Abstract
본 발명은 비알코올 지방간염 진단을 위한 MASH score에 관한 것이며, 보다 상세하게는 전체 비알코올 지방간질환에서 비알코올 지방간염 진단을 위하여 환자의 임상 소견, 혈액검사, MRI 결과를 조합하는 score에 관한 것이다. 비알코올 지방간질환은 전세계적으로 유병률이 증가하고 있고, 우리나라도 전체 인구의 30% 내외가 비알코올 지방간질환을 가지고 있다고 보고되고 있다. 비알코올 지방간질환을 가지고 있는 사람은 전체 사망률 및 간질환에 의한 사망률이 증가하는 것으로 알려져 있다. 전체 비알코올 지방간질환 환자 중에서 75%정도는 단순 지방간으로 양호한 예후를 나타내지만, 약 25%를 차지하는 지방간염을 가지고 있는 환자가 나쁜 예후를 보인다. 하지만, 지방간염을 진단하기 위해서는 간 조직검사를 통하여 간세포의 풍선변성 (hepatocyte ballooning)의 관찰이 필요하다. 하지만, 조직검사는 비용이 비싸고, 환자의 입원이 필요하고, 출혈/감염과 같은 부작용의 위험을 수반하기 때문에 모든 환자에서 진행하는데 한계가 있다. 이에 본 발명은 전체 비알코올 지방간질환에서 비알코올 지방간염 진단을 위하여 환자의 임상 소견, 혈액검사, MRI 결과를 조합하는 MR-based nonAlcoholic SteatoHepatitis (MASH) score를 제공한다.
Description
본 발명은 MR 기반 비알코올성 지방간염(MR-based nonAlcoholic SteatoHepatitis: MASH) 점수와 이의 이용에 관한 것이다.
비알코올 지방간염(Nonalcoholic steatohepatitis: NASH)은 간 섬유증(hepatic fibrosis)에 관계없이 염증 및 간세포 팽창을 동반한 간 지방증(steatosis)이 5% 이상 존재하는 것으로 정의되는 비알코올 지방간 질환(Nonalcoholic fatty liver disease: NAFLD)의 진행성 형태이다. NASH 환자는 전체 NAFLD 환자 중 20-25%에 불과하지만 NASH 환자는 예후가 좋지 않기 때문에 그 조기 진단이 중요하다. 그러나 NAFLD의 유병률은 전 세계적으로 전체 인구의 30%에 달해 모든 NAFLD 환자를 생검으로 진단하기 어려우며, 조직검사에서 결과까지 시간이 많이 소요된다. 따라서 비침습적 평가 및 진단은 NAFLD 연구에서 중요하고 유망한 분야이다.
NAFLD 진단을 위한 비침습적 바이오마커는 혈청학적 바이오마커, 조합 패널 및 영상 바이오마커와 함께 연구되었다. Cytokeratin 18(CK18)은 NAFLD의 질병 중증도에 대해 가장 많이 조사된 마커이지만, 중간정도의 정확성과 불확실한 최적 cut-off value에 의하여 임상적 사용에 제한이 있으며, 다른 염증성 마커와 아디포카인(adipokine) 역시 정확성이 낮은 문제가 있다. 또한 음향 복사력 임펄스 영상(Transient elastography)과 자기공명 탄성 검사(magnetic resonance elastography)는 간 경직도를 측정할 수 있으나 NASH 진단에 정확도가 그리 높지 않다. NASH는 매우 복잡한 질환으로 단일 매개변수로 비침습적 바이오마커를 개발하기가 어렵다. 이와 관련하여 NASHTest와 NASH Diagnostics Panel을 포함한 복합패널이 개발되었으나 비용이 비싸고 아직 충분히 검증되지 않았다.
최근 진행성 환자에서 섬유증이 있는 환자를 식별하기 위한 예측 모델로 FibroScan-AST(FAST) 점수가 개발되었다. 이 모델은 CAP(Controlled attenuation parameter) 및 LSM(liver stiffness measurement) 및 aspartate transaminase(AST)을 측정하여 점수를 계산한다. FAST 점수는 만족스러운 성과를 보였고(C-static 0.80, 95% CI 0.76-0.85) 다른 외부 코호트와 함께 검증되었다(C static range 0.74-0.95).
