Abstract
The main goal of this invention is to create a medical instrument for the purpose of tissue engineering (the collective term for stem cell transplantation therapy) in the Regenerative Medicine, e.g. for the kidney/ liver etc. Particularly for the pancreas, which can simplify the treatment of diabetic patients (people with Diabetes Mellitus type 1 and/or 2) faster, more efficient and safer.
Description
Double cannula I call this medical instrument "Double cannula". It is made of sterilizable or disposable surgery material, consisting of a combination of an "inner-cannula” and an "outer-cannula" to safely and easily transplant the harvested stem cells (either from bone-marrow, fat tissue of other sources) into the (human/ animal) pancreas under guidance of ultrasound echography. * The inner-cannula has a blunt end and the shaft has a cylindrical shape with a diameter of approximately 1-20 mm and a length of app. 5-50 cm. This cannula is app. 5-70 mm longer than the outer-cannula. Along the shaft, app. the front l/5th part, it has an opening (1 or more hole/s) for injecting the harvested stem cells. The base is provided with a female (Luer Lock) system, which should be able to be connected to an existing syringe with a male (Luer Lock) system. * The outer-cannula has to be exactly fitted over the inner-cannula so that it can slide tightly but easily over each other. The apex of the outer-cannula has a grinded sharp end to easily pierce the skin, the fasciae of the muscles and the capsula of the pancreas. At the base, the outer-cannula has a handle (e.g. for the thumb, index- and middle finger) to easily shove the outer-cannula back and forth during the punction of tight tissues.
Further description of the so-called "Setyo-Procedure" for stem cells transplantation with these double cannulas (in this case to treat Diabetes Mellitus type 1 or 2 at an adult male): 1. The appropriate length and diameter of the above described instrument must first be well determined. 2. The outer-cannula will be first placed over the inner-cannula. 3. After adequate sterilization of patient's skin area and (local or general) anesthesia, both sterile cannulas will be inserted percutaneously (whether or not with a small incision) at the level of (slightly) below the spinous process of Th 12 and ca. 4,5 cm para-vertebral (left is in my opinion more safe than right!) 4. Then the cannulas (under guidance of ultrasound sonography) will be shoved up safely up to the surface of the pancreas. The sharp point of the outer-cannula can be used to pierce the fasciae of the muscles and the capsula of the pancreas, after which it will be shoved backwards (quickly) unto the base of the inner-cannula to prevent unnecessary lesions of the surrounding organ tissues (e.g. arteria, nerves tissues) and to prevent (profuse) bleeding. 5. Next, the syringe containing stem cells will be connected appropriately at the base of the inner-cannula. 6. The blunt apex of the inner-cannula will be conducted further into the pancreas in order to transplant the harvested stem cells efficiently.
This instrument could also be used for other purposes, e.g. during the procedure of "Lipofilling or fat cell transplantation" and in the veterinary.
