Abstract

본 발명은 구형 리세스(3)를 갖는 강 삽입물(2) 및 재질이 플라스틱이고 보철물 셸(1)과 강 삽입물(2) 사이에 배열된 중간체 요소를 구비한 금속 보철물 셸(1)을 갖는 고관철 보철물에 관한 것이다.
골용해를 방지하기 위해서, 중간체 요소(4)가 상부(5) 및 하부(6)로 구성되며, 상부(5) 및 하부(6)가 서로 연결되는 것이 가능하며, 강 삽입물(2)이 상부(5) 및 하부(6) 사이에 보유되는 것이 제안된다.

Description

고관절 보철물
하나가 다른 하나 내부에 놓여있는 세 개의 구형 컬라트(calotte)를 갖는 고관절 보철물은 실용신안 FR 2 715 828호에 공지되어 있다.
구형의 외부 컬라트는 INOX-강 또는 티타늄과 같은 생교합성 경성 물질로 구성되며, 외표면에 골반 뼈의 관절강에 고정시키기 위한 방사상 함몰이 제공된다.
구형 내부 컬라트는 예를 들어 알루미늄 산화물 세락믹과 같은 단단한 생교합성 물질, 또는 대퇴부 요소의 머리가 구성되는 물질과 관련이 있는 낮은 마찰 계수를 갖는 유사한 물질로 구성된다.
구형 중간체 컬라트는 플라스틱과 같은 반강성 물질, 특히 폴리에틸렌으로 구성된다.
플라스틱으로 제조된 중간체 컬라트 또는 중간체 요소에 있어 문제점은 높은 찰상의 결과로서, 변화도는 셸 및 플라스틱 코팅제 사이의 어떠한 형태 맞물림 연결도 없는 경우에 골용해를 일으킬 수 있다는 것이다. 다른 한편으로, 형태 맞물림 연결은 EP 0 554 214 A1에 기술된 바와 같이 플라스틱 코팅제를 용융시킴으로써 달성될 수 있다. 그러나, 이것을 통해 분자 구조가 파괴될 수 있으며, 결과적으로 착용케 할 수 있다.
본 발명의 일차적인 목적은 골용해의 위험성이 상당한 정도로 제거되는 그러한 방식으로 청구항 1의 전제부에 따른 고관절 보철물을 개선시키는 데에 있다.
본 발명의 이러한 목적은 청구항 1의 특징부에 의해 본 발명에 따라 달성된다.
상부 및 하부로 구성된 중간체 요소의 결과로서, 상부 및 하부가 서로에 연결되는 것이 가능하며, 강(cavity) 삽입물이 상부 및 하부 사이에 보유되며, 중간체 요소의 착용이 기피되고, 결과적으로 골용해의 위험성이 감소된다.
바람직한 구체예에서, 강 삽입물은 중간체 요소, 즉 상부 및 하부에 의해 구형 리세스를 제외한 모든 면에 둘러싸여 마찰이 생길 수 있는 어떠한 여유 공간도 없게된다.
하부에 대한 상부의 연결은 대체적으로 다양한 방식으로 수행될 수 있다. 나사산에 의한 스크루 연결, 스냅 연결, 핀 연결, 접착 또는 용접은 유리한 것으로 입증되었다.
강 삽입물은 금속 또는 세라믹으로 생성될 수 있다. 본 발명은 모든 세라믹 강 삽입물에 적합하다.
유리한 방식으로, 회전을 방지하기 위해 삽입물이 위치한 반대편 개구부가 강 삽입물 및 인접한 하부에 제공된다. 이것에 대한 결과로서, 강 삽입물은 이의 위치에 고정되는 방식으로 중간체 요소에 놓여진다. 삽입물은 예를 들어 핀, 슬로팅 블록 및 피더-키(feather-key) 시스템, 또는 쐐기일 수 있으며, 예를 들어 금속 또는 플라스틱으로 제조될 수 있다. 개선된 고정화를 위해서, 삽입물은 개구부를 완전하게 채우는 것이 유리하다.

도 1은 골반 뼈의 관절강(도시되지 않음)에 이식될 수 있는 금속 보철물 셸(1)을 도시하는 단면도이다. 개선된 보유를 위해서, 리브 또는 나사산(10)이 보철물 셸(1)의 외부에 배열된다. 중간체 요소가 세라믹 강 삽입물(2)을 둘러싸고 있는 플라스틱으로 제조된 중간체 요소(4)가 보철물 셸(1)에 삽입된다.
중간체 요소(4)의 외부 윤곽은 보철물 셸(1)의 모양에 부합되며, 중간체 요소는 상부(5) 및 하부(6)로 구성된다.
하부(6)는 대퇴골을 면하며 강 삽입물(2)의 고정을 위한 리세스(11)를 갖는다. 이러한 연결에서, 강 삽입물(2)은 구형 리세스(3)를 제외하고는 중간체 요소(4)에 의해 완전하게 둘러싸인다. 상부(6)은 리세스(3)에 인접한 지지 표면(12)을 또한 둘러싸고 있다는 점이 특히 강조되어야 한다. 상부(5)는 강 삽입물(2)을 이들의 후면상에 둘러싸고 대체로 다양한 방식으로 하부(6)에 연결된다. 구형 리세스(3)는 대퇴부 요소의 헤드를 수용하는데 사용된다.

본 발명의 삽입물(16)을 사용하게 되면, 중간체 요소(4)내 강 삽입물(2)의 휨이 방지된다.
본 발명은 청구항 1의 전제부에 따른 고관절 보철물에 관한 것이다.

Claims (8)

  1. 구형 리세스(3)를 갖는 강 삽입물(2) 및 재질이 플라스틱이고 보철물 셸(1)과 강 삽입물(2) 사이에 배열된 중간체 요소를 구비한 금속 보철물 셸(1)을 갖는 고관철 보철물로서,
    중간체 요소(4)가 상부(5) 및 하부(6)로 구성되고,
    상부(5) 및 하부(6)가 서로 연결될 수 있으며,
    강 삽입물(2)이 상부(5)와 하부(6) 사이에 보유됨을 특징으로 하는 고관절 보철물.
  2. 제 1항에 있어서, 강 삽입물(2)이 구형 리세스(3)를 제외한 모든 면에서 중간체 요소(4)에 의해 둘러싸여 있음을 특징으로 하는 고관철 보철물.
  3. 제 1항 또는 제 2항에 있어서, 하부(6)에 대한 상부(5)의 연결이 나사산(7), 스냅 연결(8), 핀 연결(9), 접착 또는 용접에 의해 수행됨을 특징으로 하는 고관절 보철물.
  4. 제 1항 내지 제 3항 중의 어느 한 항에 있어서, 강 삽입물(2)이 세라믹 물질로 구성됨을 특징으로 하는 고관절 보철물.
  5. 제 1항 내지 제 4항 중의 어느 한 항에 있어서, 회전을 방지하기 위해 삽입물(16)이 위치한 반대편 개구부(15)가 강 삽입물 및 인접 하부(6)에 제공됨을 특징으로 하는 고관철 보철물.
  6. 제 5항에 있어서, 삽입물(16)이 핀(17), 슬로팅 블록(18) 또는 쐐기임을 특징으로 하는 고관절 보철물.
  7. 제 5항 또는 제 6항에 있어서, 삽입물(16)이 개구부(15)에 완전히 채워짐을 특징으로 하는 고관절 보철물.
  8. 제 5항 내지 제 7항 중의 어느 한 항에 있어서, 삽입물이 금속 또는 플라스틱으로 제조됨을 특징으로 하는 고관절 보철물.
