Abstract
본 발명은 TSG-6 (Tumor necrosis factor-inducible gene 6); 또는 이의 발현 또는 활성 촉진제를 유효성분으로 함유하는 간 재생 촉진 또는 간 기능 회복용 조성물에 관한 것으로, 보다 구체적으로는, TSG-6가 부분 간 절제술 후 제거된 간의 재생을 촉진시키고, 간 기능 관련 마커의 발현을 증가시켜 간 기능을 회복시키는 효과가 있음을 확인하여 간 재생 촉진 또는 간 기능 회복용 약학 조성물, 건강기능식품 조성물 등 다양하게 활용할 수 있다.
Description
본 발명은 TSG-6를 유효성분으로 함유하는 간 재생 촉진 또는 간 기능 회복용 약학 조성물을 제공한다.
간은 침묵의 장기로 불리운다. 그 이유는 간은 15 내지 20 %만 기능을 유지하여도 생존에 필수적인 대사작용을 수행할 수 있기 때문이다. 이와 같은 간의 특성 때문에 간질환의 조기 발견이 어려우며, 진단 후에도 환자들이 간의 관리를 소홀히 여기는 원인이 된다. 특히 사회적 활동이 활발한 40, 50 대의 남성에게 간질환이 주된 사망 원인이라는 통계는 간질환의 관리와 치료방법, 그리고 치료제의 개발이 매우 시급한 과제임을 시사한다.
말기 상태에 도달한 만성 간질환 환자에게 가장 유효한 치료법은 간이식이다. 그러나 간이식이 요구되는 환자의 수는 간 공여자의 수를 크게 초과한다. 이 문제를 극복하기 위해 몇 가지 혁신적인 방법이 제시되었으며 그 중 한가지인 생체부분간이식 (LDLT; Living Donor Liver Transplantation) 은 이식대기자 명단의 환자수와 장기기증자수의 차이를 크게 감소시켰다. 이 치료법에 의해 소아환자의 간이식 대기기간중의 사망률은 거의 "0"으로 감소되었으며, 성인간의 생체부분간이식 또한 급증하였다.
현재 일본에서 실시되고 있는 간이식의 99 % 는 생체부분간이식으로 보고되었다. 한편, 부분 간 절제는 간암환자에 대해서도 유일한 근원적 표준치료법으로 인정되고 있다. 그러나 과도한 간 조직 절제는 간 기능 상실을 유발할 수 있다. 간 재생이 정상적으로 일어나는 환자에서 일시적인 간 기능 부전은 신속하게 회복되나 만성적 간질환에 의해 재생기능이 저하된 환자의 경우에 간 기능 부전은 지속되어 생명이 위급한 합병증을 일으킬 수 있다. 간 세포암종 (Hepatocellular carcinoma, HCC) 환자를 대상으로 실시된 간 절제 시술이후에 발생한 간 기능 상실에 의한 사망률은 아직도 60-90 % 로 대단히 높은 상태이다.
부분 간 절제 후 간 재생은 간 세포 (hepatocyte)를 휴지상태 (quiescent state)에서 증식상태 (proliferative state)로 전환시키는 매우 복잡한 과정에 의해 조절된다. 여러 가지의 성장인자들이 간 증식을 자극하거나 또는 억제하는 신호를 제공하여 이 과정을 조절한다. 조직괴사인자 α (tissue necrosis factor α; TNF-α)와 인터루킨 6 (interleukin 6)은 간 세포가 분열을 시작하는 첫 단계에 관여하고, 간 세포 성장인자 (hepatocyte growth factor; HGF) 와 변형성장인자 α (transforming growth factor α; TGF-α)는 분열을 촉진하며 최종적으로 TGF-β 와 액티빈 (activin)이 세포성장을 억제하여 간 재생을 종료시킨다. 그러나 섬유화가 발생한 환자의 간은 절제 후에 재생능력이 현저하게 감소하는 것으로 알려져 있으며, 따라서 간이식을 통한 치료의 성공률을 저하시키는 요인이 된다. 최근에 들어와 지방간도 간 절제 환자의 수술경과를 결정하는 중요 인자로 보고되고 있으며, 그 기전은 지방간에 의한 간 세포 내 지질과산화와 쿠퍼 세포 (Kupffer cell)에 의해 매개되는 면역반응을 통한 정상 간조직의 성장저해에 의한 것으로 제시되었다.
절제된 간의 정상 간으로의 재생은 간의 수령자 (recipient) 뿐만 아니라 공여자 (donor) 에게도 성공적인 시술의 기본 요건이 된다. 생체부분간이식에서는 공여자에게서 간 기능 부전을 일으키지 않는 충분량의 간을 잔류시켜야 하며, 동시에 수령자에게는 건강회복이 가능한 충분량의 이식조직을 제공하는 것이 필수적이다. 간조직 공여자와 수령자 모두에서 합병증의 발생은 문제가 되지만, 제공자의 경우에도 최악의 경우는 사망이 보고되고 있다.
