Description
The present invention relates to the field of medical devices. In particular, the present invention relates to a cushion for a chair or bed. More specifically, the present invention relates to a cushion for a wheelchair that may be useful in preventing or treating pressure sores.
People who are unable to move may be at risk of developing pressure sores, also known as decubitus ulcers or bed sores. This problem is particularly prevalent in patients who remain in bed for long periods of time, but also manifests in patients who are confined to wheelchairs. While the following description focuses on wheelchairs, it should be understood that the same considerations may apply to users who do not have the ability to move and remain in one place for long periods of time.
People who are confined to wheelchairs may have the ability to move their upper body but not change their position in the wheelchair. As a result, blood circulation around the buttocks and upper thighs may be restricted. The exact reasons for pressure ulcers are not fully understood, but there is a high correlation between problems such as reduced blood flow, lack of movement, weight bearing, moisture and light clothing and the development of pressure ulcers. Skin damage is thought to be due to the effects of load and pressure on the skin restricting cutaneous blood flow, which is important for the viability of skin tissue.

The controller has the functionality to start and stop vibration therapy, whether through a remote link or a local control panel, and various indications to supplement operation. As described below, supplemental functions may include a graphical display of pressure (weight) spread across the cushion, a battery charge indication, and an operating program.
At least one sensor embodied as a

At least one sensor can also determine that the cushion is occupied. In a preferred embodiment, the total number of all
In a preferred embodiment of the seat cushion, four high-precision capacitive proximity sensing elements (70a, 70b, 70c and 70d) are arranged on a
The signal from each sensing element is transmitted to control
Each proximity sensing element is capable of measuring very small changes in capacitance due to small changes in proximity within the thickness of the cushion. Both the sensing area and the cushion structure are symmetrical. This allows ratiometric measurements to be made to determine the user's center of gravity, thereby determining postural symmetry.
Left/right postural symmetry can be determined by the following ratios:
(Left thigh proximity + left buttock proximity): (Right thigh proximity + right buttock proximity)
The symmetry of the front/back posture can be determined by the following ratios:
(Right buttock proximity + left buttock proximity): (Right thigh proximity + left thigh proximity)
Note that the sensor structure in Figure 7 does not directly measure weight or pressure, but rather measures postural symmetry (left-right-front-back) and cushion occupancy.
As with the embodiment of FIG. 6, each proximity detection element may also include an accelerometer, suitably a precision three-axis accelerometer. The combination of the proximity sensor and the accelerometer allows the sensor to determine when the user has changed position again. When a certain relaxation period is met, the indication and notification may be reset. Chairs fitted with active tilt-in-space (TIS) systems may also be monitored to determine when the chair cushion has been moved from its horizontal position to a specific position within a predefined range of positions. Again, when a certain relaxation period is met, the indication and notification may be reset.
Ratiometric left/right measurement also reduces measurement errors due to noise, shift, thermal drift or foam degradation, as these are common throughout the cushion body.

