該外科システムは、
導入器をさらに含み、前記アテローム切除装置は、付着物を含む目標血管に接近するように該導入器を通して挿入可能であり、
該外科システムは、
前記回転可能なシャフトおよびチップを回転させるためのモータと、
前記回転可能なチップによって除去された付着物を近位方向に差し向けるように、前記回転可能なチップの前記複数の開口部、および、前記回転可能なシャフトと前記外側部材との間の間隙を通して付着物を吸引するための吸引装置と、
前記回転可能なシャフトを通して高圧流体を注入するための流体注入装置と、をさらに含み、前記流体は、前記回転可能なシャフトの前記遠位開口部から出て、前記回転可能なチップの前記内側遠位表面に接触し、そこで、前記流体は、前記回転可能なチップによって除去された付着物を近位方向に差し向けるように後方に差し向けられる、ことを特徴とする外科システムを提供する。

長手方向軸線を有する回転可能なチップをさらに含み、前記回転可能なチップは、前記回転可能なシャフトが回転するときに、前記長手方向軸線を中心として回転できるように前記回転可能なシャフトに取り付けられており、前記回転可能なチップは、前記回転可能なシャフトの前記管腔を通して注入された流体が前記回転可能なチップの遠位内側表面に接触し、前記回転可能なチップの回転運動によって除去された付着物を近位方向に差し向けるように近位方向に方向転換されるように、前記回転可能なシャフトの前記遠位開口部が該回転可能なチップの遠位内側表面から軸線方向に間隔を隔てて配置されるように前記回転可能なシャフトの遠位部分に取り付けられており、前記回転可能なチップは、該外科アテローム切除装置のワイヤ上挿入を可能にするようにガイドワイヤを受け入れるためのガイドワイヤ管腔を含む、ことを特徴とするアテローム切除装置を提供する。

回転シャフトと、前記回転シャフトの遠位部分に設けられた回転チップと、を含む付着物除去装置であって、前記回転チップは、前記導入器シースの内径よりも大きい外径を有し、前記回転可能なチップは、前記導入器シースの内径よりも大きい外径を有し、さらに、第1および第2の対向する狭小部分を有し、前記回転シャフトは、外側ねじ山を有する付着物除去装置を準備する段階と、
皮膚切開部を通して血管内に前記導入器シースを挿入する段階と、を含み、前記導入器シースは、該導入器シースの外径に少なくとも等しい切開開口部を形成し、
除去すべき付着物に隣接して前記回転チップを前進させる段階と、
付着物に接触して付着物を血管から除去するように高速で前記回転チップを回転させ、前記回転シャフトの外側ねじ山が付着物を近位方向に差し向けるように前記回転シャフトを回転させるためのモータを作動させる段階と、
流体が前記回転チップの内側表面に接触し、そこで、流体が、前記回転チップによって除去された付着物を近位方向に差し向けるように近位方向に方向転換されるように前記回転シャフトを通して流体を注入する段階と、
前記回転チップの回転運動によって除去された付着物を近位方向に吸引するように真空を印加する段階と、を含むことを特徴とする方法を提供する。