공여자에게 나타나는 위험성은 산정하기 어려우나 European Liver Transplant Registy (ELTR)에 참여하는 서유럽국가에서는 이 비율을 0.5% 로 추정하고 있다. 이것은 신장공여에 수반되는 위험성이 0.013% 인 것과 비교하면 30 배 이상에 달한다. 아시아 지역센터에서의 시술예에서 1508 인의 공여자에게 16 %의 합병증이 발생하였으며 1%의 재수술, 그리고 최소한 1 건의 사망이 보고되었고, 449인의 공여자를 대상으로 실시된 미국 내 조사에서도 14.5%의 합병증, 4.5% 의 재수술, 그리고 1인의 사망이 보고되었다. 종합하면 단순 부분 간 절제 (간암환자 및 생체부분간이식 공여자)나 부분 간조직 수령 환자 모두에게서 신속하고 정상적인 간 재생은 치료의 성공을 결정하는 가장 중요한 요소로 판단된다.

이하, 본 발명을 보다 상세하게 설명한다.
본 발명은 TSG-6 (Tumor necrosis factor-inducible gene 6); 또는 이의 발현 또는 활성 촉진제를 유효성분으로 함유하는 간 재생 촉진 또는 간 기능 회복용 약학 조성물을 제공한다.
상기 발현 촉진제는 TSG-6 유전자의 mRNA의 발현을 증가시키는 TSG-6 코딩 유전자를 포함하는 아데노연관 바이러스 (AAV), 아데노 바이러스, 렌티바이러스 및 레트로 바이러스로 이루어진 군에서 선택될 수 있으나, 이에 한정되는 것은 아니다.
상기 활성 촉진제는 TSG-6 단백질의 활성 또는 안정성을 촉진하거나, 단백질 분해를 저해하는 저분자 화합물, 펩타이드, 펩타이드 미메틱스, 앱타머 및 천연물로 이루어진 군에서 선택될 수 있으나 이에 한정되는 것은 아니다.
상기 약학 조성물은 부분 간 절제술 (partial hepatectomy) 후 간 재생을 촉진하거나 간 기능을 회복할 수 있다.
상기 약학 조성물은 ASGR1 (Asialoglycoprotein Receptor 1)의 발현을 감소시킬 수 있으며, ASGR1 (Asialoglycoprotein Receptor 1)의 발현을 감소를 촉진시킬 수 있다.
상기 약학 조성물은 Ki-67, Cyclin D1, Cyclin B1, Pnca 또는 ß-카테닌의 발현을 증가시킬 수 있다.
상기 약학 조성물은 Alb, Cyp3a11, Cyp7a1, FoxO1, G6pc 또는 Pck1의 발현을 증가시킬 수 있다.
본 발명의 다른 구체예에서, 약학 조성물은 약학 조성물의 제조에 통상적으로 사용하는 적절한 담체, 부형제, 붕해제, 감미제, 피복제, 팽창제, 윤활제, 활택제, 향미제, 항산화제, 완충액, 정균제, 희석제, 분산제, 계면활성제, 결합제 및 윤활제로 이루어진 군에서 선택되는 하나 이상의 첨가제를 추가로 포함할 수 있다.
구체적으로 담체, 부형제 및 희석제는 락토즈, 덱스트로즈, 수크로스, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로스, 폴리비닐 피롤리돈, 물, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 및 광물유를 사용할 수 있으며, 경구투여를 위한 고형제제에는 정제, 환제, 산제, 과립제, 캡슐제 등이 포함되며, 이러한 고형제제는 상기 조성물에 적어도 하나 이상의 부형제, 예를 들면, 전분, 칼슘카보네이트, 수크로스 또는 락토오스, 젤라틴 등을 섞어 조제할 수 있다. 또한 단순한 부형제 이외에 마그네슘 스티레이트, 탈크 같은 윤활제들도 사용할 수 있다. 경구를 위한 액상제제로는 현탁제, 내용액제, 유제, 시럽제 등이 있으며 흔히 사용되는 단순 희석제인 물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등이 포함될 수 있다. 비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조제제, 좌제 등이 포함된다. 비수성용제, 현탁제로는 프로필렌글리콜, 폴리에틸렌 글리콜, 올리브오일과 같은 식물성 기름, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기재로는 위텝솔 (witepsol), 마크로골, 트윈 (tween) 61, 카카오지, 라우린지, 글리세로제라틴 등이 사용될 수 있다.
본 발명의 일실시예에 따르면, 상기 약학 조성물은 정맥내, 동맥내, 복강내, 근육내, 동맥내, 복강내, 흉골내, 경피, 비측내, 흡입, 국소, 직장, 경구, 안구내 또는 피내 경로를 통해 통상적인 방식으로 대상체로 투여할 수 있다.
