Abstract

본 발명은 척추에 이용할 수 있는 신체 내 융합 및 세팅 수단에 관한 것으로, 이와 같은 수단은 해면골(spongy bone)을 수용할 수 있는 적어도 한 개의 개방 케이지(cage)로 구성되고 디스크절제술동안에 두 개의 척추사이에 끼우도록 되어 있고, 신체 융합을 촉진하는 뼈 이식물 또는 물질을 장치할 수 있어 연관된 두 개의 척추사이에 융합을 촉진한다.
이와 같은 케이지는 FR 2 703 580; EP 493 698; EP 599 419; FR 2 124 815; EP 307 241; EP 615 428에서 공지되어 있다.
이와 같은 케이지는 일반적으로 난형(ovoid) 또는 평행육면체를 이루고 있고, 금속 또는 생체 적응성이 있는 물질로 만든 견고하고, 개방된 상단 및 바닥면을 통하여 해면골(spongy bone)을 수용할 수 있다.
이와 같은 케이지를 이용하면 양호한 결과를 얻을 수 있으나 일부 경우에 케이지의 임의 이동 또는 케이지의 부차적인 전치위험을 피하기 위해서 케이지가 움직이지 않도록 하는 것이 필수적인 경우가 있다.
EP 0 179 695에서는 해면골(spongy bone) 또는 뼈 대용물을 수용할 수 있도록 고안된 개방형 내부 케이지를 형성하는 이식물로 구성된 보철에 대해 공지하고 있다. 케이지는 케이지의 삽입면에 실제 수직인 면으로 이어지는 판에 고정되고, 두 개의 서로 인접한 척추를 고정시키기 위해 각각의 척추를 고정하는 나사가 통과하는 구멍이 각 측면에 있다. 실린더형의 이식물은 척추 부분의 일부분이 제거되는 두 개의 인접 척추에 삽입할 수 있도록 되어 있다. 이와 같은 보철은 큰 힘을 받게 될 고정 나사에 문제점을 가지고 있는데 나사가 힘을 받게되면 척추로부터 부분적으로 분리되게 되어 케이지가 상대적으로 이동하게 된다.

본 발명의 목적은 척추의 신체내 융합 및 세팅 수단을 제공하여 이와 같은 선행 기술의 단점을 해결하고자 하는데, 수단은
▶ 해면골(spongy bone) 또는 뼈 대용물을 수용하고 척추 절제술과정동안에 척추사이에 끼워넣도록 된 적어도 한 개의 개방 내부 케이지; 그리고
▶ 케이지의 양측에는 케이지의 삽입면에 수직인 면으로 이어지는 외부 판으로, 이 판에는 케이지에 의해 서로 고정될 두 개의 인접하는 척추를 고정시키는 고정 수단을 가진 것으로 구성된다.
본 발명에 따르면, 세팅 수단의 케이지에는 두 개의 인접 척추에 의해 한정되는 척추간 공간에 부합되는 정밀한 평행육면체 프로파일과 모양을 가진 전면, 후면, 측벽으로 구성된 것을 특징으로 한다.
금속 또는 생체적응성이 있는 플라스틱 물질로 만들어진 일반적인 모양이 평행육면체인 케이지는 견고하여 척추사이 공간에 적합하고, 상단 및 바닥면 또는 전방 구멍을 통하여 해면골(spongy bone) 또는 뼈 대체물질을 수용할 수 있다.
요구에 따라서는 본 발명의 수단은 일체형 케이지-판 유닛 또는 내부 케이지오 외부 판, 케이지에 판을 조립하는 수단으로 만들 수 있다.
본 발명은 또한 다음의 설명에서 나타나는 특징에 관계하나 본 발명은 단독으로 또는 임의의 다른 것과 복합하여 실행하여도 무방하다.
다음은 본 발명은 어떻게 실행하는지를 보여주어 본 발명의 이해를 돕기위함이나 본원 발명을 이에 국한시키고자 함은 아니다.
도 1에 나타낸 케이지(1)는 측벽(2, 3, 4)으로 된 단단한 평행육면체로 해면골(spongy bone) 또는 뼈 대용물을 수용하도록 고안되었고, 이의 상하측면(8, 9)은 두 개의 이어진 척추를 향하여 개방되어 있다.
전면 벽(5)과 후면 벽(3)은 적절한 척추간 공간을 유지할 수 있도록 하기 위해 결정된 높이를 가진다.
