Description
The present invention relates to a balloon-type ablation catheter for electrically isolating pulmonary veins.
Recently, as an ablation catheter for electrically isolating the pulmonary vein that is the site of occurrence of arrhythmia from the left atrium, a catheter shaft, a balloon attached to the distal end of the catheter shaft, and a balloon disposed inside the balloon A balloon-type ablation catheter including an electrode and a temperature sensor inside the balloon has been introduced (see, for example, Patent Document 1).
According to such a balloon type ablation catheter, the balloon mounted on the distal end side of the catheter shaft is expanded by supplying a liquid therein, and the expanded balloon is pressed so as to close the pulmonary vein opening, By applying a high-frequency current between the balloon inner electrode and the counter electrode plate, the temperature of the liquid supplied to the inside of the balloon is raised (for example, 60 ° C. or higher), and the surface of the balloon is heated. It is possible to cauterize the myocardial tissue around the contacted pulmonary vein mouth (the junction between the pulmonary vein and the left atrial wall and the left atrial wall around the pulmonary vein) in a zonal shape (planar shape).
This eliminates the need for repeated dozens of dot-like ablation to form an ablation line that isolates the pulmonary veins, so that one pulmonary vein can be isolated by a single ablation, reducing the procedure time. And the burden on the patient can be reduced.
However, the balloon-type ablation catheter described in Patent Document 1 has the following problems.
(1) Since the thermal energy of the balloon (heated expansion liquid) is difficult to be conducted to the inside (deep part) of the myocardial tissue in contact with the surface of the balloon, only the vicinity of the surface of the myocardial tissue is cauterized. In the deep part of the myocardial tissue, a sufficient region (cautery nest) is not formed.
(2) The entire surface of the balloon is at a high temperature (caustic temperature), and a thrombus is likely to be formed near the surface of the balloon that is not in contact with the myocardial tissue.
(3) Since the entire surface of the balloon is at a high temperature (temperature capable of cauterization), when a healthy part of the heart comes into contact with the surface of the balloon, the healthy part may be cauterized.
The present invention has been made based on the above situation.
The object of the present invention is to form a region (cautery nest) that reaches the deep part of the myocardial tissue, and it is difficult for thrombus to form near the surface of the balloon during the procedure, and the healthy part of the heart contacts the surface of the balloon. Another object of the present invention is to provide a balloon type ablation catheter in which the healthy site is not cauterized.