본 발명에 따른 유효성분의 투여량은 대상체의 상태 및 체중, 질환의 종류 및 정도, 약물 형태, 투여경로 및 기간에 따라 달라질 수 있으며 당업자에 의해 적절하게 선택될 수 있고, 1일 투여량이 0.01 mg/kg 내지 200 mg/kg, 바람직하게는 0.1 mg/kg 내지 200 mg/kg, 보다 바람직하게는 0.1 mg/kg 내지 100 mg/kg 일 수 있다. 투여는 하루에 한번 투여할 수도 있고 수회로 나누어 투여할 수도 있으며, 이에 의해 본 발명의 범위가 제한되는 것은 아니다.
본 발명에서 사용된 용어 “펩타이드”는 표적 물질에 대한 결합력 높은 장점이 있으며, 열/화학 처리시에도 변성이 일어나지 않는다. 또한, 분자 크기가 작기 때문에 다른 단백질에 붙여서 융합 단백질로의 이용이 가능하다. 구체적으로 고분자 단백질 체인에 붙여서 이용이 가능하므로 진단 키트 및 약물전달 물질로 이용될 수 있다.
본 발명에서 사용된 용어 “앱타머”는 그 자체로 안정된 삼차 구조를 가지면서 표적 분자에 높은 친화성과 특이성으로 결합할 수 있는 특징을 가진 특별한 종류의 단일 가닥 핵산 (DNA, RNA 또는 변형핵산)으로 구성된 폴리뉴클레오티드의 일종을 의미한다. 상술한 바와 같이, 앱타머는 항체와 동일하게 항원성 물질에 특이적으로 결합할 수 있으면서도, 단백질보다 안정성이 높고, 구조가 간단하며, 합성이 용이한 폴리뉴클레오티드로 구성되어 있으므로, 항체를 대체하여 사용될 수 있다.
또한, 본 발명은 TSG-6 (Tumor necrosis factor-inducible gene 6); 또는 이의 발현 또는 활성 촉진제를 투여하는 단계를 포함하는 간 재생 촉진 또는 간 기능 회복 방법을 제공한다.
또한, 본 발명은 TSG-6 (Tumor necrosis factor-inducible gene 6)을 유효성분으로 함유하는 간 기능 개선용 건강기능식품 조성물을 제공한다.
상기 건강기능식품은 여러 가지 영양제, 비타민, 광물 (전해질), 합성 풍미제 및 천연 풍미제 등의 풍미제, 착색제 및 중진제 (치즈, 초콜릿 등), 펙트산 및 그의 염, 알긴산 및 그의 염, 유기산, 보호성 콜로이드 증점제, pH 조절제, 안정화제, 방부제, 글리세린, 알코올, 탄산음료에 사용되는 탄산화제 등을 함유할 수 있다.
그밖에 천연 과일 주스, 합성 과일 주스 및 야채 음료의 제조를 위한 과육을 함유할 수 있다. 이러한 성분은 독립적으로 또는 조합하여 사용할 수 있다. 또한, 건강기능식품 조성물은 육류, 소세지, 빵, 초콜릿, 캔디류, 스넥류, 과자류, 피자, 라면, 껌류, 아이스크림류, 스프, 음료수, 차, 기능수, 드링크제, 알코올 및 비타민 복합제 중 어느 하나의 형태일 수 있다.
또한, 상기 건강기능식품은 식품첨가물을 추가로 포함할 수 있으며, "식품첨가물"로서의 적합 여부는 다른 규정이 없는 한 식품의약품안전청에 승인된 식품첨가물공전의 총칙 및 일반 시험법 등에 따라 해당 품목에 관한 규격 및 기준에 의하여 판정한다.
상기 "식품첨가물공전"에 수재된 품목으로 예를 들어, 케톤류, 글리신, 구연산칼륨, 니코틴산, 계피산 등의 화학적 합성품, 감색소, 감초추출물, 결정셀룰로오스, 고랭색소, 구아검 등의 천연첨가물, L-글루타민산나트륨 제제, 면류 첨가 알칼리제, 보존료제제, 타르색소 제제 등의 혼합 제제류 등을 들 수 있다.
이때, 건강기능식품을 제조하는 과정에서 식품에 첨가되는 유효성분은 필요에 따라 그 함량을 적절히 가감할 수 있으며, 바람직하게는 식품 100 중량부에 1 중량부 내지 90 중량부 포함되도록 첨가될 수 있다.
또한, 본 발명은 TSG-6 (Tumor necrosis factor-inducible gene 6)을 유효성분으로 함유하는 ASGR1 발현 감소용 조성물을 제공한다.
이하, 본 발명의 이해를 돕기 위하여 실시 예 등을 들어 상세하게 설명하기로 한다. 다만 하기의 실시 예 등은 본 발명의 내용을 예시하는 것일 뿐 본 발명의 범위가 하기 실시 예 등에 한정되는 것은 아니다. 본 발명의 실시 예 등은 당업계에서 평균적인 지식을 가진 자에게 본 발명을 보다 완전하게 설명하기 위해 제공되는 것이다.
