Abstract
Description
一方で、呼気延長が無く呼吸数が多い呼吸パターン(以下、「rapid-shallow」ともいう)では、運動能力は保たれるも、頻呼吸になると息切れを呈した。近年、有効性が注目されるようになってきた吸気筋トレーニング(Inspiratory Muscle Training:IMT)の効果は必ずしも一定ではないが、本発明者は、rapid-shallowの呼吸パターンの場合には、IMTにより十分な吸息が得られれば、息切れ改善に繋がると仮説を立てた(同非特許文献1)。
しかしながら、有効性が注目されるようになってきた吸気筋トレーニング(IMT)の効果は必ずしも一定ではなく、また実際のCOPD患者に対する呼気筋トレーニング(EMT)の有用性について言及された報告はない。
1)ユーザに対する運動負荷心肺機能検査(CPET)による最大運動時の1回の呼吸時間に対する吸気もしくは呼気時間割合TEXと、検査による安静時の1回の呼吸時間に対する吸気もしくは呼気時間割合TREとに関する呼気情報を入力する呼気情報入力手段。
2)TEXとTREの差分を算出し、運動中に呼気延長を来すユーザか否かを判定するユーザ判定手段。
3)運動中に呼気延長を来すユーザに対して、最大呼気筋力値に基づき、呼気圧負荷トレーニングのメニューを提示するメニュー提示手段。
4)呼気圧負荷トレーニングを所定回数行った後、上記1)の呼気情報および運動能力に関連する情報を入力して、対象ユーザの運動能力の改善効果の有無を判別してトレーニング結果を提示するトレーニング結果提示手段。
ここで、運動負荷心肺機能検査(Cardiopulmonary Exercise Test:CPET)とは、CPXとも呼ばれる検査であり、トレッドミルやサイクルエルゴメータ(自転車)などの運動負荷デバイスを用いて運動負荷試験を行うと共に、呼気ガス分析装置による呼気中のO2濃度、CO2濃度とその換気量をリアルタイムに計測して、運動能力の指標となる最大酸素摂取量などの指標を測定する検査である。CPETは、患者それぞれの運動病態生理を把握するのに有用な検査であり、1回1回の呼吸において、吸気1回換気量と呼気1回換気量を測定することが可能である。
ユーザ判定手段は、TEXとTREの差分を算出し、例えば、TEXとTREを吸気時間の割合として用いた場合、TEX−TREの値を算出し、その差分値が負の値であるか否かを判定し、負の値であるユーザを運動中に呼気延長を来たすユーザと判断する。そして、メニュー提示手段は、運動中に呼気延長を来たすと判断されたユーザに対しては、呼気圧負荷トレーニングのメニューを提供し、呼気圧負荷トレーニングの実施を促す。
呼気圧負荷デバイスとしては、例えば、EMST150(Aspire Products LLC社製)を用いることができる。現状、EMST150は日本では販売されておらず、海外のみの販売である。
呼吸圧負荷トレーニングは、高齢者であっても、自宅で簡単に実施できることから、個々のユーザに適した負荷圧でトレーニングさせて、トレーニングによる呼吸筋疲労からの筋肉痛や息切れの増悪を起こすことを最小限に抑え、ユーザにとって過度の負担が生じないように継続させる。
これにより、病態生理によりEMTとIMTを選択するテーラメイド化した呼吸圧負荷トレーニングの実現を図ることができる。
本発明の呼気圧負荷トレーニング管理システムは、上述した本発明の呼気圧負荷トレーニング管理装置が、ネットワークを介して接続されるユーザ端末と管理サーバから構成されている。ユーザ端末と管理サーバは、呼気圧負荷トレーニング管理装置の機能を分担し、各々ネットワークで接続される。多数のユーザは、自身が扱うユーザ端末を介して、自身の呼気情報および運動能力に関連する情報を入力し、テーラメイドされた呼気圧負荷トレーニングのメニューを提示され、トレーニング後の呼気情報を入力してトレーニング結果を提示される。
以下、ユーザ端末と管理サーバが備える機能について説明する。
1a)最大呼気筋力を含む呼気情報入力手段。
1b)入力された前述の呼気情報を管理サーバへ送信する手段。
