Abstract
Description
[1]自己免疫疾患の診断方法であって、
被験者から採取された糞便試料に含まれる細菌の相対存在量を測定するステップと、
以下の(1)又は(2)を実施するステップと、を備える、診断方法。
(1)16SリボソームRNA遺伝子の塩基配列が、配列番号3又は4で特定される塩基配列と99%以上の同一性を有する細菌の相対存在量が、健常人における当該相対存在量に比較して多い場合、前記被験者は自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する
(2)16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の同一性を有する細菌の相対存在量が、健常人における当該相対存在量に比較して少ない場合、前記被験者は自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する
[2]自己免疫疾患の診断方法であって、
治療前及び治療後に被験者から採取された糞便試料に含まれる細菌の相対存在量を測定するステップと、
以下の(3)又は(4)を実施するステップと、を備える、診断方法。
(3)16SリボソームRNA遺伝子の塩基配列が、配列番号3又は4で特定される塩基配列と99%以上の同一性を有する細菌の相対存在量を治療前後で比較して、治療後の当該相対存在量が治療前に比較して少ない場合、前記被験者の自己免疫疾患の病態は治療により改善されたと判定する
(4)16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の同一性を有する細菌の相対存在量を治療前後で比較して、治療後の当該相対存在量が治療前に比較して多い場合、前記被験者の自己免疫疾患の病態は治療により改善されたと判定する
[3]細菌の相対存在量の測定が、糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読することを含む、[1]又は[2]に記載の診断方法。
[4]前記自己免疫疾患が、多発性硬化症である、[1]〜[3]のいずれかに記載の診断方法。
[5]前記多発性硬化症が、再発寛解型の多発性硬化症である、[4]に記載の診断方法。
[6]16SリボソームRNA遺伝子の塩基配列が、配列番号3〜23で特定される塩基配列のいずれかと99%以上の相同性を有する腸内細菌からなる、自己免疫疾患の診断用バイオマーカー。
[7]16SリボソームRNA遺伝子の塩基配列が、配列番号3〜23で特定される塩基配列のいずれかと99%以上の相同性を有する腸内細菌の、自己免疫疾患の診断用バイオマーカーとしての使用。
[8]16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の相同性を有する細菌、及び前記細菌に由来する生理活性物質からなる群より選択される少なくとも1種を有効成分として含有する、自己免疫疾患の予防又は治療剤。
[2−1]被験者から採取された糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読した塩基配列データを取得するステップ、取得した塩基配列データから、配列番号3〜23のいずれかで特定される塩基配列と99%以上の相同性を有する塩基配列の頻度を計算し、当該塩基配列の相対存在量を算出するステップ、算出した相対存在量を予め入力された基準値と比較して自己免疫疾患の病態を判定するステップ、及び得られた判定結果を出力するステップをコンピュータに実行させるプログラムが記録されたコンピュータ読み取り可能な非一時的記録媒体。
[2−2]被験者から採取された糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読した塩基配列データを取得するステップ、取得した塩基配列データから、配列番号3〜23のいずれかで特定される塩基配列と99%以上の相同性を有する塩基配列の頻度を計算し、当該塩基配列の相対存在量を算出するステップ、算出した相対存在量を予め入力された健常人における相対存在量と比較するステップ、比較結果に基づいて、被験者が、自己免疫疾患に罹患している、又は罹患する危険性が高いかを判定するステップ、及び得られた判定結果を出力するステップをコンピュータに実行させるプログラムが記録されたコンピュータ読み取り可能な非一時的記録媒体であって、上記判定するステップは、上記相対存在量が、配列番号3又は4で特定される塩基配列と99%以上の相同性を有する塩基配列の相対存在量であり、かつ算出した相対存在量が健常人における当該相対存在量に比較して多い場合、又は、上記相対存在量が、配列番号5〜23で特定される塩基配列と99%以上の相同性を有する塩基配列の相対存在量であり、かつ算出した相対存在量が健常人における当該相対存在量に比較して少ない場合、当該被験者は、自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する、非一時的記録媒体。
[2−3]治療後に被験者から採取された糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読した塩基配列データを取得するステップ、取得した塩基配列データから、配列番号3〜23のいずれかで特定される塩基配列と99%以上の相同性を有する塩基配列の頻度を計算し、当該塩基配列の相対存在量を算出するステップ、算出した相対存在量を予め入力された治療前の被験者における相対存在量と比較するステップ、比較結果に基づいて、被験者の自己免疫疾患の病態は治療により改善されたかを判定するステップ、及び得られた判定結果を出力するステップをコンピュータに実行させるプログラムが記録されたコンピュータ読み取り可能な非一時的記録媒体であって、上記判定するステップは、上記相対存在量が、配列番号3又は4で特定される塩基配列と99%以上の相同性を有する塩基配列の相対存在量であり、かつ算出した相対存在量が治療前の被験者における当該相対存在量に比較して多い場合、又は、上記相対存在量が、配列番号5〜23で特定される塩基配列と99%以上の相同性を有する塩基配列の相対存在量であり、かつ算出した相対存在量が治療前の被験者における当該相対存在量に比較して少ない場合、当該被験者の自己免疫疾患の病態は治療により改善されたと判定する、非一時的記録媒体。