<실험 예 1> 실험 마우스
70% 부분 간 절제술은 Higgins and Anderson의 방법에 따라 아이소플루레인 마취하에 8주령 수컷 C57BL/6 WT 마우스 (Hyochang, Dae-gu, Korea)를 이용하여 시행되었다. 절차는 오전 8시에서 오후 12시 사이에 수행되었다. 마우스를 비히클 대조군으로서 인산염 완충 식염수 (PBS) 단독 (n=5/그룹) 또는 부분 간 절제술 시간에 50ng의 재조합 TSG-6 (n=5/그룹) (Tumor necrosis factor-inducible gene 6 protein)으로 복강 내 처리하고 수술후 12시간, 24시간, 48시간 또는 72시간에 희생시켰다. 절제된 정지 간을 0시간으로 사용하거나 간 절제술 (n=4)을 생략하고 동일한 방식으로 처리한 모조 수술 마우스로 사용했으며, 재생 중인 간 잔여물은 후속 분석을 위해 액체 질소에서 포르말린 고정 또는 급속 냉동되었다. 마우스는 부산대학교의 적절한 동물 시설에서 관리되었다. 모든 마우스는 12시간 명/암 주기로 수용되었고 음식과 물에 자유롭게 접근할 수 있었다. 동물 관리 및 수술 절차는 국립 보건원 (National Institutes of Health, NIH) 실험실 동물 관리 및 사용 지침의 조항에 따라 수행되었다. 사용된 동물 프로토콜은 윤리적 절차와 과학적 관리를 위해 부산대학교 동물실험관리위원회 (PNU-IACUC)의 승인을 받았다.
<실험 예 2> RNA 분석
TRIzol 시약 (Ambion, Thermo Fisher Scientific)을 사용하여 간 조직 또는 세포에서 총 RNA를 추출했다. RNA의 농도와 순도는 nanodrop을 사용하여 결정되었다. 주형 cDNA는 제조업체의 프로토콜에 따라 SuperScript First-strand Synthesis System (Invitrogen)을 사용하여 전체 RNA로부터 합성되었다. 제조사의 사양 (Applied Biosystems, Thermo Fisher Scientific, QuantStudio™ Real)에 따라 Power SYBR Green Master Mix (Applied Biosystems, Thermo Fisher Scientific)를 이용하여 실시간 정량 역전사 중합효소연쇄반응 (qRT-PCR) 분석을 수행하였다. 모든 반응은 반복되었고 데이터는 ΔΔCt 방법에 따라 분석되었다. 40S 리보솜 단백질 S9 (RPS9) mRNA를 사용하여 발현 수준을 정상화했다. 사용된 모든 프라이머의 염기서열은 표 1에 요약되어 있다. 모든 PCR 산물은 유전적 확인을 위해 직접 염기서열 분석되었다 (Macrogen, Korea).

이러한 프라이머 서열은 qRT-PCR에 사용되었다. mRNA의 발현 수준은 RPS9 mRNA의 발현 수준으로 정규화하였다.
<실험 예 3> 웨스턴 블롯
전체 단백질을 추출하기 위해 세포를 프로테아제 억제제 (Roche)가 포함된 TLB (triton lysis buffer)에서 균질화하고 13000 r.c.f에서 원심분리했다. 20분동안 전체 단백질 추출물을 함유하는 상층액을 후속 생화학적 분석에 사용하였다. 단백질 농도는 Pierce BCA Protein Assay Kit (Thermo Scientific)로 측정했다. 동량의 단백질 용해물을 10% SDS-PAGE로 분리한 다음 PVDF (polyvinylidene difluoride) 막 (Millipore, Germany)으로 옮겼다. 토끼 항-비포스포 (활성) β-카테닌 항체 (1:1000 희석, Cell Signaling Technology), 마우스 항-PCNA 항체 (1:1000 희석, Santa Cruz), 토끼 항-CD36 항체 (1:1000 희석; NB400-144, Novus Biologicals, USA), 토끼 항-AMPKα 항체 (1:1000 희석, 5832s, 세포 신호 기술), 토끼 항-포스포-AMPKα 항체 (1:1000 희석, 50081s, 세포 신호 기술)를 사용하였다. 토끼 항-GAPDH 항체 (희석 1:2000; Bio-Rad, USA)를 내부 대조군으로 사용하였다. 양고추냉이 퍼옥시다제 (HRP)-접합된 항-토끼 또는 항-마우스 IgG (Enzo Life Sciences, USA)를 2차 항체로 사용하였다. 제조업체의 사양 (ATTO Corporation, Ez-Capture II)에 따라 EzWestLumi ECL 솔루션 (ATTO Corporation, 일본)을 사용하여 단백질 밴드를 검출했다. 밴드 밀도는 CS Analyzer 소프트웨어 (Version 3.00.1011, ATTO & Rise Corporation)를 사용하여 측정되었다.