1c)管理サーバから受信した呼気圧負荷トレーニングのメニューを提示するメニュー提示手段。
1d)所定回数の呼気圧負荷トレーニング後の呼気情報および運動能力に関連する情報を入力して管理サーバへ送信する手段。
1e)管理サーバから受信したトレーニング結果を提示するトレーニング結果提示手段。
2a)上記の呼気情報をユーザ端末から受信する手段。
2b)受信した呼気情報に基づいて、差分を算出した結果、運動中の呼気延長があるユーザか否かを判定するユーザ判定手段。
2c)呼気圧負荷トレーニングのメニューを作成してユーザ端末に送信するメニュー作成手段。
2d)所定回数の呼気圧負荷トレーニング後の呼気情報および運動能力に関連する情報をユーザ端末から受信し、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を作成してユーザ端末に送信するトレーニング結果作成手段。
本発明の呼気圧負荷トレーニング管理方法は、COPDを有するユーザのトレーニング管理方法であって、以下のa)〜d)のステップを備える。
a)ユーザに対するCPETによる最大運動時の1回の呼吸時間に対する吸気もしくは呼気の時間割合TEXと、CPETによる安静時の1回の呼吸時間に対する吸気もしくは呼気の時間割合TREとに関する呼気情報を入力する呼気情報入力ステップ。
b)TEXとTREの差分を算出し、運動中に呼気延長を来すユーザか否かを判定するユーザ判定ステップ。
c)運動中に呼気延長を来すと判定したユーザに対して、最大呼気筋力値に基づき、呼気圧負荷トレーニングのメニューを提示するメニュー提示ステップ。
d)呼気圧負荷トレーニングを所定回数行った後の上記a)の呼気情報および運動能力に関連する情報を入力して、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を提示するトレーニング結果提示ステップ。
本発明の呼気圧負荷トレーニング管理プログラムは、コンピュータを、前述の呼気圧負荷トレーニング管理システムにおけるユーザ端末が備える全ての手段として、機能させるためのプログラムである。
本発明の呼気圧負荷トレーニング管理プログラムは、他の観点によれば、コンピュータを、前述の呼気圧負荷トレーニング管理システムにおける管理サーバが備える全ての手段として、機能させるためのプログラムである。
本発明の呼気圧負荷トレーニング管理プログラムは、他の観点によれば、コンピュータに、前述の呼気圧負荷トレーニング管理方法における呼気情報入力ステップ、ユーザ判定ステップ、メニュー提示ステップ、及び、トレーニング結果提示ステップを、実行させるためのプログラムである。
図1は、実施例1の呼気圧負荷トレーニング管理装置の機能ブロック図を示している。図1に示すように、呼気圧負荷トレーニング管理装置1は、呼気情報入力手段11、ユーザ判定手段12、メニュー提示手段13及びトレーニング結果提示手段14から成る。呼気情報入力手段11は、ユーザに対する運動負荷心肺機能検査(CPET)による最大運動時の1回の呼吸時間に対する吸気もしくは呼気時間の割合TEXと、検査による安静時の1回の呼吸時間に対する吸気もしくは呼気時間の割合TREとに関する呼気情報を入力するものである。ユーザ判定手段12は、TEXとTREの差分を算出し、運動中の呼気延長があるユーザか否かを判定するものである。メニュー提示手段13は、差分を算出した結果、運動中の呼気延長があると判定されたユーザに対して、最大呼気筋力値を踏まえ、呼気圧負荷トレーニングのメニューを提示するものである。また、トレーニング結果提示手段14は、所定回数のトレーニング後の呼気情報および運動能力に関連する情報を入力して、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を提示するものである。
差分を算出した結果、運動中の呼気延長がないと判定されたユーザに対しては、最大呼気筋力値を踏まえ、吸気圧負荷トレーニングのメニューを提示する(ステップS05:メニュー提示ステップ)。トレーニングを所定回数行った後、呼気情報および運動能力に関連する情報を入力して、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を提示する(ステップS06:トレーニング結果提示ステップ)。
