Abstract
Description
The present invention relates to a method and system for providing information for predicting the prognosis of breast cancer after breast reconstruction.
(Goldhirsch et al. (2001). J. Clin. Oncol. 19: 3817-27). Breast cancer is the most common cancer in women and the second most fatal cancer. In 2001, the prevalence of breast cancer was 90-100 per 100,000 in the United States and 50-70 per 100,000 in Europe. The incidence of this disease is on the rise worldwide. Risk factors for breast cancer include race, age, and mutations in the cancer suppressor genes BRCA-1, BRCA-2 and p53. Alcohol intake, high-fat diet, lack of exercise, exogenous postmenopausal hormones and ionizing radiation also increase the risk of breast cancer. Estrogen receptor- and progesterone receptor-negative breast cancer ("ER-" and "PR-", respectively), large tumor size, high-grade cytology, and poor prognosis in those younger than 35 (Goldhirsch et al. (2001). J Clin. Oncol. 19: 3817-27).
The most essential treatment for breast cancer is surgical operation to remove the tumor, and depending on the state of the tumor, prior chemotherapy using drugs before surgery, radiation therapy to prevent cancer recurrence after surgery, chemotherapy, and hormone therapy for cancer Adjuvant therapies such as
Breast cancer surgery can preserve the breast by removing only the cancer and a part of the normal tissue around the cancer if the cancer exists locally. can be implemented Reconstruction after mastectomy is divided into immediate reconstruction, which is performed simultaneously with mastectomy, and delayed reconstruction, which is performed after sufficient time has elapsed, depending on the timing of reconstruction. There is a flap technique using autologous tissues such as skin, muscle, and fat. The timing and method of such reconstruction are determined in consideration of the patient's condition, mastectomy method, breast cancer stage, metastasis, and adjuvant therapy. Breast cancer patients who underwent mastectomy experience not only physical changes but also psychological side effects such as low self-esteem and depression due to loss of femininity. Immediate reconstruction is preferred, as it has been shown to provide significant benefits to patients in both overall quality of life.
Immediate reconstruction is performed with total resection followed by the first stage surgery to insert the tissue expander, followed by the second stage surgery to remove the tissue expander and insert the implant when the expansion of the tissue expander is completed. Implants inserted in the second-stage surgery are largely divided into smooth implants and textured implants according to the texture of the surface. Previous studies on the surface texture of implants have mainly concerned their potential impact on postoperative complications including capsular contracture.
On the other hand, implant insertion has been performed not only for reconstruction purposes after mastectomy for breast cancer patients, but also during breast augmentation surgery for women. Breast Implant Associated-Anaplasic Large Cell Lymphoma (BIA-ALCL) is a type of Non-Hodgkin's Lymphoma, and is a completely different disease from the commonly known breast cancer. Since ALCL was first reported in a patient with breast implants in 1997, discussions on the possibility of an association between implants and ALCL have continued, and in 2016, the World Health Organization officially designated BIA-ALCL as ALCL. classified as one subtype.
Patients considering reconstruction surgery along with total mastectomy often had vague anxiety that the prognosis of breast cancer could change, as well as concerns about the development of BIA-ALCL depending on the surface texture of the implant. Therefore, it is necessary to study the prognosis of breast cancer according to the type of implant.

The regional recurrence means that the cancer has metastasized to the ipsilateral axilla, inner breast, supraclavicular or subclavian lymph nodes. The survival (survival) means the case of not dying regardless of whether or not recurrence or prevalence of breast cancer. The disease free survival means that a patient survives without symptoms or signs of cancer after treatment.
The pattern can predict the prognosis of breast cancer after breast reconstruction by analyzing the probability of breast cancer recurrence or disease-free survival corresponding to each type of implant by classifying and statistically analyzing the type of implant and the treatment result.

In this case, it may be predicted that the probability of breast cancer recurrence is high or the probability of disease-free survival is low, which corresponds to a poor prognosis of breast reconstruction.
본 발명의 일 실시예에 따르면, 2단계 유방 재건술을 받은 피험자의 보형물의 유형이 텍스쳐 보형물인 경우에는 유방암의 ER 양성 여부, 암의 병기, 암의 침윤성 여부와 무관하게 누적 생존(cumulative survival)이 스무스 보형물인 경우에 비하여 낮은 것을 확인하였다(도 3 참조).According to one embodiment of the present invention, when the type of implant of a subject who has undergone two-step breast reconstruction surgery is a textured implant, the cumulative survival is irrespective of whether the breast cancer is ER-positive, the stage of the cancer, or whether the cancer is invasive. It was confirmed that it was lower than that of the smooth implant (see FIG. 3).
This method of providing information for predicting the prognosis of breast cancer after breast reconstruction can predict the recurrence of breast cancer in a patient according to the type of implant during breast reconstruction, prepare appropriate treatment for this, Patients can be provided with a type of implant that is less likely to recur breast cancer.

