Description
【発明の詳細な説明】 〔産業上の利用分野〕 本発明は骨腫瘍や外傷などにより、下顎骨を切除し、欠
損部を補綴する場合に、顔面本来の形状を再現すると共
に、咬合機能を回復するための人工下顎骨に関するもの
である。

（従来の技術） 従来は、骨腫瘍や外傷などにより、下顎骨が欠損した場
合に、腸骨を切除した骨を骨移動し、ワイヤー固定を行
ったり、金属のプレートを残存骨にスクリュー固定す
る、顔面形状回復術が行われていた。

（従来技術の問題点） 上記の如く、人体の他の部位から採取した骨を移植する
ことから、下顎骨本来の形状に骨を整形して移植するこ
とが難しく、骨吸収を起こすという欠点がある。またほ
とんどの場合、腸骨から採取していたが、腸骨稜は中殿
筋の起始部であるため、採骨することにより中殿筋力が
弱くなる。その結果、下肢の筋バランスを崩し、パウエ
ルスの理論に基づくトレンデンブルグ跛行を招き易いと
いう不都合があった。
ことから、下顎骨本来の形状に骨を整形して移植するこ
とが難しく、骨吸収を起こすという欠点がある。またほ
とんどの場合、腸骨から採取していたが、腸骨稜は中殿
筋の起始部であるため、採骨することにより中殿筋力が
弱くなる。その結果、下肢の筋バランスを崩し、パウエ
ルスの理論に基づくトレンデンブルグ跛行を招き易いと
いう不都合があった。
そのために、金属プレートを曲げて、残存骨にスクリュ
ー固定する方法も用いられているが、下顎骨の形状を再
現することは難しく、また下顎骨本来の機能である、咀
嚼するための義歯を付けることが出来ないため、食生活
が満足に出来ないという欠点があった。

上記の如き問題点に鑑みて、本発明では人工下顎骨部材
の上部に義歯を冠着するためのピンを形成したことを特
徴とする。

第２図乃至第４図には本発明実施に係る人工下顎骨部材
を示し、この人工下顎骨部材は、アルミナ、アパタイト
などのセラミック材または、ステンレス鋼、Co-Cr-Mo合
金、純Ti、Ti合金などの金属材料でも良く、さらにこれ
ら金属材料にアルミナ、アパタイト、TiO2、TiNなどを
コーティングしたものから成り、外傷による骨欠損や第

人工下顎骨部材を挿入し、固定用穴としてのスクリュー
穴４を通して、スクリュー７でもって残存量骨に固定す
るか、もしくは固定用穴としてのワイヤー穴５にワイヤ
ーを貫通させ、残存骨と締結させても良く、または、両
者を併用して固定しても良い。

このような本発明に係る人工下顎骨部材３の使用態様例
を第５図に示すが、下顎骨１の欠損部に人工下顎骨部材
３を押入させることによって、下顎骨骨１の形状は正常
に修復補正が可能であり、さらに人工下顎骨部材３の上
部に設置したピン６に、義歯８を冠着させることによ
り、咬合機能が回復出来る。

欠損した部分に人工下顎骨部材を押入させることにより
患者自身の顔面の審美性を維持し、さらに、義歯を冠着
させる機能を設けたことにより、人間の食生活に不可欠
な下顎骨本来の咬合機能の回復が可能となる。さらに、
患者を自家骨採取による負担から、開放したり、腸骨稜
の切り取りに伴う中殿筋の損傷を回避できるなど人類福
祉にとって寄与するところ大である。
欠損した部分に人工下顎骨部材を押入させることにより

- 【請求項１】下顎骨欠損部を補綴すべく該欠損部の形状
に適合せしめた構造物であって、該構造物の上部に義歯
を冠着するためのピンを備え、かつ残存骨と接合するた
めの固定用穴を有することを特徴とする人工下顎骨部
材。
