Abstract
본 발명은 치료를 요하는 환자의 이상운동질환 치료 방법으로서, 환자에게 유효 함량의 베피라돌 또는 이의 약학적으로 허용되는 유도체를 투여하는 단계를 포함하고, 현기증 및 구역질의 부작용이 최소화되도록 투여 약 4 시간 이후 환자의 베피라돌의 평균 최대 혈장 농도는 약 15 ng/mL 이하이다. 본 방법에 의해 사용 가능한 서방형 약학 조성물이 또한 기술된다.
Description
본 발명은 이상운동질환 치료 방법에 관한 것으로, 치료를 요하는 환자에게 유효 함량의 베피라돌을 투여하는 단계를 포함하고, 상기 투여로 평균 환자의 베피라돌의 최대 혈장 농도는 투여 약 4 시간 이후 약 15 ng/mL 이하가 되고, 상기 방법으로 현기증 및 구역질의 부작용이 최소화된다. 또한 본 발명에 의해 사용 가능한 서방형 약학 조성물이 기재된다.

베피라돌, [(3-클로로-4-플루오로-페닐)-[4-플루오로-4-{[(5-메틸-피리딘-2-일메틸)-아미노]-메틸}피페리딘-1-일]메타논]은, 선택적이고 고효능의 세로토닌 5-HT1A 수용체 작용제이다 (Colpaert et al. 2002 Neuropharmacology 43 : 945-958), Pierre Fabre Laboratories에서 발명 (US 6,020,345; US 7,208,603). 베피라돌의 구조식은 하기된다:

본 발명의 다른 목적은 베피라돌 방출을 조절하는 약학적으로 허용되는 부형제를 포함하는 베피라돌 또는 이의 약학적으로 허용되는 유도체의 서방형 약학 조성물이고, 상기 조성물은 환자에게 유효 함량의 베피라돌을 전달하고, 환자의 베피라돌의 평균 최대 혈장 농도는 약 15 ng/mL 이하이고, 이는 투여 약 4 시간 이후에 도달된다.

실시예 3 : 4종의 베피라돌 조성물의 약동학 및 내성 임상 연구
피험 조성물:
1. IR (즉시 방출) 조성물 : 0.65 mg 베피라돌 푸마레이트 염 (0.50 mg 베피라돌와 동등) 및 하나의 정제에서 충분 함량의 부형제들 (무수인산수소칼슘 (calcium phosphate dibasic anhydrous), 셀룰로오스 미정질, 무수 콜로이드 실리카 및 마그네슘 스테아레이트).

방법론:
I 상의 단일-센터, 연구는 개방 연구, 단일 투여, 불완전 블록법으로, 4종의 베피라돌 조성물을 18명의 건강한 남성 개체에 무작위 방식으로 진행되었다.
공급 조건에서 개체에 2종의 다른 베피라돌 제제를 투여하되 다음 6종의 치료 순서 중 하나를 무작위로 선정하였다: (IR; MR1), (MR2; IR), (IR; MR3), (MR1; MR2), (MR3; MR1), (MR2; MR3).
투여는 단일, 1mg (2 x 0.5 mg 캡슐 또는 정제)의 베피라돌을 오전에 경구 투여되었다.
각각의 시기에서 단일 용량은 모든 개체에 대하여 14일 간격으로 2회 투여되었다.
약동학 분석을 위한 혈액 샘플을 오전 약물 투여 전에 (시간 0), 이어 각각의 베피라돌 투여 후 0.5h, 0.75h, 1h, 1.5h, 2h, 3h, 4h, 5h, 6h, 7h, 9h, 12h, 24h, 48h, T72h, T96h, T120h, T144h에 채취하였다.
연구 과정에서 채취한 혈장 샘플에서 베피라돌을 유효 LC-MS/MS (이중 질량분석이 동반되는 액체크로마토그래피) 생체분석 방법으로 정량화하였다 (이하 상세한 설명 참고).
이들 혈장 농도를 이용하여 정성적 프로그램 (WinNonlin®, version 5.3)으로 비 구획 약동학 분석을 수행하였다. 베피라돌에 대한 약동학 인자들을 다음과 같이 결정하였다:
Cmax (최대 혈장 농도)는 실험 데이터에서 직접 추정하였다.
Tmax (최대 혈장 농도 도달 시간)은 실험 데이터에서 직접 추정하였다.
AUClast (0에서 최종 정량 지점까지 혈장 농도-시간 곡선 아래 관찰된 면적)은 선형 상향 (linear-up) / 로그-하향 (log-down) 사다리꼴 공식에 따라 계산하였다.
AUCinf (혈장 농도-시간 곡선 아래 총 면적)은 두 면적의 합, AUClast 및 AUCext (최종 정량 지점에서 무한까지의 외삽 면적; Clast/Kel의 비율로 추정, 식 중 Clast 는 Tlast 에서 관찰된 농도이고 Kel 는 종료 상 반응속도상수)으로 추정하였다.
T½ (실험 반감기)는 ln2/Kel로 계산하되, Kel (종료 상 반응속도상수)는 곡선의 종료 선형 지점에 대한 로그-농도 대 시간 데이터의 최소자승 회귀분석의 음의 기울기를 이용하여 추정된다. 최종 시점들에서 생긴 정량화 값들의 한계 이하는 분석에서 제외하였다.
Frel (각각의 MR 제제 대 IR 제제의 상대 생체이용률, %로 표기)을 다음과 같이 계산하였다:

