The invention relates to an injection device for injecting a fluid into tissue of a body cavity, such as a bladder or an intestinal canal. The injection device comprises a hollow elongate body and an injector for injection the fluid in the tissue. The injector comprises an injection needle having an injection tip. The injector is located in the hollow elongate body 5 and is movable through said elongate body such that the injection needle is movable; into a first position in which at least the injection tip of the injection needle is located inside the elongate body and into a second position in which at least the injection tip of the injection needle is located outside the elongate body.
Said injection device is amongst others used to inject botulinum (often referred to as 10 Botox) in the tissue of the cavity of a living human being. In said situation, the elongate body is placed in the body cavity while the injection needle is located in the first position. While being located in the body cavity, the injection needle is moved into the second position. The injection tip of the injection needle is subsequently introduced in the tissue of the body cavity, after which the fluid is guided through the injector and via the injection needle 15 injected into the tissue. Many treatments require that a large number of injections are made in the tissue of the body cavity.
Above mentioned method may be used to treat incontinence. In said situation, the elongate body is placed in the bladder via the urethra while the injection needle is located in the first position. For said treatment it is required that a large number of injections with 20 botulinum are made in the bladder tissue. This is a very time consuming treatment, wherein the part of making the injections alone already takes around 45-60 minutes. Having the injection device placed in the urethra makes the treatment very unpleasant for the patient. In general the treatment is even more unpleasant for male patients due to the fact that the urethra of a male patient is much longer then the one of a female patient. To make the 25 treatment bearable, the treatment is generally performed under general anesthesia. Due to this, the treatment in general has to be performed in a hospital. For many patients, especially the elder ones and the ones who constantly have to stay in bed, it is a high burden to travel to a hospital.
The object of the invention is to provide an improved or at least an alternative 30 injection device for injecting a fluid into tissue of a body cavity. The object is achieved by a an injection device for injecting a fluid into tissue of a body cavity, which injection device comprises a hollow elongate body and an injector, wherein the injector comprises an injection needle having an injection tip, the injector is located in the hollow elongate body -2- and is movable through said elongate body such that the injection needle is movable; into a first position in which at least the injection tip of the injection needle is located inside the elongate body and into a second position in which at least the injection tip of the injection needle is located outside the elongate body, characterized in, that the injector comprises 5 multiple injection needles which are movable into the first position and the second position.
In an embodiment of the injection device according the invention, the elongate body comprises a longitudinal axis and the injection device is configured such that the injection tips of the injection needles in the second position extend traverse to the longitudinal axis.
The injection device may be configured such that the injection tips of the injection 10 needles in the first position extend substantially parallel to the longitudinal axis. In the first position the injection needles may be entirely located inside the elongate body and in the second position the injection needles may be entirely located outside the elongate body.
The elongate body may comprise a longitudinal axis and the injection device may be configured such that the injection needles in the second position extend traverse to the 15 longitudinal axis. The injection device may be configured such that the injection needles in the first position extend substantially parallel to the longitudinal axis.
The injection tips of the injection needles in the second position may define a circle. The injection tips of the injection needles in the second position may define a part of a sphere. The injection tips of the injection needles in the second position may define a half 20 sphere.
Each injection needle may comprise a blocking member for blocking a further movement of the injection needle through the tissue after the needle has entered the tissue for a specific distance. The blocking member may be located near the injection tip.
The blocking member may comprise a blocking surface for supporting on the tissue. 25 The blocking surface may be located at a distance D of between 2 and 5 mm from the injection tip. The distance D may be between 2 and 3 mm. The distance D may be around 3 mm. Each of the injection tips of the injection needles in the second position may extend in a different direction.
The injection device may comprise a needle positioner for positioning the injection 30 needles in the first position when the injector is moved in a first direction relative to the elongate body and for positioning the injection needles in the second position when the injector is moved in a second direction relative to the elongate body.
The injector may comprise an elongate fluid supply member for supplying the fluid to the injection needles and the needle positioner may comprise a pivot member via which the 35 injection needles are in fluid connection with the fluid supply member and the pivot member allows the injection needles to be moved in the first position and in the second position when the injector is moved through the elongate body.

The elongate body may be configured to force the injection needles in their first position when the injector is moved in the first direction. The elongate body may comprise a body opening and the body opening may be configured to force the injection needles in their first position when the injector is moved in the first direction.
5 The pivot member may be configured to force the injection needles in their second position when the injector is moved in the second direction.