図3は、呼気圧負荷トレーニング管理システムの機能ブロック図を示している。図3に示すように、呼気圧負荷トレーニング管理システム100は、ユーザ端末2及び管理サーバ3で構成され、ユーザ端末2には、呼気情報入力手段11、メニュー提示手段13、トレーニング結果提示手段14、送信手段17a及び受信手段18aが設けられ、管理サーバ3には、ユーザ判定手段12、メニュー作成手段15、トレーニング結果作成手段16、送信手段17b及び受信手段18bが設けられている。
ユーザ端末2は、送信手段17aにより、所定回数のトレーニング後の呼気情報および運動能力に関連する情報を入力して管理サーバ3へ送信する。管理サーバ3は、受信手段18bにより、呼気情報および運動能力に関連する情報をユーザ端末2から受信する。管理サーバ3は、トレーニング結果作成手段16により、所定回数のトレーニング後の呼気情報および運動能力に関連する情報を基に、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を作成し、送信手段17bによりユーザ端末2へ送信し、ユーザ端末2に設けられた受信手段18aにおいて受信する。ユーザ端末2は、トレーニング結果提示手段14により、管理サーバ3から受信したトレーニング結果を提示する。
運動負荷心肺機能検査装置4には、図示しないが、運動負荷デバイスや呼気ガス分析装置などが設けられており、運動中における呼気中のO2濃度、CO2濃度とその換気量をリアルタイムに計測して、運動能力の指標となる最大酸素摂取量などの指標を測定する検査が可能である。
本実施例では、運動負荷心肺機能検査装置4とスマートフォン2aは無線通信手段6aにより通信し、運動負荷心肺機能検査装置4とPC2bは有線通信手段6bにより通信する構成となっているが、通信方法はこの限りではなく、例えば、運動負荷心肺機能検査装置4とスマートフォン2aが有線通信手段6bにより通信し、運動負荷心肺機能検査装置4とPC2bが無線通信手段6aにより通信する構成でもよい。
また、無線通信手段6aや有線通信手段6bを用いずに、例えば、運動負荷心肺機能検査装置4に設けられたディスプレイ(図示せず)に表示された数値等を確認して、スマートフォン2aやPC2bに入力することでもよい。
図5(1)に示すとおり、運動中に呼気延長を来す、即ち、差分T(最大運動時−安静時)が負の値になるユーザ(T低下群)は呼気圧負荷トレーニング対象者とし、一方、呼気延長を来さない、即ち、差分T(最大運動時−安静時)が正の値になる患者(T増加群)は吸気圧負荷トレーニング対象者とした。
図5(2)に示すとおり、呼気圧負荷トレーニング対象者は、トレーニングの介入前に行ったCPETでは、最大運動時の呼気のはき残し量、即ち、吸気1回換気量と呼気1回換気量の差(吸気VTin−呼気VTex)が吸気圧負荷トレーニング対象者より有意に多かった。つまり、呼気圧負荷トレーニング対象者(T低下群)は、1回1回の呼吸で空気を完全に吐ききることが出来ず、肺に空気が溜まりそれが息切れの原因となったことが示唆され、この群ではいかに空気をはき出せるかが重要であることがわかる。従って、呼気圧負荷トレーニング対象者(T低下群)には、呼気圧負荷トレーニングを行い、十分な呼気、およびそれに伴い呼気延長改善が得られれば、運動能力の改善に繋がる可能性があるといえる。一方、吸気圧負荷トレーニング対象者(T増加群)には、運動中、肺に溜まった空気を吐き出す能力があるため、吸気能力を高め吸気量を増しても呼気への負担は少ないと考えられ、吸気圧負荷トレーニングにより十分な換気量が得られれば運動能力が改善する可能性があるといえる。
