−最適化のために標的マップ内の領域を優先させる重みマップを皮質表面上に提供し、かつ、
−標的マップおよび重みマップに基づいて、興奮性刺激、抑制性刺激、または中性刺激で同時に上記複数の皮質標的を、より一般的には、拡張された皮質パターンを大域的に刺激するために、経頭蓋電流刺激を提供することを目的とした複数の電極について最適電流および最適場所を計算する、データ処理手段を含むシステムに関する。
a)fMRI、PET、EEG、MEG、または、他のデータに基づき拡張された、重み付き皮質パターン標的マップと協働するという全体的な概念、
b)その大きさまたは強さではなく、1つ以上の電場成分の最適化の重視、
c)電場の法線および接線成分の標的とともに、皮質表面に対する座標系に基づく標的の定義、
d)適切な制約を前提として電極の電流だけではなく数および場所も最適化する高度アルゴリズムの使用、である。
(問題の概説)
非侵襲刺激問題は、以下のように、大まかに分類することができる。即ち、a)単一の孤立した局所的な標的、b)双極、または、より一般的には、多極孤立/局在的な標的、およびc)皮層パターン標的設定。単一の標的の場合について、通常生じる問題は、どのように戻り電流を処理するかである。なぜなら、物理法則では、電流の保存が必要とされ、したがって、最低2つの電極が印加される必要があるからである。帰還(つまり「基準」)電極は、通常、(例えば、「対側軌道上で」)役割を果たさないと推定される領域内に位置決めされ、時には、その影響が拡散するように「活性」領域よりも大きい領域を有するように選ばれる(Nitscheおよび他(2007))。より最新のアプローチとしては、いわゆる「高解像度tDCS」が挙げられ、電極の帰還配置は、活性電極(Dmochowskiら(2011年)およびその参考文献)の近く、または、以下で説明するモンタージュなどのより一般的な準単極モンタージュの近くに置かれ、最適に設置された帰還電極のアレイが採用される(本セクションにおいて以下で「標的設定局在式皮質領域と呼ぶ部分の説明および図1を参照されたい)。
本セクションで説明する電場計算を、Mirandaら(2013年)で説明されている現実的な頭部モデルを使用して実行した。手短に言うと、組織境界をMR画像(頭皮、頭骨、脳室を含む脳脊髄液(CSF)、灰白質および白質)から導出し、有限要素法を使用して適切な境界条件に従って頭部内の電位を計算した。組織を均一かつ等方性であると仮定して、組織の導電率の値を文献から取った。
tCS内の神経細胞の相互作用の基本的なメカニズムは、錐体細胞などの細長いニューロンの母集団との電場の結合から生じると現在考えられる(Roth(1994年)、Biksonら(2004年)、Radmanら(2009年)、Rahmanら(2013年)、Molaee−Ardekaniら。(2013)、Ruffiniら(2013年)および参考文献)。偶然ではなく、そのような母集団は、また、特定の振動数での空間的に首尾一貫した発振のプロセスにおいてEEG信号の主要な発生体であると認知されている(Merletら(2013年)および参考文献を参照されたい)。他の形式のニューロン(例えば、籠細胞などの介在ニューロン)、または、膠細胞などの他の脳細胞の役割は、十分に理解されていない。なぜなら、該細胞の分布および関係が複雑であるが、該細胞のより等方性の構造および分布のためにそのような電場による影響度は原則的に小さいからである。それにもかかわらず、本モデルによれば、tCSの効果をモデル化する差異の必要な第1のステップは、脳内に生成された電場の空間分布を判定することである。
標的マップは、皮質表面内のユーザが定義する領域または領域類とすることができる。標的マップは、ユーザが特別に定義することができるか、または、先述したように(「問題の概説」)、例えば、fMRI、PET、MEG、またはEEGデータに由来することができる。後者の場合、帯域通過フィルタリングおよび皮質マッピング(生成用双極子が皮質表面で制約されるEEGトモグラフィのより単純なバージョン)などの手法を使用して標的マップ(以下で論じる内容を参照されたい)を生成することができよう。実際、EEG結合分析は、電極空間(Rayら(2007年)を参照されたい)ではなく体素または結節レベルにて実行することができ、fcMRIの結合マップと類似の結合マップを提供できる。