Patients who received neoadjuvant neoadjuvant were excluded from the analysis because their response to chemotherapy could be heterogeneous, which could have a significant impact on the oncological outcome, and the remaining patients were followed up regularly.
In addition, when the tissue expander was removed early due to postoperative complications or discomfort, or when reconstruction was performed with autologous tissue, patients who underwent implant replacement at another hospital, and breast cancer recurrence or recurrence before the second stage surgery (implantation of implants) Patients with related deaths were excluded.
Patients who underwent the second-stage surgery were classified into two groups, either a smooth implant implantation group or a textured implant implantation group, according to the surface texture of the implant. included in the analysis.
Surgery and treatment process
Patients with breast cancer who were predicted to have early-stage cancer on preoperative examination were usually advised to undergo immediate reconstruction and refer them to reconstructive surgery. There was no specific agreement between breast and reconstructive surgeons. Patients were assigned to reconstructive surgeons who were scheduled for surgery.
Patients underwent skin- or nipple-sparing mastectomy and a tissue expander was inserted immediately for reconstruction. The tissue expander was inserted into the subpectoral space, covered on the sides with acellular dermal matrix (ADM) or serratus anterior fascia. The tissue expander was inflated within 1 month of complete wound healing. Stage 2 surgery was planned at least 3 months after tissue expander inflation was complete. In the second stage surgery, the tissue expander was removed and an implant was inserted at the location where the tissue expander was inserted.
The type of implant was determined by the preference of the patient's breast shape and the preference of the reconstruction surgeon in charge, and the oncological aspect was not considered.
After surgery, the radiation oncologist and oncologist discussed with the breast surgeon, and adjuvant radiation therapy and chemotherapy were administered. Adjuvant radiotherapy was applied to patients with T3 lesions, N2 lesions or advanced cancer stages. In addition, radiotherapy was performed in early-stage patients who underwent close resection margins or patients with high-stage disease. Adjuvant chemotherapy is usually given to patients with T2 lesions, N1 lesions, or more advanced cancer stages, or those who are estrogen receptor (ER) negative. Oncotype DX (Genomic Health), a genetic test method for breast cancer prediction, is ER-positive, human epidermal growth factor receptor (ERBB2) (human epidermal growth factor receptor 2) to determine the benefit of adding chemotherapy to hormone therapy for early-stage patients , HER2) was selectively performed in patients with N1 lesions as well as in negative cases.
1-3. Surveillance for recurrence after surgery
During the postoperative follow-up period, a breast surgeon and an oncologist diagnosed distant metastases. Patients with non-invasive cancer underwent breast ultrasonography every year after surgery and were followed for more than 5 years. Patients with invasive cancer underwent breast ultrasonography every 6 months, chest and abdomen or pelvis computed tomography (CT) every 12 months for the first 3 years, and regular physical examinations and CT scans every year for at least 2 years thereafter. received.
1-4. evaluation of results
The local regional recurrence free survival (LRRRFS) and disease free survival (DFS) were mainly evaluated. Local recurrence was defined as recurrence of cancer in the ipsilateral (same side of the breast as the previously cancer-occurring breast) including the skin, subcutaneous tissue, and chest wall. Regional recurrence was defined as metastasis of cancer to the ipsilateral axillary, medial breast, supraclavicular or subclavian lymph nodes. Recurrence of cancer in other sites was defined as distant recurrence. Cancers that developed in the contralateral breast during the follow-up period were considered as new primary tumors, not recurrences. During the follow-up period, pathological examinations were performed by surgical or radiographic methods for lesions newly identified or suspected of recurrence. Lesions that were clearly recurring or metastatic on imaging were considered recurring without pathological examination.
The date and type of recurrence, the date of the last follow-up, and the disease status at the time were recorded. For recurrence monitoring, a checkup by a breast surgeon or an oncologist was performed on the last day of follow-up. Time of first local or regional recurrence or time of death was used to determine LRRFS. Regardless of the type of recurrence, the time of first breast cancer recurrence or death was used to determine DFS.
1-5. statistical analysis
Pearson's Chi square test or Fisher's exact test was used to analyze categorical variables, and unpaired two-end t was used to compare continuous variables. Test (unpaired, 2-tailed t test) or Mann-Whitney test was adopted. Kaplan-Meier analysis was used to evaluate LRRFS and DFS by comparing differences between variables of interest using a log-rank test. Breast cancer recurrence and death were end points of follow-up. Cases in which no relevant event occurred during the follow-up period were censored.
Univariate and multivariate analyzes were performed using the Cox proportional hazards regression model to identify independent risk variables for the outcome. A backward selection model was used for multivariate analysis.
All statistical analyzes were performed using SPSS version 20 (IBM Corporation, USA), and a 2-sided p value (2-sided p) < 0.05 was considered statistically significant. Data analysis was conducted from February 15, 2020 to March 5, 2020.

