Description

1型糖尿病の被験体における非空腹時血中グルコースレベルは、約180mg/dLから約650mg/dLの間であり得る。正常な被験体における非空腹時血中グルコースレベルは、約80mg/dLから約120mg/dLの間であり得る。本開示の方法およびキットでは、血中グルコースレベルを前糖尿病レベルまで回復させることにより、血中グルコースレベルが約80mg/dL、約85mg/dL、約90mg/dL、約95mg/dL、約100mg/dL、約105mg/dL、約110mg/dL、約115mg/dL、または約120mg/dLに回復し得る。いくつかの場合には、血中グルコースレベルを前糖尿病レベルまで回復させることにより、血中グルコースレベルが約80mg/dL未満、約85mg/dL未満、約90mg/dL未満、約95mg/dL未満、約100mg/dL未満、約105mg/dL未満、約110mg/dL未満、約115mg/dL未満、または約120mg/dL未満に回復し得る。

NF−KB ODNで処理したex vivo DCを用いた1型糖尿病の処置
NF−KB ODN DCにより1型糖尿病発生を阻害することができるかどうかを決定するために、糖尿病発生に関して当技術分野において認められているモデルである非肥満糖尿病(NOD)マウスを使用した。雌NODマウスを7週齢でDCを用いて処置した。上記の実施例3に記載の方法に従ってNODマウスからDCを単離した。示されている場合では、次いで、DCを、NF−KB ODNまたはIL−4の存在下で5日にわたって増殖させ、次いで、島抗原(AG)を用いてパルスした。マウスにDC2×106個を注射し、電子グルコメーターによって糖尿病発生をモニターした。血清中グルコースレベルが350mg/mlを上回ることにより、糖尿病発生が示された。
アンチセンスオリゴヌクレオチドをローディングしたマイクロスフェアを用いた1型糖尿病の処置

2mg/kgの、蛍光タグを付けたオリゴヌクレオチドをローディングしたマイクロスフェアをマウスに注射した後、IVIS Luminaワークステーションにおける生動物イメージングを収集した(図12A;CNは、アンチセンスオリゴヌクレオチドを伴わない対照マイクロスフェアを指し、ASは、アンチセンスオリゴヌクレオチドを伴う特定の糖尿病抑制性マイクロスフェアを指す)。マウスを安楽死させた後、種々の器官およびリンパ節を採取し、別々に可視化した(図12B)。示されている通り、蛍光は膵臓の内側に濃縮され、脾臓では非常にわずかな程度までしか濃縮されなかった。図12Cおよび図12Dに、2つの異なるマウスレシピエントの膵臓における2日にわたる蛍光蓄積が要約されている。マイクロスフェアを膵臓より遠位の部位に注射したところ、膵臓の内側にも膵臓リンパ節にも蓄積しなかった(図13;IPは、腹腔内注射を指し、Sub Qは、示されている部位における皮下注射を指す)。
膵臓リンパ節への寛容原性DCの蓄積
ex vivoにおいて骨髄前駆細胞から生成した2×106個細胞を、蛍光タグを付けたスフェアと一緒に注射した後、マウスを安楽死させ、リンパ節を含めた種々の器官を採取した。器官を、IVIS Luminaワークステーションの下で可視化し(図16)、それにより、ex vivo寛容原性樹状細胞が膵臓および膵臓リンパ節に優先的に蓄積することが示された。
寛容原性ヒトDCの調製、投薬量および投与
微生物感染/汚染が存在しないこと、内毒素が体重1kg当たり5EU未満であること、生存能力が70%超であることの確認、およびDCの純度の確認(フローサイトメトリーによる)を得た後、細胞を、380×gで10分遠心分離し、次いで、体積0.5mL〜1.0mLの滅菌した5%ヒト血清アルブミンに再懸濁させ、滅菌した3mLの注射器中に吸引した。針を、27ゲージ5/8’’針と交換してリキャップ(recap)した。注射器をヒト被験体に関するコード化された情報で標識し、投与のために、被験体がいる場所に24時間以内に送達した。その後の注射のため、ならびに離れた場所での細胞送達のために、凍結保存チューブ当たり2.5×106個の細胞をアリコートにし、チューブを液体窒素条件下で保管した。最初の用量として使用する細胞は2時間以内に注射した。
