Description
The present invention relates to a dosimeter that measures radiation doses for the purpose of improving the accuracy of cancer treatment.
In recent years, minimally invasive high-precision stereotactic radiotherapy (hereafter referred to as "stereotactic radiotherapy"), which delivers a highly precise therapeutic dose to the cancerous area, has been attracting attention in the treatment of intractable cancers. This treatment maximizes the dose to the tumor while reducing the dose to the surrounding tissues, and its application is expanding to cancer diseases for which it was not previously applicable. It has shown remarkable therapeutic results, especially in early-stage lung cancer and liver cancer.
As stereotactic radiotherapy becomes more widespread, there is an increasing need for dosimeters that can measure the dose in the tumor and its surrounding tissues (hereafter referred to as the "target") in real time. In actual treatment, the position of the target changes during irradiation due to the patient's own movements and breathing, and since a high dose is irradiated, the effect of errors due to this position change is large.
The following
The
If the detector containing the stimulable phosphor can be made smaller and the optical fiber thinner so that it can be inserted into the body, it will be possible to monitor the radiation dose online during exposure.
If the small dosimeter described in the above-mentioned document could be used to measure the dose in vivo during radiation therapy, it would be possible to increase the accuracy of stereotactic radiotherapy while still being minimally invasive, and it is expected that this would lead to a significant improvement in the outcome of treatment. However, miniaturizing the detection unit generally tends to reduce detection sensitivity, and it is not easy to achieve dose measurement with sufficient accuracy. In addition, unlike the example in
The present invention provides a dosimeter that can increase the accuracy of measuring the radiation dose even when the detection unit containing the stimulable phosphor is small or when the pulse frequency of the radiation irradiation is high.
The present invention provides a dosimeter having a detection unit containing a stimulable phosphor and an optical fiber connected thereto,
a) in order to measure the amount (total amount) of radiation irradiated in a pulsed manner, stimulating light is supplied to the stimulable phosphor through an optical fiber, causing the stimulable phosphor to emit fluorescence, and the amount of emitted fluorescence is read, thereby measuring the amount of radiation irradiated to the stimulable phosphor; and
b) The conditions for supplying the stimulating light or the conditions for reading the amount of fluorescence are set according to the characteristics of the stimulable phosphor or optical fiber or the state of irradiation with radiation.
In the case of a dosimeter having the above characteristic a), after a stimulable phosphor is irradiated with radiation, the phosphor emits fluorescence in a proportion to the radiation dose when a stimulating light such as a laser beam is supplied through an optical fiber. When a stimulable phosphor is irradiated with radiation, it generates a large number of electron-hole pairs according to the energy given to it. These are trapped in separate locations and do not recombine, but when exposed to stimulating light, the electrons are reexcited and recombine with the holes, thereby emitting fluorescence. The amount of radiation irradiated can be measured by reading the amount of fluorescence through an optical fiber. As the stimulable phosphor, a group Ia-VIIb compound (alkali halide) or a group IIa-VIb compound (alkaline earth sulfide, selenide, or oxide) can be used.
However, radiation exposure conditions in cancer treatments and the like are not constant, and the type and specifications of photostimulable phosphors and other materials most suitable for dose measurement are not necessarily clear at present. Considering that there is also no clear means of ensuring sufficient detection sensitivity when the detection unit is miniaturized, at least at the current development stage, it is thought that in order to perform highly accurate dose measurement, it is not appropriate to uniformly define radiation exposure conditions or to continue to limit the use of specific photostimulable phosphors and optical fibers.
As described above in b), the dosimeter of the present invention is one in which the supply conditions of the stimulating light or the reading conditions of the amount of fluorescence are set (including being appropriately changed) according to the characteristics of the stimulable phosphor or the optical fiber or the irradiation conditions of the radiation. Therefore, even if the stimulable phosphor or the optical fiber used is changed in relation to the miniaturization of the detection unit, or even if the irradiation conditions of the radiation are changed, this dosimeter can perform accurate and appropriate dose measurement.
In the dosimeter according to the invention, it is particularly preferred that the stimulating light be supplied to the stimulable phosphor during the pulses of radiation being irradiated, after a certain delay time has elapsed since the end of a pulse irradiation, and before the start of the next pulse irradiation (for example, during the 2 ms supply time of the stimulating light as shown in FIG. 4).
In cancer treatment, radiation is usually emitted in pulses and irradiated to the target, and according to the description in the above-mentioned
In such a case, as described above, it is advisable to supply stimulating light to the stimulable phosphor during the period between the end of irradiation of a certain pulse and the start of irradiation of the next pulse after a certain delay time has elapsed, and to read the amount of fluorescence emitted during that supply time. In this way, even when using a stimulable phosphor that is prone to emitting prompt fluorescence or Cherenkov light, the dosimeter of the invention can measure the dose accurately. The delay time should be set in the range of about 100 to 1000 μs (microseconds), for example 400 μs, depending on the characteristics of the stimulable phosphor.