<실험 예 4> 간 조직학 및 면역 조직 화학
간 표본을 10% 중성 완충 포르말린 (Sigma)에 고정하고, 파라핀에 포매하고, 4㎛ 절편으로 절단하였다. 표본을 탈파라핀화, 수화 및 H&E로 염색하여 간 세포의 글리코겐 저장 검출을 위한 간 형태, PAS (Periodic acid-Schiff) 염색을 검사하였다. PAS 염색을 위해 절편을 0.5% 과요오드산 용액 (Millipore)에서 5분 동안 인큐베이션한 다음, Schiff's 시약 (Millipore)에서 15분 동안 인큐베이션했다. 그런 다음 절편을 헤마톡실린으로 대조염색했다. 면역조직화학 (IHC)을 위해 간 절편을 탈파라핀화하고, 수화하고, 내인성 과산화효소를 차단하기 위해 3% 과산화수소에서 배양했다. 항원 검색은 10mM 구연산나트륨 완충액 (pH 6.0)에서 10분 동안 마이크로웨이브에서 가열하여 수행하였다. 그런 다음 표본을 실온 (RT)에서 30분 동안 Protein Block 용액 (Dako, USA)에서 차단한 다음 4℃에서 밤새 1차 항체와 함께 인큐베이션했다. 다른 절편도 염색 특이성을 입증하기 위해 비면역 혈청에서 밤새 4℃에서 배양했다. 마우스 Ki-67 항체 (희석 1:2000; Novocastra, Leica Microsystems, UK) 및 토끼 항-ASGR1 항체 (희석 1:2000; 11739-1-AP; Proteintech)를 1차 항체로 사용하고 Protein Diluent (Dako). Polymer HRP anti-mouse (K4001; Dako) 또는 HRP anti-rabbit IgG (K4003; Dako)를 2차 항체로 사용하였다. 갈색을 나타내는 3,3'-diaminobenzidine (DAB)을 사용하여 단백질을 시각화했다. Ki-67-양성 간 세포를 정량화하기 위해, 무작위로 선택된 10개의 20X 필드/섹션을 각 마우스에 대해 Ki-67-염색된 세포/필드의 총 수를 세어 평가했다.
<실험 예 5> ALT 및 AST 측정
혈청 alanine transaminase (ALT/GPT, glutamate pyruvate transaminase) 및 aspartate transaminase (AST/GOT, glutamate-oxaloacetate transaminase) 수치는 GOT 및 GPT 시약 (ASAN PHARMACEUTICAL, Korea)을 사용하여 제조사의 지시에 따라 측정하였다.
<실험 예 6> 마우스 1차 간 세포 분리 및 세포 실험
C57BL/6 마우스 (효창, 대구, 한국)의 건강한 간에서 간 세포를 분리하였다. 간략하게, 마우스를 아이소플루레인으로 마취시켜 치료 테이블 위 누운 자세로 고정시키고 무균 상태에서 하대정맥에 캐뉼러를 삽입했다. 간은 세포를 분산시키기 위해 EGTA 및 콜라게나제 (Sigma Aldrich, St. Louis, MO, USA)로 제자리에서 관류되었다. 1차 간 세포를 Percoll 밀도 구배 원심분리를 사용하여 비실질 세포로부터 분리하였다. 트리판 블루 배제에 의해 결정된 바와 같이, 세포 생존율은 모든 실험에서 >92%였다. 1차 간 세포는 콜라겐 코팅된 10cm2 플레이트에서 페놀 레드가 없는 Williams' Medium E (Sigma-Aldrich, St. Louis, MO, USA)에서 1 x 106 세포의 밀도로 배양되었으며, 5% 태아 소 혈청 (FBS; Gibco, Thermo Fisher Scientific, Waltham, MA, USA), 1μM 덱사메타손 및 페니실린/스트렙토마이신 (P/S) 칵테일 용액, ITS + (인슐린, 트랜스페린, 셀레늄 복합체, BSA 및 리놀레산) 및 GlutaMAX ™ 및 HEPES (Gibco, Thermo Fisher Scientific, Waltham, MA, USA)가 첨가되었다. 세포 부착 후 (도금 후 약 4시간), 배양 배지를 0.1 μM 덱사메타손 및 P/S, ITS +, GlutaMAX™ 및 HEPES 칵테일 용액을 포함하는 무혈청 Williams' Medium E로 교체하여 간 세포 유지를 위해 37℃, 5% CO2를 포함하는 습한 분위기를 유지시켰다.
간 세포에서 TSG-6의 효과를 평가하기 위해, 일차 간 세포를 FBS가 포함되지 않은 배양 배지에서 밤새 배양 후, 각각 1시간 및 2시간 동안 20 ng/ml의 TSG-6 (R&D systems, Minneapolis, MN, USA) 또는 대조군으로 처리하였다.
