Description
本発明は鎮痛剤オキシコドンの経口および非経
口濫用の潜在性を減少させるためのオキシコドン
−ナロクソン組成物に関する。
強力な鎮痛剤例えばオキシコドンまたはプロポ
キシフエンは実験動物および人に身体および精神
依存性を生じることが認められている。この濫用
潜在性の故にこれらの薬剤を鎮痛剤として使用す
ることが相当に制約される。本発明は、強力な鎮
痛剤、特にオキシコドンをナロクソンと組合せて
投与して更に広大な鎮痛用途を可能とする強力な
鎮痛剤の濫用潜在性を減少させる組成物に関す
る。
1973年11月20日付けでI.J.パヒタ(Pachter)
およびM.ゴードン(Gordon)に付与された米国
特許第3773955号には、非経口投与時に無痛覚を
生じない経口で有効な鎮痛組成物が記載されてい
る。このような組成物の例(重量部)にはオキシ
コドン2−20部当りナロクソン1部およびデキス
トロプロポキシフエン(プロポキシフエン)12−
120部当りナロクソン1部を包含している。また、
鎮痛経口投与量当りナロクソン0.1−約10mgを含
む経口単位投与量形態の鎮痛剤も開示されてい
る。
1970年2月3日付けでM.J.レ−ウエンスタイ
ン(Le−wenstein)等に付与された米国特許第
3493657号には、ナロクソン(すなわち、N−ア
リル−14−ヒドロキシジヒドロモルヒノン)およ
びモルヒネまたはオキシモルフイン(すなわち、
14−ヒドロキシジヒドロノルモルフイン)の混合
物からなる組成物が開示されている。これらの組
成物は、望ましくないまたは危険な副作用例えば
錯乱、夢中状況、驚愕経験(すなわち、幻覚)等
を発現することなく、強力な鎮痛効果を生じると
言われている。この記載ではオキシコドンまたは
プロポキシフエンについて開示していないし、ま
してこの記載は任意のタイプの薬物の非経口およ
び(または)経口濫用の防止に何等関するもので
はない。
広義に記述すると、本発明は強力な鎮痛剤の経
口および非経口濫用のための潜在性を減少させる
組成物に係るものである。本発明の具体化は、鎮
痛投与量のオキシコドンを2.5:1乃至5:1重
量部の比を有し、ナロクソン1−3mgを含有する
オキシコドン−ナロクソン組成物として提供する
ことからなるオキシコドンの経口、非経口濫用の
潜在性を減少させるための組成物にある。
本発明は、強力な鎮痛剤オキシコドンの経口、
非経口濫用の潜在性が比較的特定された比で該鎮
痛剤を麻薬拮抗剤ナロクソンと組合せることによ
り減少させることができるとする知見に関するも
のである。本発明に従つてナロクソンを使用する
場合、強力な鎮痛剤の濫用潜在性が、これらの薬
剤の鎮痛活性に認めるに足る影響を及ぼすことな
く、実質的に低減される。濫用潜在性が減少した
結果、本発明により、ナロクソンと組合せた強力
は鎮痛剤オキシコドンは鎮痛剤に必要とするより
も一層低い麻酔コントロール予定にかけることが
でき、これによつてその用途が一層広大となる手
段が提供される。
本発明の好ましい態様は、強力な鎮痛剤オキシ
コドンを幻覚剤として経口、非経口使用する潜在
性またはそれによる身体または精神依存性の充足
の潜在性を減少させる組成物に関する。
本発明は、オキシコドンの鎮痛投与量を2.5−
5:1重量比を有し、ナロクソン1−3mgを含有
するオキシコドン−ナロクソン組成物として提供
することからなるオキシコドンの経口および非経
口濫用の潜在性を減少させる組成物である。
本発明の好ましい態様は、オキシコドンの鎮痛
投与量を2.5−5:1重量部の比を有し、ナロク
ソン1.4−2.5mgを含むオキシコドン−ナロクソン
組成物として提供することからなるオキシコドン
の経口および非経口濫用の潜在性を減少させる組
成物である。