MRI-PDFF 및 MRE는 지방증 및 섬유증의 검출뿐만 아니라 중증도의 등급화에서도 다른 비침습적 방법보다 우수한 성능을 보였다. 이전 연구에서 MRS, MRE, 및 T1 mapping을 포함하는 비침습적 MR index가 효과적으로 NASH를 진단할 수 있음을 확인했다. 또한 초음파가 지방간 병변의 가시화에 한계가 있는 반면 MRI를 이용하여 효과적으로 간에서 병리학적 병변을 확인할 수 있음을 확인하였다. 이러한 장점을 고려할 때 다중 매개변수 MR은 간 생검을 대체하거나 보완하는 유용한 검사 방법이 될 수 있다.
비록 NASH 평가를 위한 다중 매개변수를 이용한 MR의 가치가 이전의 연구를 통해 일부 밝혀졌지만 다중 매개변수 MR과 임상 지표를 결합하여 NASH 진단의 정확도, 민감성, 및 특이성을 향상시킬 수 있는 진단 평가 시스템은 개발된바 없다. 본 발명은 이러한 진단 평가 시스템에 관한 것이다.
본 발명은 신속하게 적절한 치료가 가능하도록 예후가 좋지 못한 비알코올 지방간염(NASH) 환자를 다중매개변수 MR 및 임상 지표를 이용한 비침습적 진단방법 제공을 목적으로 한다.
그러나 본 발명이 이루고자 하는 기술적 과제는 이상에서 언급한 과제에 제한되지 않으며, 언급되지 않은 또 다른 과제들은 아래의 기재로부터 당해 기술분야의 통상의 기술자에게 명확하게 이해될 수 있을 것이다.
상기 과제를 해결하기 위하여, 본 발명은 인구통계학적 매개변수, 실험실 매개변수, 및 MR 매개변수를 이용하여 하기 식 1의 MASH 점수를 산출하여 비알코올 지방간 질환 환자 중에서 비알코올 지방간염 환자를 진단하기 위한 시스템을 제공한다.
본 발명에서 인구통계학적 매개변수는 환자의 당뇨병(diabete) 또는 공복혈당장애(Impaired Fasting Glucose: IFG) 병력의 유무, 고혈압 상태, 나이, 및 BMI를 포함하고, 실험실 매개변수는 환자의 헤모글로빈(hemoglobin)과 혈소판(platelet) 수이며, MR 매개변수는 MRI를 이용하여 T1 mapping과 MRE를 수행하여 측정한 T1 이완시간과 간 경직도를 포함한다.

상기 식 1에서 T1 mapping은 T1 mapping을 수행하여 T1 이완시간을 측정한 값으로, 비선택적 반전 회복 펄스와 16개 반전 대비의 low-flip angle FLASH 획득을 적용하여 1) 간정맥이 하대정맥(inferior vena cave)과 합류하는 지점; 2) 간문(hepatic hilum); 및 3) 담낭와(gallbladder fossa)에서 종축 자기화를 회복하는데 걸린 시간의 평균값을 의미한다.
상기 식 1에서 MRE는 환자의 날숨에서 획득된 MRE 이미지를 이용하여 측정된 간 경직도를 의미한다.
상기 식 1에서 환자에 당뇨병 및/또는 공복혈당장애 병력이 있는 경우 상기 변수에 1을 대입하고, 환자에 고혈압이 있는 경우 hypertension 변수에 1을 대입한다.
또한, 본 발명은 (1) 환자의 당뇨병 또는 공복혈당장애(IFG) 병력의 유무, 고혈압 상태, 나이, 및 BMI 정보를 수집하는 단계;
(2) 상기 환자의 혈액 샘플에서 헤모글로빈과 혈소판 수를 측정하는 단계;
(3) 상기 환자의 MR을 이용하여 T1 mapping과 MRE(magnetic resonance elastography)를 수행하고 T1 이완시간 및 간 경직도를 측정하는 단계; 및
(4) 상기 (1) 내지 (3) 단계에서 획득된 지표를 상기 식 1에 대입하여 MASH 점수를 계산하는 단계;를 포함하는 지방간염 진단을 위한 정보제공방법을 제공한다.
본 발명의 상기 정보제공방법에서 (1) 내지 (3) 단계는 반드시 시계열적으로 수행되는 것은 아니다.
또한, 본 발명은 연산장치에 결합되어 환자의 당뇨병 또는 공복혈당장애(IFG) 병력의 유무, 고혈압 상태, 나이, BMI, 혈액 중 헤모글로빈 및 혈소판의 수, T1 이완시간, 및 간 경직도 지표를 상기 식 1을 이용하여 MASH 점수를 산출하는 단계를 실행시키기 위하여 매체에 저장된 컴퓨터프로그램을 제공하며,
상기 지표를 입력하는 입력부, 입력된 지표를 상기 식 1을 이용하여 MASH 점수를 산출하는 산출부, 및 비알코올 지방간염 진단 정보를 출력하는 출력부를 포함하는 장치를 제공한다.
