遠位分配マニホールドを有する注入スリーブカテーテル
発明の背景
1.発明の分野
本発明は、一般に、体管腔内に流体を注入する装置及び方法に関する。特に、
本発明は、バルーンカテーテルに沿って体管腔内に導入され得る注入カテーテル
の使用及び構成に関する。
経皮的管腔内血管形成術(PTA)は、患者の血管系の狭窄したアテローム性
動脈硬化部位を処置して十分な血流を回復させるために広く用いられている。通
常膨張可能なバルーンの形をした拡張可能な遠位端部を有するカテーテルは、血
管内の狭窄部位に位置決めされる。拡張可能な端部は、拡張して血管を拡げ、罹
病部上の十分な血流を回復させる。PTAは広く認められているが、相変わらず
二つの大きな問題、即ち急性閉塞及び再狭窄により制限されている。
本発明は、両方の問題に取り組むことを意図し、特に流体処置薬剤を局部的に
注入することにより再狭窄を禁止する装置及び方法に関連している。かかる流体
処置薬剤としては、特に血管形成処置後用の抗増殖及び抗再狭窄薬剤、並びにア
テローム性動脈硬化部位の救急処置に有効な抗血栓、血栓溶解、繊維素溶解薬剤
等が挙げられる。流体処置薬剤には、液体薬剤、並びに分散液内に存するマイク
ロカプセルその他の制御放出薬剤の形を含む固体薬剤がある。再狭窄とは、当初
成功した血管形成処置後に、動脈が再び狭まることを言う。再狭窄は、血管形成
患者の約3人に1人の割合で生じ、通常処置後6ヶ月以内に発生する。再狭窄に
罹った患者は、更に処置を必要とする。再狭窄の発生率を減少させるために、機
械的なもの(例えば、血管形成中のバルーン膨張の延長、レーザ血管形成、アテ
ローム切除、ステント留め等)及び薬理学的なもの(例えば、カルシウム拮抗薬
、ACE阻害剤、魚油、ステロイド、代謝拮抗薬等の注入)を含め様々な方法が
提案されている。
薬理学的処置は、全身的な或いは局部的な薬剤注入を介して行うことができる
。全身注入は患者への注入が特に容易であるが、第一に総投与量が大きくなるこ
とや全身的毒性の可能性及び部位特異性の欠如等多数の欠点を有する。これに対
し
て、流体及び分散薬剤の局部注入は、必要となる薬剤総投与量を抑制し、特に壁
内的に注入する場合全身注入に比して薬剤の局部集中を遙かに高める部位特異的
作用を提供する。
血管形成或いは他のインタベンショナル処置後に血管処置部位に薬剤を注入す
るために、種々の特殊なカテーテルが開発されている。このような薬剤注入カテ
ーテルの多くは、カテーテルのバルーン部から一定の方法で薬剤を放出するよう
に構成されたバルーン血管形成カテーテルである。しかしながら、このような二
重の機能が要求されるため、バルーンの構成とカテーテルの薬剤注入に係る構成
の点で、妥協を余儀なくされる。かかる妥協を避けるために、最初のインタベン
ショナル処置後に処置部位に別個に注入すべく他の「専用の」薬剤注入カテーテ
ルが設計されている。しかしながら、別個のカテーテルの使用は、幾つかの欠点
を招来する。第一に、薬剤注入カテーテルを主要なインタベンショナルカテーテ
ルに交換するために時間がかかり、これが、薬剤処置プロトコルの開始を遅らせ
ることになる。また、カテーテルを交換する必要があるため、処置が複雑になっ
てて長引き、時間及び患者の負担コストが増大する。更に、多くの従来の薬剤注
入カテーテルは、やや複雑で高価であるため処置のコストを更に上昇させ、有効
性が限定されるとともに、長い注入時間(30分まで)を必要とした。従って、
これらの問題の一部或いは全部を解決する改良された薬剤注入カテーテルを提供
することが望ましい。
処置薬剤を血管形成処置部位に注入するための改良されたカテーテル及び方法
は、米国特許第5336178号公報に記載されている。該特許の教示を採用し
