図2に示すように、リハビリテーション支援システム200は、リハビリテーション支援装置210と、2つのベースステーション231、232と、ヘッドマウントディスプレイ233と、2つのコントローラ234、235とを備える。ユーザ220は、椅子221に座りながら、ヘッドマウントディスプレイ233の表示に合わせて、上半身をねじったり、手を伸ばしたりすることでリハビリテーション動作を行なう。本実施形態では、椅子に座って行なうリハビリテーションを前提に説明するが、本発明はこれに限定されるものではなく、リハビリテーション動作を、立って行なっても、歩きながら行っても、ベッド上で行っても、仰臥位や腹臥位で行ってもよい。あるいはリハビリテーション動作を、走りながら、またはその他特定の動作や運動を行いながら行ってもよい。また、コントローラは足や体幹等、手以外の体の部位に保持、装着してもよい。

2つのベースステーション231、232は、ヘッドマウントディスプレイ233の動きおよびコントローラ234、235の動きを検知して、リハビリテーション支援装置210に送る。リハビリテーション支援装置210は、ヘッドマウントディスプレイ233の動きに基づいて、ヘッドマウントディスプレイ233の表示制御を行なう。また、コントローラ234、235の動きに基づいて、ユーザ220のリハビリテーション動作を評価する。なお、ヘッドマウントディスプレイ233としては、非透過タイプでも、ビデオシースルータイプでも、オプティカルシースルータイプでも、あるいは眼鏡型でも構わない。本実施形態ではVR(Virtual Reality)の仮想空間をユーザに提示するが、AR(Augmented Reality:拡張現実)のように、現実空間と仮想空間とを重畳表示してもよいし、MR(Mixed Reality:複合現実)のように、仮想空間に現実情報を反映させてもよいし、あるいはホログラム技術を代替手段として用いてもよい。

本実施形態では、ユーザの手や頭の位置または動作を検出するためのセンサの一例として、ユーザ220が手に持つコントローラ234、235や、ベースステーション231、232を示しているが、本発明はこれに限定されるものではない。ユーザの手そのものの位置または動作を画像認識処理により検出するためのカメラ(深度センサを含む)や、温度によりユーザの手の位置を検出するためのセンサを用いても良い。さらに、ユーザの腕に装着させる腕時計型のウェアラブル端末などや、モーションキャプチャなども、動作検出部211と連動させることで本発明に適用可能である。つまり、キネクト(登録商標)等の三次元トラッキング装置や動作分析装置を用いたり、体にマーカー等を装着して行うことも一つの実施形態となる。

表示制御部212は、3次元的な身体動作をユーザ220に促すための目標オブジェクト242を仮想空間240において生成し、表示する。特に、表示制御部212は、検出したリハビリテーション動作に応じて動くアバターオブジェクト241と、リハビリテーション動作の目標を示す目標オブジェクト242と、を仮想空間内に生成する。そして、それらのアバターオブジェクト241および目標オブジェクト242の画像を、動作検出部211が検出したヘッドマウントディスプレイ233の向きおよび位置に応じて、表示画面240に表示させる。アバターオブジェクト241および目標オブジェクト242の画像は、背景画像243に重畳表示される。ここでは、アバターオブジェクト241は、コントローラ234、235と同じ形状をしているがこれに限定されるものではなく、さらに左右で大きさや形状、色を変えたりしてもよい。アバターオブジェクト241は、コントローラ234、235の動きに合わせて表示画面240中を動く。コントローラ234、235には1つ以上のボタンが用意されており、ボタンを操作することにより原点設定などの初期設定を含む各種設定を行うことができるように構成されている。ボタン機能を無効にすることもでき、ボタンそのものを配置しないで、全ての設定を別途外部の操作部を用いて実行するように構成してもよい。背景画像243は、地平線244と、地表面画像245とを含んだ仮想空間から切り出したものである。

