Abstract
医療機関は、高度医療機器を貸与されて医療業務を行ない、高度医療機器を貸与した支援者が貸与料(月額対価)の支払いを受けることとし、この月額対価を高度医療機器の価格や導入後の稼動状況に基づいて計算することとした。高度医療機器導入の際に支援者が行なったサービスを貸与料に含めることができる。The medical institution conducts medical work with the lending of advanced medical equipment, and the supporter who lent the advanced medical equipment receives payment of the rental fee (monthly consideration). 技術分野
本発明は、病院などの医療機関への高度医療機器の導入促進に資することのできる、高度医療機器の貸与料の計算方法、そのプログラム、高度医療機器導入支援システム、及び高度医療機器導入支援方法に関する。
背景技術
近年、高度医療機器が注目されている。例えば、癌の検診を行なうための高度医療機器として、放射性薬剤を用いたPET(Positron Emission Tomography;陽電子放出型断層写真[断層写真装置])が注目されている。このPETによる癌検診については、先端医療技術研究所発行の「PET通信 1999 SUMMER No.27」第1頁〜第14頁などに詳細に記載されている。
PETによる癌検診を行なうためのPETシステムについては、早期に癌を発見できることなどから、病院、診療所、検診施設などの医療機関において導入が検討されている。導入に際しては、医療機関が高度医療機器を自前で購入して導入する場合と、リースで導入する場合がある。
しかしながら、PETシステムを始めとする高度医療機器は、例えば放射線技術やハイテク技術などを駆使して高度医療を実現しているためもあって、非常に高価である。つまり、高度医療機器は、高額医療機器といえるものである。このため、資金が豊富な場合を別として、高度医療機器を簡単に導入できるものではない。
また、医療機関が高度医療機器を自前で購入する場合は、エンジニアリング、運営計画、機器選定、運転・メンテナンス計画の策定、立上げ、官庁に対する種々の書類の提出(官庁申請)、建物設計を行なわなければならない。
ところで、PETなどの高度医療機器は、例えば癌を極めて早期に発見することができるので、早期に発見されない場合よりもトータルの治療費(つまり患者に対する金銭的負担)を安く済ませることが可能になる。このことは、患者の家計ばかりでなく、国民経済の観点からしても大変好ましいことである。また、高度医療機器により、従来不可能であった難病の治療が行なえるようになると、看病を行なう者の負担を大幅に軽減したり、負担を全くなくしたりすることができるという、大変大きなメリットがある。また、高度医療機器により、病気の事前予測ができれば、大変都合がよい。よって、高度医療機器の導入を促進する必要がある。
発明の開示
従って、本発明は、高額な高度医療機器の導入を促進することができる、高度医療機器の貸与料の計算方法、そのプログラム、高度医療機器導入支援システム、及び高度医療機器導入支援方法を提供することを主たる課題とする。
前記課題を解決した本発明においては、医療機関は、高度医療機器を貸与されて(貸し出されて)医療業務を行ない、高度医療機器を貸与した(貸し出した)者が貸与料(対価)の支払いを受けることとし、この貸与料(対価)を高度医療機器の価格や導入後の稼動状況に基づいて計算することとし、もって医療機関への高度医療機器の導入を促進・支援する。
なお、「医療機関」は、例えば病院、診療所、療養所、研究所などであり高度医療機器を利用して検診、治療、健康診断、研究などを行なうものである。また、「高度医療機器」は、PET、重粒子線照射装置、MRI(Magnetic Resonance Imaging;核磁気共鳴診断装置)などである。
また、「医療機関に貸与する」とは、実際に貸与している場合と、今後貸与する場合の双方を含むものである。今後貸与する場合に計算される貸与料は、例えば試算あるいはシミュレーションとしての位置付けを有する。
発明を実施するための最良の形態
以下、本発明の実施形態を、添付図面を参照して詳細に説明する。
≪第1実施形態≫
まず、本発明の第1実施形態を、図面を参照して詳細に説明する。
なお、第1実施形態は、高度医療機器を支援者(貸与元)から貸与されて導入した後における、医療機関(貸与先)の支援者への対価(貸与料)の支払いに関する実施形態に係るものである。ちなみにこの第1実施形態は、本発明の「高度医療機器の貸与料の計算方法」、「高度医療機器の貸与料の計算プログラム(グラフ化部分を除く)」、「高度医療機器導入支援システム」を具現化したものである。
〔高度医療機器導入支援サービス〕
まず、第1図を参照して高度医療機器導入支援方法により実現される高度医療機器導入支援サービス(高度医療支援サービス)を説明する。
第1図は、高度医療機器導入支援方法などにより実現される高度医療機器導入支援サービスのイメージを模式的に示した図である。
(支援者)
第1図に示す支援者1は、資金力及び技術力を有しており、その資金力及び技術力で高額な高度医療機器及びオペレーションサービス及びメンテナンスサービスを医療機関2に提供し、高度医療機器導入支援サービスを行なう団体である。
支援者1は、高度医療機器を医療機関2に提供(貸与)することにより、医療機関2から対価(貸与料)を受けるようになっている。また、支援者1は、オペレーションサービス及びメンテナンスサービスを提供することによっても、医療機関2から対価を受けるようになっている。なお、導入時に行なうサービス(役務)の費用を貸与料に含めることができる。この導入時に行なうサービスは、例えば、エンジニアリング、運営計画提案、機器選定提案、運転・メンテナンス計画提案、官庁申請支援、建物設計支援などである。例えば、エンジニアリングなどは、専門的な知識や技術が必要であり、医療機関2は持ち合わせていない。また、官庁申請などは、医療機関2にとって手間のかかる作業である。
