患者モニタ装置のためのデータ収録ポッド装置
発明の背景 本発明の分野
本発明は、医療用装置、そして特定化すれば患者に関する医療データを収集し
、蓄積し、そして表示するための患者モニタ装置に関する。従来技術の説明
病院および他のヘルスケア環境においては、患者からの医療データの変化を連
続的に収集し、そして分析することがしばしば必要となる。それらのデータには
、心電図信号、体温、血圧、呼吸、脈拍および他のモニタされるべき生命信号が
含まれる。
関連する技術におけるモニタ装置は、標準的には2つの総体的カテゴリのうち
のどちらか一方に属するものであって、その1つはマルチ機能の監視、記録およ
び表示装置であって、これは必要なデータのすべてを処理し、そして収集するも
のであるが、しかしこれらは大きくそして運搬が難しい、そして他は運搬が容易
な小さい、携帯可能な装置であるがしかし、より少ない形式のデータを処理し、
そして収集するものであり、そして制限された蓄積容量を有している。(たとえ
ば
救急車または緊急処置室における)初期時点には、患者には簡単な、携帯可能な
モニタが接続されて、EKGまたは非侵入血圧のような、限られた数の医療用特
性が観察される。患者が(たとえば集中治療室または手術室のような)より高度
な治療施設に移るに従い、それらの簡単なモニタを強化して付加的なパラメータ
も観察することが望ましい、一般的には、このことは患者から簡単なモニタを外
し、そして患者により大きな能力を持つモニタ装置を接続する事によって行われ
る。
データ収集および表示の連続性に関する必要は、緊急事態において最も強く求
められる。病院職員は、付加的なパラメータをモニタすることを、観察されてい
るパラメータの選択を変えることを、または患者の履歴から付加的なデータを取
り戻すことを望む。同時に、患者が別の治療室に移動させられるかもしれない、
非常事態においては、患者がベッドから手術室または集中治療室に移送されるス
ピードが、実質的に患者の生死を分ける強い影響を与えるものである。病院職員
は、装置に機能を迅速に付け加えて、そして別の治療室に行くことを可能にする
必要がある。
モニタ装置の設計における2つの重要な配慮は、装置の構成変更の容易さおよ
び速度である。特に、移送の直前に患者にセンサを接続するか、またはそれらを
取り外すことや、あるいはクリティカルな処理の管理
は望ましいものではない。カダヒー他による米国特許第4,715,385号お
よび第4,895,385号は、固定された場所の表示ユニットおよび携帯でき
る表示ユニットを含むモニタ装置を説明している。ディジタル収録および処理モ
ジュール(DAPM)は、患者に取り付けられたセンサからのデータを受け取り、そ
してそのデータを固定された、および携帯できる表示ユニットの一方または両方
に提供する。DAPMは患者移送の間患者に取り付けられたままであり、移送に先だ
って患者から邪魔な装置を取り外したり、移送の後に装置を再接続する必要は排
除されている。通常は、DAPMは患者のベッドの近くに設けられたベッドサイド表
示ユニット内に挿入されている。ベッドサイド表示への電気的接続は、DAPMがベ
ッドサイド表示内に挿入された時に形成されている。
患者を移送する必要が生じたとき、以下の2つの構成変更ステップが実行され
る。最初にDAPMがケーブルを取り付けることによって携帯できる表示に接続され
、そして次にこれをベッドサイド表示ユニットから取り外すことによって、ベッ
ドサイド表示から接続外しされる。いったんDAPMがベッドサイド表示から接続外
しされると、携帯可能な表示およびDAPMを含む可搬形モニタ装置が形成される。
ベッドサイドモニタにDAPMを取り付けるために、ベッドサイド表示ユニットに達
するようセンサとDAPMとの間には充分なケーブル長が設
けられる。
ベッドサイドモニタ内にDAPMを挿入するために、患者からDAPMまでアナログデ
ータ信号を伝達する線は、患者からベッドサイドモニタに達するよう充分長くさ
れる。このケーブル長は、アナログ信号間の望ましくない干渉を生じさせ、そし
