聴覚刺激および体性感覚刺激:Open ExとRX8 DSP(TDT、Alachula、FL)とを用いてサンプリング周波数を100kHzに12ビットの精度で設定し、余弦ウインドウでゲート制御されるトーン信号(持続時間50ミリ秒、2ミリ秒の上昇/下降時間)を生成した。HB7アンプ(TDT、Alachula、FL)により動かされるシールドスピーカー(DT770、Beyer)により、音を中空耳棒を通じて左側の耳に伝えた。外耳道を近似する1/4インチ細長チューブにより中空耳棒に取り付けられたコンデンサーマイクロホンを用いてシステム応答を測定した。システム応答を説明するために、プログラマブル減衰器(PA5、TDT、Alachula、FL)を用いて音レベルを調整し、較正されたレベル(dB SPL)を200Hz〜24kHzの周波数で伝えた。同心状のバイポーラ電極を介してSp5へ伝えられる1000Hzの3つの二相(100μs/相)電流パルスを用いて、DCNに向けて突出すると知られている体性感覚脳幹核(somatosensory brainstem nuclei)内のニューロンを活性化した。動きアーチファクトを引き起こさない最も高いレベル(範囲:50〜70μA)に電流振幅を設定した。
Claims (27)
- 被験体の聴覚疾患を治療するためのシステムであって、
音声信号発生器(908)と体性感覚刺激信号発生器(910)とプロセッサ(902,14)とメモリ(16)とを有するバイモーダル刺激システムとを備えており、
該バイモーダル刺激システムが、
音声信号発生器内で、第一の発火点および第一の発火周期ならびに/または継続期間を有する聴覚刺激信号を生成し、
被験体の体性感覚系を刺激するための体性感覚刺激信号であって、第二の発火点および第二の発火周期、刺激オンセットならびに/または継続期間を有する体性感覚刺激信号を生成し、
聴覚疾患を軽減するように前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差を確定するように構成されており、
聴覚疾患を軽減するために、前記第一の発火周期のオンセットが前記第二の発火周期のオンセットと重ならないように、前記第一の発火周期および前記第二の発火周期が非同期に維持されるように構成されてなる、システム。 - 前記バイモーダル刺激システムが、前記第一の発火点と前記第二の発火点との間のタイミング順番を変更して所望の聴覚疾患軽減を達成するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、前記第一の発火点と前記第二の発火点との間のタイミング差を調節して所望の聴覚疾患軽減を達成するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、前記体性感覚系を前記被験体の脳に刺激を加えて刺激することにより、前記被験体に前記体性感覚刺激信号を伝えるようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、顔刺激を通じて前記被験体の三叉神経に刺激を加えて前記体性感覚系を刺激することにより、前記被験体に前記体性感覚刺激信号を伝えるようにさらに構成されてなる、請求項4に記載のシステム。
- 前記バイモーダル刺激システムが、前記被験体の深部脳領域に刺激を加えて前記体性感覚系を刺激することにより、前記被験体に前記体性感覚刺激信号を伝えるようにさらに構成されてなる、請求項4に記載のシステム。
- 前記バイモーダル刺激システムが、前記被験体の脳の表面領域に刺激を加えて前記体性感覚系を刺激することにより、前記被験体に前記体性感覚刺激信号を伝えるようにさらに構成されてなる、請求項4に記載のシステム。
- 前記聴覚刺激信号および前記体性感覚刺激信号のうちの少なくとも1つが機械的刺激または電気的刺激を通じて前記被験体に伝えられてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、前記被験体のタイミングプロファイルであって、
前記第一の発火点と前記第二の発火点との間のさまざまなタイミング順番およびタイミング差のタイミングデータを含み、さらに、これら異なるタイミング順番およびタイミング差についての知覚された聴覚疾患データを含む、タイミングプロファイルを求め、
前記聴覚刺激信号および前記体性感覚刺激信号の推奨聴覚疾患治療計画を求める際にアクセスするために前記タイミングプロファイルを格納するようにさらに構成されてなる、請求項1に記載のシステム。 - 前記バイモーダル刺激システムが、前記被験体のための初期タイミングプロファイルであって、
前記第一の発火点および前記第二の発火点の初期タイミング順番および初期タイミング差を含み、オンセット後の聴覚疾患を軽減すると判断される、タイミングプロファイルを求めるようにさらに構成されてなる、請求項1に記載のシステム。 - 前記バイモーダル刺激システムが、聴覚疾患のオンセットを示す情報を受け取り、前記聴覚疾患を軽減するように前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差を自動的に確定するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差をある期間にわたって調節して該ある期間にわたる聴覚疾患軽減プロファイルを求め、該聴覚疾患軽減プロファイルを格納するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、聴覚疾患軽減プロファイルにアクセスし、前記聴覚疾患軽減プロファイルから前記タイミング順番および前記タイミング差を求めることにより、前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差を確定するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、治療時間を特定し、聴覚疾患軽減プロファイルにアクセスし、前記聴覚疾患軽減プロファイルから前記治療時間に対応する前記タイミング順番および前記タイミング差を特定し、特定された対応する前記タイミング順番および前記タイミング差に基づいて、生成された前記聴覚刺激信号および前記体性感覚刺激信号を伝えることにより、前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差を確定するようにさらに構成されてなる、請求項1に記載のシステム。
- 特定された前記治療時間が現在の時間である、請求項14に記載のシステム。
- 特定された前記治療時間が将来の時間である、請求項14に記載のシステム。
- 前記バイモーダル刺激システムが、聴覚疾患を軽減するように前記第一の発火周期および/または前記第二の発火周期を調節するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、耳鳴りを軽減するように前記第一の発火周期および/または前記第二の発火周期を周期的に調節するようにさらに構成されてなる、請求項1に記載のシステム。
- 前記バイモーダル刺激システムが、前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差を調節して特定のニューロンの発火頻度を選択的に増大または減少させるようにさらに構成されてなる、請求項1に記載のシステム。
- 前記聴覚疾患が耳鳴りである、請求項1に記載のシステム。
- 前記聴覚疾患が聴覚過敏である、請求項1に記載のシステム。
- 被験体の聴覚疾患を治療するためにプロセッサにより実行可能な一組のインストラクションを格納するコンピュータ読取可能格納媒体であって、
前記一組のインストラクションが、
第一の発火点および第一の発火周期、刺激オンセットならびに/または継続期間を有する聴覚刺激信号を生成するためのインストラクションと、
被験体の体性感覚系を刺激する体性感覚刺激信号であって、第二の発火点および第二の発火周期、刺激オンセットならびに/または継続期間を有する、体性感覚刺激信号を生成するためのインストラクションと、
聴覚疾患を軽減するように前記第一の発火点と前記第二の発火点との間のタイミング順番およびタイミング差を確定するためのインストラクションとを含んでおり、
聴覚疾患を軽減するために、前記第一の発火周期のオンセットが前記第二の発火周期のオンセットと重ならないように、前記第一の発火周期および前記第二の発火周期が非同期に維持されるように構成されてなる、コンピュータ読取可能格納媒体。 - 前記聴覚疾患が耳鳴りである、請求項22に記載のコンピュータ読取可能格納媒体。
- 前記聴覚疾患が聴覚過敏である、請求項22に記載のコンピュータ読取可能格納媒体。
- 前記インストラクションが、聴覚刺激信号および体性感覚刺激信号の最適なパラメータ値を求めて前記聴覚疾患に関与する聴覚伝導路および非聴覚伝導路内のニューロンの発火頻度を変更するためのインストラクションをさらに含んでなる、請求項22に記載のコンピュータ読取可能格納媒体。