In the above-mentioned dosimeter, depending on the type of stimulable phosphor or the radiation irradiation conditions, it is preferable to supply the stimulating light to the stimulable phosphor only during the interval between pulses of radiation being irradiated, and to cause the energy (all or almost all) stored in the stimulable phosphor by the radiation irradiated up to that point to be released as the above-mentioned fluorescence during the supply of the stimulating light, so as to read the amount of fluorescence.
For example, when the stimulable phosphor is BaFBr:Eu, the time constant for emitting fluorescence is small, about 0.2 ms (milliseconds). Therefore, in the current general pulse interval of radiation irradiation, it is sufficient to supply stimulating light to the stimulable phosphor only in the one interval between the irradiated radiation pulses. In other words, if stimulating light is supplied only in that one interval, as shown in Figure 4, the entire amount of energy accumulated in the stimulable phosphor by the radiation irradiated up to that point can be made to emit as fluorescence during that interval. The amount of irradiated radiation can be measured by reading the amount of fluorescence.
That is, when the time constant during fluorescence emission of the stimulable phosphor is not large or the pulse interval of radiation irradiation is long, preferable dose measurement can be performed by setting the supply conditions of the stimulating light in the dosimeter of the present invention as described above.
In the above-mentioned dosimeter, when the type of stimulable phosphor or the radiation irradiation conditions are different from those described above, the above-mentioned stimulating light is supplied to the stimulable phosphor in each of a plurality of intervals between the irradiated pulses of radiation and two or more subsequent pulses, and during these plurality of supplies of stimulating light, the energy (all or almost all) stored in the stimulable phosphor by the radiation irradiated up to that point is released as the above-mentioned fluorescence, and the total amount of emitted fluorescence is read.
For example, when the stimulable phosphor is KCl:Eu, the time constant for the emission of fluorescence is large, about 1 ms. Therefore, unless the pulse interval of the radiation irradiation is considerably long, the stimulable phosphor cannot release all of the stored energy in only one interval between one pulse and the next. Therefore, as shown in Fig. 5, for example, stimulating light is supplied to the stimulable phosphor in each of the intervals between a pulse and two or more subsequent pulses. During these multiple supplies of stimulating light, the total amount of energy stored in the stimulable phosphor by the radiation irradiated up to that point is caused to be emitted as the above-mentioned fluorescence, and the amount of emitted fluorescence can be read to measure the amount of radiation irradiated.
In other words, when the time constant of the stimulable phosphor for emitting fluorescence is large or the pulse interval of radiation irradiation is short, the dosimeter of the present invention can perform preferable dose measurement by setting the conditions for supplying the stimulating light as described above.
In the dosimeter according to the invention, it is preferable that the wavelength of the stimulating light is set in accordance with the characteristics of the stimulable phosphor.
The characteristics of stimulable phosphors vary depending on their chemical components, and the peak wavelength of the stimulating light that easily induces fluorescence is not the same for all of them. For example, the peak wavelength of the stimulating light for BaFBr:Eu is 600 nm, and for KCl:Eu it is 560 nm. Therefore, if it is possible to determine the wavelength of the stimulating light in a dosimeter to match the stimulable phosphor used, it is possible to improve the accuracy of dose measurement even when the stored energy decreases as the detector is made smaller.
In the above dosimeter, it is preferable that the detection unit and the optical fiber (the part inserted into the living body) each have a maximum dimension in cross section of 1 mm or less and are covered with a biocompatible material such as silicone. The maximum dimension in cross section of 1 mm or less means that the optical fiber has a diameter of 1 mm or less, and the detection unit can be entirely contained within a circle of 1 mm diameter or less in a cross section perpendicular to the optical axis. Covered with a biocompatible material includes the case where the detection unit is coated with the material and the case where the outer member is made of the material.
If the detector and the optical fiber are of such a nature, there is a low risk of damaging the living body when they are placed in the living body, and therefore they can be left in the living body to perform accurate dose measurements repeatedly, thereby improving the effectiveness of radiation therapy.
Furthermore, it is advantageous if the surface of the detection portion is coated with a gold film having a thickness of 1 μm to 10 μm.
Gold is sometimes embedded in the living body as a so-called marker and used for alignment with a target (such as a tumor) during radiation therapy. Therefore, when the detection unit is coated with a gold film as described above, the detection unit can also serve as a marker for alignment during radiation therapy. If the detection unit contains a large amount of metal components, artifacts (noise) tend to occur during imaging with X-rays, CT, or MRI. On the other hand, if the gold film is too thin, it may not function as a marker, but if the film thickness is within the above range, these inconveniences do not occur.
