Abstract
Description
本発明は、例えば角膜の切開によって該角1151層面
上又は角Jul内に付設されるリフラクティテヶラI・
プラスティ川レンズに関するものである。The present invention provides a refracticle I.

従来より、視力矯正の手段としてコンタクトレンズが利
用されていることは周知の通りである。
尚、このコンタクトレンズは、ポリメチルメタクリレ−
1・、アクリル変性シリコーン樹脂のようないわゆるハ
ードコンタクトレンズ材料、ポリ−2−ヒドロキシエチ
ルメタクリレート、ポリビニルピロリドンといった含水
性樹脂のようないわゆるソフトコンタクトレンズ材料、
その他シリコーンラバーのような軟性材料から構成され
ている。
しかし、患者によってはこのようなコンタクトレンズを
装着できない者もおり、コンタクトレンズが絶対的なも
のでもない。
又、一部では角膜移植手術も実施されている。
しかしながら、角膜は人手が困難であり、しかも長期保
存ができず、さらには角膜に対する消毒法に完全なもの
がなく、免疫的な生体反応などにより安心して利用でき
ない等といった角膜を用いることによる固有の欠点があ
る。
尚、摘出された角膜を用いるのではなく、上記したコン
タク!・レンズ材料と同じような材料を用いて構成した
人工角膜を角膜移植手術に用いることも一部で提案され
ている。
最近に至り、コンタクトレンズ、角膜又は人工角膜移植
手術による視力矯正手段とは異なる手段が提案され、注
目を浴び始めている。
すなわち、角膜を切開し、角JIIJI内又は角膜層面
上にプラスチック状の切片を付設する手段が注目を浴び
始めている。
例えば、患者の角膜を層状ボタン型に切り取り、あらか
じめ所望のレンズに作製した他人の角膜又は合成高分子
化合物を角膜実質内の眉間に配置した後、角膜を元の位
置へ縫着するケラトファキア(keraLopl+II
kin)が提案されている。
又、患者自身の角膜を層状ボタン型に切り取り一冷凍さ
せ、旋盤で所望のレンズに切削し、再び患者の眼に縫着
するケラトミロイシス
(keratoe+i 1eusis)が提案されてい
る。
又、他人の角膜を切り収った後、冷凍下で旋盤加工を行
なってレンズを作製し、患者の角膜上皮を取り除いた−
にへ移植するエビケラトファキア(epikeraLo
phakia)も提案されている。
その他、角膜実質層を剥離切開して袋を作り、レンズを
挿入するコーニアルインレイ(cornealinl楓
y)も提案されている。
これらの手段による視力矯正は、第1図及び第2図に示
されるような形態のものであって、これまでの角膜(又
は人工角膜)移植手術の手段とは異なっている。
尚、第1図及び第2図中、1は角膜、2は角膜層内に埋
め込まれた切片であるリフラクティブケラトプラスティ
用レンズ、3は角膜層表面に取り付けられた切片である
リフラクティブケラトプラスティ用レンズである。
そして、このリフラクティブケラトプラスティ。
用レンズに対する研究が各方面で行なわれている。
例えば、小沼、林等によって、ポリメチルメタクリレ−
1・製のりフラクティブケラトブラスティ用レンズを家
兎眼に試みた報告(1眼、1963年”4月、17巻、
4号、第453〜457ページ)がなされている。
しかしながら、このポリメチルメタクリレート製のリフ
ラクティブケラトプラスティ用レンズは、組織反応等の
点において問題が残されていると指摘されている。
又、天理等は、リフラクティブケラトプラスティ用レン
ズをガラクトース系の材料、グリセロール系の材料及び
ヒドロキシ系の材料で構成し、これらの中ではヒドロキ
シ系材料で構成したものが最も優れていたと報告(日本
コ・し学会誌、1971年、第13巻、第12号、第1
40〜144ページ)している。
ずなわち、ガラクトースメタクリレ−) (GAN^)
、グリセリルメタクリレ−1−(に114^)、2−ヒ
ドロキシ10ビルメタクリレート(2II P N^)
、エチレングリコール(HG)及びN、N−ジメチルホ
ルムアミド(DMF)を用いた含水率52%(吸水率1
10%)のガラクトース系材料のもので構成したリフラ
クティブケラトプラスティ用レンズは、角PAR内は埋
め込み術1月月半後には透明性が低下し、又、角膜内血
管の侵入が始まり、角膜の混濁が起きていることから、
このガラク!・−ス系の材料で構成されたリフラクティ
ブケラトプラスティ用レンズは失格であると指摘されて
いる。
又、メチルメタクリレート(88^)、GIN^、2
II P M^、テトラエチレングリコシメタクリレ−
1−(TETR^)及びECを用いた含水率45〜76
%(吸水率81〜318%)のグリセロール系材料のも
ので構成したりフラクティプケラトプラスティ用レンズ
は、角MNI内に埋め込み術1月月後には透明性が低下
するものもあり、又、角膜内血管の侵入が始まり、角膜
の混濁が起き、さらには角膜に潰瘍を生じるものもあっ
て、これまた良いものではないと指摘されている。
そして、NN^、2−ヒドロキシエチルメタクリレ−)
(211HMA)、N−ビ= ルー2−t? o !