(医療機関)
第1図に示す医療機関2は、医療スタッフ、医療技術、及び土地・建物を有しており、支援者1から高度医療機器及び必要に応じてオペレーションサービス及びメンテナンスサービスの提供を受けて、検診者や患者などに高度医療サービスを行なう団体である。
具体的には、医療機関2は病院などであり、支援者1から提供(貸与)されて導入した高度医療機器を使用して患者の検査・診断を行なう。また、近隣医療機関から紹介された患者の検査・診断を行なう。さらに、健康診断(健診)を行なう。医療機関2は、例えば、保険の対象となる医療行為については、診療報酬請求書(レセプト)を社会保険診療基金や国民健康保険団体連合会に提出して診療報酬を得る。また、自由診療として、患者などから全額の報酬を得る。なお、患者などは、特定承認保険医療機関において高度先進医療を受けた場合は、高度の医療技術を除いた部分(特別料金[自費])に対して特定療養費制度が適用され、健康保険がきく。
次に、第2図を参照して高度医療機器導入支援サービスをさらに説明する。
第2図は、高度医療機器導入支援サービスのスキームを模式的に示す図である。なお、ここでの高度医療機器25はPETシステムである。ちなみに、PETシステム25は単独の機器・装置ではなく、加速器251、合成装置252、カメラ253を含んでなるシステム構成を有する。
第2図に示す支援者1は、医療機関2の要望を加味して調達したPETシステム25を医療機関2に提供(貸与)する。また、支援者1は、医療機関2との取り決めや契約に基づいて、加速器251の運転及び保守、合成装置252の保守、並びにカメラ253の保守を行なう。また、支援者1は、加速器251の運転用に、技師及び運営管理者を提供する。ちなみに、合成装置252の運転は医薬品の製造に該当し、カメラ253の運転は診断行為に該当するため、支援者1はその運転を行なわない。また、支援者1は、合成装置252及びカメラ253についての技師及び運営管理者の提供を行なわない。
第2図に示す医療機関2は、支援者1から提供されたPETシステム25について、支援者1が加速器251を運転した結果得られた陽電子放出核種(例えば18F[フッ素18])を原料として合成装置252を運転して陽電子放出核種を含む放射性薬剤としての医薬品(例えばFDG[フルオロデオキシティグルコース])を製造(製薬)する。医療機関2は、製造した医薬品を患者などに投与した後、カメラ253を運転(操作)して断層写真を撮影し、診断を行なう。医療機関2での、合成装置252を運転しての製薬、並びにカメラ253を操作しての写真撮影及び診断には、医師、診療放射線技師、看護婦、薬剤師があたる。なお、この第2図のスキームでは、PETシステム25を設置する土地・建屋は医療機関2のものである。
医療機関2は、PETシステム25を使用した結果、研究用の収入を得る。また、臨床用の収入として、それぞれ保険の報酬(診療報酬)、高度先進の報酬、健診の報酬(自由診療報酬)を得る。これらが、対価(貸与料)支払いの原資となる。
〔対価の計算・支払い(支援システム)〕
医療機関2は、PETシステム25を貸与された対価(貸与料)を支援者1に支払う。換言すると、支援者1はPETシステム25を医療機関2に貸与した対価の支払いを、医療機関2から受ける。
以下、高度医療機器導入支援サービスにおける対価の計算及び支払い(収受)について詳細に説明する。
第3図は、高度医療機器導入支援サービスを実現するための支援システムの全体構成を示すブロック図である。
第3図に示すように、支援システムは、支援者1が管理する支援サーバ11を中核としたシステムであり、通信回線を介して複数の医療機関2の端末21が接続されている。なお、支援サーバ11が請求の範囲の「高度医療機器導入支援システム」に相当する。
この第3図において、符号111は支援サーバ11の処理装置であり、符号112は同記憶装置であり、符号113は同入出力装置であり、符号114は同通信装置である。また、符号211は端末21の処理装置であり、符号212は同記憶装置であり、符号213は同入出力装置であり、符号214は同通信装置である。また、符号CNは、検診者数をカウントするカウンタである。
支援サーバ11のCPUやRAMなどから構成される処理装置111には、医療機関2の端末21が送信した検診者数に基づいて「月額対価」を計算するプログラムが起動・展開している。このプログラムは、請求の範囲の「期間ごとの貸与料」及び「支払回数の最終回」を計算・設定するものである。このプログラムの機能については、後にフローチャートなどを参照して詳細に説明する。なお、「期間」はこの実施形態では月であるが、週、年、半期、四半期などでもよい。支援サーバ11のハードディスクなどから構成される記憶装置112には、「月額対価」を計算する計算式が記憶されている。この記憶装置112は、請求の範囲の「計算式記憶手段」に相当する。ちなみに、計算式を大別すると、「支払総額(対価の総額)」を計算する式と、月ごとに支払う「月額対価」を計算する計算式とに分けられる。
(計算式)
「支払総額(対価の総額)」は、一例として次の計算式により計算される。
(1)支払総額=高度医療機器の購入金額+マージン+サービスの対価+金利分
(2)マージン=f(総支払回数)
(3)金利分=f(総支払回数)
(4)総支払回数=INT(〔支払総額の前回値−支払済み金額〕/過去6ヶ月の平均月額対価)+既支払回数
但し、2年度までは、総支払回数=72回