1型糖尿病の安定した処置は寛容原性DCの多数回注射によって達成される
多数回の寛容原性DC注射(n=8)により、大多数の新規発症糖尿病NODマウスにおけるグルコースレベルが100〜280mg/dL範囲内に安定に維持された(図20A)。多数回の対照DC注射(n=8)では、少数のレシピエントにおいて、一過性にではあるが、1型糖尿病が一過性に逆転した。単回の寛容原性DC注射は、多数回注射と比較して、血中グルコース安定性の回復に有効ではなかった。線は、個々のNODマウスにおける非空腹時血中グルコースレベルを示す。糖尿病性高血糖が確認されたら、マウスに、グルコースレベルが280mg/dL未満に低下するまでインスリンを投与した(平均で5〜8日)。この時点で、インスリンを中止した。次いで、DC(対照または寛容原性)2×106個を、胃腸器官を覆う側腹部に皮下注射した。0時間は、糖尿病が確認された時点を表した。x軸の下の黒い矢印は、インスリンを中止し、同時に1回目のDC注射を行った時点を示した。灰色の矢印は、DCを投与した時点を示した(単回または多数回)。グラフ内の破線は、逆転したiDC処置群において測定された最小および最大非空腹時血中グルコースレベルを示した。新規発症糖尿病NODマウスを他のいかなる処置にも供さなかった場合、血中グルコースはインスリンを中止してから1日以内に280mg/dL閾値を超えた(図20B)。
寛容原性DC送達により、in vivoにおける抑制性B細胞の頻度の上昇が促進される
DC(対照または寛容原性)2×106個を、NOD雌マウスにおいて胃腸器官を覆う側腹部に皮下注射した。3日後、脾臓を採取し、CD19+B220+CD11c−IL−10+B細胞ならびにB10 Bregの頻度をフローサイトメトリーによって測定した。絶対数を、組織から回収された生存可能な単一細胞の総数のヘマトクリット測定値に基づいて算出した。
抑制性B細胞は、in vitroにおける同種異系混合リンパ球反応において機能的に抑制性である
脾臓T細胞、照射した脾細胞(単独で、CD19+B220+CD11c−IL−10+B細胞と一緒に、またはその存在下で)の5連ウェルを5日間インキュベートした(図22A)。最終日にBrdUを添加した。6日目にBrdU+細胞の数をフローサイトメトリーによって測定した。培養物は、1×105個の、NOD雌マウス(8週齢)の脾臓由来のT細胞、照射した同種異系脾細胞(C57BL/6雄、8週齢)および精製したB220+CD19+CD11c−IL−10+B細胞からなった。T細胞または脾細胞のみの増殖をこれらの分析における100%増殖を表すものとみなした。バーは、n=5ウェルの平均を示し、エラーバーはSEMを示す。CD19+B220+CD11c−IL−10+B細胞の、cDCまたはiDCの不在下での共培養物における増殖のDCの存在下での共培養物における増殖と比較した差異は統計学的に有意であった(p値がグラフのバーの上に単一のアスタリスク、2重のアスタリスク(p<0.05、ANOVA)または3重のアスタリスク(p<0.01、ANOVA)で示されている)。バーの最後の2つのセットは、DC−Breg:T細胞数の比1:10と1:1でのT細胞の増殖を比較したものである。
寛容原性DCにより、抑制性B細胞の増殖が促進される
野生型マウス系統から新たに採取した脾細胞からフロー選別されたCD19+B220+CD11c−細胞の蛍光特性に基づいて自己蛍光性細胞を排除するための非常にストリンジェントなゲーティングを用いて、IL10gfpトランスジェニックマウスから新たに採取した脾細胞をフロー選別して、CD19+B220+CD11c−GFP+またはCD19+B220+CD11c−GFP−集団にした(右側のパネルの挿入図)(図23A)。本実験において実施した全ての選別の転帰を表す、選別後のGFP+の純度(一番上の四分円プロットにおけるIL−10対FSCで示される)ならびに細胞の生存能力(一番上の四分円プロットの隣のヒストグラムの生/死染色)を示した。選別された細胞(GFP+またはGFP−)5×104個をPBS、または同数のcDCまたはiDCとの共培養物に入れた。cDC、iDCおよび培地を伴う共培養物における5日後のB細胞の代表的なGFP蛍光をヒストグラムに示し、GFP+細胞を培養物中の総細胞に対する%として表した(3回の別々に行った実験を表すこの特定の実験の値がヒストグラムの中に示されている)。
