【考案の詳細な説明】 [産業上の利用分野] この考案は主に歯科用パノラマX線撮影装置に患者を切
端咬合位置を保持して位置付けするためのマウスピース
に関するものである。

[従来の技術] 従来,歯科用パノラマX線撮影装置用マウスピースとし
ては,上下顎歯列を歯列弓に沿った5mm程度の厚みのパ
ノラマX線断層域の同一面上に位置付けるため,また上
下の歯顎の重複を避けるため,切端咬合位と呼ばれる位
置付け技術が行われており,例えば実開昭56−363
06や実開昭60−17705で開示される顎載せ台に
上下調整自在に支持された切端ゲージの先端部に前歯部
上下を咬み合わせるための溝を設けたものがある。(以
下先行技術Aという)また,実公昭57−57689の
第6図で表された断面ほぼT字形で歯列弓に沿った曲線
を有するマウスピースが知られている。(以下先行技術
Bと言う) [考案が解決しようとする課題] 上記の先行技術Aでは,切端ゲージの先端部に前歯部上
下を咬み合わせるための溝を対向して設けている。しか
し溝形状は曲線状ではなく浅い直線状とされ,せいぜい
門歯分の巾しかなく歯列弓形状に合致させづらいばかり
か咬合部と顎載せ台とで患者頭部はある程度位置決めで
きるものの,顎の位置は顎載せ台上で前後するため,頭
部の前傾や後傾については術者がチェックするしか対応
できなかった。

(Hereinafter, referred to as Prior Art B) [Problems to be Solved by the Invention] In the above Prior Art A, a groove for engaging the upper and lower front tooth portions is provided at the tip of the incision gauge so as to face each other. However, the groove shape is not a curved shape but a shallow linear shape, and at most there is only the width of the incisor, which makes it difficult to match the shape of the dental arch. Since the position moves back and forth on the chin rest, the operator could only check the forward and backward tilt of the head.

また,先行技術Bでは,断面ほぼT字形で歯列弓に沿っ
た曲線を有する咬合部を有するため,歯列弓へのフィッ
トはするものの口腔内に挿入される部分の厚みは咬合部
の厚みと同じで溝は形成されていない。従って,患者が
自然にマウスピースを咬んだ場合,通常は下顎歯列が上
顎歯列の内側に位置する自然咬合位となるが,自然咬合
位でマウスピースを咬むことも出来る構造であるため,
時にはX線断層域から下顎歯列がはずれ鮮明なX線像を
得られない事があった。また,パノラマX線撮影には約
20秒前後の時間がかかるため,この間に咬合位が変化
するとX線像の画像が乱れるもとになるが先行技術Aお
よびBにおいて,咬合位の変化をほぼ完全に押さえるこ
とは出来なかった。したがって本考案の目的は,パノラ
マX線装置用マウスピースを患者が咬合した状態におい
て歯列弓がずれてしまうことを確実に防ぎ,高精度で歯
列弓の位置を位置決めすることができるパノラマX線装
置用マウスピースを提供することである。

[作用] 本考案に従えば,パノラマX線装置用マウスピースは上

方向に対向して形成される。このような各凹溝を咬合し
た状態では,少なくも門歯および側切歯の一部が嵌まり
込み,上顎および下顎の各前歯を中心とした角変位を阻
止することができ,これによってX線撮影における歯列
弓のずれを確実に防ぐことができる。

大略的に直方体状であって,その前端部2寄りの上下面
には,前歯により咬み合せ溝3を歯列弓形状に近似して
凹設した曲線状凹溝3が上下面にマウスピース1の厚み
方向(第2図の上下方向)対向するよう設けられてい
る。凹溝3の形状は望ましくは,第2図に示すように頬
側はほぼ垂直面5とし,舌側は歯牙形状に合わせて約4
5°の傾斜面6とし,凹溝3の深さは約5mm前後であ
り,また幅も第3図に示されるように,歯列弓mの前歯
部である門歯(第1切歯ともいう)10a,10bおよ
び側切歯(第2切歯ともいう)11a,11bの一部に
もかかるように,すなわち門歯10a,10bおよび側
切歯11a,11bの一部を含む範囲として,その幅L
=約20〜40mmとすれば,溝3の中に前歯部の上顎
7と下顎8とが確実に保持され,特に溝の舌側側傾斜面
6により,下顎8が舌側側へ移動する事を防ぐことがで
きる。

り,患者の咬合位置のずれを確実に防止し,正確に位置
決めして撮影することができる。