In Table 1, a represents two patients who had previously undergone breast augmentation surgery, and textured implants were inserted 1 year (patient 1) and 3 years (patient 2) before breast cancer diagnosis. b is a patient who had previously undergone breast augmentation surgery, but for which related information could not be collected. c is patients who underwent Oncotype DX, a genetic test for breast cancer prediction, all younger than 50 years of age.
Age, BMI, type of lymph node surgery, cancer stage, rate of adjuvant radiotherapy and chemotherapy, and type of chemotherapy appeared to be similar between the smooth implant group and the texture implant group. However, the incidence of N0 lesions tended to be higher in the textured implant group compared to the smooth implant group (330 [79.9%] vs. 197 [71.9%]; p = 0.06). The median follow-up period was longer (55.0 [range: 31 to 101] months and 50.0 [range: 31 to 106] months) in the smooth implant implantation group compared to the texture implant implantation group.
Overall, 28 patients (4.1%) were diagnosed with breast cancer recurrence during the follow-up period. The 5-year LRRFS was 96.7% and the 5-year DFS was 95.2%. In the textured implant group, the incidence of all types of recurrence (23 cases and 5 cases) was higher and lower LRRFS (95.9% and 97.8%) and DFS (93.3% and 97.8%), respectively, compared to the smooth implant group. .
In the Kaplan-Meier analysis, LRRFS did not differ between the textured and smooth implanted groups (97.5% and 98.9% at 3 years, 97.3% and 98.5% at 4 years, 95.9% and 97.8% at 5 years, p = 0.25). However, DFS was significantly lower in the textured implant group than in the smooth implant group (96.1% and 98.9% at 3 years; 94.7% and 98.5% at 4 years; 93.3% and 97.8% at 5 years; p = 0.01) (Fig. 2). ). No statistically significant difference between the two groups was observed in the DFS survival curve for the specific type of implant.
유방 보존 수술 후 유방암이 재발한 이력이 없는 원발성 유방암 환자로 제한하여 분석한 경우에도 이와 유사한 연관성이 관찰되었다(조정 HR, 3.072; 95% CI, 1.120-8.097; p = 0.02).Univariate or multivariate analysis did not show any difference in LRRFS according to the surface texture of the implant. ER status appeared as an independent factor related to LRFS. However, in the textured implant group, compared to the smooth implant group, the DFS value was significantly lower in univariate analysis (hazard ratio [HR], 3.221; 95% CI, 1.224-8.477; p = 0.02). This association remained significant after adjustment for other variables including tumor stage and ER status (HR, 3.054; 95% CI, 1.120–8.051; p = 0.02) (Table 2). Other factors, including reconstruction, adjuvant radiotherapy, and adjuvant chemotherapy, did not appear to alter DFS significantly. A similar association was observed when the analysis was limited to patients with primary breast cancer who had no history of breast cancer recurrence after breast-conserving surgery (adjusted HR, 3.072; 95% CI, 1.120-8.097; p = 0.02).
Table 2 shows the results of univariate and multivariate analyzes using the Cox proportional hazards regression model for factors related to LFFRS and DFS.

Table 2a indicates that age, BMI, cancer stage, ER, PR, ERBB2, adjuvant chemotherapy, adjuvant radiotherapy, and type of implant were initially included in the multivariate analysis for the DFS evaluation. b means that the HR of ER-positive was calculated based on ER-negative. c means that the HR of the textured implant was calculated based on the smooth implant. d means that the HR of each stage was calculated based on the cancer stage being 0.
Analysis of subgroups
To reduce potential confounding of these variables, subgroup analyzes were performed according to ER status, tumor invasiveness and cancer stage.
In a univariate analysis of ER-positive breast cancer patients similar to the entire study population, the textured implant group showed a statistically significantly worse DFS than the smooth implant group (96.4% and 99.1% at 3 years, 95.1% and 95.1% at 4 years, respectively). 98.7%, 94.3% and 98.0% at 5 years; p = 0.03) (FIG. 3). This difference was also maintained in multivariate analysis (adjusted HR, 3.130; 95% CI, 1.053-9.307; p = 0.04). A similar trend was observed in patients with ER-negative cancer, showing an increased risk of recurrence in the texture implant implantation group compared to the smooth implant implantation group (DFS: 94.0% and 96.9% at 3 years, 92.5% and 96.9% at 4 years; 88.9% and 96.9% at 5 years, p = 0.21), which did not reach statistical significance.
For patients with in situ stage cancer, only one recurrence (local and regional lymph node recurrence) was observed in the textured implant group, so no statistical analysis was performed.
In the multivariate analysis of patients with invasive cancer, a trend similar to that determined in the analysis of all patients was confirmed in the textured implant group (HR, 3.044; 95% CI, 1.120-8.039; p = 0.03).
(HR, 8.874; 95% CI, 1.120-68.748; p = 0.04).The type of implant did not show a significant association with recurrence for