Claims (12)
- 血管の内部からプラークのような付着物を除去するための外科アテローム切除装置であって、該外科アテローム切除装置は、
外側部材と、
前記外側部材内に回転運動できるように位置決めされる回転可能なシャフトと、を含み、前記回転可能なシャフトは、ガイドワイヤを受け入れるように寸法形状決めされた内部を貫通して延びる管腔と、前記管腔と連通した遠位開口部と、を有し、前記管腔は、前記回転可能なシャフトの内径と、前記ガイドワイヤとの間で流体注入を可能にする横断面形状寸法を有し、
長手方向軸線を有する回転可能なチップをさらに含み、前記回転可能なチップは、前記回転可能なシャフトが回転するときに、前記長手方向軸線を中心として回転できるように前記回転可能なシャフトに取り付けられており、前記回転可能なチップは内側管腔を有し、かつ、前記回転可能なシャフトの前記遠位開口部が前記回転可能なチップの遠位端壁の内側表面から軸線方向に間隔を隔てて配置されるように前記回転可能なシャフトの遠位部分に取り付けられており、前記内側管腔は、前記回転可能なシャフトの前記管腔を通して前記シャフトの開口部から外に注入された流体が、前記内側管腔を通して流れ、前記回転可能なチップの前記遠位端壁の内側表面に接触し、前記回転可能なチップの回転運動によって除去されて前記チップ内に受けられた付着物を近位方向に差し向けるように、前記回転可能なシャフトと外側部材との間の隙間に向かって前記チップ内を通して近位方向に逆向きに方向転換されるように構成されており、前記回転可能なチップは、該外科アテローム切除装置のワイヤ上挿入を可能にするようにガイドワイヤを受け入れるためのガイドワイヤ管腔を含み、
前記回転可能なチップは、付着物を除去するための複数の側方開口部を含み、前記複数の側方開口部は、前記チップを通して前記付着物を吸い込み、前記付着物を前記回転可能なシャフトと外側部材との間の隙間を通じて差し向けるように構成され、前記側方開口部は前記遠位端壁よりも近位に位置している、
ことを特徴とする外科アテローム切除装置。 - 前記回転可能なチップは、遠位部分と近位部分の間の中間部分と、該中間部分に設けられたスカラップ状部分と、を有する、ことを特徴とする請求項1に記載の外科アテローム切除装置。
- 前記回転可能なシャフトは、外側表面上に、該回転可能なシャフトが回転されるときに付着物を近位方向に差し向けるためのねじ山領域を有する、ことを特徴とする請求項1または2に記載の外科アテローム切除装置。
- 前記回転可能なチップの近位端は、前記外側部材の遠位縁から軸線方向遠位方向に間隔を隔てて配置される、ことを特徴とする請求項1ないし3のいずれか1項に記載の外科アテローム切除装置。
- 前記回転可能なチップの遠位部分は、弾丸状先端部を有する、ことを特徴とする請求項1ないし4のいずれか1項に記載の外科アテローム切除装置。
- 切除表面を形成するように前記回転可能なチップの外側表面に形成された複数の長手方向に延びる溝をさらに含む、ことを特徴とする請求項1ないし5のいずれか1項に記載の外科アテローム切除装置。
- 前記回転可能なシャフトおよびチップは、モータによって回転される、ことを特徴とする請求項1ないし6のいずれか1項に記載の外科アテローム切除装置。
- 導入器シースと組み合わされた請求項1ないし7のいずれか1項に記載の外科アテローム切除装置であって、該外科アテローム切除装置は、付着物を含む目標血管に接近するように前記導入器シースを通して挿入可能である、ことを特徴とする外科アテローム切除装置。
- 付着物を吸引するように、前記外側部材の外径と前記導入器シースの内径との間に間隙が設けられる、ことを特徴とする請求項8に記載の外科アテローム切除装置。
- 前記導入器シースは、付着物が前記外側部材と該導入器シースとの間の間隙で吸引されるように吸引源に接続可能な側方アームを有する、ことを特徴とする請求項8または9に記載の外科アテローム切除装置。
- 前記回転可能なチップは、前記導入器シースの内径よりも大きい外径を有し、さらに、第1および第2の対向する狭小部分を有する、ことを特徴とする請求項8ないし10のいずれか1項に記載の外科アテローム切除装置。
- 前記導入器シース内に前記回転可能なチップが挿入されることによって、前記導入器シースは、前記回転可能なチップのより大きな外径を収容するように変形され、前記回転可能なチップを前記導入器シースの遠位開口部を通して外へ前進させた後、前記導入器シースは、変形されない形状に戻る、ことを特徴とする請求項11に記載の外科アテローム切除装置。
