治療使用のためのその場で生成するゲル
本発明は、特に眼科分野における、治療使用のための徐放薬デリバリーシステ
ムに関し、生理溶液と接触すると、患者に問題を起こさずに、投与区域、例えば
眼の中でかなりの時間滞留することが見いだされているゲルマトリックスを形成
する水性液体組成物を提供する。このゲルは、薬の放出をコントロールしながら
長時間にわたって連続投与するのに利用することができる。
本発明による組成物は、徐放システムが有利であるとき、いくつかの状況、例
えば経口、舌下、鼻内及び膣内投与において使用することができるが、諸要件が
特に高いので、以下に主としてその眼内使用に関連して考察する。
眼への薬の局所投与の場合の最も効率のよい治療形態であるいくつかの状況が
あり、この目的で種々の点眼液が開発されている。しかし、本来のメカニズム、
例えば涙の量及び瞬きは、眼の表面からほこり及び汚れを除去することによって
眼を保護するのに重要であるが、薬のデリバリーを困難にする。
患者のかなり大部分が溶液を自分で眼に点眼するのにかなりの問題があり、し
たがって徐放システムの必要性が長い間認められていた。また眼との接触時間が
長い程、薬の効果が有利になるいくつかの治療状況がある。一つの考え方として
、活性の薬物を含有する固形インプラントを製造して、医師又は看護婦によって
好ましくは患者の眼瞼の一方の下、結膜に施用する。このタイプのインプラント
は、それを通して薬物が拡散される外膜層を有することが多い。放出速度は、膜
の構造及び薬物が組み入れられている内部マトリック
スに依存する。他の一つの考え方として、室温及びそれ以下において液体(非固
体)である組成物を眼に点眼し、温度上昇のためにポリマーマトリックス、例え
ばゲルを形成する。いくらか類似のアプローチは、塩イオンと接触するとある種
のポリマー溶液はゲルを形成するという事実を利用する。生理的に使用可能であ
り、そして生理塩溶液、例えば涙液と接触してゲルを形成するいくつかのシステ
ムが開示されている。この考え方によれば薬のデリバリーのために潜在的にきわ
めて有用であるシステムが得られることが容易に認められる。Laboratories Mer

与されると、試験物質フルオレセインは点眼5時間後でもなお持続することが挙
げられている。
これらの刊行物においては、ゲランゴムの名によって知られ、Gelrite(登録
商標)の商品名で市販されている、細菌Pseudomonaselodeaによって産生される
細胞外アニオン性ヘテロ多糖類が最も好ましいゲル形成成分として提示されてい
る。適当な形態の薬物及び場合によっては保存剤と共にイオン性物質を含有しな
いか又は極めて少量含有するこのゲル形成成分の水溶液が調製される。非固体組
成物は好ましくは等張性であるべきであり、強度調節物質、例えばマンニトール
が添加される。
驚くべきことに本発明者らは、先行技術の教示に反して上に挙げたタイプの組
成物が低張性に作られる場合かなり改善された薬デリバリーシステムが得られる
ことを見いだした。これら条件下形成さ
れるゲルは、比較的高い張度値、例えば等張条件下に形成されるゲルに比して眼
の中にかなり長く滞留するのでさらに効率的なデリバリーシステムを与えること
が見いだされた。
したがって本発明は、医薬として活性な薬物及び眼内投与のために使用される
とき生理溶液、例えば涙液又は体液と接触するとその場で液−ゲル相転移をする
タイプの少なくとも1種の親水性ポリマーよりなる低張性眼用溶液に関する。
本発明の他の一面では、本明細書中定義される組成物を使用する薬物の投与法
が提供される。
親水性ポリマーは、イオン強度を生理値に上げるときポリマーの水溶液が固形