ここで、「サービスの対価」は、PETシステム25の導入時に支援者1が行なったエンジニアリングなどのサービスの費用である。また、「マージン」は支援者1の手数料であるが、式(2)では「総支払回数」が多くなると「マージン」は増えるように設定されている。「金利分」も、式(3)では「総支払回数」が多くなると増えるように設定されている。
なお、式(2)、式(3)における「総支払回数」は、式(4)のように、例えば2年度までは暫定的な初期値として72回(6年)が設定してあり、3年度以降は、「支払総額の前回値」、「支払済み金額」、「過去6ヶ月の平均支払い月額」、「既支払回数」に基づいて、計算式により「総支払回数」を推定するようにしてある。但し、ここでの「総支払回数」は、「マージン」と「金利分」を設定するためだけに使用される変数である。また、「支払総額の前回値」と「支払総額」という二つの変数があるのは、「支払総額」が「総支払回数」により変動するからである。
ちなみに、「高度医療機器の購入金額」と「サービスの対価」は、本実施形態では固定値である。
第1実施形態では、対価(月額対価)の支払いは月ごとに行なわれるが、この「月額対価」は、一例として次の計算式により算出される。
(5)月額対価=K×検診者数
ここで、Kは係数(円/月/人)である。
第1実施形態では、この「月額対価」の計算式は、顧客(医療機関)2ごと、あるいは高度医療機器(PETシステム)25ごとに個別に設定されており、Kの値が医療機関2やPETシステム25により異なるものになっていたりする。なお、Kは、例えば検診者一人当たりの医療機関2としての売上金(あるいは利益など)であったり、該売上金などに関連した係数であったりする。
また、医療機関2(あるいはPETシステム25)によっては、「月額対価」が、「基本月額+K’×検診者数」という、固定・従量併用制になっていたり、あるいは、インセンティブの項が付加されていたりする。また、医療機関2(あるいはPETシステム25)によっては、「検診者数」が、PETシステム25を構成する機器の稼働時間(例えば加速器251の稼働時間)になっていたりする。どのように「月額対価」を計算するかは、支援者1と医療機関2との契約による。
ちなみに本実施形態では、「検診者数」が多くなると「月額対価」が多くなるように設定されている(式(5)参照)。なお、支払う「月額対価」が多くなると、当然として、「総支払回数」(支払期間)を少なくすることができる(式(4)参照)。「総支払回数」を少なくすることができると、その分、前記した式(2)や式(3)からわかるように、「マージン」や「金利分」を減らして式(1)で算出される「支払総額」を少なくすることができ、医療機関2にとって都合がよい。ちなみに、「総支払回数」を少なくすると「マージン」や「金利分」を少なくできるのは、支援者1にとって資金回収が速やかに行なわれるからである。
第3図に示す医療機関2の端末21の処理装置211は、所定の時期(例えば毎月20日〔しめ日〕)になると、PETシステム25の期間ごとの稼動状況として検診者数カウンタCNの検診者数を取得し、これを検診者数情報として支援サーバ11に送信するプログラムが展開している。支援サーバ11に送信する検診者数情報は、次のフォーマットをしている。なお、医療機関IDは医療機関2ごとにユニークなものになっている。検診者数は、検診者数カウンタCNの今回値と前回値の差である。前回値は前回データ採取日(例えば前月20日)における検診者カウンタCNの数である。この検診者数は、カメラ253が複数ある場合でも一台ごとのものではなく、医療機関2全体としてのものである。もちろん、カメラ253の一台ごとの検診者数としてもよい。なお、検診者数は、例えば手入力の値でも、医療機関2の事務処理システムで管理されている値でもよい。
〔対価の計算・支払に係る動作〕
次に、第4図及び第5図を参照して対価の計算・支払いに係る動作を説明する。
第4図は、対価の計算に係るシーケンスチャートである。第5図は、支援サーバ11の対価の計算に係る動作を説明するフローチャートである。
第4図に示すように、支援サーバ11は、S11で送信される検診者数情報を受信する。そして、S12で受信した検診者数情報に基づいて「月額対価」を計算し、S13でこの計算した「月額対価」の額を、医療機関2に通知する。通知を受けた医療機関2は、S14で「月額対価」を支払い、これを支援サーバ11に通知する(支払通知、S15)。なお、このように支援サーバ11が「月額対価」の額を事前に通知するのは、「月額対価」が固定ではなく、その都度計算されるものだからである。もちろん、「月額対価」の通知を省略する構成としてもよい。なお、検診者数情報の受信は、請求の範囲の「高度医療機器の期間ごとの稼動状況を、貸与料の計算要素として取得するステップ」などに相当する。
処理装置111が実行する第4図のS12の処理を、第5図のフローチャートを参照して詳細に説明する。即ち、第4図のS12では、以下に説明する処理がなされる(貸与料を計算するプログラムが実行される)。
まず、支援サーバ11が検診者数情報を受信し(S121)、検診者数情報に含まれる医療機関IDに基づいて、記憶装置112に記憶されている「支払済み金額」、「既支払回数」、「支払総額の前回値」を取得する(S122)。続いて、医療機関IDに基づいて「支払総額(今回値)」を計算する式(1)〜(4)を取得し、「支払総額(今回値)」を計算する(S123)。医療機関IDと計算式とは、紐付されて支援サーバ11の記憶装置112に記憶されている。
なお、記憶装置112には、予め入出力装置113や通信装置114を介して、「高度医療機器の購入金額」、「サービスの対価」が、医療機関IDと紐付けして記憶されている(取得されている)。
次に、S124で今回支払う「月額対価」を計算する。「月額対価」の計算は次のように行なう。即ち、医療機関IDに基づいて「月額対価」を計算する式(5)を記憶装置112から取得し、この式(5)にS121で受信した検診者数情報に含まれている「検診者数」を代入して「月額対価」を計算する。