<실험 예 7> LC-MS/MS
TSG-6의 잠재적인 상호 작용 파트너를 확인하기 위해 TSG-6 처리 마우스 1차 간 세포의 총 단백질을 추출하여 IP를 수행했다. 비특이적 결합을 줄이기 위해 아가로스 A/G 비드를 사용하여 세포 용해물을 사전 제거했다. 마우스 항-His 태그 1차 항체 (sc-8036; Santa Cruz Biotechnology, Inc., Dallas, TX, USA)를 사용하여 단백질을 밤새 4℃에서 면역 침전시키고 단백질을 2시간 동안 A/G Plus-Agarose (sc-2003; Santa 크루즈)와 함께 배양했다. 용해 완충액으로 세 번 세척한 다음 5× 시료 완충액에서 10분 동안 끓였다. 상층액을 SDS-PAGE에 로딩하고, 탈이온수 (DW)로 3회 세척한 다음, 쿠마시 브릴리언트 블루 (B0770; Sigma-Aldrich. MO, USA)와 함께 1시간 동안 인큐베이션하였다. 밴드가 명확하게 관찰될 때까지 젤을 DW로 세척했다. LC-MS/MS를 위해 전체 겔을 8개의 조각으로 절단했다. LC-MS/MS 분석은 TSG-6과 상호작용하는 단백질을 확인하기 위해 National Instrumentation Center for Environmental Management (NICEM; 서울대학교, 서울, 대한민국)에서 수행되었다. Dionex U 3000 RSLCnano HPLC 시스템이 장착된 Thermo Scientific Orbitrap Exploris 240 질량 분석기를 사용하여 질량 분석을 수행했다. Xcaliber 소프트웨어 버전 4.4를 사용하여 MS 데이터를 수집했다. Orbitrap 분석기는 m/z 200에서 60,000 분해능으로 350-1800 m/z의 질량 범위로 전구체 이온을 스캔했다. 질량 데이터는 proteome Discoverer 2.5 (Thermo Scientific, USA)를 사용하여 자동으로 수집되었다.
<실험 예 8> 단백질 네트워크 분석
단백질의 분자 기능 및 생물학적 과정을 기반으로 단백질의 기능을 분류하기 위해 UniProt 데이터베이스 (https://www.uniprot.org/)가 사용되고 전자 온톨로지 (GO) 표준을 준수하는 생물 정보학 도구로 PANTHER (Protein Analysis Through Evolutionary Relationships) 데이터베이스 버전 16.0 (www.pantherdb.org)을 사용했다.
<실험 예 9> 분자 도킹 예측
TSG-6과 ASGR1의 상호작용을 예측하기 위해 TSG-6 (PDB ID: 1O7B)과 ASGR1 (PDB ID: 1DV8)의 3차원 구조를 RCSB PDB (Protein Data Bank)에서 PDB 형식과 분자 도킹으로 얻었다. HADDOCK 2.4 (2023년 4월 1일에 액세스한 https://wenmr.science.uu.nl/haddock2.4/)로 수행되었다. 도킹 매개변수 섹션에서 기본 매개변수가 선택되었다. 기본 매개변수는 HADDOCK 서버 웹사이트 https://wenmr.science.uu.nl/haddock2.4/에서 찾을 수 있다. HADDOCK 결과에서 얻은 최상의 클러스터의 3D 구조는 PyMOL (Version-2.5.4)을 사용하여 시각화되었다.
<실험 예 10> 면역 침전 분석
면역 침전 분석을 위해, 1시간 및 2시간동안 TSG-6를 처리한 마우스 1차 간 세포로부터 추출된 총 단백질을 토끼 항-ASGR1 1차 항체 (11739-1-AP; Proteintech) 및 단백질 A/G 플러스 아가로스 (sc-2003; Santa Cruz로 밤새 4℃에서 면역 침전시켰다. 용해 완충액으로 3회 세척하고 5× 시료 완충액에서 10분간 끓인 후 원심분리하였다. 생성된 맑은 상층액을 SDS-PAGE에 적용했다. 웨스턴 블롯에 적용한 것과 동일한 방법을 사용하여 다음 단계를 수행했다.
<실험 예 11> 간 트리글리세리드 분석
제조업체의 지침에 따라 Triglyceride Colorimetric 분석 키트 (Cayman Chemical)를 사용하여 간 트리글리세라이드 (이하 TG라 함.) 수치를 측정했다. 간략하게, 간 조직을 NP40을 사용하여 균질화하고 조직 균질물을 10,000 r.c.f, 4℃에서 10분 동안원심분리했다. 트리글리세라이드를 포함하는 상층액은 후속 생화학 분석에 사용되었다. 10 μl의 상층액과 150 μl의 효소 용액을 각 웰에 첨가한 후 15분 동안 배양하였다. Glomax 다중 검출 시스템 (Promega)을 사용하여 560 nm의 파장에서 흡광도를 측정하였다.