表示制御部212は、目標オブジェクト242を仮想空間内に生成し、ユーザ220の上方向から下方に向けて移動させる。これにより、ヘッドマウントディスプレイ233においては、表示位置および大きさが徐々に変わるように(例えば徐々に大きくなり、その後小さくなるように)表示される。ユーザ220は、コントローラ234、235を動かして、画面中のアバターオブジェクト241を、目標オブジェクト242に近づける。なお、目標オブジェクトの移動方向は、例えば床面方向から頭上方向へ上昇していくように表示させてもよい。また、上下方向の動きに加えて奥行き方向や左右方向への移動を含め、3次元的にどのような移動方法を伴ってもよいし、あるいは移動せずにある特定の座標位置に固定されていてもよい。

ユーザ220は、コントローラ234、235を動かして、画面中のアバターオブジェクト241を、目標オブジェクト242に近づける。アバターオブジェクト241が目標オブジェクト242にぶつかると、表示制御部212は目標オブジェクト242を消滅させ、フィードバック部214は、目標動作が達成されたとして、メッセージを表示する。具体的には、アバターオブジェクト241に含まれるセンサオブジェクトと目標オブジェクト242との最短距離が所定範囲内になると、目標達成となり、目標オブジェクト242は消滅する。このとき、アバターオブジェクト241に含まれるセンサオブジェクト(例えば、アバターオブジェクト241の中心点を含む球体オブジェクト)と目標オブジェクト242との最短距離が所定の閾値S1以下になれば目標の完全達成とする。そして完全達成の場合、「あっぱれ」と表示すると同時に対応する音声を出力してフィードバックする。同時に課題を達成した方の手で保持するコントローラ234,あるいは235を振動させてもよいし、嗅覚や味覚に対して刺激を与えてもよい。センサオブジェクトと目標オブジェクト242との距離がどこまで縮まったかによってリハビリテーション動作を3段階以上に分けて評価してもよい。また、目標オブジェクトを同時に仮想空間内に2つ以上生成し、表示してもよい。五感刺激に対する課題達成の報知は、動作種別に応じて等、いかように組み合わせて行ってもよい。アバターオブジェクト241に含まれるセンサオブジェクトと目標オブジェクト242との最短距離が閾値S1以上閾値S2以下であれば、目標の達成として「お見事」と表示すると同時に対応する音声を出力してフィードバックすればよい。なお、出力される音声は表示メッセージと同一のものでなくてもよく、例えば「ピローン」というような非言語的な効果音を用いてもよい。

表示制御部213は、レーダスクリーン画像250をヘッドマウントディスプレイ233の表示画面240に表示させる。レーダスクリーン画像250は、目標オブジェクト152の発生を報知するための報知画像である。レーダスクリーン画像250は、発生する目標オブジェクト242の位置が、仮想空間内の基準方向(ここでは椅子にまっすぐに腰掛けたユーザの正面方向となるように初期設定されている)に対して、相対的にどちらの方向であるかを報知する。レーダスクリーン画像250は、さらに、発生する目標オブジェクト242の位置が、ユーザ220からどの程度離れているかも報知する。なお、報知画像はレーダースクリーン画像に限定されず、文字や矢印、記号やイラスト、光や色の種類、強弱、点滅等によって報知されてもよい。また、報知方法は画像に限定されず、音声、振動または音声、振動、画像のいずれかの組み合わせによって行われてもよい。表示制御部213は、ユーザ220の頭の向きにかかわらず、レーダスクリーン画像250をヘッドマウントディスプレイ233の表示画面240の中央部分(例えば-50度~50度の範囲内)に表示させる。ただし表示部分は中央に限定されず、例えば画面の四隅や上端、下端、左端、右端の任意の場所でも構わない。

レーダスクリーン画像250は、上方から見たユーザの頭を表わす頭画像251と、頭画像251の周囲を複数のブロックに分割したブロック画像252と、ユーザの視野領域を示す視野領域画像としての扇型画像253と、を含む。目標オブジェクトの位置を示す目標位置画像は、ブロック画像252のどのブロックが着色または点滅、点灯されるかによって示される。これにより、ユーザ220は、自分が向いている方向に対して左側に目標オブジェクトがあるのか、右側に目標オブジェクトがあるのかを、知ることができる。なお、本実施形態では、ブロック画像252が固定されて、扇型画像253が動く構成としているが、本発明はこれに限定されるものではなく、扇型画像253や頭画像251を固定しつつ、ブロック画像252を頭の向きに応じて動かしてもよい。具体的には頭が左に向けば、ブロック画像252が右に回転する構成でもよい。