処理装置111は、「月額対価」を計算すると、S125で、「月額対価」と「支払済み金額」と「支払総額」とから、今回の支払いが最終回か否かを判断する。具体的には、「月額対価」+「支払済み金額」<「支払総額(今回値)」か否かを判断して、不等式の左辺が右辺の「支払総額」に満たない場合(YES)は、S124で計算した「月額対価」を正式な「月額対価」として当該医療機関2に通知する(S126)。通知に対応して支払いがあると、支払確認後、「支払済み金額」を更新する(S127)。そして、「既支払回数」をインクリメント(S128)して記憶し、S121で、当該医療機関2についての検診者数情報の受信を待つ。
一方、S125で、不等式の左辺が右辺の「支払総額」以上の場合(NO)は、S129で「月額対価」を再計算し、再計算した「月額対価」を当該医療機関2に通知する。計算式は、「月額対価」=「支払総額」−「支払済み金額」である。つまり、S129では、「月額対価」と「支払済み金額」の合計が、「支払総額」を超えてしまうことがないようにしている。また、S129では、今回の支払いが最終回であることも、当該医療機関2に通知する。医療機関2からの支払いがあると、支払確認後、「支払済み金額」を更新すると共に、「支払終了」を設定する(S130、最終回の設定)。「支払終了」を設定すると、次回の処理が行なわれなくなる。
このようにして、毎回の「月額対価」の支払いが行なわれ、「支払終了」が設定される。この「月額対価」の支払いによれば、支払う「月額対価」が多くなるほど「支払総額」が少なくなるようになっており、医療機関2にとって都合がよい。一方、「検診者数」が少ないと、PETシステム(高度医療機器)25による収入も少ないが、その分、支援者1に毎月支払う「月額対価」も少なくて済むので、医療機関2にとって都合がよい。また、計算式を医療機関2(あるいは高度医療機器25)ごとに設定できることから、医療機関2(あるいは高度医療機25)ごとに種々の支払プランを組むことができ、この点からも医療機関2にとって都合がよい。つまり、この例では、式(1)〜式(5)を使用して「支払総額」、「月額対価」を計算したが、医療機関2や高度医療機器25によって、係数Kが異なったものにしたり、式自体が異なったものにしたりできる。ちなみに、Kの値が大きいと早期に支払終了になる。一方、Kの値が小さいと、長い期間にはなるが、毎月少ない額を無理なく支払うことができる。なお、支払終了後のPETシステム25の所有権などの取扱は、契約により定められるものとする。
〔支払パターンの比較〕
次に、第6図は、医療機関2が年度ごとに支援者1に支払う金額を、高度医療機器25を、購入、定額リース、本件方法の、3つ導入の仕方により導入した場合の支払いパターンを比較した図である。
実線で示す本件方法の場合は、初期の支払い(出費)を抑えることができるので、破線で示す購入(医療機関2が自前で購入)と比較した場合はもちろん、一点鎖線で示すリースの場合と比較しても、医療機関2は、高度医療機器25を断然導入し易いことが判る。また、リースの場合は、導入後に検診者数が伸びないときでも契約時に決められた定額のリース料を支払わなければならないが、本件方法の場合は、導入後に検診者数が伸びないときでも検診者数に応じた支払いであるので、高度医療機器25により得られる利益(売上など)の範囲内で支払いを行なうことも可能であり、医療機関2にとって負担が少なく大変都合がよい。よって、高度医療機器25の医療機関2への導入が促進される。
ちなみに、第6図では、本件方法の支払期間がリース期間よりも長いように表現(設定)されているが、契約内容にもよるが、検診者数が多ければ、リースよりも支払いの終了(満了)期間を短くすることができる。しかも、期間を短くすれば、前記した説明の通り支払総額を少なくすることができるので、医療機関2の自己努力が促される。この点からも本件方法は優れているといえる。
〔エンジニアリング、機器の選定、各種計画の策定、各種契約など〕
ところで、医療機関2がPETシステム(高度医療機器)25を導入するためには、購入の場合についていえば、第7図に示すように、エンジニアリング、運営計画策定、機器選定、運転・メンテナンス計画、機器の据付・調整、機器の立上げ、官庁申請、建物設計などを行なわなければならない。備品の調達も行なわなければならない。また、地方自治体への申請手続きなどや、金融機関、保険会社、運転会社などとの契約も行なわなければならない。これらは、専門の技術を必要とする大変な作業であったり、手間のかかる作業であったりする。なお、第7図は、医療機関2が高度医療機器25を自前調達(購入)で導入するとした場合の契約関係・手続関係を模式的に示す図である。
同様に、リースの場合についていえば、第8図に示すように、地方自治体への固定資産税の支払い、金融機関からの融資・返済、保険会社との保険契約はリース会社が行なうものの、なおもエンジニアリング、運営計画策定、機器選定、運転・メンテナンス計画、機器の据付・調整、機器の立上げ、官庁申請、建物設計などを行なわなければならない。備品の調達も行なわなければならない。また、リース会社との契約もあるし、運転会社との契約もある。なお、第8図は、医療機関2が高度医療機器25をリースで導入するとした場合の契約関係・手続関係を模式的に示す図である。
一方、本件方法についていえば、第9図に示すように、支援者1が、エンジニアリング、運営計画提案、機器調達、運転・メンテナンス計画提案、機器の立上げ支援、官庁申請支援、建物設計支援、ファイナンスなどのサービスを行なう。また、支援者1が、運転会社との契約、機器の売買契約、メンテナンス契約、地方自治体への固定資産税の支払い、保険会社との契約も行なう。従って、本件方法の場合は、医療機関2が行なうのは、支援者1との契約だけでよい。つまり、医療機関2は、購入(自前調達)の場合やリースの場合における種々の煩いから開放される。なお、第9図は、医療機関2が高度医療機器25を本件方法で導入するとした場合の契約関係・手続関係を模式的に示す図である。