抑制性B細胞の表面マーカーの特徴付け
ゲーティングを示されている通り確立した(CD19+B220+CD11c−)(図24)。このゲーティング内のヒストグラムに示されている表面タンパク質のそれぞれを発現する細胞の頻度により、NOD雌マウス(10週齢)の新たに単離した脾臓におけるDC−Bregの表現型をさらに確立した。示されているデータは、年齢が一致した3匹の異なるNODマウスから新たに得た脾細胞のフローサイトメトリー分析を表す。表面マーカーにはIgD、IgM、CD10、CD21、CD27、CD38およびCD40を含めた。
in vitro同種異系混合リンパ球反応(MLR)における抑制性B細胞
BrdU+T細胞の頻度をフローサイトメトリーによって測定した(図25)。詳細には、IL10gfpトランスジェニックマウスの脾臓から新たに単離したIL10gfpT細胞を、同系のCD1d、CD5、IL−10枯渇B細胞(グラフ中に「IL−10−CD1d−CD5−B細胞」と示されている)および同種異系の照射した脾細胞(Spl)の存在下または不在下で培養した。データは、BrdU+T細胞の対照に対する%として示されており、ここで、対照とは、照射した同種異系脾細胞のみの存在下でのT細胞増殖の頻度を指す(100%とみなす)。B細胞を、B細胞:T細胞の比1:1または1:10で添加した。バーは平均を表し、エラーバーはSEMを表す。共培養物間のT細胞の増殖の差異は統計学的に有意ではなかった(p=0.118、ANOVA)。
抑制性B細胞は、レチノイン酸(RA)受容体を発現する
リアルタイム半定量RT−PCRにより、高度に精製された(フロー選別した)DC−BregおよびIL10gfpトランスジェニックマウスの新たに単離した脾臓由来のB10 Bregにおいて、RA受容体アルファ(図26A)ならびに低レベルのレチノイドX受容体(図26B)の定常状態mRNAの存在が確認された。図26Aおよび図26Bのゲル画像にフロー選別したB10 BregからのRT−PCR産物が示されている。B10 Breg由来のRARアルファおよびRXRの定常状態mRNAレベルを対照として使用し、値を、100%受容体発現を表すものとみなした。非B10 Breg集団におけるRARアルファおよびRXRの定常状態mRNAレベルをゲル画像の下のグラフに示し、B10 Breg値に対して正規化し、過少発現または過剰発現の倍率として示した。定常状態RARアルファmRNAはB10 Breg(グラフ中の最初のバー;左端)、IL−10−CD19+CD5+CD1d+細胞(グラフ中の2番目のバー)、DC−Breg(グラフ中の3番目のバー)、およびIL−10−DC−Breg(グラフ中の最後のバー;右端)において検出された。バーは中央値を示し、エラーバーは標準偏差を示す。これらのデータは、年齢が一致したマウス(10週齢、雌)の2つの異なる脾臓由来のフロー選別された細胞からの定常状態mRNAを示す。
樹状細胞はin vitroにおいて生物活性のRAを産生する
ALDEFLUOR試薬は、アルデヒドデヒドロゲナーゼ発現細胞、したがって、RA産生細胞を染色するものである。cDCはALDEFLUOR+であり、CD11c+ALDEFLUOR+cDCの頻度がCD11c+ALDEFLUOR+iDCと同様であった(cDC:8.7%の細胞がCD11c+ALDEFLUOR+、それに対してiDCでは9.5%)にもかかわらず、1細胞あたりのベースでは(平均蛍光強度;MFI)、iDCのALDEFLUORとの反応性はcDCの2倍であった(84374と比較して44186)(図27A)。示されているフローサイトメトリー分析は、年齢および性別が一致したNOD雌マウス(7週齢)4匹の2連の培養物を表す。