The invention further relates to an injection system comprising an injection device according to the invention, wherein the injector is connected to an injector fluid supply. The elongate body may be connected to a cavity fluid supply.
10 The invention further relates to the use of the injection device according to the invention for injecting a fluid into tissue of a body cavity.

The figures 1 and 2 show a first embodiment of the injection device according the invention. The injection device 1 comprises a hollow elongate body 5 and an injector 7. The injector 7 comprises multiple injection needles 8. Each injection needle 8 comprises an injection tip 9. The injector 7 is located in the inner space 6 of the hollow elongate body 5 35 and is movable through said body 5. By moving the injector 7, the injection needles 8 are movable into a first position 11 in which at least the injection tips 9 of the injection needles 8 are located inside the elongate body 5 (see fig. 1) and into a second position 12 in which at -4- least the injection tips 9 of the injection needles 8 are located outside the elongate body 5 (see fig. 2). The injection needles 8 are movable from the first position 11 into the second position 12 and visa versa.
The elongate body 5 comprises a cavity fluid opening 25 which can be connected to 5 a cavity fluid supply 32 (see fig. 6). The injection device 1 is configured such that a cavity fluid (such as a saline solution) from the cavity fluid supply 32 can flow via the cavity fluid opening 25 into the inner space 6 of the elongate body 5, pass the injector 7 and flow out of the body opening 29.
The injector 7 comprises an injector opening 30 which can be connected to an 10 injection fluid supply 31 (see fig. 6). The injection fluid can flow through the interior 33 of the hollow elongate injector 7 to the injection needles 8 and subsequently flow trough the hollow injection needles 8 and flow out their injection tips 9. The injection device 1 comprises a sealing 24 between the injector 7 and the elongate body 5. The sealing 24 allows the injector 7 to move relative to the elongate body 5 without leakage of cavity fluid located in 15 the inner space 6 the elongate body 5.
In the first position 11 shown in fig. 1, all the injection needles 8 are located in the inner space 6 of the hollow elongate body 5. The injection needles 8 extend substantially in the direction of the longitudinal axis 10 of the elongate body 5.
In the second position 12 shown in fig. 2, all the injection needles 8 are located 20 outside the elongate body 5. The injection tips 9 of the injection needles 8 extend traverse to the longitudinal axis 10 of the elongate body 5. In the shown embodiment the injection needles 8 extend along a substantially straight line and traverse to the longitudinal axis 10 of the elongate body 5. The injection needles may also have a different configuration, such as a curved form, wherein the injection tips 9 extend traverse to the longitudinal axis 10 of the 25 elongate body 5.
For moving the injection needles 8 from the first position 11 into the second position 12, the injector 7 is moved relative to the elongate body 5 in a second direction 22 (fig. 1). For moving the injection needles 8 from the second position 12 into the first position 11, the injector 7 is moved relative to the elongate body 5 in a first direction 21 (fig. 2).
30 The injection device 1 comprises a needle positioner 17 for positioning the injection needles 8 in the first position 11 when the injector 7 is moved in the first direction 21 relative to the elongate body 5 and for positioning the injection needles 8 in the second position 12 when the injector 7 is moved in a second direction 22 relative to the elongate body 5.
The injector 7 comprises a hollow elongate fluid supply member 20 for supplying the 35 injection fluid to the injection needles 8. The needle positioner 17 comprises a pivot member 18 via which the injection needles 8 are in fluid connection with the fluid supply member 20.

The pivot member 18 allows the injection needles 8 to be moved in the first position 11 and in the second position 12 when the injector 7 is moved through the elongate body 5.
The pivot member 18 is configured to force the injection needles 8 in the second position 12 when the injector 7 is moved in the second direction 22 relative to the elongate 5 body 5. The pivot member 18 comprises elastic material. The injection needles 8 are held by the elastic material of the pivot member 18 such that their preferred orientation is the second position 12. This means that if the injection needles 8 are not subjected to any additional forces, the elastic material of the pivot member 18 positions the injection needles 8 in the second position 12.
10 The elongate body 5 is configured to force the injection needles 8 in the first position 11 when the injector 8 is moved in the first direction 21 relative to the elongate body 5.
When the injector 8 is moved in the first direction 21, the injection needles 8 come in contact with the part of the elongate body 5 surrounding the body opening 29. Said part of the elongate body 5 is configured to subject the injection needles 8 to a force such that the 15 injection needles 8 are moved in the second position when the injector 7 is moved in the first direction 21.