LC-MS/MS 생체분석 방법:
샘플 처리
1mL ChemElut® 카트리지 (VARIAN)를 이용하여 고체상 추출에 의해 혈장 샘플 (500μL)을 제조하였다. 2 x 3mL 에틸 아세테이트로 용출하였다. 추출물을 질소 기류 중에 건조하였다. 마른 잔류물을 이동상에 용해시키고 LC/MS-MS 시스템에 주입하였다.
크로마토그래피
크로마토그래피 분리는 Chromolith Fast Gradient RP 18e, 50 x 2mm I.D. 컬럼에서 pH 3의 아세토니트릴 I 암모늄 아세테이트 (20/80, v/v)의 이동상으로 진행하였다. 샘플을 600°C로 설정된 Turbo V ESI 인터페이스에 주입하였다. 이중질량분석으로 체류 시간 700 ms에서 검출하였다.
교정 범위
응답은 베피라돌0.1 - 100 ng.mL-1 범위의 농도에서 선형적이었다.
방법론은 특정되고 정밀하고 정확한 것으로 입증되었다. 응답 대 농도 데이터를 최소자승 선형 회귀분석으로 1/X² 가중계수로 피팅하였다.
정량화 한계
베피라돌에 대하여 정량화 한계는 0.1 ng.mL-1 이었다.
결과:
총 18명의 건강한 남성 개체들이 참여하여 연구를 완료하였다. 평균 연령은 30.7 세 (22세에서 42 세), 평균 체중은 78.02 kg 및 평균 키는 180.1 cm이었다.
9 개체들에서 공급 조건으로 각각의 제제1 mg의 단일 경구 투여 후 베피라돌에 대한 평균 혈장 농도-시간 프로파일 (기하평균으로 표기)을 도 1에 표기한다.
각각의 제제에 대한 베피라돌의 주요 약동학 인자들은 기하평균 (기하 변동 계수 %) 및 [범위]로 표기하되, Tmax에 대하여는 중앙값 및 [범위]로 표기한다. 결과를 하기 표에 요약한다:

IR 제제와 비교할 때, 베피라돌의 약동학적 거동에서 관찰되는 모든 변경은 MR 제제 코팅제의 Surelease-clear E-7-19040®의 비율 증가와 관련된다. 모든 MR 제제는 "변형 방출" 제제 특성을 보였고 Cmax 감소와 관련된 Tmax 지연으로 느리고 지연된 흡수를 나타내고, 상대적 생체이용률 감소 및 약동학 인자들에서 개인 간 변산도 증가는 없었다.
5명 (5/18) 개체들 (2명은 IR 제제 및 3명은 MR1 제제)은 7종의 가벼운 약물 투여 후 발생된 이상반응들을 경험하였다.
각각의 개체는 가벼운 현기증의 1 내지 4-시간 반응 (episode)을 보고하였고, 이들 5 개체들 중 하나 (IR 제제)는 또한 두통 및 구역질을 보고하였다.
이들 이상반응은 베피라돌의 약리적 프로파일과 일치하였다.
MR2 및 MR3 제제에 대하여는 약물 투여 후 발생된 이상반응들이 발생되지 않았다.
결론:
본 연구는 각각 9.88% 및 14.49%의 Surelease-clear E-7-19040® 로 코팅된 베피라돌의 MR 제제 (MR2 및 MR3)를 투여하여, 평균 Cmax 를 약15 ng/mL 이하로 중앙값 Tmax를 약 4h 이상으로 제공하면, 평균 Cmax 가 약 15 ng/mL보다 크고, 중앙값 Tmax는 약 4 h보다 작고 따라서 시간에 대한 환자의 베피라돌의 혈장 농도는 기울기가 급한 베피라돌의 IR 및 MR1 (4.75%의 Surelease-clear E-7-19040®) 제제와 비교할 때 성공적으로 현기증 및 구역질의 부작용을 크게 최소화 할 수 있다는 것을 보였다.