電極場所の1組が指定されたと仮定して、標的マップおよび重みマップを考慮して電流の強さの最適化のプロセスをここで説明する。仮想N−電極システムを解く一般的な連立方程式は、[E1(x)...EN−1(x)}・I=E0(x)である(簡潔さのために、法線成分を示すために使用される記号を落した)、式中、En(x)は、(メッシュにおいて各点xにてE電場の法線成分を指定する)特定の双極結合の基底関数解であり、Iは、求められる電流のアレイ、Ε0(x)は、t−マップに関係する標的値である。
一般に、使用される電極の数を制限することを実際的な理由から望まれることになることから、電極場所の空間の追求を実行する必要がある。遺伝的アルゴリズム(GA)が、ここで当てはまるように、導かれた追求の問題を解くために使用されることが多い。手短に言うと、GAでは、進化および自然淘汰に従う染色体が授けられた個人としての最適化問題の候補ソリューションを処理することにより性質を模倣する(導入については、例えば、Mitchell(1998年)を参照されたい)。ここで実行される遺伝的アルゴリズムは、つまり、使用されるべき電極を指定する(この場合、次元N−1)「DNA」2進列による解の定義に基づく。また、最適化関数として、最小二乗誤差、即ち、選ばれた電極場所について可能な限り良好な電流構成を有するソリューションを使用する。交差関数および突然変異関数は、解の子孫が解において電極の最大数の制約に違反しないことを確保するために定義される。DNA列(即ち、特定のモンタージュ)が指定されると、その適合性は、その特定のモンタージュの解を逆変換することにより簡単に計算される。望まれる電極の最大数を上回る解は、強力に罰される。アルゴリズムは、特殊設計の適合性で実行されると、交差関数および突然変異関数は、むしろ素早く(2、3時間で)かつ確実に、解に収束する。
次に、上述した技術を使用する一部のソリューションを示す。表1において、「フルキャップ」27チャンネルソリューションを含め、各モンタージュの特性の要約を示す。電極の数を増大させて8つを越えると、これらの特定の標的マップについて、特に単純なマップほどわずかにだけソリューションのパフォーマンスが向上することを観察することができる。
先に論じたように、典型的なtDCS研究において、特定の脳領域を標的にするために2つの電極を頭皮上に設置する。選ばれたモンタージュの効果は、灰白質(GM)および白質(WM)において誘導されたベクトル電場の空間分布に左右され、双極モンタージュにおいて第2の電極は1次電極として電流の同一量を通電することになることから、望ましくない副作用が、「戻り」つまり「基準」電極上に発生し得る。例えば、標的設定を興奮、つまり、脳卒中リハビリテーションにおける一般的なアプローチ)(Mahmoudiら(2011))の左運動皮質と検討してみよう。ここでは、運動皮質領域内の重みマップの重さは、皮質の残りの2倍の大きさとなるように選ばれ、電場標的は、ゼロである。図1に、C3およびFP2(対側軌道)にわたる25平方cmスポンジでの従来のモンタージュを使用する電場のシミュレーションを示す。誘導電場の広範囲にわたる性質を観察することができ、GA最適化8電極モンタージュと比較すると高い結果的に生じた誤差を観察することができる(上表1を参照されたい)。重み付け相互相関係数は、最良のソリューションについてさえ相対的に低いままであり、所要の重み付け標的マップに適合するために利用可能な自由の限界が反映されていることに注目することができる。同様に、図2は、脳卒中リハビリテーションにおいて使用される双極標的マップを例示し、(例えば、Lindenbergら(2010年)、Mahmoudiら(2011))、一方の興奮性標的は、左運動皮質上にあり、他方の(抑制性)標的は右運動皮質上にある。また、多電極ソリューションは、中性効果標的領域のより良い報告とともに、優れた適合を提供する。
図3に、PETデータ(Maybergら(2005年))に基づく皮質標的マップのソリューションを示す。標的は、治療が効かない大鬱病の脳深部刺激療法に応じた脳血流量(CBF)変化を反映している。したがって、最適化問題は、CBFの変化の大きさに比例した重みマップの標的重さで、CBFが増加した領域を刺激して、CBFが減少する領域を抑制するように設計される(標的マップ)。表1でわかるように、ソリューションは、一部の場所で標的マップに「ヒット」し、一方、伝統的な標的マップは、その成果がかなり劣るため、多部位ソリューションは、有効な重み付き誤差および相関係数の向上(表1)を提供する。