If the detection unit allows accurate dose measurement and can also be used as a marker as described above, this is extremely preferable for performing highly accurate stereotactic radiotherapy.
It is also preferable that the dosimeter is one in which a plurality of the detection sections are connected, or a plurality of stimulable phosphors are built in the detection section.
Such a dosimeter would be able to simultaneously measure the dose delivered to the target as well as the dose delivered to the surrounding area, making it possible to perform minimally invasive, highly accurate stereotactic radiation therapy.

The control method of the present invention is a method for controlling a dosimeter that uses a dosimeter having a detection unit containing a stimulable phosphor and an optical fiber connected thereto for measuring the amount of radiation irradiated in a pulsed manner, the method comprising the steps of:
A) supplying stimulating light to the stimulable phosphor through an optical fiber and causing the stimulable phosphor to emit fluorescence, and measuring the amount of radiation irradiated to the stimulable phosphor by reading the amount of emitted fluorescence; and
B) The conditions for supplying the stimulating light or the conditions for reading the amount of fluorescence are set according to the characteristics of the stimulable phosphor or optical fiber or the state of irradiation with radiation.
This control method has the above characteristic A), so when the stimulable phosphor is irradiated with radiation and then stimulating light such as laser light is supplied through an optical fiber, the phosphor emits fluorescence that is roughly proportional to the radiation dose. Therefore, the amount of radiation irradiated can be measured by reading the amount of emitted fluorescence. In addition, because it has the above characteristic B), appropriate dose measurement can be performed with the dosimeter even if the stimulable phosphor or optical fiber used is changed, or if the radiation irradiation conditions change.
In the above-mentioned control method, it is particularly preferable to supply the stimulating light to the stimulable phosphor during a period between pulses of radiation to be irradiated, after a certain delay time has elapsed since the end of irradiation of a certain pulse, and before the start of irradiation of the next pulse.
In this way, even when using stimulable phosphors or optical fibers that emit noise such as prompt fluorescence and Cherenkov light almost immediately upon exposure to radiation, accurate dose measurement without being affected by noise is possible.
According to the dosimeter of the present invention, even when the stimulable phosphor or optical fiber used is changed in order to reduce the size of the detection section, or when the radiation irradiation conditions are changed, it is possible to perform accurate and appropriate dose measurement by simply setting the conditions for supplying the stimulating light, etc.
For example, it is possible to set the stimulating light to be supplied to the stimulable phosphor after a certain delay time has elapsed since the end of a certain pulse of radiation irradiation and before the start of the next pulse of irradiation, to set the interval at which the stimulating light is supplied between the irradiated pulses of radiation, or to set the wavelength of the stimulating light in accordance with the stimulable phosphor used, thereby enabling accurate dose measurement.
Furthermore, dosimeters in which the detection unit and optical fiber have a maximum cross-sectional dimension of 1 mm or less and are covered with a biocompatible material, dosimeters in which the surface of the detection unit is coated with a gold film, or dosimeters in which multiple detection units are connected or multiple photostimulable phosphors are built into the detection unit all enable minimally invasive, highly accurate stereotactic radiotherapy.
The dosimeter control method of the present invention makes it possible to perform appropriate dose measurement with high accuracy even when the stimulable phosphor or optical fiber used in the dosimeter is changed, or when the radiation irradiation conditions change.

When the

Examples of the mode of radiation irradiation, the
In a certain device for stereotactic radiotherapy, the timing of irradiation of radiation (X-rays) is 360 pps (Pulse Per Second), the pulse width is 5 μs, and the pulse interval is 2.78 ms. In this case, a
In this case, in the
FIG. 4 shows data relating to the radiation (X-ray) pulse, the supply pulse of stimulating light, and the fluorescence emission intensity in the above example.
As shown in Fig. 4(a) and (b), when the number of X-ray pulses reaches n (i=n), a stimulating light having a wavelength of about 600 nm is supplied to the
FIG. 5 shows data for an example in which the radiation exposure was the same as in the above example, but a different KCl:Eu was used as the
As shown in Fig. 5(a), X-ray irradiation is performed at 360 pps, the pulse width is 5 µs, and the pulse interval is 2.78 ms, which is the same as the above example, but the KCl:Eu used as the
Therefore, in this second example, a stimulating light with a wavelength of about 560 nm is supplied. In addition to when the count value of the X-ray irradiation pulses reaches n, the stimulating light is supplied in pulse form for only 2 ms after resetting (i.e., three times when i = n, 0, and 1), as shown in Figure 5 (b), with a delay time of 400 μs as above. The operation in the control system of Figure 3 involves changing some of the settings of the
As a result of the above, the