MASH 점수가 0.73 이상인 경우 비알코올 지방간염으로 진단하고, 0.37 이하인 경우 비알코올 지방간염이 아닌 것으로 판단할 수 있다.
본 발명의 MASH 점수는 비알코올 지방간 질환 환자 중에서 비알코올 지방간염 환자를 비침습적 방법으로 빠르고 정확하게 진단하기 위하여 이용될 수 있다.

NASH는 NAFLD의 진행형이고 NASH 진단을 위해서는 간 생검이 필수적이기 때문에 NASH 진단을 위한 비침습적 바이오마커의 개발이 시급하다. 본 발명자들은 당뇨병/IFG(공복혈당장애) 및 고혈압, 나이, BMI, 헤모글로빈, 혈소판 수, T1 mapping 및 MRE를 상태변수로 NASH를 진단할 수 있는 MASH 점수를 개발하였다.
NASH 환자는 NAFL 환자와 비교하여 예후가 좋지 않고 NASH 환자만 약물치료를 시행하기 때문에 NASH의 진단은 매우 중요하다. NASH의 진단은 아직까지 간 조직검사가 기준 진단 검사이다. 그러나, 간 조직검사는 고비용, 침습성, 및 합병증 발생의 위험이 있다. 또한, 적은 간 조직으로는 전체 간 상태를 반영할 수 없고 아직까지 단일 병리 진단 기준이 없으며 병리 검사자의 진단 일치도가 낮을 수 있다. 이에, 간 조직검사 대신 영상의학 검사와 생화학 검사를 통한 비침습적 바이오마커의 개발이 요구되고 있다. 초기 NASH 바이오마커는 단일 마커였으며 세포자멸사, 염증, 산화스트레스와 같은 NASH 질병 경로를 표적으로 삼았다. 그러나, NASH는 질병이 진행되는 동안 복잡한 병리학적 과정을 갖고있기 때문에 단일 매개변수의 바이오마커는 전체 NAFLD 환자와 NASH 환자를 구별하는데 한계가 있었다. 단일 매개변수 바이오마커의 한계를 극복하기 위하여 NASH Test, NASH Diagnostic Panel 및 CK-18과 표면항원 Fas의 조합 등이 복합 바이오마커로 개발되었으나, 이러한 조합 패널은 진단의 정확도는 높였으나 CK-18, 아포지단백(apolipoprotein), 아디포넥틴(adiponectin), 및 레지스틴(resistin)과 같은 정규적으로 확인되지 않는 변수를 포함한다. 본 발명자들은 접근이 용이한 인구 통계학적 매개변수(당뇨병/IFG, 고혈압 상태, 연령, BMI)과 측정이 용이한 실험실 매개변수(헤모글로민 및 혈소판의 수)를 이용하여 NASH 진단을 위한 MASH 점수를 개발하였으며, MASH 점수는 0.892로 높은 정확도를 나타낸다.
당뇨병/IFG 및 HTN은 NAFLD의 발달 및 진행과 관련된 것으로 알려진 대사 질환이다. 당뇨병 및 당뇨병의 가족력은 NASH와 유의한 관련이 있다. 당뇨병에서 인슐린 저항성 및 지방 조직 기능 장애는 간세포에서 지방 독성을 유발하고, 전염증 경로를 활성화한다. 당뇨병의 당독성은 지방독성 및 인슐린 저항성과 관련되어 NASH를 촉진한다. 또한 고혈압은 NAFLD의 위험 및 NAFLD의 질병 중증도와 관련이 있습니다. 즉, 당뇨병/IFG 및 고혈압 모두 MASH 점수의 양의 상관관계가 있다.