治療が効かない大鬱病の治療の実施例について説明を続ける。下側帯状皮質(SG)に関する皮質rs−fcMRI相関統計的t−マップパターンに基づいて皮質の異なる領域を刺激および抑制する電極モンタージュが生成され、標的重さは、t―マップ大きさに比例している。この場合、目標は、関連のシードの抑制であるため、rs−fcMRIt−マップは、標的マップを生成するために符号を逆にする必要がある。反相関領域を刺激して相関領域を抑制することにより、本件発明者らはこの刺激がSGに伝搬し、かつ、最大限にSGを抑制することになり、抗鬱応答が向上すると仮定することになる。尚、この標的マップに基づいて、左の側背前頭前皮質(DLPFC)にわたる陽極刺激による従来のモンタージュを使用することに対する明らかな理論的根拠はなく、例えば、rs−fcMRI標的マップは、かなり左右対称である。図4に、従来のモンタージュを使用するソリューションではなく8電極モンタージュを使用するこの問題のソリューションを示すが、Fp2上に戻り部位を伴って左Brodmann領域BA46(F3)により示すように左DLPFCを標的にする(Palmら(2012年)、Fregniら(2006年)を参照されたい)。やはり、多電極ソリューションでは、伝統的なモンタージュよりも重み付き誤差は低く、相関係数は高い(表1)。
本発明は、皮質表面により画定されるような電場の成分の現実的な頭部モデル化に基づいて拡張された標的を伴うtDCSモンタージュの最適化の新しい方法および新しいシステムを提供する。これは、より概念的な視点から、および、(本セクションにおける)より特定の視点から先述した。皮質表面上の電場と協働する利点は、電場の皮質面法線(または垂線)成分、または、接線成分、または、例えば、全体的な大きさの最適化を可能にするというものである。この方法は、tCS電場および皮質の一次相互反応に関する現行の知見に基づく。最適化問題は、重さを異なるメッシュ点に帰する標的マップの観点から定義される。この概念は、本発明の方法を非常に柔軟性のあるものにし、かつ、単純なまたは任意の形状を有する1つまたは少数の拡張された均一な標的との協働、または、より重要なことに、刺激に利用可能な電極の数を指定することができるとともに、様々な撮像治療法を使用して取得される「起動」または「結合」など当該の何らかの目安により重み付けされた拡張された標的との協働、を可能にする。一例として、焦点性が、標的外側の結節にて比重量も指定することができるゼロフィールド値を規定することにより達成される。プロトコル最適化における安全が、各電極を通る電流、および、脳に注入される全電流を介して電流を制限することにより対応される。
tACSの症例に対する提案する方法の一般化は、たとえ低周波数(<1kHz)の電場の計算のプロセスがtDCSの場合と本質的に同じである(同じものが、低周波数tRNSに適用される)としても、重要なことである。即ち、E(x)が特定のモンタージュのDC電流および各種電流のソリューション電場である場合、E(x、t)=E(x)cos(2πtf)は、各電流にcos(2πtf)が掛けられる類似のAC事例の解である。ここの実際の問題点は、生理学的に意義がある最適化問題の選択にある。

前記標的マップ内の前記複数の皮質標的のそれぞれについての相対重要度を指定する、前記脳の皮質の表面の重みマップを提供し、
前記標的マップおよび重みマップに基づいて、興奮性刺激、抑制性刺激、または、中性刺激で同時に前記複数の皮質標的を大域的に刺激するために経頭蓋電流刺激を提供することを目的とした複数の電極について最適な電流と、最適な数および場所とを計算する、
データ処理手段、を備えることを特徴とする、システム。 - 前記データ処理手段は、前記脳の皮質の誤差許容差マップまたは誤差許容差オーバーオール値を更に提供しかつ、少なくとも前記標的マップおよび前記誤差許容差マップまたは前記誤差許容差オーバーオール値の組み合わせに基づいて前記経頭蓋電流刺激を提供するために必要とされる電極の最小数を計算する、請求項28に記載のシステム。
- 前記処理手段の制御下で所望の誤差許容差が満たされるかどうかを示す表示手段を更に含む、請求項29に記載のシステム。
- 前記システムは、請求項1〜27のいずれかに記載の前記方法を実行するように適合されることを特徴とする、請求項28〜30のいずれかに記載のシステム。
- コンピュータが請求項1〜27のいずれかに記載の前記方法を実行するためのプログラム命令を含むコンピュータ可読媒体。
- コンピュータが請求項1〜27のいずれかに記載の前記方法を実行するためのプログラム命令を含むコンピュータプログラム。
- 前記複数の電極についての最適な数および場所の前記計算が、交差関数および突然変異関数を有する遺伝的アルゴリズムを用いて行われ、ここで、2進DNA列が、電極の数および位置のモンタージュを指定する、請求項1に記載の方法。