フィードバック部214は、表示制御部212を介して、メッセージ種別を、リハビリテーション動作の評価に応じて変化させることが好ましい。例えば、センサオブジェクトが目標オブジェクト242の中心に接触すれば、「あっぱれ」と表示し、センサオブジェクトが目標オブジェクト242の中心の周囲部分のみに接触すれば「お見事」と表示するなどである。この目標オブジェクト242の大きさや、周囲部分の大きさは、設定部217により設定可能である。また、センサオブジェクトの大きさも設定部217により設定可能である。

フィードバック部214は、目標オブジェクト242に仮想的に触れたユーザに対して、五感(視覚、聴覚、触覚、味覚、嗅覚)のうち2つ以上の感覚を刺激するフィードバックを行う。センサオブジェクトが目標オブジェクト242の中心から所定距離内に入ったタイミング、または、センサオブジェクトが目標オブジェクト242と接触したタイミングとほぼ同時にフィードバックを行う(リアルタイムマルチチャネルバイオフィードバック)。それらのタイミングからフィードバックまでの遅延は、例えば1秒以内であれば効果が高く、ユーザの動作タイミングとフィードバックのタイミングとの間隔が近ければ近いほど(遅延が小さいほど)効果が大きい。フィードバック部214は、「あっぱれ!」という画像により、ユーザの視覚を刺激するフィードバックを行ないつつ、同時に、スピーカ236から出力する音声により、ユーザの聴覚を刺激するフィードバックを行なう。これら五感(視覚、聴覚、触覚、味覚、嗅覚)に対する刺激フィードバックは、1回の課題ごとにその達成の有無をユーザに報知するために行われる。

このようにコントローラ234、235を動かすユーザ220の動作がリハビリテーション動作であり、ユーザ220が行なうべき1回のリハビリテーション動作を促す目標オブジェクトの表示をタスク、あるいは課題と呼ぶ。1つのタスクを表わす情報(タスクデータ)は、目標オブジェクト242の仮想空間240における属性としての、移動速度、表示数、大きさ、表示位置および表示間隔を少なくとも含む。タスクデータには、目標オブジェクトの出現方向(椅子の正面方向に対して右90度、右45度、正面、左45度、左90度)、目標オブジェクトまでの距離、目標オブジェクトの形状(大きさ)、出現位置(ユーザからの奥行方向距離)、出現間隔(時間間隔)、落下または上昇等の移動速度、目標オブジェクトの色、左右どちらのコントローラで取得すべきか、同時に出現する目標オブジェクトの数、センサオブジェクトの大きさなどが含まれてもよい。ユーザ220から目標オブジェクト242の落下位置までの奥行き方向の距離を連続的に設定してもよいし、例えば3段階のいずれかに設定してもよい。例えば、ユーザ220の直ぐそばに落下させたり、ユーザ220が大きく前のめりにならないと届かない位置に落下させたり、と変更することができる。これにより、ユーザに与える運動負荷、および空間認知能力、あるいは空間把握能力に対する負荷を制御することができる。