ポリマー構造物、例えばゲルを形成しうるかどうかによって機能的に定義される
。この群に属する成分の例は、多糖類、並びに天然及び合成多糖類誘導体を含む
。現在のところ好ましい成分は、ゲランゴムの名によって知られ、Gelrite(登
録商標)の商品名で市販されている、細菌Pseudomonas elodeaによって産生され
る細胞外アニオン性ヘテロ多糖類である。ゲル形成成分は、より一般的には、投
与前液体であるが、生理流体と接触すると該液体と生理流体との間の成分の交換
のためにゲルを形成する任意の成分であってもよい。上記成分の交換は、蛋白、
例えばリゾチームと上に論じたイオン性成分とを含むが、それらに限定されない
。Gelriteが使用される場合のゲル形成成分の濃度は、好ましくは0.1〜10重量%
、特に0.1〜2重量%の範囲である。
本発明の思想は、上に挙げたとおり親水性ポリマー溶液を含有する非固体組成
物が低張性であるということである。しかし、組成物を使用するとき低すぎる張
度は望ましくない副作用、例えば大なり
小なり激しい初期刺激を起こすので、張度は低すぎるべきでない。
非固体組成物のモル浸透圧濃度は、したがって等張液のもの(約290mOs/kg)
より低くなるべきであり、本発明によって選択された範囲は、約25〜200mOs/kg
、好ましくは25〜150mOs/kg、特に50〜150mOs/kgである。現在のところ最も好
ましい組成物は100mOs/kgの値を有する。
組成物中含有される薬物成分は好ましくは、粒子のサスペンジョンとしてか又
はエマルジョンとして、分散系として存在するべきである。薬物は、粒子の形態
の担体物質、例えばポリマー(生分解性、非生分解性又は生理流体と接触すると
溶解又は分解するポリマー)、脂質又はイオン交換樹脂中組み入れるか又はそれ
と組み合わすこともできる。
EP424042中組成物が点眼されるときpHの変化のために溶解する粒子の形態で薬
物が存在するGelriteタイプの組成物をベースにしたシステムが開示され、組成
物のpH及び緩衝力は、涙液が固体微粒子を溶解するのに十分なものである。
眼科活性物質を含有する脂質粒子の製法の例は、EP437368に示されている。
組成物中に含有される薬物成分は、溶液としても存在することができる。それ
は、表面活性剤、脂質によるか又はコンプレックス形成剤によって可溶化してよ
い。
本発明によって投与される活性薬物は、例えば眼科使用のため、多数の成分の
中で選ぶことができる。上記成分の例は次のものを含む。
−抗ヒスタミン剤及びうっ血除去剤、例えばピリラミン、テトラヒ
ドラゾリン、アンタゾリン及びそれらの類縁体、
−抗炎症剤、例えばフルビプロフェン、ジクロフェナック、アセチルサリチル酸
、コーチゾン、ヒドロコーチゾン、デキサメタゾン、プレドニゾン、インドメタ
シン及びそれらの類縁体、
−抗寄生虫及び(又は)抗原生動物化合物、
−抗菌物質、例えばテトラサイクリン、ペニシリン、バシトラシン、スルフォン
アミド及びそれらの類縁体、
−散瞳剤、例えばシクロペンタノール及びトロピカミド、
−細胞毒、例えば5−フロロウラシル、
−抗緑内障薬、例えばEP0364417に開示されているオメガ鎖修飾誘導体のような
プロスタグランジン(特に13,14−ジヒドロ−17−フェニル−18,19,20−トリノ
ル−PGF2a−イソプロピルエステル)及びチモロール、ピロカルピン、エピネフ
リン、ジピバリルエピネフリン及びそれらの同族体、
−抗ビールス剤、
−抗酸化剤、
−抗白内障剤、
−抗アレルギー剤、
−麻酔剤。
組成物は、他の添加剤、例えばポリマー、保存剤、非イオン張度剤、可溶化剤
、緩衝剤、コンプレックス形成剤及びキレート剤を含有してよい。