<실험 예 12> 통계 분석
결과는 평균 ± sem으로 표시되었다. 대조군과 치료군 또는 하위군 사이의 통계적으로 유의미한 차이는 two-sample Student’s t-test으로 분석되었다. p-값이 <0.05일 때 차이가 유의한 것으로 간주되었다.
<실시 예 1> TSG-6 처리에 의한 간 재생 효과 확인
간의 70%를 제거하는 부분 간 절제술 (partial hepatectomy, 이하 PH라 함.) 또는 모조수술을 수행한 생쥐에 TSG-6 또는 PBS (phosphate buffered saline) 처리에 의한 간 조직의 변화를 거시적으로 관찰하였다.
그 결과, 도 1에 따르면, PH 이후 PBS를 처리한 그룹의 경우 48시간 이후부터 간이 부풀고, 창백해지는 모습이 관찰되었다. 그러나 TSG-6를 처리한 그룹의 간은 12시간과 24시간부터 색이 옅어진 것이 보였고, 이는 PBS를 처리한 그룹의 간보다 빨리 나타났다. 모조 수술만 진행한 그룹은 PBS 또는 TSG-6를 처리에 의한 변화는 보이지 않았다.
또한, PH 이후 TSG-6 처리의 간 재생 촉진 효과를 확인하기 위해, 생쥐의 간 무게 (Liver weight; LW)와 체중 (Body weight; BW), 간 손상의 지표로 활용되는 혈청 아스파르테이트 아미노전달효소 (aspartate aminotransferase, AST) 및 알라닌 아미노전달효소 (alanine aminotransferase, ALT) 수준을 비교하였다.
그 결과 도 2에 따르면, PH이후, PBS를 처리한 그룹에서 간 무게 증가와 지질의 축적이 48시간이후부터 관찰되지만, TSG-6를 처리한 그룹에서는 간 무게 증가가 24시간 후부터 유의성 있게 관찰되었고, 간 세포 내 지질 축적도 확연하게 증가되었다. PH에 의해 급격하게 증가하였던 간 손상 지표인 혈청 AST와 ALT 또한, TSG-6를 처리한 경우 PBS를 처리한 그룹에 비해 먼저 완화됨을 확인하였다.
이를 통해 TSG-6가 간 재생 및 회복을 촉진함을 알 수 있었다.
나아가, TSG-6 자체에 의한 간 손상이 있는지 확인하기 위해 모조 수술 그룹의 간 무게 및 체중, 혈청 AST 및 ALT 측정 결과를 확인하였다.
그 결과, 도 3에 따르면, 모조 수술 그룹의 경우 조직학적, 생리학적 간 손상이 관찰되지 않으며, PBS와 TSG-6의 처리에 따른 차이가 없음을 확인하였다. 이를 통해 수술과정에서의 간섭없이 PH가 성공적으로 진행되었고, TSG-6 자체에 의한 간 손상이 없음을 알 수 있었다.
<실시 예 2> TSG-6 처리에 의한 간 세포 분열 증진 효과
TSG-6 처리에 의한 간 세포 분열 증진 효과를 확인하기 위해 PH 이후 간 세포 분열을 세포분열 마커인 Ki-67, Cyclin D1, Cyclin B1, Pcna (Proliferating cell nuclear antigen), E2f1 (E2F Transcription Factor 1) 및 β-카테닌의 발현을 비교하였다.
그 결과, 도 4에 따르면, PH 이후 TSG-6를 처리한 그룹에서 PBS를 처리한 그룹에 비해 간 내 Ki-67-양성 세포의 수가 더 빠른 시간인 12시간부터 유의하게 증가하기 시작하여 24시간과 48시간까지 더 많은 세포가 Ki-67를 발현함을 확인하였다. 세포분열 관련 마커들의 RNA 발현 또한 TSG-6를 처리한 그룹에서 PBS를 처리한 그룹에 비해 더 높은 발현을 보였다. Pcna의 단백질 발현은 TSG-6 처리에 의해 증가함을 확인하였고, 세포분열 마커들의 발현을 조절한다고 알려진 β-카테닌의 단백질 발현 역시 TSG-6 처리에 의해 PBS처리 그룹 보다 빠른 24시간에서 증가함을 확인하여, TSG-6가 간 세포 분열의 증식에 영향을 줌을 알 수 있었다.
그에 반해, 모조 수술 그룹에서는, 도 5에 따르면, β-카테닌과 Pcna의 발현이 유의하게 증가하지 않음을 확인하여 모조 수술 그룹에서 TSG-6는 간 세포 분열에 영향을 주지 않음을 알 수 있었다.
<실시 예 3> TSG-6 처리에 의한 간 기능 회복 효과
증식한 간 세포가 정상적으로 기능하는지 확인하기 위하여, 간 기능 관련 마커들인 Alb (albumin), Cyp3a11 (Cytochrome P450 3a11), Cyp7a1 (Cytochrome P450 7a1)을 비교하였다.