また、既に説明したように、この高度医療機器25を医療機関2に貸し出す際に支援者1が行なうエンジニアリングなどのサービスの費用を、毎月支払う月額対価に含めることができる。なお、高度医療機器(PETシステム)25の導入に際して行なわれるエンジニアリングや運営計画提案などは、例えば以下の内容である。
(エンジニアリング)
PETシステム25には、薬剤合成に必要な核種製造装置(加速器251)、薬剤合成装置(合成装置252)、薬剤検定装置、PETカメラシステム(カメラ253、画像解析装置)、その他各種付属機器が必要となる。それらの機器は、病院における検診内容及び研究の有無に大きく依存するものである。
エンジニアリングは、病院(医療機関2)の計画に応じて、最適な機器を選定すると共に、必要な付属機器も選定する。また、選定した機器を購入すると同時に、適切な工程及び品質管理の基で設置する。なお、機器選定提案もこのエンジニアリングに含めることができる。
(運営計画提案)
PET検診は、使用する放射性薬剤の種類、検診者数によって、薬剤合成・PETカメラシステムのスケジュールが大きく異なる。ここでは、日、週、月、年間の運営スケジュールを、検診予定者数、使用薬剤の種類から最適化するための提案を行なう。
(運転・メンテナンス計画)
PETシステム25で使用する主要機器である核種製造装置(加速器251)、薬剤合成装置(合成装置252)、PETカメラシステム(カメラ253、画像解析措置)などは、定期検査により常時、機器の信頼性を維持する必要がある。運転・メンテナンス計画は、そのために必要な定期検査の合理的な実施回数、年間の運転スケジュールを考慮した定期検査実施時期の設定及びその費用の低減策を計画する。
(官庁申請支援業務)
PETシステム25の運転にあたっては、文部科学省に対するRI(Radio Isotope)使用許可申請、厚生労働省に対する診療放射線施設届けなど、種々の許認可申請が必要である。官庁申請支援業務では、申請書作成業務などを補助する。
(建物設計支援)
PETシステム25は、RI及び加速器251を使用する施設のため、「放射線障害防止法」に従った放射線遮蔽、密閉構造で設計する必要がある。また、薬剤製造施設であることから、日本核医学会と日本アイソトープ協会からでている管理基準に準拠した設計であることが望まれている。建物設計支援は、これらの各種基準をとりまとめて、建物設計に反映させると同時に、医療従事者の外部被曝をできるだけ最小にする配置計画などを実施する。
〔その他〕
なお、第1実施形態の説明において、「月額対価」をPETシステム全体として計算したが、加速器など構成機器ごとに計算するようにしてもよい。また、対価は月ごとではなく、半年ごとや1年ごと、あるいは1週間ごとに計算するようにしてもよい。また、支払終了(支払の満期)を設けることとしたが、PETシステムが医療機関に設置されている限り、検診者数(稼動状況)に基づいた対価を支払うようにしてもよい。
以上説明した第1実施形態の高度医療機器導入支援サービス(高度医療機器の貸与料の計算方法、そのプログラム、高度医療機器導入支援システム)によれば次のような効果があり、医療機関への高度医療機器(PETシステム)の導入が促進される。
即ち、支援者が高度医療機器を購入するので、医療機関は、大きな投資をすることなく高度医療機器による診療、健診などを行なえる。しかも、支援者が高度医療機器を購入するので、医療機関は、地方自治体に対する固定資産税の申告手続などの煩い、金融機関に対する資金調達などの煩い、保険会社に対する保険手続の煩いから開放される。さらに、支援者がエンジニアリング、各種計画の策定など、専門性・技術性を有する作業を行なう。このため、医療機関は、専門性・技術性を有する作業を行なわなければならないという大きな煩いからも開放される。
また、医療機関は、検診者数に応じた対価(月額対価、貸与料)を支援者に支払えばよいので、検診者数が少なければ支払う金額も少なくて済む。この点、検診者数などの高度医療機器の稼動状況に無関係に対価を支払わなければならない購入やリースとは異なる。このため、検診者数が伸びないために、高度医療機器から得られる収益が少ないときも、検診者数に応じた(収益に応じた)対価の支払いができるので、医療機関にとって大変都合がよい。
また、導入に際して行なうエンジニアリング、各種計画の策定などのサービスに対する対価を、医療機関の選択により、月額対価に含めることができる。つまり、エンジニアリングなどの費用も、検診者数(高度医療機器の稼動状況)に応じた支払いとすることができる。従って、導入時の費用の支出が抑制され、この点からも高度医療機器の医療機関への導入が促進される。
また、医療機関ごと、高度医療機器ごとに計算式が設定可能であり、多様なプランに対応して支援者に支払う対価を計算(設定)することができる。換言すると、支援者は、医療機関に対して多様なプランでの対価の支払いを可能とする。
また、検診者数に基づいて対価を計算するに際して、通信回線を介して検診者数を確実に入力することができる。また、検診者数は加速器の稼働時間、放射性薬剤の使用量などの稼動状況に簡単に置き換えることができる。
また、検診者数を送受することにより、支援者1は、統計データを作成することもできる。例えば、支援者1が複数の医療機関2のデータを集めて統計化することにより、検診者数の少ない医療機関2の問題点を浮き彫りにしたり、改善点を提案したりすることができる。
なお、高度医療機器の導入が促進されると、国民の健康増進、国家財政における医療費の支出の抑制につながるという公益的な効果も生じる。
≪第2実施形態≫
