Claims (1)
- 封書下紙片を中央にして封書下紙片の一方の側
辺に折り用ミシン目を介して表面に宛名印字部を
有する封書上紙片を連接し、かつ封書下紙片の他
辺の側辺に折り用ミシン目を介して表面に宛名に
対応する通知情報を印字する通知情報印字部を有
する封書中紙片を連接してなり、封書下紙片およ
び封書中紙片の折り用ミシン目側の辺と封書上紙
片の折り用ミシン目の反対側の辺にそれぞれ開封
用ミシン目が設けられてなり、封書上紙片裏面の
上下辺および開封用ミシン目側の辺と封書下紙片
裏面の上下辺に封着部が設けられ、かつ封書中紙
片は折り用ミシン目側の辺表面に封着部が設けら
れるとともに上下辺の少なくとも一方の表裏面に
、前記封書上紙片および／または封書下紙片の封
着部に対応して切取り用ミシン目と封着部とが設
けられ、通常の状態では接着することがなく熱、
圧、または水などの接着条件を付与することによ
り接着する接着剤が前記所定の封着部に設けられ
ていることを特徴とする封書用フオーム。