たカテーテルは、カリフォルニア州パロアルト市のローカルメッド・インコーポ
レイティッドにより商標名「カプラン−シンプソン・インフサスリーブ」として
開発されている。これらの装置は、複数の薬剤注入管腔をその全長に亘り形成し
たスリーブカテーテルである。カテーテルの遠位部は、半径方向に拡張可能であ
り、バルーンの膨張により薬剤注入口を血管壁に係合させるためにバルーンカテ
ーテルのバルーン上に位置決めしてもよい。従来の血管形成カテーテルによる血
管形成に続いて、スリーブカテーテルの薬剤注入管腔を同一の血管形成カテーテ
ルのバルーン上に整列させる。次に、処置部位で注入管腔を拡張するためにバル
ーンを用い、注入管腔を通って薬剤を導入する。
他の薬剤注入カテーテルに比して著しい改良を示しているが、かかるスリーブ
注入カテーテルは、幾つかの欠点を有する。特に、全長に亘り注入管腔を形成し
たバルーンカテーテル上の同軸スリーブの存在は、組み合わせた装置の有効断面
形を増加させる。この横断面積の増加は、バルーンカテーテル及び薬剤注入カテ
ーテルを冠状動脈まで導入するために用いる従来の8F案内カテーテル(最も一
般に使用される大きさ)の管腔を通って組み合わせた装置を導入するとき、問題
が生じる。血管形成カテーテル及び注入スリーブカテーテルを「案内する」こと
に加えて、案内カテーテルの管腔は、冠状動脈に造影剤を注入し(不透明化)、
管腔内の造影剤と血液の血圧を測定する(変換)ために使用される。 内腔の横
断面積が限られた案内カテーテル(例えば8F案内カテーテル)を用いて十分な
不透明化及び変換を達成するためには、血管形成バルーンカテーテルと注入スリ
ーブカテーテルを組み合わせたものが占める横断面積を最小にしなければならな
い。しかしながら、注入スリーブカテーテルの横断面積の限定可能性は、バルー
ンカテーテルを通過させる必要性並びに薬剤注入管腔の横断面積を最大化する必
要性により制約される。圧力降下を最小にして所与の近位注入圧力に対して管腔
を通る流体流量を最大にするために、薬剤注入管腔の横断面積は大きい方が好ま
しい。
これらの理由から、バルーンカテーテルと組み合わせて採用される型の改良さ
れた流体注入スリーブカテーテルを提供することが望ましい。特に、案内カテー
テル内で注入スリーブカテーテルとバルーンカテーテルを組み合わせたものが占
める全横断面積を減少し得る改良された注入スリーブカテーテルを提供すること
が望ましい。しかしながら、かかる横断面積の減少は、薬剤注入管腔の内側横断
面積の実質的な減少無しに達成されるべきである。即ち、改良された注入スリー
ブカテーテルは、所与の注入流量に対してカテーテルの長さに亘る注入流体の圧
力降下の増大を回避すべきである。更に、本発明のカテーテルは、比較的低コス
トで製造可能であるべきであり、任意に、上述した目的及び課題のうちの少なく
とも幾つかと取り組むべきである。
2.背景技術の説明
米国特許第5336178号公報は、先に説明した。米国特許第525408
9号公報は、膨張可能なバルーン内に複数の薬剤注入管腔を一体に形成した薬剤
注入カテーテルを説明している。各注入管腔は、カテーテル軸内の流体移送管腔
に接続されている。
本願は、共通の譲受人に譲渡された同時係属出願である米国特許出願第08/
222143号、米国特許出願第08/241428号、08/221613号
、及び08/305250号の主題に関連している。
発明の概要
本発明は、治療用流体やその他の流体を体管腔内の目標部位に移送するための
装置及び方法を提供する。本方法及び装置は、患者の冠状血管系内での血管形成
処置に関連して有効な抗再狭窄、抗増殖、血栓溶解、腺溶解その他の薬剤の血管