次に、本発明の第2実施形態を、図面を参照して詳細に説明する。
なお、第2実施形態は、高度医療機器としてのPETシステムの導入を検討している医療機関(貸与先)に対して、支援者(貸与元)が第1図及び第2図に示すイメージやスキームに基づいた導入プランを提示する実施形態に係るものである。ちなみにこの第2実施形態は、本発明の「高度医療機器の貸与料の計算方法」、「高度医療機器の貸与料の計算プログラム」、「高度医療機器導入支援方法」を具現化したものである。
以下の説明において、第1実施形態と重複する構成などについては同一の符号を付して、その説明を省略することとする。
〔高度医療機器導入支援サービス(プラン提示システム)〕
第10図は、第2実施形態の高度医療機器導入支援サービスを実現するためのプラン提示システムの全体構成図である。医療機関2に対してPETシステムの導入プランの提示を行なう貸与額試算サーバ12は、第10図に示すように、処理装置121、記憶装置122、入出力装置123、通信装置124を含んで構成される。貸与額試算サーバ12は、PETシステム(第2図の符号25参照)の導入を検討する者(医療機関2)や、PETシステムの導入を医療機関2に説明する者が操作するWeb端末22からの依頼(機器選定要求)に対して、PETシステムの機器の構成案を選定して提示する機能(機器構成案提示機能)を有する。また、Web端末22からの依頼に対して、年度別の貸与料を計算(額試算)してWeb端末22に回答する機能(額試算回答機能)を有する。なお、貸与額試算サーバ12には、Web端末22との情報の送受信を可能とするためのWWW(World Wide Web)サーバプログラムが、CPUやRAMなどから構成される処理装置121に起動・展開している。ちなみに、Web端末22には、閲覧用のブラウザソフトがインストールされているものとする。
貸与額試算サーバ12のハードディスクなどから構成される記憶装置122には、後で説明する第11図〜第14図に示すような情報が記憶されている。ルータなどから構成される通信装置124は、インターネットでの情報の送受信を可能とするため、TCP/IP(Transport Control Protocol/Internet Protocol)による通信をサポートする機能を有する。
(機器情報)
ここで、第11図〜第14図を説明する。
第11図は、貸与額試算サーバ12の記憶装置122に記憶された粒子発生装置(第2図の加速器251に相当)に関する機器情報を示す。この第11図に示すように、粒子発生装置に関しては、粒子発生数(単位時間当たり)、RIの種類、電源容量、金額、設置面積及び重量が機器情報として複数の粒子発生装置A、B、…各々に対して記憶されている。なお、第11図の粒子発生装置A、B、…は、加速器のメーカ名と型式番号を示す情報であり、ここでは機器特定情報と呼ぶ。
第12図〜第14図は、貸与額試算サーバ12の記憶装置122に記憶されたそれぞれ合成装置、カメラ及び画像処理装置に関する機器情報を示す図であり、第12図に示すように、合成装置(第2図の符号252参照)に関しては、薬剤の種類、薬剤の生成効率、処理時間、電源容量、金額、設置面積及び重量が記憶されている。また、第13図に示すように、カメラ(第2図の符号253参照)に関しては、感度、分解能、信号処理時間、金額、設置面積及び重量が記憶されている。さらに、第14図に示すように、画像処理装置に関しては、画像処理時間、主メモリの容量、金額、設置面積及び重量が記憶されている。なお、第1実施形態では、画像処理装置はカメラの構成の一部として記載した。
このように、記憶装置122には、PETシステムの装置ごとに金額、処理能力、設置面積、重量などの情報が記憶されている。なお、第11図〜第14図の機器情報は一例であり、機器情報として設定される項目やその内容はこれに限られるものではない。
〔高度医療機器の選定・支払プラン提示に係る動作〕
以下、医療機関2がPETシステム(高度医療機器)の導入を検討する場合を、第15図のシーケンスチャートに基づいて、第11図〜第14図、第16図〜第18図を参照して説明する。
まず、支援者1は、特定のメーカに偏ることなく、いろいろなメーカや代理店などから最新の機器情報を広く収集し、これを第11図〜第14図に示すようなフォーマットで記憶装置122に記憶する(S201,S202)。もちろん、特定のメーカの機器情報のみを記憶するものとしてもよい。次に、医療機関2から支援者1に対してPETシステムの構成案の提示を依頼(機器選定要求)する。本実施形態では、支援者1が貸与額試算サーバ12をWebサーバとしてホームページを開設しているので、医療機関2は、そのホームページ上でPETシステムに対する要求(条件)を入力して支援者1にシステム構成案の提示を依頼する(機器選定要求情報の送信、S203)。そして、その条件に適合した機器を貸与額試算サーバ12が選定し(S204)、選定したPETシステムに関する情報をWeb端末22に送信する(選定機器情報の送信、S205)。
ここで、第15図のステップS203〜S205で行なわれる処理を、第10図〜第14図、第16図〜第18図を参照して説明する。
第16図は、支援者1が開設しているホームページの初期画面を示し、医療機関2から支援者1のホームページにアクセスしたときに医療機関2のWeb端末22の図示しない入出力装置に接続されたモニタに表示されるものである。第16図に示すように、初期画面上には「PETシステム構成案提示依頼」という文字が表示され、その文字をマウスポインタでクリックすることにより条件入力画面が現われる。
第17図は、入力選択画面を示す。この図に示すように、条件入力の第1段階として「PETシステム全体の条件を設定」と「機器ごとの条件を設定」の2つを表示させ、条件の設定をPETシステム全体に対して行なうか、機器ごとに行なうかをマウスポインタによるクリックで選択する。なお、第17図の画面において、「戻る」をクリックすると1つ前の画面(ここでは第16図)が表示され、「Topへ」をクリックすると常に第16図に示す初期画面が表示される。これは以下に説明する各表示画面においても同様である。
