Sheet guides 6 and 7 are arranged in the conveyance path of the form 2, and the form 2 is conveyed along the sheet guides 6 and 7. In FIG. 2, the reading apparatus according to the first embodiment includes an upper transport unit 21 and a lower transport unit 22.