内移送に特に効果的であるが、その他広範な種類の薬剤を他の体管腔に移送する
ためにも使用し得る。
本発明による装置は、バルーンカテーテルと組み合わせて使用される注入スリ
ーブカテーテルから成る。注入カテーテルは、近位端部と遠位端部とを有する軸
と、軸の遠位端部に取り付けられた半径方向に拡張可能な注入スリーブとを備え
ている。注入スリーブ上には、複数の流体注入管腔が配設され、軸内に配設され
た流体移送管腔に接続されている。軸内の流体移送管腔とスリーブ上の流体注入
管腔との間には、該注入管腔に流体を配分するためにマニホールド構造を配置し
ている。注入スリーブは、更に、バルーンカテーテルを収容するための中央収容
部と、軸の遠位方向即ち通常はスリーブの近位端部又はその近傍に位置するバル
ーン用の導入口とを備えている。このようにして、注入スリーブは、バルーンカ
テーテル上に同軸的に取り付けてその遠位端部近傍に位置決めすることができる
。一方、注入カテーテルの軸は、バルーンカテーテルの本体と同軸ではなく平行
に配設され、軸と本体の組み合わせ体により占められる合計横断面積を最小にす
ると共に、流体を流体注入管腔に移送するための流体移送管腔内に十分な横断面
積を確保するようにしている。
本発明の方法によれば、体管腔内の目標部位に複数の注入管腔を位置決めする
。
注入管腔の内側でバルーンを膨張させ、注入管腔上の流体移送口を体管腔の内壁
に係合させる。バルーンを流体注入管腔とは別個に形成することにより、バルー
ンと流体注入管腔との間の相対的な軸線方向移動を可能にしている。流体は、体
管腔内に配置された流体移送管腔を通って移送され、目標部位の近位位置に在る
複数の流体注入管腔に移送管腔から配分される。
本発明の装置及び方法は、多くの点で有利である。第一に、単一又は少数の流
体移送管腔を用いて流体をより多数の流体注入管腔まで搬送しているため、注入
スリーブカテーテルの近位部分により占められる横断面積を減少させることがで
きる。しかしながら、このような横断面積の減少は、移送管腔内での流れ抵抗(
所与の流量での圧力降下)を実質的に増加させることなく、達成可能である。ス
リーブカテーテルの横断面積の減少は、また、カテーテルの押し込み可能性及び
追跡可能性の改善により達成されるべきである。即ち、改良されたスリーブカテ
ーテルは、血管系内の目標部位まで従来のバルーン血管形成カテーテルと共に且
つ/或いは該カテーテル上で導入され、従来のやり方で当該部位で操作(即ちバ
ルーンカテーテル上で前進後退させる)可能であるべきである。本発明のカテー
テルは、また、組み立てが簡単で、製造のコスト及び複雑さを減少させる。

好適な実施形態の説明
本発明は、患者の血管等の体管腔内の対象部位に処置、診断、その他の流体媒
体を注入する装置及び方法を提供する。注入し得る流体媒体は、例えば薬剤それ
自体が液状であったり薬剤を生理食塩水その他従来のキャリヤ流体と組み合わせ
る場合は液体であり、薬剤が固体状であったり(任意にマイクロカプセル或いは
リポソームその他の小胞を含む制御解放注入系内に包含される)適当な液体キャ
リヤ内に分散される場合は分散体である。処置は、「管腔内で」行われる。「管
腔内で」とは、普通は患者の血管系内でありより普通には冠状動脈、末梢動脈、
脳動脈を含む動脈系内である体管腔内の対象部位内で処置が行われることを意味
する。本発明の方法及び装置は、本発明による注入カテーテルをバルーン血管形
成カテーテルと同時に従来の案内カテーテルを通って導入する冠状血管系内の部
位まで、抗再狭窄症薬剤、抗血栓症薬剤、血栓溶解剤、線維素溶解剤、抗増殖剤
等の流体処置薬剤を導入する際に最大の効用を発揮する。しかしながら、本発明