てたとえば、外部源からの無線周波数干渉(RFI)による雑音によってアナログ
信号を劣化させる恐れがある。
さらに、カダヒー他によるディジタル収録および処理モジュールの装置は、固
定されたパラメータ構成を持っており、そして患者の状態の変化によって必要と
されるパラメータが変化するならば、ディジタル収録および処理モジュールが接
続外しされねばならず、そしてモニタされる必要のある新しいパラメータを含む
異なるモジュールが接続されなくてはならない。この処理は、センサの、そして
患者とモジュールとの間のケーブルの再接続によって時間を消費するだけでなく
、最初の処理モジュールにおいて収録されていた患者のデータが、モジュールが
接続外しされたときに失われてしまうので、データの破壊をも生じさせる。さら
に、カダヒー他の処理モジュールは、大きくそして患者の近くに位置させること
は難しい。加えて、カダヒー他の処理モジュールは、異なる患者のセンサへの拡
張ケーブリングを必要とし、これはさらに複雑さと装置のセットアップ時間を加
えるものとなる。
モニタ構成にセンサを加えるときに遭遇するであろう時間遅延を別にしても、
従来技術による装置はまたケーブル管理に関して必要となる多くの課題を残して
いる。標準的な患者モニタ装置においては、多数のケーブルが患者とモニタとの
間に存在する。従来、モニタされる各パラメータに関して少なくとも1つのケー
ブルが加えられてきた。たとえば、EKGのために5本のケーブルが、心臓出力
のために2本のケーブルが、温度のために2本のケーブルが、そして侵入センサ
を用いる血圧測定のための4本のホースが存在する。このケーブルとホースの配
列は、患者のベッドの周囲を移動する人にとっては妨害となる。ケーブルとホー
スの数がより多くなれば、誰かが偶然にそれらの1つを中断させる危険もより大
きくなる。このことは幾つかの売り主から市販されている従来装置における共通
的な問題である。
従来技術のデータ収録装置の別の特色は、それらがスタンドアロン装置ではな
いことである。1つの例は、シーメンス医療装置社によって製造されているSire
custTMカートリッジ装置である。患者の医療データは1つまたはそれ以上のマル
チパラメータカートリッジによって収集される。データを表示上に表示するため
に、このカートリッジはSIREMTMモジュールボックス内に挿入される。モジュー
ルボックスの大きな寸法は、ボックスをベッド上またはバッドの上方に設けるこ
とを
実際的ではなくさせ、これは標準的にはベッドの横に置かれる必要があり、そし
て患者を取り扱う病院職員の通行を妨害する。ボックスが道をふさぐのみならず
、前に説明したとおりカートリッジと患者との間のケーブルの配列は病院職員の

装置ではない。
手術室または集中治療室に運ぶために患者およびモニタ装置を準備する時間を
減少させるため、付加的な単純さが望まれる。収集されたアナログ信号における
雑音およびRFIの減少もまた望まれている。(モジュールボックスを含めた)全
体のデータ収録装置の全体寸法の減少が望まれる。
本発明の概要
本発明は、表示を含む装置において用いることが適切なデータ収録ポッドにお
いて実施される。このポッドは、医療患者に結合された複数のセンサを用いて患
者の状態の少なくとも2つのパラメータを表す複数の
アナログデータ信号を連続的に収集する。データは、データ収録ポッドを用いて
収集されるが、患者は固定されていてもよく、また患者は運搬されても良い。
データ収録ポッドは、複数のセンサからの少なくとも2つの異なるパラメータ
を表す患者データ信号を受け取る。本発明の1つの特色によれば、患者データ信
号はデータ収録ポッド内の回路によって表示装置に伝送されるよう条件づけされ
る。回路は、センサの近くに選択的に設けられる。本発明のべつの特色によれば
、データ収録ポッドは、スタンドアロン装置であり、そして選択的に表示装置に
直接的に結合される。回路は表示装置とは独立して位置決めされる。

実施例の説明
第1図は、本発明による2つの例としてのデータ収録ポッド10および30を
含む装置を示している。ポッド10は、EKG電極に結合するための複数の端子1
