n-butane was then supplied through the gas inlet.
The solution was homogenized at 23000 rpm for 10 minutes while the rotor-stator agitator was held with the openings slightly above the liquid surface. The result was a white colored creamy dispersion, which was transferred to a sealable container and rinsed with perfluoro-n-butane. The dispersion was then transferred to a separatory funnel and centrifuged at 12,000 rpm for 30 minutes, so that a creamy layer of microbubbles was obtained at the top and a turbid infranatant. The infranatant was removed and replaced with water. The centrifugation was then repeated twice, but now at 12,000 rpm for 15 minutes. After the last centrifugation, the supernatant was exchanged with 10 wt% sucrose. Aliquots of 2 mL of the resulting dispersion were dispensed into a number of 10 mL flat-bottomed lyophilization vials, and the vials were cooled to -47°C and lyophilized for approximately 48 hours, resulting in a white, fluffy solid. The glasses were transferred to a vacuum chamber and the air was removed with a vacuum pump and replaced with perfluoro-n-butane gas. Before use, water was added and the jars were shaken lightly by hand for several seconds to create microbubble dispersions suitable as ultrasound contrast agents.

This solution was pumped through a 4 ml sonicator flow cell and subjected to ultrasound at 20 kHz with amplitude 90 µm. The diameter of the sonicator home was 1.3 cm, the inner diameter of the cell was 2.1 cm, and the distance between the horn and the bottom of the cell was 1 cm. The lipid solution was mixed with perfluoro-n-butane to a volume ratio of 1:2 before being introduced into the sonicator cell (20 ml/min lipid solution and 40 ml/min perfluoro-n-butane gas). The temperature was kept stable at 33 °C. The result was a white, creamy dispersion which was filled into a container and rinsed with perfluoro-n-butane.
Karakterisering Characterization
The microbubbles' size distribution and volume concentration were measured with a Coulter Counter Mark H apparatus with a 50 :m aperture and measuring range 1-30 :m. Samples were diluted 20:1 in 200 mL of saline saturated with air at room temperature and allowed to stand for 3 minutes to equilibrate before measurement.
The ultrasound characterization was performed with an experimental setup slightly modified from that described by de Jong, N. and Hoff, L. in "Ultrasound scattering properties of Albunex microspheres", Ultrasonics 31(3), pp. 175-181 (1993) . This instrumentation measures the effectiveness of the attenuation in the ultrasound frequency range 2-8 MHz for a dilute suspension of contrast agent. During measurement of the attenuation, a pressure stability test was performed by exposing the sample to an overpressure of 120 mmHg for 90 seconds. Typically 2-3:1 of the sample was diluted in 55 ml of Isoton II and the diluted sample suspension was stirred for 3 minutes before analysis. As the primary response parameter, the attenuation at 3.5 MHz was used, together with the value of the recovery attenuation at 3.5 MHz after release of the overpressure.

Prosedyren ble gjentatt med nye prøvepar for hver av de utvalgte gassene. trifluoromethylsulfur pentafluoride and tetramethylsilane, according to the following procedure: Two samples containing lyophilized product from example 2(b) were placed in a desiccator with inlet and outlet for gas. The desiccator was connected to a Btichi 168 vacuum/distillator control in order to supply the samples with a selected gas and control the evacuation. The pressure above the samples was lowered to approximately 10 mbar and held there for 5 minutes, then it was increased to atmospheric pressure by adding the selected gas. The test tubes were then tightly closed. The procedure was repeated with new sample pairs for each of the selected gases.
Resultatene er oppsummert i tabell 3.1. 2 ml of distilled water was added to each glass and the glasses were lightly shaken by hand before use. The resulting microbubble dispersions were characterized by measurements of the size distribution as described in example 2. The results are summarized in table 3.1.
As can be seen from the results above, there were no significant differences in the size distribution for the different gases.
Resultater in vivo Results in vivo
An aliquot produced with each of the five gases was tested in vivo to record improvements in Doppler imaging at 10 MHz. The dispersions were injected into chinchilla rabbits via a vein in the ear and measured using a Doppler technique with an ultrasound probe directly on a carotid artery. Recording of signal intensity and duration was carried out and the integral of the Doppler curve was calculated. The results (see Table 3.2 below) showed that microbubbles containing perfluorobutane produced the strongest Doppler intensity increase. Microbubbles containing sulfur hexafluoride, trifluoromethylsulfur pentafluoride or tetramethylsilane did not give significantly worse Doppler enhancement than those containing perfluorobutane, with integrals of around 60-80% of the values for perfluorobutane.