[2−4]被験者から採取された糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読した塩基配列データを取得する入力手段と、取得した塩基配列データに基づいて、被験者が自己免疫疾患へ罹患している、又は罹患する危険性が高いことを判定する演算手段と、上記演算手段で得られた判定結果を出力する出力手段を備える、自己免疫疾患の診断システム。
[2−5]治療後に被験者から採取された糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読した塩基配列データを取得する入力手段と、取得した塩基配列データに基づいて、被験者の自己免疫疾患の病態は治療により改善されたかを判定する演算手段と、上記演算手段で得られた判定結果を出力する出力手段を備える、自己免疫疾患の診断システム。
本実施形態に係る自己免疫疾患の診断方法は、被験者の腸内細菌叢の構成に基づく判定を行うものであるため、例えば、自己免疫疾患への罹患の有無又は罹患の危険性を判定すること(第1の実施形態)、及び自己免疫疾患の治療効果を判定すること(第2の実施形態)に用いることができる。
(1)16SリボソームRNA遺伝子の塩基配列が、配列番号3又は4で特定される塩基配列と99%以上の同一性を有する細菌の相対存在量が、健常人における当該相対存在量に比較して多い場合、前記被験者は自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する。
(2)16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の同一性を有する細菌の相対存在量が、健常人における当該相対存在量に比較して少ない場合、前記被験者は自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する。
(3)16SリボソームRNA遺伝子の塩基配列が、配列番号3又は4で特定される塩基配列と99%以上の同一性を有する細菌の相対存在量を治療前後で比較して、治療後の当該相対存在量が治療前に比較して少ない場合、前記被験者の自己免疫疾患の病態は治療により改善されたと判定する。
(4)16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の同一性を有する細菌の相対存在量を治療前後で比較して、治療後の当該相対存在量が治療前に比較して多い場合、前記被験者の自己免疫疾患の病態は治療により改善されたと判定する。
上述した本発明に係る診断方法は、コンピュータを自己免疫疾患の診断システムとして機能させるプログラムとして提供することもできる。
本実施形態に係る自己免疫疾患の診断用バイオマーカーは、16SリボソームRNA遺伝子の塩基配列が、配列番号3〜23で特定される塩基配列のいずれかと99%以上の相同性を有する腸内細菌からなる。
本実施形態に係る自己免疫疾患の予防又は治療剤は、16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の相同性を有する細菌、及び前記細菌に由来する生理活性物質からなる群より選択される少なくとも1種を有効成分として含有する。
[1.対象]
20名のMS患者(平均年齢:36.0±7.2歳、男性6名、女性14名)と、50名の健康対照(HC)(平均年齢:27.2±9.2歳、男性23名、女性27名)を対象とした。McDonaldの診断基準にしたがって診断した結果、全てのMS患者が再発寛解型MS(RRMS)であった。また、全てのMS患者は、一次進行型MS、二次進行型MS、他の疾患を併発していなかった。MS患者及び健康対照を含む全ての対象はいずれも、糞便試料を回収する間、抗生物質を投与されることはなかった。なお、本試験は、精神・神経医療研究センター、順天堂大学附属病院、麻布大学附属病院及び東京大学附属病院の各ヒト研究倫理委員会に認可されたプロトコルで実施された。全ての対象から事前にインフォームドコンセントを得た。
対象から回収した糞便は、直ちに使い捨ての酸素吸収剤及び二酸化炭素発生剤を含有するプラスチックバッグ(プラスチックバッグ内は、酸素感受性嫌気性細菌が生存可能な環境である。)に入れて、4℃の温度を維持しながら、実験室へ運搬した。実験室において、20%グリセロールを含有するリン酸緩衝生理食塩水に糞便を懸濁させ、直ちに液体窒素で凍結させ、使用するまで−80℃で保管した。
16S rRNA遺伝子のV1−V2領域を、フォワードプライマー27Fmod(バーコード配列を含む、配列番号1:5’−agrgtttgatymtggctcag−3’)及びリバースプライマー338R(配列番号2:5’−tgctgcctcccgtaggagt−3’)を用いて、PCRで増幅した。250μM dNTPs及び1U Ex Taq ポリメラーゼ(タカラバイオ社製)の存在下、10mM Tris−HCl(pH8.3)、50mM KCl、1.5mM MgCl2、フォワードプライマー(0.2μM)、リバースプライマー(0.2μM)及び鋳型DNA(<20ng)を含有する1×Ex Taq PCRバッファー(50μL)を用いて、PCRを実施した。PCRは、9700 PCR System(ライフテクノロジージャパン社製)を用いて、初期変性(96℃、2分間)を行った後、変性(96℃、30秒間)、アニーリング(55℃、45秒間)及び伸長(72℃、1分間)を25サイクル繰り返し、最終伸長(72℃、1分間)で実施した。