나아가, 연령의 증가는 MASH 점수 체계에서 NASH와 연관된다. 고령의 NAFLD 환자는 보다 심각한 조직학적 변화를 보였으며, 세포 노화는 지방간 및 NAFLD의 중증도와 상관관계가 있었다. 한편, BMI는 MASH 점수에서 NASH와 역의 상관관계가 있었다. 비만과 높은 BMI는 NAFLD 및 NASH의 위험을 증가시키는 것으로 알려져 있다. 그러나, Hagstrm et.al.는 마른 NAFLD 환자가 BMI가 높은 NALFD 환자보다 심각한 간 질환 위험이 더 높다고 보고한바 있다. 또 다른 연구에서도 마른 NAFLD 환자가 과체중 또는 비만 NAFLD 환자보다 사망률이 높으며 좋지못한 임상 결과를 나타냄을 보고하였다. 또한, 근감소증(Sarcopenia)은 NASH의 위험인자로서 제지방량 (lean body mass) 증가의 원인이고, 이는 본 발명의 MASH 점수에서 NASH와 BMI 사이에 음의 상관관계에 대한 설명이 된다. 만성 염증은 빈혈의 원인 중 하나이며, 실험실 매개변수는 NASH 환자가 NAFL 환자와 비교하여 낮은 헤모글로빈 수치를 보임으로부터 도출되어 헤모글로빈 수치는 NASH의 위험을 유의하게 증가시킨다.
NAFLD의 질병 중증도를 평가하기 위한 본 출원의 MASH 시스템과 다른 스코어링 시스템 간의 주요 차이점은 T1 mappin 및 MRE와 같은 다중 매개변수 MR 구성요소의 적용 여부이다. 다중 매개변수 MR은 NAFLD 마우스모델에서 NAFLD 활성 점수를 예측할 수 있었고 염증, 섬유증 및 풍선화(hepatocyte ballooning)와 높은 상관관계를 보였다. 본 발명자들은 이전의 연구에서 다중 매개변수 MR 지수가 지방증- SAF(steatosis-activity-fibrosis) 점수에 따른 NASH 진단에 높은 정확도 확인하였다. MASH 점수의 MR 매개변수는 T1 mapping 및 MRE 이다.
본 발명자들의 이전 연구에 따르면 MRE는 NASH 또는 진행성 섬유증 단계(3기 또는 4기)를 높은 정확도로 진단하는데 효과적인 반면에(AUROC 0.86), MRE의 진단 정확도는 NASH(AUROC 0.77)에 제한적이었다. 본 발명의 MASH 점수는 전체 NAFLD 환자에서 NASH를 진단하는 것을 목적으로 하며, MASH 점수 체계는 NASH 환자의 중증도와 무관하게 NAFL 환자 중에서 NASH 환자를 구별할 수 있도록 하고 MASH 점수에 따른 NASH 진단 정확도는 섬유화 단계에 영향을 받지 않는다.
또한, 본 발명은 진단 cut-off 값(0.73)과 제외 cut-off 값(0.37)을 설정하여 MASH 점수의 정확도를 극대화하였다. 본 발명자들이 설정한 cut-off 값에서 MASH 점수의 음성 예측값은 0.87이고 양성 예측값은 0.89를 나타낸다. 그레이 존에 위치한 환자는 추가 진단 평가가 요구되지만 MASH 점수에 따른 그레이 존(gray zone) 환자는 소수에 불과하다(22/130, 17%)
본 발명은 다양한 변환을 가할 수 있고 여러 가지 실시예를 가질 수 있는 바, 이하 특정 실시예들을 도면에 예시하고 상세한 설명에 상세하게 설명하고자 한다. 그러나, 이는 본 발명을 특정한 실시 형태에 대해 한정하려는 것이 아니며, 본 발명의 사상 및 기술 범위에 포함되는 모든 변환, 균등물 내지 대체물을 포함하는 것으로 이해되어야 한다. 본 발명을 설명함에 있어서 관련된 공지 기술에 대한 구체적인 설명이 본 발명의 요지를 흐릴 수 있다고 판단되는 경우 그 상세한 설명을 생략한다.
[실험방법]
1. 연구 대상
간 생검으로 NAFLD로 진단된 130명의 환자를 대상으로 연구를 수행하였다. 모든 환자는 만성 B형 간염, 만성 C형 간염, 자가면역 간질환 등 다른 만성 간질환은 없었다. 남성의 경우 주당 140g, 여성의 경우 주당 70g 이상 음주한 환자, 대상성 간경변증(decompensated liver cirrhosis) 환자, MRI 사용이 금지된 환자, 기타 중증 전신 질환 또는 악성 종양 환자도 포함되지 않았다. 모든 환자는 간 생검 후 6개월 이내에 실험실 검사와 MRI를 받았다. 실험실 검사에는 헤모글로빈, 백혈구 수, 혈소판 수, 알라닌 아미노전이효소(alanine aminotransferase, ALT), 알칼리성 인산분해효소(alkaline phosphatase, ALP), 감마-글루타밀 전이효소(gamma-glutamyl transferase, GGT), 총 빌리루빈(bilirubin), 알부민(albumin), 프로트롬빈 시간(prothrombin time, PT), 혈액요소질소(Blood Urea Nitrogen, BUN), 크레아티닌(creatinine) 및 C반응단백(C-reactive protein )가 포함되었다.