J トン(NVP)及びD14Fを用いた含水率20〜
33%(吸水率25〜50%)のヒドロキシ系の材料で
構成したリフラクティブケラトプラスティ用レンズは、
」ユ記した吸水率の大きなガラクトース系及びグリセロ
ール系の場合には良好な結果が得られなかったのに対し
、吸水率が小さなヒドロキシ系の場合には良好な結果が
得られたと報告している。
しかし、このヒドロキシ系の材料で構成したりフラクテ
ィブケラトプラステイ用レンズの場合にあっても、脱水
現象による為か皺がよるといった問題点が起きると指摘
されていた。
尚、E記材料の他にも、例えば22−11E^で構成し
たものが提案されているが、この2− II E M^
製のリフラクティブケラトプラスティ用レンズでは、角
膜が不透明になってしまうと報告されている。
又、シリコーンで構成したりフラクティブゲラトプラス
ティ用レンズも提案されたが、このものでは1力月程度
で角膜内血管の侵入が起き、これまた17!Ij!fl
があるものであった。
このように、眼科用の医療具として同じような目的を達
成するものではあるが、コンタクトレンズ及び人]二角
膜として優れた特長を発揮すると言われてきた材料でリ
フラクティブケ・ラドプラスティ用レンズを構成しても

本発明者は、上記情報を基にして研究を進めていった段
階においては、すなわち火爆等の報告(日本コ・し学会
誌、1971年、第13巻、第12号)を基にして検討
を進めていった段階では、リフラクティブクラドプラス
ティ用レンズの構成材料としては含水率(吸水率)の低
い材料が望ましいものであるとの観点に沿って材料開発
を進めていたのであるが、含水率の小さな材料が必ずし
も望ましいものではないとの知見を得るに至った。
すなわち、角膜自体が基本的に高含水率のものである以
上、やはり高含水率の材料でリフラクティブケラトプラ
スティ用レンズを構成するのが望ましいのではないかと
の啓示を得た。
そこで、まず第−I’m FJとして、高含水性の材料
を用いて構成したりフラクテイブケラトプラステイ用レ
ンズが提案されていないかとのリサーチを行。

この結果は、グリセリルメタクリレートを用いた含水率
88%のリフラクティブケラトプラスティ用レンズがD
ol+Iman等によって提案されていて”、そしてこ
のリフラクティブケラトプラスティ用レンズを家兎眼に
移植した実験が報告(A rchivesor (l
phLl+alehology vol、フッ、Fe
b、、Z5Z〜25〕、1967)されており、このリ
フラクティブゲラドブラスティ用レンズは優れた材料で
構成されているが、移植初期の段階では角膜浮腫等の生
体反応が起きることが指摘されている。
又、グリセリルメタクリレートは架橋剤としてトリエチ
レングリコールジメタクリレートを加えた材料で構成し
た場合には、角膜浮腫、角膜浸潤や血管新生までも生じ
たことも報告している。
又、ポリビニルピロリドンを主成分とした含水率71%
のリフラクティブケラトプラスティ用レンズが1ler
nard等によって提案され、そしてこのリフラクティ
ブケラトプラスティ用レンズを家兎眼に移植した実験が

981)されていて、このリフラクティブケラ]・プラ
スティ用レンズは脱水現象によって角膜実質との不均衡
が生じた旨の指摘がなされている。
このようなことから、高含水性の材料でリフラクティブ
ケラ!・プラスティ用レンズを構成することは無理なの
かとの思いを持ったのである。
それ故、この上は、各種の高含水性材料を用いてリフラ
クティブゲラドブラスティ用レンズを作成し、この高含
水(1材料を用いて構成したリフラクティブケラトプラ
スティ用レンズを家兎眼の角膜層内及び角膜層面上に付
設し、その結果を観察するしかないとの結論に到達し、
これを試みた。
この結果、コラーゲン製、特に可溶性コラーゲンで構成
した含水率約70〜90%のリフラクティブケラトプラ
スティ用レンズが用いられた場合には、角膜透明性等の
問題は全く起きず、浸れたものであることがI’ll明
した。
尚、リフラクティブケラi・プラスティ用レンズとして
ではないが、特開昭57−200152号公報において
、発酵解重合あるいは分散精製した皮膚コラーゲン、又
は牛の鍵から調整された透明フィルムが角膜成形用とし
て検討されたが、臨床的診療には認容されなかったと報
告されている。
このコラーゲンは動物の皮膚等から比較的安価に、かつ
大陸に得られ、他の蛋白質に比べて抗原性が弱い特徴が
ある。この抗原決定基は主にテロベプタイドにあること
から、テロペプタイドを除いたアテロコラーゲンは免疫
活性がほとんど問題ない、さらには、紫外線や放射線の
照射等により滅菌処理を行なうと、レンズとして長J(
Jl保存が可1mとなる。
可溶性コラーゲンの意味は次の通りである。
大部分の天然のコラーゲンは不溶性であるが、例えば酢
酸のような希薄な酸、水酸化ナトリウムのような塩基及
び食塩のような希薄な塩の水溶液中では溶かすことが出
来、すべて比較的低い収率の生成物を得る。
コラーゲン抽出技術のずべてのプロセスは、以。
1ぐに説明するが、公知である。
(a)酸抽出法
小生の皮のような若い動物のコラーゲン組織は約1〜2
%の酸ii)溶性コラーゲンを含んでいる。
このコラーゲンは0.1規定の酢酸又はp I+ 3
、6の0.15モルのクエン酸緩衝液のようなpl+が
2〜4の酸の水溶液によって抽出される。
(b)塩基抽出法
獣皮の真皮層は室温でlO〜15日間、0.2モルのモ
ノメチルアミンと15%の硫酸ナトリウムとを含む4%
の水酸化すl・リウム溶液に浸される。
この獣皮は塩基を除去する為に水で洗われ、その後0.