の方法及び装置は、血管系内での使用に限定されず、尿道(例えば、良性前立腺
肥大症、前立腺炎、腺癌を処置するため)、輸卵管(例えば、狭窄症を処置する
ため)、脳実質(パーキンソン病を処置するため)等を含む他の体管腔内でも好
適に採用し得る。本発明の方法及び装置は、従来の薬剤の投与に限定されず、遺
伝子療法その他の処置方法を目的として通常リポソーム又は他のキャリヤ内に存
する遺伝子、核酸、DNA構成体等を注入するために使用してもよい。
体管腔内の「対象部位」は、通常罹病しているか罹病が疑われている。血管処
置の場合、対象部位の例としては、アテローム性沈積即ちプラークの結果として
血流が制限される狭窄部が挙げられる。他の体管腔内の罹病部位は、良く知られ
ており、医学文献に記載されている。
本発明の注入カテーテルは、血管等の体管腔を拡張することを目的とした型の
バルーンカテーテル等の半径方向に拡張可能なカテーテルと組み合わせて使用さ
れる。例えば、本発明での使用に適した血管形成装置及びカテーテルは、米国特
許第5041089号、4762129号、4775371号、4323071
号、4292974号に記載されており、上記公報の開示内容は全て出典を明記
することにより本願明細書の一部とする。適当な血管形成カテーテルは、カリフ
ォルニア州、テマキュラ市のアドヴァンスト・カーディオヴァスキュラー・シス
テム社、フロリダ州マイアミ市のコーディス・コーポレーション、マサチューセ
ッツ州ナティック市のボストン・サイエンティフィック・カンパニー・インコー
ポレイティッドその他から市販されている。
本発明の注入カテーテルでは、注入流体移送管腔を内部に有する軸の遠位端部
上に、半径方向に拡張可能なスリーブ部材を取り付けている。注入スリーブは、
従来のカテーテル上にバルーンその他の拡張可能な装置を受容する中央収容部、
並びにその外面上又はその壁内に配設された複数の流体注入管腔を有する。スリ
ーブは、バルーンその他の拡張可能な装置をその内部で拡張したとき各管腔に供
給された流体を解放可能とする流体注入口を含む部分に亘り半径方向に拡張可能
である。注入スリーブは、所望寸法のバルーンその他の拡張可能なカテーテルを
受容するに十分な、且つその上面又はその壁内に流体注入管腔を収容するに十分
な大きさを有する。半径方向の拡張性は、様々な方法で付与することができる。
例示的実施形態では、半径方向の拡張を可能とするために、スリーブは軸線方向
に分割されている。かかる拡張性は、また、弾性材料の使用、拡張可能な編組材
料の使用、バルーンを内部で膨張させたとき展開する折り畳まれた追従性又は非
追従性材料の使用により付与することもできる。拡張可能なバルーン収容部を構
成する特定の構造が、出典を明記することによりその開示内容全体を本願明細書
の一部とした同時係属出願第08/401541号に多数記載されている。
半径方向に拡張可能なスリーブは、拡張時にカテーテルバルーンの全長に亘り
延びるに十分な長さを有する。通常、スリーブは、以下詳細に説明するように、
スリーブの軸への接続を容易にするために、バルーンの長さより大きい長さを有
する。従って、スリーブの長さは、通常少なくとも2.5cmであり、代表的に
は2.5cm乃至50cm、通常は5cm乃至25cmである。スリーブの直径
は、案内カテーテルから冠状動脈小孔を通り冠状血管系に貫入するに十分小さく
、代表的には2mm以下、通常1.5mm乃至2mmの範囲内である。
流体注入管腔は、スリーブの半径方向に拡張可能な部分の少なくとも一部に亘
り延び、通常、バルーンカテーテルにより半径方向に拡張される部分に亘り延び
る。構成を単純にするため、及び特に使用時に案内カテーテル内で注入カテーテ