量を3:1−4:1重量部の比を有し、ナロクソ
ン1−3mgを含有するオキシコドン−ナロクソン

ある。
更に好ましい態様は、オキシコドンの鎮痛投与
量を3.5:1重量部の比を有し、ナロクソン1.4−
2.5mgを含むオキシコドン−ナロクソン組成物と
して提供することからなるオキシコドンの経口お
よび非経口濫用の潜在性を減少させる組成物であ
る。
なお更に好ましい態様は、オキシコドンの鎮痛
投与量をオキシコドン2.5−10mgおよびナロクソ
ン1−3mgを含有するオキシコドン−ナロクソン
組成物として提供することからなるオキシコドン
の経口および非経口濫用の潜在性を減少させる組
成物にある。
オキシコドン2.5−15mgおよびナロクソン1−
3mgを含有する製薬組成物を通常の製薬手段に従
つて処方すると単位投与量形態を提供することが
でき、これらの形態には経口投与に適する固形製
剤例えば錠剤、カプセル剤、粒剤、粉剤、乳剤お
よび懸濁剤、ならびに非経口投与に適する液状製
剤が包含される。例えば、オキシコドン−ナロク
ソンの固形製剤は無機担体例えばタルクまたは有
機担体例えば乳糖またはデンプンと共に調製さ
れ、一方非経口投与に適した液状製剤は通常の希
釈剤例えば水または懸濁媒質例えばワセリン、ポ
リオキシエチレングリコール、植物油等と共に調
製される。組成物はまたその他の追加成分例えば
吸収剤、安定剤および緩衝剤を含有していてもよ
い。特に好ましい組成物はオキシコドン2.5−10
mgおよびナロクソン1−3mgを含有している。
オキシコドン−ナロクソン2.5:1−5:1重
量部からなる本発明の組成物は、動物での経口鎮
痛活性に関して、オキシコドン単独とほとんど区

示し、本発明のオキシコドン−ナロクソン混合物
の鎮痛活性および濫用潜在性の減少を明らかにす
る。
例 1
モルヒネ依存性猿での禁断抑制
モルヒネ依存性はモルヒネの連日皮下注射によ
り雄のリーサス猿に誘起させた。当初の投与量
7.5mg/Kg体重を14日間隔で徐々に増加して最終
維持投与量60mg/Kgとした。身体依存性は、モル
ヒネの中止に続く48時間後に徐々に発現する禁断
徴候で示された。これらの徴候には起毛、異常姿
勢の採択、発声の増大、歯のむき出し、しんせ

の証拠なし
1/4多分に極めて緩和な投薬中止徴候
1/4極めて緩和な投薬中止徴候、4日後に
は明白でない
上掲の結果によれば、オキシコドンは高度の身
体依属症傾向を有するものと分類され、これに対
して3.5:1の比のオキシコドン−ナロクソン混
合物は低度の身体依存性を有するものと分類され
る。

ラツトにおけるオキシコドン−ナロクソン鎮痛
反応
オキシコドンの鎮痛活性に影響を及ぼさないオ
キシコドンと組合せて投与するナロクソンの投与
量は、ラツトについて行つたダーマー
(D′Amour)およびスミス(Smith)の輻射熱
法、ジヤーナル・オブ・フアマコロジイ・アン
ド・エクスペリメンタル・セラピユテイクス(J.
Pharmacol.Exp.Therap.)72:74(1941年)に従
つて測定した。この方法では、痛覚欠如に用いる
終末点は経口もしくは皮下投与後30、60および
120分で求められる尾のふりであつた。鎮痛
ED100は、任意の試験間隔の1点において動物の
100%に個々のプレドーズ反応時間の100%の増加
を生じる投与量と定義された。痛覚欠如の遮断
は、付随して投与されるナロクソンの投与量が反
応時間で個々の鎮痛反応を100%の増加以下に減
少させた場合に明らかであるとみなした。次の結