4および、温度、鼻呼吸または心臓出力熱希釈のような、抵抗センサに結合して
いる複数のマルチ機能端子12とを含んでいる。それらのセンサからのリードは
、複数の受信線16によって端子12に接続される。圧力ポッド30は、(示さ
れていない)圧力トランスジューサからの信号を受け取るための端子23a-23
d、および温度センサからの信号を受け取るための端子24aおよび24bを含ん
でいる。電極および(示されていない)センサは、患者に取り付けられ、そして
患者が固定されているときも、患者が運ばれているときも、患者にそしてポッド
に取り付けられたままである。ポッドは、ベッドサイドテーブル上に置かれるか

従来技術によって知られているデータ収録カートリッジとは異なり、データ収
録ポッド10および30は前もって構成された、スタンドアロン(内蔵型)の、
マルチパラメータユニットである。前もって構成された装置としてポッド10お
よび30は、センサからの
アナログ圧力信号を受け取り、その信号をフィルタし、それらを単独の多重化さ
れたアナログ信号に組み合わせ、そして単独の信号をアナログ形式からディジタ
ル形式に変換するために必要なすべての電子回路を含んでいる。このディジタル
出力信号は、電線34または(示されていない)(たとえば赤外線の)ワイヤレ
スリンクによって(データ処理機能を含むことができる)表示装置20に直接的
に転送されることができる。(従来技術カートリッジとは異なり)スタンドアロ
ン装置として、ポッド10および30は表示装置への電気的なパスを形成するた

データ収録ポッドは、携帯型表示であっても良い表示20に単独の結合用線1
8によって選択的にそして取り外しできるように結合される。何らかの理由があ
れば、表示20をさらに別の選択された(示されてい
ない)表示に置換する事も望ましく、このことは古い表示から単独の結合用線1
8を取り外し、そして線18を新しい表示に取り付けることによって行われる。
別のデータ収録ポッド30もまた示されている。ポッド30は、血圧または圧力
および温度の両方を表すデータを受け取るように適用される。EKGポッド10お
よび圧力ポッド30の各々が本発明の特色を含むものであっても、それらは第1
図に示されるように1つのデータ収録装置内に互いに他と組み合わせて用いるこ
とができる。ポッド10および30は互いに他に関して、そして表示20に関し
て独立的である。ポッド10および30は患者の近くに都合良く設けることがで

センサが取り付けられている受信線16に長さによって決められるような、患
者とポッドの間の電気的パスは、実質的にポッドを表示20に結合する結合線1
8または34の長さよりも短い。以下に説明される回路によって構成されるデー
タ収録ポッド10または30は、センサと共に配列される程充分にコンパクトで
ある。EKGポッド10は、患者の近くのベッド上に置かれることが出来、そして
(示されていない)圧力トランスジューサは、それらをポッド30のハウジング
のチャンネル38内に挿入することによってポッド30上に取り付けることがで
きる。
データ収録ポッド10と表示20との間を結合する
データケーブルは、赤外線リンクであるあるような、(示されていない)ワイヤ
レス通信リンクのような、収録されたデータ信号を表示20に通信することので
きるあらゆる方法を含むことを意図している。
第2図は、例として示されるデータ収録ポッド11のブロック図を示している
。ポッド11は、第1図に示されているEKGポッド10および圧力ポッド30の
どちらにも共通な機能を発揮する。ポッド11は、複数のセンサ410a−41
0nからの患者データを受け取る。センサ410a−410nは、患者に取り付け
られたセンサのサブセットであっても良い。これらのセンサは、EKG血圧、脈拍
、温度、EEGまたは他の物理的パラメータを測定する。各入力データ流は、雑音
およびセンサが収録するかもしれない(たとえば無線周波数(RF)歪みのような
)いかなる望ましくない信号を除去するためにフィルタされる。このフィルタさ
れた信号は増幅される。フィルタされた出力信号420a−420dは、単独の信