250 mg of hydrogenated phosphatidylserine from eggs was added to 50 ml of water for injection containing 5.4% by weight of a mixture of propylene glycol and glycerol (weight ratio 7:20). Sentrifugeringen ble så gjentatt to ganger, a separatory funnel and centrifuged at 12,000 rpm for 30 minutes, so that a creamy layer of microbubbles was obtained over a turbid infranatant. To 6 ml of the resulting dispersion was added 6 ml of a 30% by weight trehalose solution. Portions of 2 ml of this dispersion were distributed among a number of flat-bottomed 10 ml lyophilization glasses, and the glasses were cooled to -47 °C and stored at this temperature for 24 hours.
Half of the portions were thawed after 24 hours at -47 °C and gave homogeneous creamy white dispersions of gas microbubbles which were suitable as ultrasound contrast agents. The printed dispersions were characterized by measuring the size distribution as described in example 2 above (see table 4.1). The remaining portions were lyophilized for approximately 48 hours, resulting in a fluffy white solid. The glasses were transferred to a vacuum chamber, the air was pumped out with a vacuum pump and replaced with perfluoro-n-butane gas. Before use, they were added to water and the jars were lightly shaken by hand for several seconds to create bubble dispersions suitable as ultrasound agents. The reconstituted products were characterized by measuring the size distribution and the acoustic attenuation with the methods described in example 2 above. The results are presented in table 4.1.

Dispersion of microbubbles with <p>erfluorobutane exposed to air-saturated liquid A glass containing lyophilized material under an atmosphere of perfluorobutane was prepared as described in Example 2(b). Water was added to the glass just before use to make a microbubble dispersion.
200 ml of Isoton n liquid was exposed to air for several days at room temperature so that the solution was completely saturated with air. Another portion of 200 ml of the liquid was degassed in a vacuum bottle at 60 °C for one hour and cooled to room temperature under vacuum. Air was released into the bottle just before use.
Aliquots of 10:1 of the microbubble suspension were added to each of the liquid samples and the resulting mixtures were incubated for 5 min before size characterization (Coulter Multisizer Mark II).
In the degassed environment, where there was no reason to expect gas diffusion from the liquid into the microbubbles, the mean diameter of the microbubbles was 1.77 µm, and 0.25% of the microbubbles were larger than 5 µm. In the air-saturated liquid, the corresponding values were 2.43 :m and 0.67%. New measurements taken after 5 minutes indicated that the microbubble size had reached a stable value.
Denne undersøkelsen viser at gjennomsnittsdiameteren til mikroboblene bare økte med 37 % da de ble utsatt for en luftmettet væske tilsvarende arterieblod, og svært få 28 This investigation shows that the average diameter of the microbubbles only increased by 37% when exposed to an air-saturated fluid equivalent to arterial blood, and very few 28
This can be compared to the doubling of the size of perfluorohexane-containing microbubbles in a similar environment (i.e. a highly dilute dispersion of microbubbles in water with dissolved air), which is reported in Example II of WO-A-9503835.
Eksempel 6 Example 6
Trykkstabiliteten til en dispersjon av perfluorbutanmikrobobler The pressure stability of a dispersion of perfluorobutane microbubbles
Glass was prepared as described in example 2(a) with lyophilized material under an atmosphere of perfluorobutane. Just before use, water (2 ml) was added to the vials to make microbubble dispersions.
Attenuation spectra were recorded for 1.5-8 MHz before, during and after an overpressure of air of 300 mmHg was applied. The pressure was removed after 90 seconds. The results are shown in fig. 2 of the illustrations, showing that although the attenuation at 4 MHz (the contrast agent peak without pressure loading) dropped to less than one-third with overpressure, it was almost completely (85%-95%) restored when the overpressure was removed.
Overpressure of air of up to 300 mmHg was used for a duration of 90 seconds. The attenuation was measured at 3.5 MHz. The results are shown in fig. 3 and indicates that the attenuation was well restored (at least approx. 95%) after the pressure was removed for all the overpressure values used.
The size distributions were determined by Coulter analysis for a sample that was not under pressure and for samples that were exposed to overpressure of air of 150 and 300 mmHg for a duration of 90 seconds. The results are shown in fig. 4 and indicates that there were no significant differences between the distribution curves in the range 1-10 :m.