図6に示す呼気圧負荷トレーニング後の持続運動の改善結果は、トレーニング前の平均運動時間が454秒(標準偏差211秒)であったのが、トレーニング後の平均運動時間が764秒(標準偏差313秒)となり、持続運動時間の著明な改善が確認された。また、図7に示す吸気圧負荷トレーニング後の持続運動の改善結果は、トレーニング前の平均運動時間が458秒(標準偏差203秒)であったのが、トレーニング後の平均運動時間が799秒(標準偏差495秒)となり、こちらも持続運動時間の著明な改善が確認された。特に、呼気圧負荷トレーニングを受けたユーザは、すでに十分な吸入薬治療が導入され、これまでに呼吸リハビリテーションを受けたユーザも多かったにも関わらず、著明な改善が得られたことは興味深い結果であった。
実際のCOPD患者の症例について説明する。患者は、64歳の男性で、身長が174cm、体重が69kgであり、既往歴(喘息など)は無い。
病歴としては、COPDとして、長時間作用性抗コリン薬、長時間作用性β刺激薬、吸入ステロイドの吸入薬3剤による治療が行われるも、労作時の息切れのため、平地歩行は、せいぜい200m程度であった。呼吸リハビリテーションなどの非薬物療法を上乗せするも効果は不十分であった。そのため、呼気圧負荷トレーニングを含む臨床研究に参加頂いた。この患者は、呼吸圧負荷トレーニングの前評価では、前述の吸入薬3剤の吸入下で肺機能は著明に低下しており、1秒量(FEV1)、0.89L、%FEV1,26.9%と、COPDの病期はIV期:最重症(病期分類はI〜IVの4つに分類)と診断された。また、この患者は、運動中、呼気延長の程度により呼気・吸気圧負荷トレーニングを行うかを判断する臨床研究に参加され、エルゴメータによるCPETにより、運動中の1回の呼吸時間に対する吸気時間の割合を確認すると、運動するにつれて、吸気時間の割合が低下(呼気時間が延長)したため、呼息が十分でないことが息切れに関連すると推察し、呼気圧負荷トレーニングメニューを提示した。
胸部レントゲンを用いての検査所見では、横隔膜は平定化し、呼息が不十分で空気が肺に鬱滞し無効換気に陥っている可能性が示唆されていた。
30回の呼気圧負荷トレーニングを1日2回(朝、夕)行った。初回は、最大呼気筋力の20%から開始し、2週毎に5%ずつアップし、3ヶ月をかけて50%に到達することを目標にした。本症例は50%の目標に到達しトレーニングを終了した。
結果は次のとおりである。肺機能検査において、3ヶ月の呼気圧負荷トレーニング後、下記表1に示すように若干の改善が得られた。なお、表1において、FEV1は1秒量、VCは肺活量を示す。
図8に示すように、呼気圧負荷トレーニング後、定常負荷において呼気の1回換気量は低下するも、吸気1回換気量−呼気1回換気量が低下、即ち、十分な呼息が可能になり、1回の呼吸時間に対する吸気時間の割合Tは、運動中に低下することなく、つまり、呼気時間が延長することなく運動が終了した。その結果、運動中の息切れの程度を表すボルグスケールの傾きは緩やかとなり、運動時間の延長は約9分と驚くべき効果が得られたのである。なお、ボルグスケールは0〜10段階で、息切れの程度を表すものであり、運動能力を表す最高酸素摂取量は、分時換気量を用いて算出される。ここで、分時換気量は、1回換気量と呼吸数の積で算出される。

Claims (15)
- 慢性閉塞性肺疾患(COPD)を有するユーザのトレーニング管理装置であって、
ユーザに対する運動負荷心肺機能検査(CPET)による最大運動時の1回の呼吸時間に対する吸気もしくは呼気の時間割合TEXと、前記検査による安静時の1回の呼吸時間に対する吸気もしくは呼気の時間割合TREとに関する呼気情報を入力する呼気情報入力手段と、
TEXとTREの差分を算出し、運動中の呼気延長があるユーザか否かを判定するユーザ判定手段と、
運動中の呼気延長があると判定されたユーザに対して、最大呼気筋力値に基づき、呼気圧負荷トレーニングのメニューを提示するメニュー提示手段と、
所定回数の前記トレーニング後の前記呼気情報および運動能力に関連する情報を入力して、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を提示するトレーニング結果提示手段、