号を形成するよう組み合わせられる。この組み合わせは、時分割マルチプレクサ
414によって実行される。その代わりに、当業技術者にとっては理解されてい
るように、(たとえば周波数分割多重のような)単独信号を形成するためにデー
タを組み合わせる他の方法も用いられる。組み合わせられた信号は次に、A/Dコ
ンバータ412によってアナログ形式からディジタル形式に変換される。以下の

明においては、組み合わせ装置はマルチプレクサとして参照される。A/Dコンバ
ータ412は、表示20への単独のデータ通信結合19を含んでいる。この結合

よび30は、第2図において示されるポッド11の全ての機能を含んでいること
ができる。
ポッド11に結合された各センサによって提供されるデータは、それぞれ異な
るレートにおいてサンプルされる。マルチプレクサ414は、クロック信号を提
供するための、そしてそのそれぞれに割り当てられたサンプリングレートに従っ
て少なくとも1つの時間分割の間にそれぞれのセンサに結びついたアナログ信号
をサンプルするために設けられている制御装置430によって制御される。セン
サの出力信号のそれぞれを表すデータは、時分割の一部分の間の単独の時分割多
重された出力信号415に含まれる。この方法において、各々がその望ましいサ
ンプルレートにおいて、全ての信号が単独の線上で運ばれる。
第1図の説明において指摘したように、患者とポッド10との線16は短く、
一方ポッド10と表示20との間の結合線18は長い。この構成は、患者とアナ
ログのフィルタ/増幅器418a-418nとの間のアナログ信号が運ばれる電気
的パスを短くする。増幅に先だって、各信号に加えられる雑音は、増幅よりも前
に信号が運ばれる電気的パスを短くすることによって減少する。さらに患者から
の信号の増幅が実行され、そして患者への結合のインピーダンスが不均衡である
ならば発生し得る、雑音および信号伝達による変化が回避される。信号条件づけ
回路と患者から離れた雑音源との間の容量を減少させることによってEKG回路の
ためのポッド10において電気的な絶縁を設けることにより、干渉がさらに減少
される。
EKG信号をモニタする上で、このことは特に重要である。室内には(照明にま
たは電気的サージの大きなユニットによる)電界傾斜が存在する。もし線16が
極めて長く、そしてポッド10が患者から離れたモニタ20の近くに置かれてい
るならば、この電界傾斜は患者とポッドとの間との電位に著しい差異を生じさせ
得る。電流がポッドと患者との間を流れる。もし電極が等しいインピーダンスを
持っているならば、これは差動増幅器によって除去されるコモンモード電流とな
る。しかし、もし電極抵抗が(普通そうであるように)互いに異なっているなら
ば、電極間には差電圧コンポーネントが存在する。この差電圧は増幅され、そし
てEKG波形における雑音となる。
反対に、この実施例においては、ポッド10は患者に接近して設けられ、そし
て低い容量絶縁がポッド内に設けられており、患者に結合した回路とモニタ20
に結合した回路とを絶縁する。患者とポッド10との
距離を短くすれば、電界傾斜による電位差が小さくなり、そしてそのため非均等
電極インピーダンスによる差電圧雑音コンポーネントが小さくなる。
ポッド10においては、信号は、ポッド11と表示20との間の比較的長い結
合線を通して伝送される前に、A/Dコンバータ412a−412nにおいてディジ
タル形式に変換される。一旦ディジタル形式に変換されると、データは、信号と
RFIとの間のクロストークによる劣化に関して影響を受けにくくなる。
不揮発性メモリ装置432がポッド11内に含まれている。メモリ432は一
般的な電気的に消去可能なプログラム可能な読み出し専用メモリ(EEPROM)であ
っても良い。メモリ432は、患者の統計的データ、センサに関するアラーム限
界、および患者の傾向データを蓄積することができる。それらのデータをポッド
11内に蓄積することによって、(たとえば表示20の代わりに別の表示を用い
るような)機器変換が簡単になる。表示20から他の表示へのデータダウンロー