Referring to FIG. 9, a flowchart of the process of programming and operating the controller to control the vibration of the cycloidal vibration device is shown. The first part of the flowchart in FIG. 9 describes the program setup. This may be done as a factory pre-setting or can be performed by the user and/or caregiver. The selected program can be customized to adjust the intensity and/or duration and/or location of the vibration. The first step to start the setup process is to select the location of the vibration. If only one cycloidal vibration device is provided, this step may be unnecessary, but if more than one cycloidal vibration device is provided, the following steps may be performed for each cycloidal vibration device, whereby the vibration program is different at different positions and/or different times.
After the location of vibration is selected, the intensity of the vibration is selected, which may also be preset. The intensity may be selected on a suitable scale, such as a 10-point scale ranging from minimum intensity to maximum intensity. The intensity of the vibration is determined by the frequency. The frequency may range from about 30 Hz to about 50 Hz. The duration of the vibration therapy is selected, which typically spans a period of 15 to 50 minutes. This duration may be repeated, for example 40 minutes of vibration followed by a 1-2 hour break followed by another 40 minutes of vibration, which may also be repeated three times.
Finally, a start time is selected. This may be an immediate start, a programmed start after a delay, or a start at a specific time (e.g., 9:00 AM each day). To facilitate this function, the controller may incorporate a real-time clock and calendar integrated circuit. Such devices are readily available. Such devices may also provide event logging capabilities, as described below.
In one embodiment, the controller includes pre-programmed settings for vibration position, frequency/speed, time and duration that are activated by pressing a single button on the control panel and/or smartphone application, suitably at the start of the day.
The designed program may be saved and numbered appropriately (Program 1, Program 2, etc.). The saving step is optional, so that the program can simply be run as a one-off application with respect to the parameters selected by the user.
For automated actions, a program may be selected and initiated from stored programs, which may be pre-configured and provided to the user, or may be programs specifically designed by or for the user.
The program periodically checks the duration via a time loop and continues until the full duration has elapsed.
The inventors have found that programmable vibration has certain advantages over the continuous motion of the prior art. By way of example, the following program is believed to be beneficial and to provide certain advantages:
- Three 30-minute treatments per day, operating at specified times with 1.5-2 hour intervals, at a frequency of 30 Hz to 40 Hz.
This may be operated with a single push button on the control panel, which will immediately begin the first 30-40 minutes of treatment. The operator may be able to change or adjust the frequency/speed and/or duration of the motor vibration.
- The frequency increases slowly from 30 Hz to 50 Hz over a period of 90 seconds, but this is not easily noticeable by the patient.
It is expected that the treatment will improve blood flow by applying a range of vibration frequencies for 30 minutes.
- Four 15 minute treatments spread over an 8 hour timed program.
This may allow for improved and more regular blood flow in the at-risk location.
Treatment frequency and duration selected based on body weight.
A person of average weight may receive 30 minutes of treatment at 30 Hz, while a heavier person may receive 35 minutes of treatment at 40 Hz, and a larger person may receive 40 minutes of treatment at 50 Hz.
The programs outlined above are provided by way of example only. Other programs may be appropriate for particular patients in particular situations and may have particular innovative advantages. The following variations are available:
Location: right hip; right thigh; left hip; left thigh; any combination of right hip, left hip, right thigh and left thigh.
Frequency: 30Hz to 50Hz
Intensity: 0.1 mm to 0.8 mm working amplitude Duration: 30 minutes to 3 hours in 15 minute increments, which can be repeated with pauses between 30 minutes to 6 hours of treatment in 30 minute increments.
The controller and/or computer application program records and/or provides on a graphical user interface a pressure distribution history log of the
Green spot - balanced (<+/- 5%).
Yellow - moderately misaligned (>5% <15%).
Red - extreme position, immediate repositioning required (>15%).
The data and history logs may be automated and the user and/or caregiver/nursing staff may receive a daily, weekly or monthly summary report on usage, i.e., whether the support cushion is activated each day, and a report on posture trends. The log may provide a percentage analysis of how much time the user spent in a balanced or unbalanced posture. The user and/or caregiver/nursing staff may then evaluate the data and correct and modify the user's posture using the reference support structure.
The controller also provides an indication to the user and/or the caregiver and/or the nursing staff via the user's and/or the caregiver's and/or the nursing staff's smartphone/tablet or device. The indication may be visual, audible, tactile, or a combination of vibration and ringtone, etc. The indication may indicate, for example, that the pressure sensor timer has expired and the patient needs to be repositioned to relieve pressure. Alternatively, it may indicate that an uneven distribution of pressure on the
The controller may also log the time taken from activating the pressure indication to repositioning and resetting. The response data may form part of an optional summary report showing how quickly the caregivers or nursing staff tend to the patient and may be a useful supervision and management tool. Family members of the user of the support cushion may also have the summary report sent to them for supervision. This data may also be useful for home care services providing multiple and diverse caregivers to individuals in their own homes to monitor staff compliance with mandated use of the support cushion.
The controller may also incorporate networking facilities so that multiple support cushions may be monitored from a central storage location. This may be particularly useful in nursing home facilities that have multiple cushion products in use. The central monitor may display all operating units in the nursing home facility so that operation may be checked. Reports may provide information on operations such as usage, response times and effectiveness.
The control panel may also include a flashing indicator light to provide an on-the-spot visual indication that it is time for the user of the support cushion to reposition to relieve pressure. It has been said that the optimal time for a patient to reposition is every 15-20 minutes.
The controller may also be programmed to provide an indication to a user of the support cushion that the support cushion and the user have been in the same position for an extended period of time and should reposition. The indication that it is time to move may be a short, intense vibration provided by a cycloidal vibration motor.
A number of other features may be incorporated into the controller to enhance the effectiveness of the support cushion. For example, the vibration program may be interrupted or stopped if pressure is released from the cushion, for example because the user is no longer sitting in the chair. Various parameters may be logged over time and used for diagnostics. These parameters may include pressure and position, movement, lack of movement, etc.
As discussed above, although a particular design is depicted, it is not essential to the present invention that the support cushion have a particular size or shape. The