評価更新部215は、ユーザ220が達成したタスクの量および質に応じて、ユーザのリハビリテーション動作を評価し、ポイントを加算する。ここで達成したタスクの質とは、「あっぱれ」か「お見事」か、つまり、どこまで目標オブジェクトにアバターオブジェクトを近づけることができたかを含む。評価更新部215は、達成したタスクに対して、それぞれ、異なるポイント(遠いオブジェクトには高いポイント、近いオブジェクトには低いポイント)を付与する。評価更新部215は、積算されたポイントに応じて、タスクを更新することができる。例えば、課題達成率(目標達成数/課題数)などを用いてタスク(目標オブジェクトの属性)を更新してもよい。評価更新部215は、動作検出部211が検出したリハビリテーション動作と、表示制御部212によって表示された目標オブジェクトが表わす目標位置とを比較して、ユーザ220のリハビリテーション能力を評価する。具体的には、動作検出部211が検出したリハビリテーション動作に対応して移動するアバターオブジェクト241と目標オブジェクト242とが重なったか否かを、3次元仮想空間中の位置の比較により決定する。これらが重なれば、一つのリハビリテーション動作をクリアしたものと評価し、ポイントを加算する。表示制御部212は、目標オブジェクト242を、奥行き方向について異なる位置(例えば3段階の位置)に出現させることができる。評価更新部215は、それぞれ、異なるポイント(遠いオブジェクトには高いポイント、近いオブジェクトには低いポイント)を付与する。

評価更新部215は、積算されたポイントに応じて、目標課題を更新する。例えば、課題達成率(目標達成数/課題数)などを用いて目標課題を更新してもよい。

タスクセットデータベース216は、複数のタスクのセットを格納している。タスクとは、ユーザが行なうべき1回のリハビリテーション動作を示す。具体的には、1つのタスクを表わす情報として、どの位置にどのような速度で、どのような大きさの目標オブジェクトを出現させたか、その際、アバターオブジェクトはどのような大きさだったかなどを格納している。タスクセットデータベース216は、そのような複数のタスクをどのような順番でユーザに提供するかを決めるタスクセットを格納している。例えば、病院毎のテンプレートとしてタスクセットを格納してもよいし、実行したタスクセットの履歴をユーザごとに格納してもよい。リハビリテーション支援装置210は、インターネットを介して他のリハビリテーション支援装置と通信可能に構成されていてもよい。その場合、1つのタスクセットを、同じユーザが複数の場所で実行することもできるし、様々なテンプレートを離れた複数のユーザ同士で共有することもできる。

設定部217は、目標オブジェクト242の仮想空間240における属性として、移動速度、表示数、大きさ、表示位置および表示間隔の少なくともいずれか1つを設定する。推定部218は、目標オブジェクト242に対する身体動作をユーザ220が達成できた場合に、当該目標オブジェクト242の属性に応じて、ユーザ220の認知能力レベルを推定する。設定部217は、目標オブジェクト242の発生を報知したタイミングから、目標オブジェクト242を発生させるタイミングまでの遅延時間を設定してもよく、これによりユーザ220に対して与える認知的な負荷を制御することができる。つまり、ユーザは、レーダスクリーン画像250などによって目標オブジェクトが発生する仮想空間内の位置(ヘッドマウントディスプレイをどの方向に向ければ表示されるかを表す位置)を知ってから、実際に目標オブジェクトが発生するまでの時間、自分が行なうべき動作を継続的に記憶保持しなければならない。この「記憶時間」が、ユーザにとっての認知負荷となる。また設定部217は、「目標オブジェクト152を発生させるタイミングまで」ではなく、「目標オブジェクト152がユーザ220の届く範囲に接近するまで」の時間を変更することにより、認知的な負荷を制御してもよい。設定部217は、目標オブジェクト242以外の背景画像243をヘッドマウントディスプレイ233に表示させることにより、ユーザ220に対して認知的な負荷を与えてもよい。

なお、認知負荷を変更する場合、ユーザに対してあらかじめ認知負荷を上げるまたは下げることを通知することが望ましい。通知方法は視覚的に文字や記号を用いて行っても、音声によって行っても、例えば肩や肘、腕や足を叩くなど、体の一部に触れるような形で行ってもよい。 When changing the cognitive load, it is desirable to notify the user in advance that the cognitive load is increased or decreased. 図3は、オペレータが操作するための画面(操作パネル)300を示す図である。設定部217が、このような操作パネル300を表示させる。本実施形態では、直感的な操作パネル300によって7つのパラメータ(距離、高さ、角度、大きさ、スピード、感度、間隔)を設定することで、姿勢バランス能力と二重課題型の認知処理能力とを総合的に評価する。測定はマニュアルモード(タスク一つ一つのパラメータを設定して課題ごとに操作する方法)と、テンプレートモード(複数のタスクセットにパラメータがあらかじめ設定されている方法)、あるいは機器に自動的にタスクを生成させるお任せモード(オートモード)のいずれか、あるいはそれらを組み合わせて行うことができる。なお、操作パネルからは、ユーザの基本情報から各種の認知および運動機能評価指数、検査結果の確認、テンプレートの作成やオートモードの設定や指示なども行うことができる。