ルのスリーブと軸との間の移行部を位置決めするために、代表的には、各注入管
腔は、通常は少なくとも2.5cmの長さ、代表的には2.5cm乃至50cm
の範囲の長さ、最も普通には10cm乃至25cmの範囲の長さを有する注入ス
リーブの全長に亘り延びる。注入管腔から注入液を解放するためにスリーブ内に
形成した灌流口は、通常スリーブの遠位部のみに亘り延び、普通は、膨張すべき
バルーン上に位置して半径方向に拡張する部分上にのみ配設される。
注入カテーテルの軸部の直径は、半径方向に拡張可能な注入スリーブの直径よ
り小さく、代表的には1mm以下、好ましくは0.8mm以下であり、0.7m
m以下であることも多い。第一の特定の実施形態では、軸は、体管腔内でのカテ
ーテルの操作を可能とするに十分な軸線方向強さ並びに曲げ強さを有する単純な
管状部材から形成してもよい。
例示的管状構造は、以下詳述するように、円形又は非円形横断面を有する皮下
注射管その他の金属管を含む。また、軸構造は、一、二、又はそれ以上の管腔を
有して内部ロッドにより補強した押出ポリマ管から構成してもよい。かかる構造
は、先細の補強ロッドを使用することにより軸が可変可撓性を有するように構成
され得るので、有利である。特に、かかるロッドは、それらの遠位端部近傍で極
めて可撓性に富むように先細にすることができ、場合によってはカテーテル全体
の押し込み可能性を高めるために半径方向に拡張可能なスリーブ構造の少なくと
も近位部分に延出させることができる。軸内の流体移送管腔は、代表的には0.
2mm2乃至0.5mm2好ましくは0.25mm2乃至0.35mm2の範囲
の横断面積を有する。軸は、代表的には90cm乃至150cm、好ましくは1
00cm乃至125cmの範囲の長さを有する。
半径方向に拡張可能な注入スリーブは、種々の生物学的適合性を有する材料、
代表的には、天然ポリマ或いはポリアミド(ナイロン)、ポリ塩化ビニル、ポリ
ウレタン、ポリエステル、ポリエチレン、ポリテトラフルオロエチレン(PTF
E)、ナイロン等の合成ポリマから形成することができる。スリーブ用の好適な
材料は、ポリアミド(ナイロン)である。
注入スリーブは、通常、非追従的であり、使用時に注入管腔の開存性を維持す
るために任意に補強してもよい。通常、スリーブ構造は所望の高分子材料の従来
の押出により形成され、中央収容部と各注入管腔とを同時に形成する。注入管腔
と中央収容部の横断面積及び形状は、高精度押出機内に取り付けられた精密工具
を用いて精密に調整してもよい。
注入管腔を4本設ける場合、各管腔は、代表的には0.05mm2乃至0.1
5mm2、通常0.08mm2乃至0.12mm2の範囲の横断面積を有し、総
(合計)横断面積は0.2mm2乃至0.6mm2、通常0.32mm2乃至0
.48mm2の範囲にある。
注入スリーブの少なくとも一部は、半径方向に拡張可能である。代表的には、
スリーブは、管状ウェブとして形成され、各注入管腔は押出工程時に形成される
。注入管腔は、比較的薄い管状ウェブ又は壁「上に」設けられる突出部即ち突起
として形成することができ、代表的には約0.05mm乃至0.1mmの厚さを
有する。また、管状ウェブ又は壁は、代表的には0.2mm乃至0.4mmの厚
さを有する比較的厚い構造として形成してもよく、この場合、各注入管腔は壁の
外
面から突出することなく壁内に形成される。いずれの場合でも、半径方向の拡張
可能性は、代表的には、拡張可能であることが望まれる長さに略等しい長さに亘
り管状壁又はウェブを分割することにより、実現される。かかる分割は、最初の
押出後に行われる。半径方向の拡張可能性を付与する別の構造は、本願の別の箇
所で説明する。
軸内の流体移送管腔は、軸と注入スリーブ間の結合部に配設されたマニホール