ドのための時間消費の必要なく、ポッド11が表示20から接続外しされ、そし
て別の表示に再接続できる。
メモリは、(波線で示されているように)ポッド11のハウジングの外側に設
けられた、そしてポッド11に接続されることができる別個のメモリ装置434
であっても良いことは、当業技術者にとっては理解で
きる。メモリ434は選択的にポッド11から取り外しできる。単独のメモリ4
34が、センサ410nおよびフィルタ418nに結合されて示されている。同様
なメモリが各センサに結合されることができる。もし各センサがそれぞれのメモ
リ434を持っているならば、患者が移動するときにメモリ434をセンサに添
付することができるので好都合である。こうして、ある1つの部門が、(たとえ
ばポッドが戻されないと予想される自部門以外の)病院の別の部門に患者と共に
そのポッド11を送ることを望まなければ、メモリ434はポッド11から取り
外され、そして病院の別の部門への移送のために別のポッドに接続する事ができ
る。このことは、選択的にポッド11を表示20に結合させることができるとい
う付加的な素子の柔軟性を提供する。
第3a図は、EKGデータ収録ポッド10を形成する回路を示すブロック図である
。基本的なポッド構成は、3つの印刷配線板(PCB),EKG PCB100、SPO2 P
CB200およびオプト−絶縁パワーボード134、を含んでいる。EKG PCB10
0は、EKGおよび温度信号を受け取り、そして処理するための回路を含んでおり
、ここでは温度信号は呼吸または心臓出力データを代表している。オプト−絶縁
パワーボード134は、電源および、残りのセンサ装置によって用いられるリタ
ーン回路から分離されたEKG PCB100のためのリターン
回路ろを備えることによって、患者の安全を確実にする。SPO2 PCB200は血
液酸素飽和圧力センサからのデータを受け取り、そしてポッド10と表示20と
の間の通信を取り扱う。付加的なPCB280および290は、一方では脳波(EEG
)データを、そして他方では経口酸素および二酸化炭素圧力(TCP O2/CO2)デー
タを、それぞれ収集するために設けることができる。
第3b図は、ポッド10のEKG印刷回路ボードPCB100の機能ブロック図であ
る。EKG PCB100は、5つのEKG電極(102、104、106、108および
110)、2つの心臓出力サーミスタまたは鼻サーミスタ、および2つの体温サ
ーミスタからのデータを受け取り、そして処理するための回路を含んでいる。EK
G PCB100は、それぞれの電極の各々から受け取られたEKG信号を処理するため
のフィルタおよび増幅器(112、114、116、118および120)を含
んでいる。一般的な信号処理アルゴリズムがそれらの電極からの信号に加えられ

EKGリードが2つの電極間の電圧差を比較することによって形成されることも
、当業技術者にとってよく知られている。5つのフィルタ/増幅器回路(112
、114、116、118および120)からの出力信号は、マルチプレクサ1
22および124内で多重化され、そして差動増幅器128に提供される。この
構
成を用いることによって、5つの電極(102、104、106、108および
110)の選択された7対から7つのEKG信号が得られる。
マルチプレクサ122からの出力信号123は、バッファ126内に蓄積され
、そして遅延された信号127がバッファによって備えられる。マルチプレクサ
122からの出力信号123はまた、差動増幅器128にも提供される。バッフ
ァ126は差動増幅器128によって生じる信号遅延を補償する遅延を提供し、
その結果信号127および129は同じ時間周期の間に収集されたデータを表現
する。差動増幅器は、マルチプレクサ122および124、それぞれからの入力
信号123および125を受け取る。差動増幅器128は、信号123から信号

126からの出力信号127はマルチプレクサ/アナログ−ディジタル(A/D)

マルチプレクサ122もまた、温度センサ150、152、154および15
6からの入力信号を受け取る。センサ150および152は、2組の体温データ