また、各種パラメータ設定領域321~324は、タスクを規定する複数のパラメータを設定するための画面である。設定部217は、各種パラメータ設定領域321~324に対する入力を、不図示の入力デバイスから受け付けることもできる。入力デバイスは、マウスやテンキー、キーボードでもよいし、各種コントローラやゲーム用のジョイスティック、タッチパネル等でも良く、その技術構成を問わない。

各種パラメータ設定領域321~324は、左右の目標オブジェクトの大きさを決める入力領域321と、アバターオブジェクト241の感度範囲の大きさを決める入力領域322と、目標オブジェクトの移動スピードを決める入力領域323と、次に現われる目標オブジェクトの位置を決める入力領域324とを含む。さらに入力デバイス(ホットキー)による目標オブジェクトの出現位置の操作を受け付けるか否かを設定するチェックボックス325を有している。

入力領域321は、右と左のそれぞれにおいて、ユーザに目標オブジェクトの位置を見やすくするための視認用オブジェクトの半径(視認サイズ)と、アバターオブジェクト241と反応する目標オブジェクトの半径(評価サイズ)とを設定可能である。つまり、図3の例では、ユーザには半径20cmの円が見えているが、実際にはその円の中央に位置する半径10cmのボールにタッチしなければタスクを正確に完了したことにならない。視認サイズが小さければ、ユーザは目標オブジェクトを見つけるのが困難になる。視認サイズを大きくすれば、ユーザは目標オブジェクトを見つけやすくなる。評価サイズを大きくすれば、アバターオブジェクト241のずれの許容量が大きくなる。評価サイズを小さくすれば、アバターオブジェクト241のずれの許容量が小さくなり、よりシビアにリハビリテーション動作を評価できる。これらの視認サイズと評価サイズとを一致させることもできる。

入力領域322では、アバターオブジェクト241の左右のセンササイズ(センサオブジェクトのセンサ範囲のサイズ)をそれぞれ設定できる。センササイズが大きければ、手の位置が目標オブジェクトから大きくずれていてもタスクを達成したことになるため、リハビリテーション動作の難易度は下がる。逆にセンササイズが小さければ、手を目標オブジェクトの中央領域(評価用サイズ)に、正確に動かさなければならないため、よりリハビリテーション動作の難易度が上がる。図3Aの例では、センササイズは左右それぞれ2cmとなっている。

入力領域324は、次のタスクの位置(タスクまでの距離と角度)を入力するための画像であり、レーダスクリーン画像250を拡大した形状となっている。チェックボックス325がチェックされているので、入力領域324における複数のブロックのいずれかをクリックまたはタップする操作が行なわれた場合に、操作が行なわれたブロックの位置に対応する仮想空間内の位置に、目標オブジェクト242を発生させる。

つまり、図3の例では、ユーザ220は仮想空間の中で、2cmのセンサ部分を含むアバターオブジェクト(コントローラ)を、左側の遠い場所において45cm/sの速度で落下する半径10cmの目標オブジェクト(ボール)にタイミング良く接触させることがタスクとなる。図3は、ユーザ視野領域301に目標オブジェクト242が出現した状態を示している。この状態で、領域306に示されているようにユーザ220が左腕を伸ばすと、ユーザ視野領域301に、アバターオブジェクト241が現われる。目標オブジェクト242の周囲には、目標オブジェクト242の視認性をよくするための視認用オブジェクト312が表示されている。入力領域321で設定した視認サイズとは、このドーナツ形状の視認用オブジェクト312の半径を示している。