INの酢酸−0,1モルHaCl(pH2,8)中で撹
拌することによって抽出される。
はとんど全ての獣皮は溶解し、粘性溶液が得られる。こ
のコラーゲンを遠心分離器で集め、pl+を4〜5に増
加さぜることで沈澱さぜ、水洗する。
エタノールによって脱水させた後、コラーゲンは脂肪を
除去する為にエタノール−エーテル混合fI液で処理す
る。
空気92燥した後、コラーゲンは0112〜4の酸性水
溶液に溶解させ、フィルターを通してろ過する。
コラーゲンはp114〜5で沈澱し、遠心分離器で集め
られ、最終的には0112〜4の酸性水溶液中で4〜1
0%のコラーゲンが11Nされる。
(e)塩抽出法
コラーゲンは例えば希薄なNa1l溶液のような塩の溶
液で処理することによって組織から抽出することが出来
る。
(d)酵素抽出法
洗浄して除毛した獣皮又は皮膚は加工できるサイズの小
片に切り刻まれ、蛋白性酵素(コラ−ゲナーゼ以外)の
存在のもとて酸性水溶液中で溶がし、粘ちょうな溶液に
する。尚、適当な酵素はペプシン、1−リブシン、プロ
ナーゼ、プロクターゼ等である。
この消化によって2つのフラクションが得られ、その一
つは不溶性の固体のフラクションで捨てられるものであ
り、もう一つは次の如く作り上げられる溶液の又は可溶
性のフラクションである。
この溶液は残りの活性な酵素を変性させる為にp11約
10.0にされ、そして911約6.0〜7.0にされ
て中性化する。
アテロコラーゲンはこの9+1で沈澱し、消化されたテ
ロベブタイド、他の1u悪蛋白質、サツカライド、ムコ
ポリサッカライド等を溶液中に残す。
このアテロコラーゲンは通常pH2〜3での溶解及びp
116〜7での再沈澱を繰返して精製される。
そして取出されたコラーゲンは溶媒脱脂プロセスの為は
、そして脂質成分及び余分の水を除去する為にエタノー
ルで洗われる。
このコラーゲンはエタノール−エチルエーテル混合溶液
による処理で脱脂され、チーズクロスのフィルターによ
り粘性のある固体として取出され、そして空気乾燥され
、次いでp+1約3の酸の水溶液中で溶解することによ
りゲルに変換される。
尚、リフラクティブケラトプラスティ川レンズを構成す
る為に、可溶性コラーゲンの他に、例えばヒロアル酸、
コンドロイチン硫酸等のムコ多糖類、その他の強度向上
材を副成分として用いても良いものであった。
(実施例11
コラーゲンをp++約3の酸性溶液に溶解することによ
り、10%の透明なコラーゲンゲルを得た。
これをプラスティック製の成形型の四部に入れ、減圧下
で脱泡し、成形型の凸部をコラーゲンゲルに押し付け、
そして成形型全体を窒素で満たした後、コバルト60を
211i間照射することによって架橋反応を行なわぜな
、尚、コバルト60の照射総線喰は1メガラツトであっ
た。
このようにして得たコラーゲンレンズをリン酸塩緩衝液
で膨潤させ、生理食塩水に移して保存した。
尚、このコラーゲンレンズは、含水率が90%、光学的
に透明、柔軟性に富んでいる。
上記保存になるコラーゲンレンズを取り出してi・レフ
インで直径6輪輪に打ち抜き、コーニアルインレイ用の
レンズとし、これを白色家兎の角膜実f1層の剥離した
ポケットに挿入した。
挿入(li後、スリットランプ等による経時変化のrr
J4察と術後20力月目に角1罠を摘出し、走査型電子
顕微鏡により角膜組mbこおよぼす影響を検討した。
その結果、このリフラクティブゲラドブラスティ用レン
ズは、透明が保証されており、光学性は良く、炎症反応