または1組の体温データと1組の鼻呼吸データを収集する事ができる。もし呼吸
データが望まれるのであれば、センサ152は、気道を出入りする空気の温度変
化に応答する、鼻または口に置かれた迅速に応答する温度センサとする事ができ
る。こうして、吸入された、そして排気された空気の間の温度の差による鼻通過
温度における変化が測定される。センサ150および152から収集されたデー
タは、それぞれ一般的なフィルタ装置160および162によってフィルタされ
る。
第1図の説明において、指摘されたように、端子12は温度、鼻呼吸、または
心臓出力を表すデータを受け取ることができる。第3b図は、第1図の実施例の
変形を示しており、ここでは分離された端子12a−12dが2組のセンサ、温度
150、152および心臓出力または鼻呼吸のいずれかである154、156、
からのデータを受け取る。付加的端子が温度、呼吸および心臓出力データの同時
収集のために設けられるような、さらに別の変形が可能であることは理解できる
。
センサ154および156はエドワード型カテーテル内に設けられ、そして熱
希釈方法を用いて心臓出力データを提供する。(示されていない)エドワード型
カテーテルは、冠状動脈内に冷却されたまたは室温の、いずれかの水を注入して
使用される。センサ154および156を校正するために、エドワード校正抵抗
器180もまた(示されていない)カテーテルプローブ内に設けられる。下流血
液温度および注入温度が測定
される。心臓出力の測定で得られたそれらの比較は、当業技術者にとってはよく
知られている。
それぞれのフィルタ160および162からの出力信号161および163は
、マルチプレクサ168に提供され、これはそれら信号の互いに時分割多重を行
う。マルチプレクサ168からの出力信号169は次に、増幅器172に提供さ
れる。同様に、フィルタ164および166からの心臓出力信号が校正信号と共
に多重化される。マルチプレクサ170の出力信号171は増幅器174に提供
される。増幅された信号173および175は、互いに時分割多重され、そして
マルチプレクサ122に提供される。
EKG新Bぞう出力、温度および鼻呼吸センサからの測定値を取り扱うために、
マルチプレクサ122、124および176はそのそれぞれのサンプルレートに
従って各センサからの信号をサンプルするよう制御される。たとえば、EKGチャ
ンネルは、心臓出力のために用いられるレートの100倍でサンプルされる。
マルチプレクサを制御するために用いられる制御およびクロック信号133は
、EKG通信用用途特定集積回路(ASIC)132を通して提供される。EKG通信用AS
IC132は、EKG PCB100とオプト−絶縁パワーPCB134との間にデータ転送
させる。オプト−絶縁パワーPCB134は、第3c図に示されるようにEKG/呼吸回
路とSPO2 PCB200との間に5000ボルトの電気
的絶縁を維持することによって患者を保護する。同時に、SPO2 PCBを通して携
帯可能な表示20とEKG PCBとの間で電源およびデータの転送することによって
ハードウェア効率が得られる。ラインレシーバ144、ラインドライバ146お
よび直流電源148の、表示20への接続は、EKGリターン136および表示2
0のメインエンクロージャの両方から絶縁されている分離されたリターンを共有
する。
第3c図は、SPO2 PCB200を示すブロック図である。SPO2 PCBは、たとえ
ばネルコール社によって製造されたような市場で入手できるセンサである、SPO
2センサ250からのデータを受け取り、そして処理するための回路を含んでい
る。SPO2センサ250は、2つの発光ダイオード(LED)252および254を
含んでおり、それらは(たとえば耳たぶ、指先、またはつま先のような)患者の
体の一部をそれぞれ光線が通過するよう赤色光及び赤外光線を発散する。フォト
ダイオード260は、患者の組織の中を通過下周波数の帯域において特に光線の
量を測定し、そして患者の血液における酸素の相対的な飽和を表す信号を提供す
る。