第17図の表示画面において「PETシステム全体の条件を設定」が選択されると、次に第18図に示すPETシステム全体の条件設定画面が表示される。第18図に示すように、この画面では条件として「予算」、「検診人数」、「設置面積」及び「重量」という文字が表示されると共に、数字を入力するための入力欄が表示される。更にその横には、各条件の優先順位を入力するための入力欄が設けられている。この画面において、Web端末22の操作者は、マウスポインタにより入力欄をクリックするとその入力欄に数字が入力可能となるので、各条件を設定すると共に、どの条件を優先すべきかを示す優先順位を設定する。なお、数値が入力されていない条件については、貸与額試算サーバ12はその条件についての要求がないものとして扱う。このように条件及び優先順位の設定が完了したら、マウスポインタにより「決定」をクリックする。
条件入力画面において「決定」がクリックされると、その時点で入力されている条件と優先順位の情報(以下「機器選定要求情報」という)が支援者1の対価貸与額試算サーバ12に送信され(機器選定要求情報の送信、S203)、処理装置121は送信された機器選定要求情報を記憶装置122に記憶させる。また、処理装置121は、記憶装置122に記憶させた機器選定要求情報から優先順位の一番高い条件を確認する。また、処理装置121は、条件と優先順位にマッチした粒子発生装置、合成装置、カメラ、画像装置を含んだPETシステムの組合せを全て抽出する(機器選定、S204)。
処理装置121は、システム構成案の抽出処理が終了したら、その時点で記憶装置122に記憶されているシステム構成案を表にまとめ、表情報(選定機器情報)として記憶装置122に表No.を付けて記憶させると共に、医療機関2のWeb端末22に送信する(S205)。第19図にシステム構成案をまとめた表を示す。第19図に示すように、この選定機器情報たる表には、選択された各機器の機器特定情報(粒子発生装置A、B、…)、1日の検診可能人数、総額、総設置面積及び総重量を表示する欄が設けられており、各欄にはシステム構成案と共に記憶されている情報が記入される。表情報が与えられたWeb端末22は、表情報に基づいて図示しないモニタのブラウザ画面に第19図に示す表を表示させる。医療機関2では、モニタに表示された表を参考にして、導入するPETシステムの構成を検討する。
第19図のシステム構成案No.の欄の各番号はリンク設定されており、番号をマウスポインタでクリック(PETシステム選択決定)すると第20図に示す画面が表示されるようになっている。第20図は、貸与額試算条件入力画面である。第20図に示すようにこの画面は、システム構成案No.、総額、注意書きが表示されている。また、年度別の検診者数(想定される期間ごとの稼動状況)を入力する年度別検診者数入力欄が設けられている。この検診者数入力欄は、マウスポインタでクリックした後、例えば、初年度の欄には5(人数/日)、2年度の欄には7(人数/日)などと、見込みの数値を順次入力して行く。入力が完了したら、マウスポインタにより「決定」をクリックする。
説明を第15図に戻す(適宜図10など参照)。
「決定」をクリックすると、クリックした番号の情報(PETシステム選択決定情報)と入力した年度別検診者数を含む貸与額試算設定情報が、第15図に示すように、Web端末22から貸与額試算サーバ12に送信される(S206)。貸与額試算サーバ12は、貸与額試算設定情報を受信すると貸与額の計算を、CGI(Common Gate−way Interface)処理により行なう(額試算、S207)。なお、ここでの貸与額の計算は、第1実施形態で説明した式(1)〜式(5)を用いて、第1実施形態と同様にして行なわれる。ここで、式(5)は「月額対価」を算出するものであるから、月ごとに「月額対価」が計算される。「月額対価」は12ヶ月分をまとめることで貸与額としての「年度別支払額」に変換される。ちなみに、式(1)〜式(5)は、顧客(医療機関2)の要望に応じて、Web端末22の側から計算式のパラメータ(例えば係数K)を設定することができるようにしてあってもよい。
また、貸与額試算サーバ12は、選択決定されたPETシステムを購入した場合と、定額リースの場合の年度別支払額を、同様にCGI処理により計算する。
計算(額試算)した、購入の場合の年度別支払額、定額リースの場合の年度別支払額、本件方法(高度医療機器貸出支援方法)の場合の年度別支払額のデータとこれらをグラフ表示するWebページを共に、支払プラン情報としてWeb端末22に送信する(S208)。
Web端末22には、第1実施形態で参照した第6図のグラフが画面表示される。これにより、PETシステムを貸与される医療機関2は、対価をどのように支払えばよいのかが、購入の場合、リースの場合と対比して理解される。このグラフから、本プラン(本件方法)による場合は、初期投資を大幅に押さえることができるのが一目瞭然で判る。もちろん、初期から稼働率が高ければ、その分多くの対価を無理なく支援者1に支払うことができる。
〔その他〕
(変形例1)
なお、第20図において「決定」をマウスポインタでクリックすると、Web端末22上で額試算(本件方法の試算、必要に応じて購入及びリースの試算)を行なうと共にその結果をグラフ化する小さなプログラム(アプレット)を貸与額試算サーバ12がWeb端末22に送信し、前記した貸与額試算サーバ12側のCGI処理によらずとも、Web端末22側にて、額試算とその結果のグラフ化を行なうようにしてもよい。再度の額試算やグラフ化の処理が、通信回線を介さないで行なえる分、早くなるからである。また、第20図において、Web端末22で年度別の検診者数(見込みの値)を入力する操作を省いて、貸与額試算サーバ12の側で年度別の検診者数を設定(標準パターンを設定)するようにしてもよい。
