Description

【発明の詳細な説明】[Detailed description of the invention]

本発明は子宮頚癌等の温熱療法に使用する温熱
療法装置に関する。 癌に対する治療法としては、手術療法、放射線
療法、化学療法および免疫療法があり、近年で
は、温熱療法が試みられ、その臨床成積について
の報告もあり、温熱が正常組織を破壊することな
く、選択的腫瘍組織を破壊することが明らかにな
つた。さらに温熱は腫瘍に対して放射線感受性を
高め、また、制(抗)癌剤の効果を増強させるこ
とも判明している。 ところで、現在、温熱を行なう手段として、全
身温熱療法、局所潅流療法、温水療法、超短波療
法或いは超音波療法などが試みられ、これら各療
法に使用する装置も開発されているが、いずれの
装置も実験段階的なもので、末だ完全なものはな
く、今後の研究により解決しなければならない問
題を有している。 また、上記温熱療法装置は可成り大がかりにな
り、また或る種のものは、たとえば子宮頚癌等、
癌の種類によつては、その温熱療法装置として適
さないものもある。 本発明は上記のような点に着目し、簡易な構成
により、病巣を含む生体の限局した部分を一定の
温度に保持でき、特に子宮頚癌、食道癌或いは直
腸癌などの温熱療法に適した斬新的な温熱療法装
置を提供することを目的とするものである。 本発明は上記の目的を達成するために、耐熱性
を有し、かつ適度の可撓性を有する素材で造ら
れ、適当の径及び長さに形成した挿入体内にヒー
ターとセンサーを内蔵して成る加温器と、該加温
器の温度制御器とより成り、温度制御器により加
温器の温度を自動的に制御させるように構成した
ものにおいて、加温器の挿入体の外周面に制
(抗)癌剤等の薬剤を添着させるように構成した
ものである。 上記本発明によれば、加温器の温熱療法と薬剤
の化学療法の相乗作用によつて患部を治療するこ
とになる。 以下、図面を参照して本発明の実施例につき説
明する。図面は本発明の実施例を示すもので、図
において、aは加温器を示し、1は耐熱性を有
し、かつ適度の可撓性を有する素材で造られ、適
当の径及び長さに形成した挿入体で、挿入体1内
には先端1a側に位置させてニクロム線その他の
ヒーター2とセンサー3が内蔵されている。実施
例の挿入体1は先端1aを閉塞した管体状に形成
すると共に先端1aを球面状に形成して治療部位
への挿入を円滑に行なえるように構成してある。
挿入体1の径及び長さは治療する部位に応じて決
めるもので、たとえば子宮頚癌用の場合には、直
径5ミリ程度、長さ250ミリ程度が良い。 上記挿入体1は、可撓性を有する素材で造り、
子宮頚部等の挿入部に合わせて適当に湾曲させる
等、変形性を付与してあるが、同時にまた、挿入
体は子宮頚部等に挿入するものであるため、柔ら
かすぎると挿入を困難にする虞れがある。さらに
また、挿入体1にはヒーター2を内蔵しているの
で、その素材としては絶縁材が好ましい。そこ
で、挿入体1の素材の選択に際しては上記の点を
考慮して決定すれば良く、たとえば、テフロン系
樹脂、シリコン系樹脂あるいはポリプロピレン系
樹脂などが適している。しかしながら挿入体1の
素材は上記に例示したものに限定されるものでは
ない。 また、上記センサー3としては、熱電対、サー
ミスタ、白金電極、ダイオードその他所望のもの
を選択して採用できる。 4は吸盤状等に形成し、挿入体1に摺動自在に
装着したストツパー部材で、該ストツパー部材4
は、治療部所に挿入した加温器aを所定の位置に
安定して定置させるためのもので、必要に応じて
設ける。5は挿入体1の基端を連結した握部、6
はコードを示し、コード6の先端にはコネクタ7
が取付けてある。 bは加温器aの温度を制御する温度制御器を示
し、温度制御器bの前面には、電源スイツチ8、
コネクタ7の差込口9、加温器aの温度を表示す
る指示計10、加温器aの温度を目的の温度に設
定する温度設定用ダイヤルおよび加温器aが一定
以上の温度になつた時(故障等の原因により)、
報知器で警報し、かつ加温器aとの回路を遮断さ
せるための温度を設定するダイヤル12等が設け
てあり、この温度制御器bは所望の形式のものを
採用できる。 而して、本発明は上記加温器aの挿入体1の外
周面に薬剤Cを添着させるもので、以下、これに
つき第5図乃至第7図を用いて説明する。 第5図の実施例では、挿入体1の外周面に、ヒ
ーター2の内蔵部分に位置させて抗癌剤、制癌剤
等の薬剤Cを塗布或いは吹き付け等により付着さ
せたものである。 また、第6図の実施例では、挿入体1に被嵌さ