본 발명의 실시예에서, 케이지(1)의 측벽(2, 4)에는 이에 상응하는 유사한 모양으로 관통하는 큰 슬롯(10, 11)이 있다.
따라서, 케이지(1)에는 네 개 면이 개방되어 있다. 또한 케이지(1)의 전면 벽(5)에는 케이지(1)의 삽입면에 실제 수직으로 이어지는 외부 스트랩-형성 요소(12)(이하 "판"이라 칭함)를 가지며, 판의 양단에는 케이지(1)를 통하여 적어도 두 개의 인접하는 척추를 함께 연결하도록 하기 위해 척추에 고정되는 고정 수단(13, 14)이 있다.
원래, 외부 스트랩 또는 판(12)은 케이지-판 유닛으로 주조 또는 주형함으로써 내부 케이지(1)와 일체형으로 만들 수 있다.
그럼에도 불구하고, 본 실시예에서와 같이 판(12)과 내부 케이지(1)는 별도로 만들어 조립 수단을 이용하여 서로 고정시키는 것이 적절하다.
도 1의 예시를 참고로 하면, 케이지(1)에 판(12)을 조립하는 수단은 케이지(1)의 수직 전면 벽(5)에 형성된 실린더형 스터드(15)와 스터드(15)의 원형 헤드(16)아래에 형성된 원주 홈(17)으로 구성되고, 헤드(16)는 판(12)을 관통하는 열쇠구멍형상의 장방형 슬롯(19)의 넓은 부분(18)에 삽입하기에 적합하고, 홈(17)은 스터드(15)의 헤드(16)가 판(12)의 바깥쪽 면(21)에 닿을 때까지 열쇠구멍의 좁은 부분(20) 양측사이에 미끄러져 들어가 축방향으로 고정될 수 있는 크기를 가진다.
열쇠구멍에 있는 스터드의 연결은 스터드가 좁은 부분에 들어갈 수 있도록 고안된다.
원래, 전술한 케이지의 높이, 폭, 깊이 등을 다양하게 할 수 있다.
다음의 실시예에서 적절한 해부학적 모양을 가질 수 있다.
해면골(spongy bone) 또는 뼈 대용물을 척추사이에 위치한 케이지(1)의 앞 또는 뒤에 넣는다.
도 2에 나타낸 실시예는 선행 실시예와는 약간 상이한 것으로써, 케이지(1A)에 판(12A)을 조립하는 수단은 열장이음형 슬라이드방식으로 구성되는데, 장붓구멍-형성 부분(30)은 수직방향으로 슬라이드를 하기 위해 판(12A)의 내면(31)에 형성되고, 이에 상응하는 장부-형성 부분(32)은 케이지(1A)의 바깥쪽 전면(5A)에 형성되고, 역으로도 가능하다.
현재 실시예에서, 케이지(1A)에는 선행 실시예에 있는 측면 구멍을 가지지는 않는다.
케이지(1A)의 외측 전면(5A)과 이에 상응하는 판(12A)의 면(31)은 동일한 장방형 슬롯(33, 34)을 가지고 있어, 판(12A)이 케이지(1A)에 조립되었을 때, 슬롯(33, 34)이 일치되어, 케이지(1A)가 위치한 후에, 해면골(spongy bone)을 정면으로 삽입할 수 있도록 한다.
도 2의 실시예에서 케이지(1A)의 모양 및 특정 프로파일로 척추간 공간에 적절히 고정될 수 있게 된다.
도 3에 나타낸 실시예는 일체(one piece)로 만들어졌다는 것이 도 2의 실시예와 다른 점이다.
케이지-판 유닛의 전면(5A)에는 장방향 슬롯(33)이 있어서 케이지-판 유닛을 정치시킨 후에 케이지 부분으로 해면골(spongy bone)을 정면으로 삽입할 수 있다.
도 1, 2, 3의 실시예에 공통적으로, 케이지(1, 1A)를 고정시킨 후에, 척추에 판(12, 12A)을 고정시키는 고정 수단은 스트랩(12, 12A)의 단부를 관통하여 형성된 상응하는 구멍(13, 14)을 통과하는 페디큘라(pedicular) 나사(도시되지 않음)를 이용한다.
본 발명의 또 다른 특징은 판(12, 12A)의 고정 구멍(13, 14)은 대각선 Y, Y'에서 조립체의 중앙 수직 축 X, X'의 양측에 배치되어 있다는 것이다.
