Description
1.ABCトランスポーター阻害剤を含む、細胞増殖性疾患細胞からの放射性治療薬または放射性診断薬の排出を阻害し得る、細胞増殖性疾患治療用薬剤または細胞増殖性疾患診断用薬剤。
2.ABCトランスポーター阻害剤と、放射性治療薬を含む、細胞増殖性疾患の治療用のキット。
3.ABCトランスポーター阻害剤がmultidrug resistance-associated protein(MRP)阻害剤である、前項2に記載の細胞増殖性疾患治療用キット。
4.MRP阻害剤が、multidrug resistance-associated protein 1(MRP1)およびmultidrug resistance-associated protein 4 (MRP4)の少なくともいずれか1つのトランスポーターを阻害する薬剤であり、放射性治療薬が放射性元素により標識された3-ヨードベンジルグアニジン(MIBG)、その塩およびそれらの誘導体から選択される化合物である、前項3に記載の細胞増殖性疾患治療用キット。
5.MRP阻害剤が、MK-571またはプロベネシド(PBC)である、前項3または4に記載の細胞増殖性疾患治療用キット。
6.ABCトランスポーター阻害剤と、放射性診断薬とを含む、細胞増殖性疾患の診断用のキット。
7.ABCトランスポーター阻害剤が、multidrug resistance-associated protein(MRP)阻害剤であり、放射性診断薬が放射性元素により標識されたテトロホスミン(TF)、2-メトキシイソブチルイソニトリル(MIBI)、それらの塩およびそれらの誘導体から選択される化合物である、前項6に記載の細胞増殖性疾患診断用キット。
8.MRP阻害剤が、multidrug resistance-associated protein 1(MRP1)、multidrug resistance-associated protein 2(MRP2)およびmultidrug resistance-associated protein 3 (MRP3)の少なくともいずれか1つのトランスポーターを阻害する薬剤であり、放射性診断薬が放射性元素により標識されたTF、その塩およびそれらの誘導体から選択される化合物である、前項7に記載の細胞増殖性疾患診断用キット。
放射性診断薬としてカーディオライト(R)第一(富士フイルムRIファーマ株式会社)及びマイオビュー(R)「注射用」(日本メジフィジックス)を用いた。それぞれの添付マニュアルに記載されている方法に従い、99mTc注射液(370 MBq、2 mL)を用いて標識を行った。薄層クロマトグラフィー(TLC)により、各診断薬の純度が95 %以上あることを確認した。
ヒト由来神経芽細胞腫培養細胞株SK-N-SH (Riken Cell Bank)、ヒト由来肺腺癌培養細胞株H441 (American Type Culture Collection,ATCC)、A549 (ATCC)、ヒト由来前立腺癌培養細胞株PC3 (ATCC) の4種の癌培養細胞株を使用した。SK-N-SHはFetal Bovine Serum (FBS,Gibco) 10 %を混合したMEM(Wako)、H441およびPC3はFBS10 %、sodium pyruvate (Sigma) 1 %を混合したRPMI-1640 Medium (Sigma)、A549はFBS 10%を混合したD-MEM High Glucose with L-Glutamine、Phenol Red、Pyruvate (DMEM,Wako) の培地を用いて培養した。