(変形例2)
また、第2実施形態では、例えば営業マンが携帯できるようなノート型のコンピュータに、第11図〜第14図に示されるような情報を記憶し、さらに、額試算を行なうための計算式(第1実施形態の式(1)〜式(5))を記憶し、通信回線としてのインターネットを介して貸与額試算サーバ12にアクセスすることなく、機器の選定及び額試算を行なえるように構成してもよい。つまり、ノート型のコンピュータに、貸与額試算サーバ12が有している機器構成提示機能、額試算回答機能を持たせてもよい。併せて、額試算の結果をグラフ化する機能を持たせてもよい。
(変形例3)
また、第2実施形態では、貸与額試算サーバ12が第11図〜第14図に示される機器情報を参照してPETシステムの提案を行なうこととしたが(貸与額試算サーバ12がいわば見積りを行なうこととしたが)、第21図(a)(b)に示すように、予めメーカなどから取っておいた見積り額を、直接Web端末22から入力して、額試算結果をWeb端末22のモニタにグラフ表示するようにしてもよい。つまり、貸与額試算サーバ12が第11図〜第14図に示すような機器情報を記憶し、第16図〜第19図に示されるような機器の選定を行なうことを省く構成としてもよい(請求の範囲における「高度医療機器の貸与料の計算プログラム」に相当)。
ここで、第21図(a)は、年間費用(貸与額)の額試算の条件を入力する表示画面である。この第21図(a)では、1.導入PETシステムの価格を入力(加速器、合成装置、カメラ、画像解析装置)し、2.耐用年数を入力し、3.リース期間とリース料を入力し、4.減価償却方法を定率か定額かのチェックボックスにチェックマークを付け、計算ボタンをマウスでクリックする。リース期間とリース料は予めリース会社に見積りを取っておいて入力するか、妥当なリース期間とリース料を適宜入力する。また、額試算の条件としての機器の金額は、例えば予め機器メーカなどから見積りを取っておく。
第21図(a)の計算ボタンをマウスでクリックすると、次の第21図(b)の額試算結果をグラフ化した画面が表示される。この第21図(b)は、第6図と同様の図であり、医療機関2にとって、導入初期における出費が少なくて済むことがわかる。なお、第21図(a)(b)の両画面は、第2実施形態でいえばWeb端末22に表示されるものである。
また、第2実施形態では、第22図に示すように、建物設計支援などの見積りを、Web端末22を介して支援者1に依頼できるようにしておいてもよい。なお、第22図では、依頼する項目のチェックボックスにチェックを付け、詳細設定ボタンをマウスでクリックすると、見積り依頼の詳細設定ができるようになっている。
(変形例4)
同様に第2実施形態では、第23図に示すように、運転・メンテナンス計画提案の見積りを、Web端末22を介して支援者1に依頼できるようにしておいてもよい。なお、第23図でも、依頼する項目のチェックボックスにチェックを付け、詳細設定ボタンをマウスでクリックすると、見積り依頼の詳細設定ができるようになっている。
(変形例5)
また、第2実施形態では、第16図に画面で示されるように、「PETシステム発注依頼」という文字をマウスでクリックすると、選定した機器を本方法の導入プランに基づいて発注することができるようにしてあってもよい。
以上説明したこの第2実施形態によれば次のような効果があり、医療機関への高度医療機器(高額医療機器)の導入が促進される。
即ち、医療機関の要望に応じた高度医療機器(PETシステム)の選定を、インターネットを介して行なうことができる。このため、機器選定という大きな負担のかかる作業の工数を、大幅に削減することができる。
つまり、病院や検診施設などの医療機関において高度医療機器を導入する場合、高度医療機器を構成する機器の選定や機器メーカへの発注などを行なわなければならないが、そのためには各機器メーカの製品について性能や金額などを逐一調査しなければならず、高度医療機器導入時の医療機関の負担は非常に大きかった。殊に、PETシステムの場合、ユーザとなる医療機関が、電子・機械・情報処理などの技術を駆使したPETシステムの各機器の性能を把握して、必要とされる適切な機器を選定することは容易ではなかった。しかし、この第2実施形態によれば、PETシステムの機器を容易に選定することができる。
なお、機器の選定に際しては、入力する情報量が少なくても、貸与額試算サーバが医療機関の要求を満足する高度医療機器のシステム構成を、金額を含めて提示する。よって、医療機関などは希望に添ったシステム構成の提案と見積りを、労力をかけずに簡単に取ることができる。しかも、機器が特定のメーカのものに偏ることがないので、医療機関として選択範囲が広がり非常に都合がよい。
また、高度医療機器の価格が判ったところで、本件方法による導入プランでの支払パターンを知ることができる。しかも、本件方法と購入とリースとを比較することができるので、医療機関はどのようにして高度医療機器を導入するかを適切に決定することができる。また、比較は、Web端末に表示されたグラフにより行なうことができる。よって、視覚的に導入プランを検討することができる。また、貸与額試算サーバから額試算及び結果をグラフ化するアプレットを送信すると、再試算、再グラフ化(グラフの更新)を迅速に行なうことができる。よって、高度医療機器の導入を検討している医療機関や販売を行なっている営業マンなどは、能率よく導入の検討や説明を行なうことができる。
産業上の利用可能性
以上説明した通り、本発明は以下の優れた効果を奏する。即ち、本発明によれば、高度医療機器の医療機関への導入が促進される。

通信回線を介して前記計算した対価を送信するステップ、