이 연구는 고려대학교 구로병원 기관심사위원회(2016GR0302)의 승인을 받았으며 이전 연구와 함께 ClinicalTrials.gov(NCT03725631)에 등록되었다. 모든 환자는 연구 참여에 동의하고 사전 동의서를 작성했다. 이 연구는 헬싱키(Helsinki) 선언에 따라 수행되었다.
2. 조직병리학적 평가(Histopathologic evaluation)
숙련된 방사선 전문의가 18 게이지 Tru-cut needle(TSK Laboratory, Thochigi, Japan)을 사용하여 늑간 공간을 통해 간 생검을 수행하였다. 2개의 간 표본을 포르말린으로 고정하고 파라핀 블록을 만들었다. 슬라이스 섹션을 H&E 염색하고, 경험이 풍부한 병리학자가 NAFLD를 진단하고 NASH Clinical Research Network 조직학적 점수를 평가하였다. NASH는 간 지방증이 5% 이상 존재하고 섬유증과 상관없이 염증과 간세포 팽창이 있는 경우로 진단하였다.
3. 다중 매개변수 자기공명 영상(Multiparametric magnetic resonance imaging)
3 T MR scanner (MAGNETOM Skyra, Siemens Healthcare, Erlangen, Germany)을 이용하여 모든 환자의 MRI를 촬영하였다. 다중 매개변수 MR 시퀀스는 MRI-PDFF, MRS, T1 mapping, MRE, T1강조영상(T1-weighted image), T2강조영상(T2-weighted image), 및 확산 강조영상(diffusion-weighted image)의 총 7개 시퀀스를 포함한다.
MRI-PDFF와 MRS 기술, Modified DIXON 기술을 사용하여 MRI-PDFF를 측정하였다. MRS는 20 X 20 X 20 mm 단일 복셀(voxel)에서 수행되었으며 데이터는 MR 공급업체의 온라인 프로그램을 사용하여 분석하였다.
T1 mapping: T1 이완시간은 FLASH(Fast Low-Angle Shot)를 기반으로 한 shMOLLI(Modified Look Locker Inversion) 복구 시퀀스를 사용하여 수행하였다. 각 이미지는 3가지 수준에서 획득되었다: 1) 간정맥이 하대정맥(inferior vena cave)과 합류하는 지점; 2) 간문(hepatic hilum); 3) 담낭와(gallbladder fossa). T1 이완 시간은 비선택적 반전 회복 펄스와 16개 반전 대비의 low-flip angle FLASH 획득을 적용하여 측정하였다. 이미지 매개변수는 하기 표 1에 나타내었다. 서로 다른 이미지에서 4개의 관심 영역(regions-of-interest: ROI)을 측정하고 평균 값을 T1 이완시간(milliseconds)으로 표시하였다. 본 명세서에서 T1 이완시간은 T1 mapping으로 표시하였다.
MRE(magnetic resonance elastography): 간 경직도는 환자의 간 오른쪽 전흉벽에서 pneumatic driver system (Resoundant, Rochester, MN, USA)을 이용한 MRE를 통해 측정하였다. 드리아버에서 생성한 60-Hx shear-wave는 유연한 비닐 튜브를 통해 전달된다. 시퀀스 매개변수는 하기 표 1에 나타내었다. 환자의 날숨에서 4개의 MRE 이미지를 획득하였으며, 획득한 이미지를 elastogram으로 처리하고 elastogram에 표시된 ROI를 측정하여 간 경직도를 측정하였다. 각 이미지에서 4개의 ROI를 측정하였고 간 경직도의 평균값을 킬로파크칼(kPa)로 표시하였다. 본 명세서에서 MRE를 통해 도출한 간 경직도의 평균값은 MRE으로 표시하였다.

4. 통계분석
인구통계학적 특성과 실험실의 특성은 범주형 변수에 대한 백분율이 포함된 환자의 수와 1번째 및 3번째 백분위수의 중앙값으로 표시되었다. 기저 특성(Baseline Characteristics)을 비교하기 위해 SPSS 소프트웨어 (version 25; IBM Corporation, Armonk, NY, USA)을 사용하여 Pearson's chi-squared test 및 the Man-Whitney U-test를 수행하였다(P< .05).