定量的PCRには、Brilliant III FastSYBR Green QPCR Master Mix (Agilent Technologies) を用いた。反応溶液の組成は、12.5 μLの2×Brilliant III Fast SYBR Green QPCR Master Mix、至適濃度のプライマー、テンプレート(1 チューブ当たり10 ng cDNA(RNA 換算)もしくは検量線用人工合成プラスミド、RNase-free 水である(最終容量25μL)。RT-PCR により得られた結果はMxPro QPCR Software Version 4.10 (Agilent Technology) を用いて解析を行った。
ABCトランスポーターが発現している癌培養細胞としてSK-N-SH、H441、A549およびPC3 の4種類のヒト由来癌培養細胞を用いて実験を行った。細胞培養用175 cm2セルカルチャーフラスコ (Falcon) において、参考例2と同様の方法で、各細胞を培養した。SK-N-SHは12ウェル細胞培養用マルチウェルプレート(IWAKI) に1 ウェルあたり2×105 cells/mL、その他の3種類の細胞は1 ウェルあたり1×105 cells/mL になるように播種した。SK-N-SH は播種から約48 時間後に、その他の癌培養細胞は約24時間後に実験を行った。インキュベーション用緩衝液としてModified Hanks’ balanced salt solution (MHBS) (0.952 mM CaCl2,5.36 mM KCl,0.441mM KH2PO4,0.812 mM MgSO4,136.7 mM NaCl,0.385 mM Na2HPO4,25 mM D-glucose)を用いた。各培養液をインキュベーション用緩衝液であるMHBSで置換し、10 分間のプレインキュベーションを行った。阻害剤無負荷時の実験として、1 ウェルあたり、約37 kBqの99mTc-MIBIまたは99mTc-TFを投与後、5、10、30、60分間のインキュベーションを行った。阻害剤負荷時には、P-gp阻害剤のVERとMRP阻害剤のMK571を用いた。VERとMK571の最終濃度はそれぞれ50 μMとなるように99mTc-MIBIまたは99mTc-TF溶液とMHBSを用いて調整し、阻害剤無負荷時の実験と同様の時間でインキュベーションを行った。阻害剤の有無に関わらず、インキュベーション後に、99mTc-MIBIまたは99mTc-TF溶液を含んだMHBSを除去し、氷冷したMHBSで細胞が入ったウェルを2回洗浄した。0.1N NaOHで細胞を溶解させオートウェルカウンタで放射能を測定した。また解析結果は各癌細胞内の蛋白量1 mgあたりの集積薬剤物質量で表した。蛋白の定量は、PierceR BCA Protein Assay Kit (ThermoScientific) を用いて行った。吸光度は、Multiskan FC 吸光マイクロプレートリーダ (Thermo Scientific)により測定し、SkanItTM Software 2.5.1 (Thermo Scientific) で解析を行った。統計解析には、Student’s t-testを用いた。また、有意差の判定はP < 0.05 またはP < 0.01 とした。
H441における両放射性診断薬の集積量の経時変化を図4C,Dに示す。99mTc-MIBIの集積量は、controlでは投与後30 分まで経時的に増加したが、その後若干減少傾向を示した。MK571負荷時には集積量が経時的に増加し、投与後10 分を除いてMK571負荷時はcontrolに比べて集積量が有意に増加した。99mTc-TFの集積量は、controlでは投与後30 分まで経時的に増加したが、その後若干減少傾向を示した。また、すべてのインキュベーション時間でVER負荷時にはcontrolに比べて、集積量が有意に増加した。99mTc-MIBIと99mTc-TFの結果を比較すると、SK-N-SHと同様に、MK571負荷時は99mTc-MIBIが99mTc-TFに比べて集積量の増加が大きく、集積量の経時変化において両者の違いが見られた。また、投与後60 分におけるcontrolに対するMK571負荷時の集積量は、99mTc-MIBIで約12.5 倍、99mTc-TFで約8.6 倍高くなった。
放射性治療薬として131I-MIBG (富士フイルムRIファーマ株式会社) を用いた。本参考例3は、参考例1と同様にして行った。MRP 阻害剤としてMK571、およびprobenecid (PBC,Invitrogen) を使用した。なお両阻害剤ともMops-Tris 緩衝液で溶解し、1 サンプル当たりに加える阻害剤の最終濃度がそれぞれ50 μMと1 mMとなるように調整した後、阻害剤無負荷時の薬液調整においてMops-Tris 緩衝液を0.5 μL を注入した代わりに、阻害剤の溶液を0.5 μL 加えた。

製薬株式会社) の5種類の癌培養細胞株を使用し、参考例2で用いたプライマーに加えて以下のプライマーを使用した以外は、参考例2の手法と同様にして各ABCトランスポーターの発現を、RT-PCRを用いて行った。

ABCトランスポーターが発現している癌細胞として、131I-MIBGによる治療対象となるヒト神経芽細胞腫由来細胞株SK-N-SHと、MRP1とMRP4の高発現モデル細胞であるヒト由来結腸腺癌細胞株DLD-1の2種類のヒト由来癌細胞を用い、実施例1と同様の方法で、実験を行った。阻害剤無負荷時の実験として、各細胞を播種したプレートの1 ウェルあたりに、約37 kBqの131I-MIBGを投与して、SK-N-SHにおいては1 分間から180 分間、DLD-1においては1 分間から5 分間のインキュベーションを行った。阻害剤負荷時の実験には、MRP阻害剤としてMK571およびPBCを用いた。MK571とPBCの最終濃度はそれぞれ50 μMと1 mMとなるように131I-MIBG 溶液とMHBSを用いて調製し、阻害剤無負荷時の実験と同様の時間でインキュベーションを行った。解析は実施例1と同様の方法で行った。
臨床で131I-MIBGの治療対象となっているSK-N-SH における131I-MIBGの滞留性を検討するため、長時間の阻害剤負荷による排出阻害実験を行った。投与後1分から180分までの131I-MIBG集積量の経時変化を図8に示した。controlおよび阻害剤負荷時の集積量は投与後120 分にピークに達した。また、controlとMK571負荷時の集積量を比較すると、ほとんどのインキュベーション時間において、MK571負荷時にはcontrolに比べて、集積量が有意に増加した 。さらにPBC負荷時の集積量は、投与後全ての時間でcontrolの集積量に比べて有意に増加した。また、MK571負荷時に比べPBC負荷時の集積量が高い傾向を示した。

- 放射性治療薬として放射性元素により標識された3-ヨードベンジルグアニジンとABCトランスポーターの阻害剤としてMK-571又はプロベネシドを含む細胞増殖性疾患の治療用のキット。