せるように構成したテフロン系樹脂、ポリプロピ
レン系樹脂その他で造つた肉薄の被嵌袋13を設
け、この被嵌袋13の外周面における所定の位置
(被嵌袋13を加温器aに被嵌した際、ヒーター
部に位置する箇所)に抗(制)癌剤等の薬剤Cを
塗布或いは吹き付け等の手段により付着させ、こ
の被嵌袋13を挿入体1に着脱自在に被嵌させる
ように構成してある。 さらにまた、第7図の実施例では、第6図の実
施例と同様の被嵌袋13を設け、この被嵌袋13
の外周面における所定の位置(第6図と同じ箇
所)に流出孔14aを有する薬剤収容部14を形
成し、該収容部14内に抗(制)癌剤等の薬剤
C1(顆粒状)を収容し、この被嵌袋13を挿入体
1に着脱自在に被嵌させるように構成したもので
ある。 実施例の温熱療法装置は上記のように構成した
もので、次にその使用方法の一例につき、第1図
および第4図を参照して説明する。第4図は子宮
頚癌に使用している例を示したもので、まづ、加
温器aのコード6のコネクタ7を温度制御器bの
差込口9に差込み、電源スイツチ8をONにする
と共にダイヤル11,12を回動して加温器aの
温度、および警報温度を所定値(たとえば、ダイ
ヤル11を50℃、ダイヤル12を55℃に)設定す
る(第1図参照)。そして、第4図示のように、
子宮口Aより、加温器aの挿入体1(ただし、第
6図、第7図示のものにあつては被嵌袋13を挿
入体1に被嵌させた状態で)を子宮頚部Bに挿入
し、加温器aのヒーター部を子宮頚部Bの中央に
位置させて定置し、加温器aによる温熱療法と薬
剤による化学療法とを併用して治療するものであ
る。 なお、第6図、第7図の実施例のように構成す
ると、被嵌袋13のデスポ的使用が可能になる。 次に実験例につき説明する(於;岩手医科大学
部産婦人科学教室)。 実験例 1 本発明の装置(ただし、薬剤は使用せず、以下
同じ)を用い、正常子宮頚部、子宮頚部上皮内
癌、初期浸潤癌、子宮頚癌b期の子宮頚部を加
温した後に子宮を摘出し、その病理組織的変化を
検討して次の結果を得た。 (1) 48.5℃では、癌組織、正常扁平上皮、結合織
はすべてが壊死となつていた。 (2) 45.6℃では、癌組織、扁平上皮基底層は壊死
におちいるが、扁平上皮表層や結合織には類壊
死の像がみられた。 (3) 43.5℃では癌細胞のPyknosisが著じるしい。 また、正常扁平上皮基底層細胞にPyknosis
をみとめたが、表層細胞や間質組織には、特記
すべき変化をみとめなかつた。 実験例 2 本発明の装置を用い、子宮頚癌温熱療法への基
礎実験を行なつた。すなわち、卵白およびソーセ
ージを用いて子宮頚部のフアントームを作製し、
本発明装置使用時の温度分布を針形熱電温度計に
より測定した。その結果得られたデータは以下に
示す表のとうりであつた。次いで、摘出子宮を用
いて、同様の方法で加温、温度測定を行なつた結
果、フアントームに一致した温度分布が得られ、
子宮頚癌の温熱治療へ本願発明装置を応用できる
ことを示唆する成績が得られた。 以下に示す表は、ソーセージを用いて作製した
フアントームの中心に加温器aを挿入し、加温器
aの設定温度を順次変え、加温されるフアントー
ムの温度分布状況を計測して得た温度分布表(摂
氏温度)で各表中のx欄は加温器aのヒーター2
の中心部における挿入体1の外壁面より、フアン
トームの肉厚方向(横方向)への距離(単位ミ
リ)を、y欄は同じくフアントームの軸線方向
(縦方向)への距離(単位ミリ)を示す。

Depending on the type of cancer, some types of cancer may not be suitable as thermotherapy devices. The present invention focuses on the above-mentioned points, and has a simple configuration that allows a localized part of the body, including a lesion, to be maintained at a constant temperature, and is particularly suitable for thermotherapy for cervical cancer, esophageal cancer, rectal cancer, etc. The purpose is to provide a novel thermotherapy device.