[실험 결과]
1. 기저 특성(Baseline Characteristics)
2016년 9월부터 2019년 3월까지 고려대학교 구로병원에 간 조직검사를 통해 NAFLD로 진단된 130명의 환자를 등록하였다. 인구통계학적 및 실험실적 특성을 NAFL 환자와 NASH 환자를 정리하여 표 2에 비교하였다. NASH 환자는 NAFL 환자와 비교하여 고령이고 낮은 BMI 수치를 나타내었다. 당뇨병/IFG, 고혈압 및 이상지질혈증의 유병률은 NAFL 환자보다 NASH 환자에서 더 높았으며, 특히 당뇨병/IFG의 유병률에서 유의한 차이가 확인되었다. 실험실 테스트에서 NASH 환자는 NAFL 환자보다 낮은 헤모글로빈 및 혈소판 수와 높은 AST를 나타냈다. 다중 매개변수 MR 시퀀스 중에서 MRE는 NAFL 환자와 비교하여 NASH 환자에서 더 높은 T1 mapping(p = .089) 및 MRE(p < .001)를 보였다.

2. 데이터 검증
먼저 각 변수 간의 다중공선성(multicollinearity)을 확인하였으나, 분산팽장계수에서 10을 초과하는 변수는 없었으며 변수간 다중공선성은 관찰되지 않았다. 이어서, 각 피험자간의 영향을 확인하였으나, DIFCHISQ 100 이상인 관련 환자는 없었다.
3. Model Specification
NASH 진단 모델에 대해 4개의 범주형 변수와 19개의 연속형 변수를 평가하였다(표 3). p-값이 0.1 이하를 기준으로 연령, BMI, 당뇨병/IFG 및 고혈압과 같은 4개의 임상 매개변수, 헤모글로빈 및 혈소판 수와 같은 2개의 실험실 매개변수 및 T1 mapping과 같은 2개의 MRI 매개변수 및 MRE가 선택되었다. 4개의 임상 파라미터와 2개의 실험실 파라미터로 구성된 진단 모델은 0.759의 C-statistics (95% CI, 0.834-0.950; P<0.001)을 보인 반면, 4개의 임상 파라미터, 2개의 실험실 파라미터 및 2개의 MR 파라미터를 포함하는 진단 모델은 0.860의 C-statistics (95% CI, 0.834-0.950; P<0.001). 이 두 모델 사이에 C- statistics 의 유의한 차이를 확인할 수 있었으며, 이는 MR 매개변수가 진단 정확도를 유의하게 증가시킴을 의미한다.

4. 진단 정확성 확인
도 1 및 2에서 확인되는 바와 같이, MASH 점수는 NASH 진단에 높은 진단 정확도를 보였다(C-statistics, 0.892; 95% CI, 0.834-0.950; P<0.001). cut-off value를 Youden index 0.698로 0.324로 설정했을 때 민감도와 특이도는 각각 0.914와 0.783이었다. 낮은 특이성을 보상하기 위해 민감도 ≥.9에 대한 제외 컷오프와 특이도 ≥.9에 대한 진단 컷오프를 설정하였다(도 3 및 표 5). NASH 진단을 위한 컷오프로 0.73을 설정했을 때 민감도는 0.67, 특이도는 0.90(PPV=0.89, 47/53)이었다. 0.37을 NASH 배제에 대한 cut-off로 설정했을 때 민감도는 0.90, 특이도는 0.78(NPV=0.87, 47/54)이었다. 환자의 17%(22/130)만이 그레이 존에 위치하였다.

Claims (9)
- (1) 환자의 당뇨병 또는 공복혈당장애(IFG) 병력의 유무, 고혈압 상태, 나이, 및 BMI 정보를 수집하는 단계;
(2) 상기 환자의 혈액 샘플에서 헤모글로빈과 혈소판 수를 측정하는 단계;
(3) 상기 환자의 MR을 이용하여 T1 mapping과 MRE(magnetic resonance elastography)를 수행하고 T1 이완시간 및 간 경직도를 측정하는 단계; 및
(4) 상기 (1) 내지 (3) 단계에서 획득된 지표를 하기 식 1에 대입하여 MASH 점수를 계산하는 단계;를 포함하는 지방간염 진단을 위한 정보제공방법으로서,
상기 (1) 내지 (3) 단계 수행의 선후는 무관한 것을 특징으로 하는, 정보제공방법.