を備えたことを特徴とする呼気圧負荷トレーニング管理装置。 - 前記呼気圧負荷トレーニングは、一方向弁付きの呼気圧負荷デバイスを用い、
前記デバイスは、設定圧力に達すると前記弁が開いて呼気が流入することを特徴とする請求項1に記載の呼気圧負荷トレーニング管理装置。 - 前記設定圧力は、30cmH2O以下の設定幅において設定可能であり、ユーザの前記最大呼気筋力の20〜30%の呼気圧負荷からトレーニングを開始し段階的に増加し得ることを特徴とする請求項2に記載の呼気圧負荷トレーニング管理装置。
- 前記ユーザ判定手段において、前記差分を算出した結果、運動中の呼気延長がない場合には、
前記メニュー提示手段において、吸気圧負荷トレーニングのメニューを提示することを特徴とする請求項1〜3の何れかに記載の呼気圧負荷トレーニング管理装置。 - 前記メニュー提示手段における前記メニューは、呼吸器専門医により、確認され承認されたものであることを特徴とする請求項1〜4の何れかに記載の呼気圧負荷トレーニング管理装置。
- 請求項1〜5の何れかの呼気圧負荷トレーニング管理装置が、ネットワークを介して接続されるユーザ端末と管理サーバから構成されており、

前記呼気情報入力手段と、
入力された前記呼気情報を管理サーバへ送信する手段と、
管理サーバから受信した前記呼気圧負荷トレーニングのメニューを提示するメニュー提示手段と、
所定回数の前記トレーニング後の前記呼気情報および運動能力に関連する情報を入力して管理サーバへ送信する手段と、
管理サーバから受信した前記トレーニング結果を提示するトレーニング結果提示手段、
を備え、
管理サーバは、
前記呼気情報および運動能力に関連する情報をユーザ端末から受信する手段と、
受信した前記呼気情報に基づいて、前記差分を算出した結果、運動中の呼気延長があるユーザか否かを判定する前記ユーザ判定手段と、
前記呼気圧負荷トレーニングのメニューを作成してユーザ端末に送信するメニュー作成手段と、
所定回数の前記トレーニング後の前記呼気情報および前記運動能力に関連する情報をユーザ端末から受信し、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を作成してユーザ端末に送信するトレーニング結果作成手段、
を備えたことを特徴とする呼気圧負荷トレーニング管理システム。 - 前記ユーザ端末は、患者ユーザの自宅内、トレーニング施設内、又は、医療クリニック施設内の何れかに設置されることを特徴とする請求項6に記載の呼気圧負荷トレーニング管理システム。
- 慢性閉塞性肺疾患(COPD)を有するユーザのトレーニング管理方法であって、
ユーザに対する運動負荷心肺機能検査(CPET)による最大運動時の1回の呼吸時間に対する吸気もしくは呼気の時間割合TEXと、前記検査による安静時の1回の呼吸時間に対する吸気もしくは呼気の時間割合TREとに関する呼気情報を入力する呼気情報入力ステップと、
TEXとTREの差分を算出し、運動中の呼気延長があるユーザか否かを判定するユーザ判定ステップと、
運動中の呼気延長があると判定されたユーザに対して、最大呼気筋力値に基づき、呼気圧負荷トレーニングのメニューを提示するメニュー提示ステップと、
前記トレーニングを所定回数行った後、前記呼気情報および運動能力に関連する情報を入力して、該ユーザの運動能力の改善効果の有無を判別してトレーニング結果を提示するトレーニング結果提示ステップ、
を備えたことを特徴とする呼気圧負荷トレーニング管理方法。 - 前記呼気圧負荷トレーニングは、一方向弁付きの呼気圧負荷デバイスを用い、

- 請求項6の呼気圧負荷トレーニング管理システムにおける前記ユーザ端末が備える全ての手段として、コンピュータを機能させるためのプログラム。
- 請求項6の呼気圧負荷トレーニング管理システムにおける前記管理サーバが備える全ての手段として、コンピュータを機能させるためのプログラム。
- 請求項8〜12の何れかの呼気圧負荷トレーニング管理方法における前記呼気情報入力ステップ、前記ユーザ判定ステップ、前記メニュー提示ステップ、及び、前記トレーニング結果提示ステップを、コンピュータに実行させるためのプログラム。