그 결과, 도 6에 따르면, 성숙한 간 세포가 갖는 마커들의 RNA 발현은 PH를 한 간에서 TSG-6를 처리 후 48시간에서 유의하게 증가함을 확인하였다. 이는 TSG-6가 간 재생의 마지막 단계인 간 기능의 회복 역시 촉진함을 의미한다.
나아가, 도 7에 따르면, 글리코겐 저장능력 (PAS염색)과 포도당신생합성 또는 당분해 관련 유전자인 FoxO1, G6pc 및 Pck1의 발현을 확인하여 TSG-6를 처리 후 24시간에서 유의하게 증가함을 확인하여 간 기능이 더 빨리 회복됨을 확인할 수 있었다.
<실시 예 4> TSG-6와 상호작용하는 수용체 확인
TSG-6와 상호작용하는 수용체를 찾기 위해 구조프로그램인 HADDOCK 을 이용하였고 통해 TSG-6와 ASGR1간의 상호작용을 예측하였고, IP를 통해 실제로 상호작용한다는 것을 확인하였다.
또한, ASGR1은 정지 간 세포에서 발현되는 것으로, 증식을 하는 간 세포에서의 ASGR1의 발현은 낮은 것으로 알려져 있는데, PH 이후 TSG-6가 ASGR1의 발현을 감소시키는지 확인하였다.
그 결과, 도 9에 따르면, ASGR1은 간 세포에서 발현됨을 확인하였고, ASGR1은 PH 후 발현이 점차 감소하여, 간 세포 증식이 활발한 48시간과 72시간에서 거의 발현되지 않음을 알 수 있었다. 하지만, TSG-6를 처리한 그룹에서는 PH후 간의 반응을 24시간 앞으로 당긴 것과 유사하게, ASGR1의 발현이 48시간에서 가장 낮았고, 72시간에서는 오히려 ASGR1이 다시 발현됨을 확인하였다.
ASGR1은 하부 타겟인자인 Pcsk9를 활성화하고, Pcsk9는 지질인자를 간 세포로 받아드리는 Cd36과 Ldlr를 파괴시키는 작용을 하는 것으로 알려져 있는데, TSG-6에 의한 ASGR1의 억제 작용으로 인해 Pcsk9 발현이 감소되어, TSG-6가 Cd36와 Ldlr의 발현 증가를 가져오는지 확인하였다.
그 결과 도 10에 따르면, Pcsk9에 의해 감소되는 지방대사 인자인 Cd36와 Ldlr의 발현이 증가되었고 Cd36와 Ldlr에 의해 지방 공급이 증가되었음을 간 조직내 TG함량을 통해 확인하였다. 또한 이렇게 공급된 지방이 세포 증식에 필요한 에너지로 전환되었음을 pAMPK의 발현을 통해 알 수 있었다.
전술한 본 발명의 설명은 예시를 위한 것이며, 본 발명이 속하는 기술 분야의 통상의 지식을 가진 자는 본 발명의 기술적 사상이나 필수적인 특징을 변경하지 않고서 다른 구체적인 형태로 쉽게 변형이 가능하다는 것을 이해할 수 있을 것이다. 그러므로 이상에서 기술한 실시예들은 모든 면에서 예시적인 것이며 한정적이 아닌 것으로 이해해야만 한다.
본 발명의 범위는 후술하는 청구범위에 의하여 나타내어지며, 청구범위의 의미 및 범위 그리고 그 균등 개념으로부터 도출되는 모든 변경 또는 변형된 형태가 본 발명의 범위에 포함되는 것으로 해석되어야 한다.

- TSG-6 (Tumor necrosis factor-inducible gene 6); 또는 이의 발현 또는 활성 촉진제를 유효성분으로 함유하는 간 재생 촉진 또는 간 기능 회복용 약학 조성물.
- 청구항 1에 있어서,
상기 발현 촉진제는 TSG-6 유전자의 mRNA의 발현을 증가시키는 TSG-6 코딩 유전자를 포함하는 아데노연관 바이러스 (AAV), 아데노 바이러스, 렌티바이러스 및 레트로 바이러스로 이루어진 군에서 선택된 것을 특징으로 하는 약학 조성물. - 청구항 1에 있어서,
상기 활성 촉진제는 TSG-6 단백질의 활성 또는 안정성을 촉진하거나, 단백질 분해를 저해하는 저분자 화합물, 펩타이드, 펩타이드 미메틱스, 앱타머 및 천연물로 이루어진 군에서 선택된 것을 특징으로 하는 약학 조성물. - 청구항 1에 있어서,
상기 약학 조성물은 부분 간 절제술 (partial hepatectomy) 후 간 재생을 촉진하거나 간 기능을 회복하는 것을 특징으로 하는 약학 조성물. - 청구항 1에 있어서,