ド構造により、注入スリーブ内の個々の流体注入管腔に接続されている。代表的
には、マニホールドは、軸の単一管腔から管状管腔を経て注入スリーブの軸線方
向の各管腔に至る移行部を形成する。例えば、注入スリーブの押出成形品は、最
初にその近位端部から注入管腔を除去してもよい。かかる除去により、注入管腔
の近位端部に管腔入り口が形成される。次に注入スリーブの近位端部上にカバー
シースを配設し、内部マンドレルを用いて組立体を適切に熱収縮させることによ
り、環状管腔から個々の管腔への移行部を形成することができる。同様にして、
軸の単一管腔から環状管腔への移行部は、スリーブ構造の近位端部に形成するこ
とができる。通常、バルーン入り口は、組立工程時に同時に形成される。
マニホールド構造は、代表的には1本であるが場合によっては2本である軸内
の管腔と、代表的には少なくとも2本であるが通常少なくとも3本或いはしばし
ば4本であり場合によっては8本以上にものぼる注入スリーブ内のより多数の管
腔との間の必要な移行部を構成する。
以下、図1乃至図6を参照して、本発明の原理に従って構成された例示的注入
カテーテル10を説明する。注入カテーテル10は、半径方向に拡張可能な注入
スリーブ12と、該スリーブ12内の半径方向に拡張可能な部分13と、マニホ
ールド部14と、軸16とから構成されている。ハブ18は、軸16の近位端部
に取り付けられており、注射器、ポンプ等の注入液源に接続してもよい。スリー
ブ12の遠位端部には、非外傷性先端部19が固定されている。軸の遠位端部2
0は、マニホールド構造14の近位管状延長部22内に固定されている。図1乃
至図6に示したように、軸16は、円形横断面積を有する金属皮下注射管である
。軸の長さは、カテーテル10の他の部分の長さに応じて決まり、カテーテルの
全長は、以下詳述するように大腿動脈を経て導入される冠状用途の場合、約
90乃至150cmである。
半径方向に拡張可能なスリーブ12は、中央収容部14(図2及び図3)と4
本の注入管腔26とを有する。スリーブ12の拡張可能部13の最遠位の2.5
乃至10cmに亘り、注入口28が形成されている。通常、スリーブの拡張可能
な部分13は、後述する図9に示すように半径方向に拡張可能とするために、線
32(図2)に沿って軸線方向に分割される。管腔26の各遠位端部は、代表的
には先端部19により封止されている。半径方向の拡張可能性を付与する他の構
造については、先に説明した。
マニホールド構造14は、内側管42上に同軸的に収容される外側シース即ち
管40を備えている。環状管腔44は、注入液を各注入管腔26内に誘導する。
環状管腔44は、バルーンカテーテル入り口56の近傍に形成された三日月形の
移行管腔部52(図5)を介して、管腔50及び軸16(図6)に接続されてい
る。バルーン入り口56は、カテーテル58に連通し、該カテーテル管腔58は
、代表的には0.5mm2乃至2mm2の範囲、代表的には約1.25mm2の
横断面積を有するバルーン収容部24に連通する。
次に図7乃至図9を参照すると、膨張バルーンBを有するバルーンカテーテル
BCを入り口56から導入し、バルーンBがスリーブ12の遠位先端部を通って
外側に延出させる。次に、注入スリーブ12を後退させたまま、バルーンを膨張
させて収縮させる。バルーンBを収縮させた後、図8に示すようにバルーン上を
遠位方向にスリーブ12を前進させる。次に、バルーンを膨張させることにより
、図9に示したようにスリーブ12の拡張可能部分13を拡張させる。
注入スリーブ12は、図9A及び図9Bに示したような代わりの横断面を有し
てもよい。スリーブ12′は、図2及び図3に示したようにカテーテルの外面上