16S rRNA遺伝子の全長配列データベースを、RDP(http://rdp.cme.msu.edu/)、CORE(http://microbiome.osu.edu./)及びNCBI(http://www.ncbi.nlm.nih.gov/)のデータベースに登録されている全長16S rRNA遺伝子(FL−16S)の塩基配列から構築した。
(1)帰属分類群の推定
16S rRNA遺伝子のV1−V2領域の塩基配列の読み取りデータ(リード)の確立された解析パイプライン(DNA Res.,2013年,20巻,pp.241−253;DNA Res.,2014年,21巻,pp.15−25参照)を使用して、各試料の細菌叢を解析した。
UniFrac解析により腸内細菌叢の全体構成の違いを解析した。Chao1推定量に基づく各試料のOTUの豊富さは、R(version 2.15.2)に実装されたVegan package(v.2.0−5)を使用して計算した。
全ての統計解析には、R(version 2.15.2)を使用した。細菌種の豊富さ、均等度及び多様性は、R Vegan packageを使用して評価した。統計検定は、ウェルチのt検定により行った。また、Benjamini−Hochberg法により多重検定のp値を補正した。系統樹は、近隣結合法により作製した。系統樹中の各節の長さは、ブートストラップ法(1000反復)で評価した確率を示す(図10左上に示した「−」の長さが確率0.01に相当する)。
[1.16Sリードの帰属]
454 GS FLX Titaniumを使用して、MS20群及びHC40群の糞便試料から、それぞれ141,549リード(1試料あたり7,080±825リード)及び303,585リード(1試料あたり7,590±616リード)の高品質16Sリードを得た(表3及び表4参照)。
図1(a)は、MS20群及びHC40群のOTU及びクラスタ数の平均値を示す図である。縦軸はOTU及びクラスタの数を示す。図1(b)は、MS20群及びHC40群のOTU及びクラスタ数のChao1推定値を示す図である。縦軸はOTU及びクラスタの数を示す。図1(c)は、MS20群及びHC40群のShannon値を示す図である。縦軸はShannon値を示す。
次に、MS20群とHC40群との間で腸内細菌叢における相対存在量に差のある細菌種を同定するために、種々の分類レベルで細菌種組成を分析した。
同定された21種の細菌種の相対存在量の有意差を評価するために、HC18群について、2週間毎に9回回収した糞便試料を用いて、長期に亘る各細菌種の相対存在量の変化を分析した(以下、長期HC18群という。)。図12は、MS20群と長期HC18群との間での、細菌の相対存在量の差(Log10(MS20群の平均リード数/長期HC18群の平均リード数))を示す図である。図12の縦軸は、細菌の相対存在量の差を示す。
Claims (8)
- 自己免疫疾患の診断方法であって、
被験者から採取された糞便試料に含まれる細菌の相対存在量を測定するステップと、
以下の(1)又は(2)を実施するステップと、を備える、診断方法。
(1)16SリボソームRNA遺伝子の塩基配列が、配列番号3又は4で特定される塩基配列と99%以上の同一性を有する細菌の相対存在量が、健常人における当該相対存在量に比較して多い場合、前記被験者は自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する
(2)16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の同一性を有する細菌の相対存在量が、健常人における当該相対存在量に比較して少ない場合、前記被験者は自己免疫疾患に罹患している、又は罹患する危険性が高いと判定する - 自己免疫疾患の診断方法であって、
治療前及び治療後に被験者から採取された糞便試料に含まれる細菌の相対存在量を測定するステップと、
以下の(3)又は(4)を実施するステップと、を備える、診断方法。
(3)16SリボソームRNA遺伝子の塩基配列が、配列番号3又は4で特定される塩基配列と99%以上の同一性を有する細菌の相対存在量を治療前後で比較して、治療後の当該相対存在量が治療前に比較して少ない場合、前記被験者の自己免疫疾患の病態は治療により改善されたと判定する
(4)16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の同一性を有する細菌の相対存在量を治療前後で比較して、治療後の当該相対存在量が治療前に比較して多い場合、前記被験者の自己免疫疾患の病態は治療により改善されたと判定する - 細菌の相対存在量の測定が、糞便試料に含まれる細菌の16SリボソームRNA遺伝子の塩基配列を網羅的に解読することを含む、請求項1又は2に記載の診断方法。
- 前記自己免疫疾患が、多発性硬化症である、請求項1〜3のいずれか一項に記載の診断方法。
- 前記多発性硬化症が、再発寛解型の多発性硬化症である、請求項4に記載の診断方法。
- 16SリボソームRNA遺伝子の塩基配列が、配列番号3〜23で特定される塩基配列のいずれかと99%以上の相同性を有する腸内細菌からなる、自己免疫疾患の診断用バイオマーカー。
- 16SリボソームRNA遺伝子の塩基配列が、配列番号3〜23で特定される塩基配列のいずれかと99%以上の相同性を有する腸内細菌の、自己免疫疾患の診断用バイオマーカーとしての使用。
- 16SリボソームRNA遺伝子の塩基配列が、配列番号5〜23で特定される塩基配列のいずれかと99%以上の相同性を有する細菌、及び前記細菌に由来する生理活性物質からなる群より選択される少なくとも1種を有効成分として含有する、自己免疫疾患の予防又は治療剤。