실시예에서(도시되지 않음), 세팅 수단은 두 개이상의 케이지(1)를 가지고 실행할 수 있는데, 두 개이상의 케이지는 서로 고정되고, 케이지(1)에 두 가지 조립 수단(15, 18 또는 30, 32)을 가지는 판(12, 12A), 척추에 고정하기 위한 세 개이상의 나사와 중간 고정 구멍등으로 연속하는 척추에 사용된다.
판은 한 척추당 하나의 고정 나사를 가지는 것으로 나타내었으나, 필요에 따라서는 둘 이상을 가질 수 있다.
원래, 세팅 수단은 도 1, 2, 3의 실시예에 나타낸 것과 같이 판을 선택적으로 겹쳐놓아 서로 쌍으로 연결된 두 개 이상의 케이지-판 유닛으로 구성될 수도 있다.
본 발명의 세팅수단은 티타늄 합금 또는 이의 등가의 물질 또는 생체적응성이 있는 플라스틱물질로 만들 수 있다.

Claims (9)

  1. 신체 내 척추 융합 및 세팅 수단에 있어서, 상기 수단이
    ▶ 해면골(spongy bone) 또는 뼈 대용물을 수용할 수 있는 하나이상의 개방 케이지(cage)(1A)로 구성되고 디스크절제술동안에 두 개의 척추사이에 끼워지고;
    ▶ 수단에는 케이지(1A)의 삽입면에 실제 수직인 면으로 이어지는 외부 판(12A)이 케이지의 양측면에 있고, 케이지(1A)에 의해 서로 고정될 둘 이상의 척추에 고정되는 고정 수단이 판의 양 단부에 배치되도록 구성되고,
    세팅 수단의 케이지(1A)는 두 인접 척추에 의해 한정되는 척추간 공간에 부합되는 정밀한 평행육면체 프로파일과 모양을 가진 전면 벽(5), 후면 벽(3), 측벽(2, 4)으로 구성되는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  2. 제 1 항에 있어서, 케이지(1A)의 전면 벽(5)에는 케이지의 전면으로 해면골(spongy bone)을 삽입할 수 있는 구멍(33)이 있는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  3. 제 1 항 또는 2항에 있어서, 케이지(1A)는 단부에 외부 판(12A)에 고정되는데, 판의 단부에는 정면 척추에 연결될 수 있도록 각 나사가 통과할 수 있는 각 한개 구멍으로 구성된 고정 수단이 있고, 각 구멍(13, 14)은 조립체의 중앙 수직 축(x-x')의 양측에서 대각선방향(y-y')으로 배치된 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  4. 제 1 항에 있어서, 외부 판(12A)은 내측 케이지(1A)와 일체로 만들 수 있는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  5. 제 1 항에 있어서, 외부 판(12)과 내측 케이지(1A)를 별도로 만들고 조립 수단에 의해 서로 고정시키는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  6. 제 5 항에 있어서, 케이지(1A)에 판(12A)을 조립하는 수단은 열장이음형 슬라이드방식으로 구성되고, 장붓구멍-형성 부분(30)은 수직방향으로 슬라이드를 하기 위해 판(12A)의 내면(31)에 형성되고, 이에 상응하는 장부-형성 부분(32)은 케이지(1A)의 바깥쪽 전면(5A)에 형성되고, 역으로도 가능한 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  7. 제 6 항에 있어서, 케이지(1A)의 외측 전면(5A)과 이에 상응하는 판(12A)의 면(31)은 동일한 장방형 슬롯(33, 34)을 가지고 있어, 판(12A)이 케이지(1A)에 조립되었을 때, 슬롯(33, 34)이 일치되어, 케이지(1A)가 위치한 후에, 해면골(spongy bone)을 정면으로 삽입할 수 있도록 하는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  8. 제 1 항에 있어서, 개방된 상측 및 바닥면(8, 9)에 추가하여, 내측 케이지(1)에는 해면골(spongy bone)을 삽입하기 위한 삽입 슬롯(10, 11)이 있는 측벽(2, 4)이 있는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
  9. 제 1항에 있어서, 세팅 수단은 둘 이상의 케이지(1A)를 가지고 실행할 수 있고, 둘 이상의 케이지가 서로 고정되며, 케이지(1A)에 이셈블리하기 위한 두 가지 조립 수단(30, 32)을 가지는 판(12, 12A), 두 개의 단부 고정 구멍(13, 14), 그리고 척추에 고정하기 위한 셋 이상의 나사와 중간 고정 구멍등으로 연속하는 척추에 사용하는 것을 특징으로 하는 신체 내 척추 융합 및 세팅 수단.