付随する配列表に列挙されている核酸配列およびアミノ酸配列は、37 C.F.R.1.822において定義されている通り、ヌクレオチド塩基については標準の文字略語、およびアミノ酸については3文字コードを使用して示されている。各核酸配列の一方の鎖のみが示されているが、示されている鎖への言及のいずれにも相補鎖が含まれると理解される。
Claims (15)
- 新規発症糖尿病を有する哺乳動物において血中グルコース濃度を低下させるための組成物であって、寛容原性樹状細胞を含み、
前記新規発症糖尿病を有する哺乳動物の膵臓の近位にある1カ所または複数カ所の注射部位に前記組成物の注射が2回またはそれ超投与されることを特徴とし、
前記血中グルコースが、少なくとも24時間の期間にわたって正常な血中グルコース濃度まで低下する、組成物。 - 前記寛容原性樹状細胞が、前記哺乳動物から、または同じ種の異なる哺乳動物から単離されている、請求項1に記載の組成物。
- 前記寛容原性樹状細胞が、予め凍結されている、請求項1から2のいずれか一項に記載の組成物。
- 前記1カ所または複数カ所の注射部位のうちの1カ所が、
(1)前記膵臓から約3.25〜約2.25インチ左外側かつ前記膵臓から約2〜約1インチ上部であるか、
(2)前記膵臓から約1.75〜約0.75インチ左外側かつ前記膵臓から約2〜約1インチ上部であるか、
(3)前記膵臓から約3.25〜約2.25インチ左外側かつ前記膵臓から約3〜約2インチ上部であるか、または
(4)前記膵臓から約1.75〜約0.75インチ左外側かつ前記膵臓から約3〜約2インチ上部である、
請求項1から3のいずれか一項に記載の組成物。 - 前記投与が、少なくとも4カ所の注射部位を含む、請求項1から3のいずれか一項に記載の組成物。
- 第1の注射部位が、前記膵臓から約3.25〜約2.25インチ左外側かつ前記膵臓から約2〜約1インチ上部であり、第2の注射部位が前記膵臓から約1.75〜約0.75インチ左外側かつ前記膵臓から約2〜約1インチ上部であり、第3の注射部位が前記膵臓から約3.25〜約2.25インチ左外側かつ前記膵臓から約3〜約2インチ上部であり、第4の注射部位が前記膵臓から約1.75〜約0.75インチ左外側かつ前記膵臓から約3〜約2インチ上部である、請求項5に記載の組成物。
- 前記組成物の前記注射が少なくとも3回、4回または5回投与されることをさらに特徴とする、請求項1から3のいずれか一項に記載の組成物。
- 前記血中グルコースが、正常な血中グルコース濃度まで低下し、少なくとも約7日の間にわたって前記正常な血中グルコース濃度まで回復する、請求項1〜7のいずれか一項に記載の組成物。
- 前記投与が、追加的な免疫抑制療法の投与を含まない、請求項1から4のいずれか一項に記載の組成物。
- 2回またはそれ超の前記組成物の前記注射が、少なくとも1つの粒子を含み、前記粒子が、配列番号4、配列番号5、配列番号6、もしくは配列番号7として記載されている核酸配列、またはそれらの組合せを含むオリゴヌクレオチドを含む、請求項1から9のいずれか一項に記載の組成物。
- (1)前記少なくとも1つの粒子が少なくとも1つのマーカーで標識されており、任意選択で、前記マーカーが蛍光マーカーであり、好ましくはpH指示薬であるか、または
(2)前記少なくとも1つの粒子の全てがマーカーで標識されているわけではなく、任意選択で、前記マーカーが蛍光マーカーであり、好ましくはpH指示薬である、
請求項10に記載の組成物。 - 前記哺乳動物がヒトである、請求項1から11のいずれか一項に記載の組成物。
- 前記哺乳動物が、最大で1年にわたって1型糖尿病の臨床発症を有する、請求項1に記載の組成物。
- 前記臨床発症が、(i)高血糖、(ii)前記哺乳動物の前記血中グルコース濃度の制御の不能、または(iii)それらの組み合わせを含む、請求項13に記載の組成物。
- 前記寛容原性樹状細胞がオリゴヌクレオチドを含み、前記オリゴヌクレオチドが、配列番号4、配列番号5、配列番号6または配列番号7として記載されている核酸、あるいはそれらの組み合わせを含む、請求項1から9および12から14のいずれか一項に記載の組成物。