In the injection device 1 shown in the fig. 1 and 2, the needle positioner 17 is formed by the pivot member 18 and the part of the elongate body 5 surrounding the body opening 29. It will be clear to the skilled person that also different configurations may be used.
20 The injection device 1 comprises a guiding member 23 for guiding the injection device 1 into the body cavity. The guiding member 23 is movable connected to the elongate body 5. The guiding member 23 can be moved from a guiding position 27 (fig. 1) into a displaced position 28 (fig. 2) and visa versa. The guiding member 23 is connected to the elongate body 5 via a hinge 26 provided at the body opening 29. In the guiding position 27 25 the guiding member 23 member is used to facilitate the movement of the elongate body 5 through the body of the patient and into the body cavity. In de displaced position 28 the guiding member 23 allows the movement of the injection needles 8 into the second position 12.
Due to the fact that the injection device 1 the invention comprises multiple injection 30 needles 8, multiple injections can be made simultaneously in the tissue of the body cavity. This reduces the time to make the (large number of) injections required for a specific treatment. This means for example that the time that the injection device is located in the urethra of the patient is reduced, which makes the treatment more bearable for the patient. It may even allow that a specific treatment does not have to be performed under general 35 anesthesia. In said situation, the patient does not have to travel to a hospital and the treatment may be performed at home.

The injector 7 comprises around 35 injection needles 8 (for clarity reasons, less injection needles are shown). The injector 7 may comprise between 20 and 50 injection needles 8. The injector 7 may comprise between 30 and 40 injection needles 8.
Figure 3 shows an enlarged view in cross section of the injection needles of the 5 injection device of the figures 1 and 2. Figure 4 shows an enlarged top view of the injector needles of figure 3 in the direction of arrow IV. The injection needles 8 are located in the second position 12. Each of the injection tips 9 of the injection needles 8 extend in a different direction. The injection tips 9 of the injection needles 8 define a part of a sphere 34.
Figure 5 shows a further enlarged view in cross section of one of the injection 10 needles of figure 3. In the injection device 1, each injection needle 8 comprises a blocking member 15 for blocking a further movement of the injection needle 8 through the tissue after said needle 8 has entered the tissue for a specific distance D. The blocking member 15 is located near the injection tip 9. The blocking member 15 comprises a blocking surface 35 for supporting on the tissue and the blocking surface 15 is located at a distance D from the 15 injection tip 9. The distance D may be between 2 and 5 mm. The distance D may be between 2 and 3 mm. The distance D may be around 3 mm.
Figure 6 shows an injection system according the invention comprising the injection device of the figures 1 and 2. The injector 7 is connected to an injector fluid supply 31. The injector fluid supply 31 comprises a first fluid duct 39 connected to the injector opening 30.
20 The first fluid duct 39 is connected to an injector fluid holder 37. A first pump 41 is provided to the injector fluid holder 37 to pump the injector fluid trough the first fluid duct 39. The elongate body 5 is connected to a cavity fluid supply 32. The cavity fluid supply 32 comprises a second fluid duct 40 connected to the cavity fluid opening 25. The second fluid duct 40 is connected to a cavity fluid holder 38. A second pump 42 is provided to the cavity 25 fluid holder 38 to pump the cavity fluid trough the second fluid duct 40. The first pump 41 is controlled by a first control 43 provided on the elongate body 5. The second pump 42 is controlled by a second control 44 provided on the elongate body 5.
The injection system is configured such that each injection needle 8 releases around 200 IU (International Unit) of injection fluid during one injection. The injection system may be 30 configured such that each injection needle 8 releases between 100 and 400 IU (International Unit) during one injection.
Figure 7 shows a view in cross section of an alternative embodiment of the injector of the injection device of the figures 1 and 2. Figure 8 shows a top view of the injection needles 8 of the injector 7 of fig. 7 in the direction of arrow VIII. The injection needles 8 are in the 35 second position 12. The injection tips 9 of the injection needles 8 define a circle 36. The pivot member 18 comprises the same elastic material as discussed in relation with the injector device of the fig. 1 and 2. The injector 7 comprises eight injection needles 8. The -7- injector 7 may comprise between four and six injection needles 8. The injector may be used for injecting a fluid in an intestinal canal or an oesophagus.
In figure 9 the injection device 1 of the figures 1 and 2 is shown with the injection needles 8 in the first position 11.
5 In figure 10 the injection device 1 of the figures 1 and 2 is shown with the injection needles 8 in the second position 12.
For clarity reasons, the guiding member is left out of the fig. 9 and 10.