(1) The ablation catheter device of the present invention is an ablation catheter device comprising a balloon type ablation catheter for electrically isolating a pulmonary vein and an electrode catheter for measuring pulmonary vein potential;
The balloon ablation catheter includes a catheter shaft having a plurality of lumens;
A tip attached to the tip of the catheter shaft;
A balloon that is attached to the catheter shaft so as to enclose the distal end portion of the catheter shaft, and expands when liquid flowing through any lumen of the catheter shaft is supplied to the inside thereof;
A balloon surface electrode comprising a metal thin film formed on at least a part of the outer surface of the balloon, to which a high-frequency current is passed between a counter electrode attached to the patient's body surface,
A wire that is electrically connected to the balloon surface electrode and that is inserted into any lumen of the catheter shaft (a lumen different from the lumen through which the liquid flows),
In order to irrigate the balloon surface electrode with a liquid for expanding the balloon in at least a part of the formation area of the balloon surface electrode, a large number of irrigations are performed from the inner surface of the balloon to the surface of the balloon surface electrode. A through hole is formed ,
The catheter shaft has a lumen for inserting the electrode catheter together with the lumen for allowing the liquid to flow and the lumen for inserting the conducting wire,
An opening communicating with the lumen for inserting the electrode catheter is formed in the distal tip ;
The electrode catheter comprises a catheter body, and a ring-shaped catheter tip portion connected to the distal end side of the catheter body and having a plurality of electrodes,
The catheter tip of the electrode catheter can be extended from the opening formed in the tip of the balloon-type ablation catheter .
(2) In the ablation catheter device of the present invention, the distal tip of the balloon-type ablation catheter communicates with the lumen through which the electrode catheter is inserted, and is a side hole that opens on the side peripheral surface of the distal tip Formed,
It is preferable that the distal end portion of the electrode catheter is inserted into the side hole formed in the distal end tip of the balloon type ablation catheter and can extend from the opening of the side hole.
According to the balloon-type ablation catheter having such a configuration, the myocardial tissue in contact with the balloon surface electrode is caused by a high-frequency current that is passed between the balloon surface electrode and the counter electrode attached to the patient's body surface. Since the deep portion is also sufficiently heated (high-frequency heating), a region (cautery nest) extending from the surface of the myocardial tissue to the deep portion can be reliably formed.
Further, according to the balloon type ablation catheter having such a configuration, the vicinity of the balloon surface electrode in contact with the myocardial tissue to be ablated becomes a high temperature (temperature capable of ablation), but the balloon surface electrode is not formed. Since the surface of the balloon and the vicinity of the balloon surface electrode spaced apart from the myocardial tissue to some extent do not become high temperature, the formation of thrombus in the vicinity thereof can be prevented.
In addition, since the balloon surface electrode is irrigated with the liquid supplied through the irrigation through-hole, the formation of a thrombus is ensured even in a place where the blood flow is stagnant due to a short distance from the myocardial tissue. Can be prevented.
Further, in the conventional balloon type ablation catheter described above, since the myocardial tissue is cauterized by the thermal energy of the balloon (heated expansion liquid), even if the supply of heat energy is stopped, the heated expansion catheter is used. It takes time to lower the temperature of the liquid, and when a healthy part of the heart contacts the surface of the balloon, the healthy part may be cauterized. In contrast, according to the balloon-type ablation catheter of the present invention, the surface of the balloon on which the balloon surface electrode is not formed becomes high temperature even when the healthy part of the heart contacts the surface of the balloon (balloon surface electrode). In addition, when a healthy part comes into contact with the balloon surface electrode, the temperature in the vicinity of the balloon surface electrode can be immediately reduced by stopping energization of the high-frequency current, so there is a risk that the healthy part is cauterized. Can be reduced.
(3) In the balloon-type ablation catheter provided in the ablation catheter device of the present invention, the metal thin film constituting the balloon surface electrode is a thin film made of gold, a platinum alloy or a cobalt chromium alloy and having a thickness of 2.5 to 10.0 μm. Preferably there is.
(4) In the balloon type ablation catheter provided in the ablation catheter device of the present invention, the balloon at the time of expansion has a ratio (D / L) of the balloon diameter (D) to the balloon length (L) of 1.1 to 5.0. A substantially spheroid (flat ellipsoid) is preferable.
(5) In the balloon-type ablation catheter provided in the ablation catheter device of the present invention, it is preferable that the balloon surface electrode is formed on the outer surface on the distal end side of the maximum diameter portion of the balloon.
(6) In particular, a ring-shaped balloon surface electrode may be formed around the central axis of the balloon on the outer surface on the tip side of the largest diameter portion of the balloon.
(7) In the balloon-type ablation catheter provided in the ablation catheter device of the present invention, the balloon surface electrode is composed of a plurality of electrode segments divided at equal angular intervals along the circumferential direction of the balloon. It is preferable that different power can be supplied.
(8) In the balloon ablation catheter comprising the ablation catheter device of the present invention, it is preferable that the distal tip is an electrode.
According to the balloon-type ablation catheter of the present invention, a region (cautery nest) reaching the deep part of the myocardial tissue can be formed, and thrombus is hardly formed near the surface of the balloon (balloon surface electrode) during the procedure. Even if a healthy part of the heart comes into contact with the surface (balloon surface electrode) of the heart, the temperature in the vicinity of the balloon surface electrode can be immediately reduced by stopping energization of the high-frequency current, so that the risk of the healthy part being cauterized Can be reduced.

On the other hand, when the value of the ratio (D / L) exceeds 5.0, the outer diameter (wrapping diameter) when such a balloon is folded and wound around the catheter shaft may be excessive.

As ablation treatment (procedure) performed using the
Note that it is possible to determine whether or not sufficient cauterization has been performed by measuring the electric potential before and after cauterization using the electrode catheter 150 (not shown in FIG. 9). When it is determined that sufficient cauterization has been performed, Complete this cauterization treatment in the pulmonary veins. In addition, when it is determined that the cauterization is insufficient and the touch-up is necessary, the
According to the
More specifically, in the conventional Baarn type ablation, the liquid inside the balloon is heated to about 60 ° C., and the heat causes the myocardial surface to be cauterized. Therefore, the surface of the myocardium in contact with the balloon is cauterized, but the heat is not easily transmitted to the deep part. On the other hand, in the case of high-frequency heating, the myocardial tissue is heated by a high-frequency current between a counter electrode on the body surface (for example, one that is applied to the entire back of the patient) and the balloon surface electrode. Therefore, the high-frequency current flows in the deep direction when viewed from the myocardial tissue. Then, due to the difference in surface area between the counter electrode plate and the balloon surface electrode, when a high-frequency current is applied, the current density near the myocardial tissue increases, and heating near the myocardial tissue (in the deep direction) is performed. Therefore, in the case of high-frequency heating, a region in the deep direction of the myocardium is reliably formed.
In addition, the surface of the
In addition, since the surface of the
Further, in the conventional balloon type ablation catheter described above, since the myocardial tissue is cauterized by the thermal energy of the balloon (heated expansion liquid), even if the supply of heat energy is stopped, the heated expansion catheter is used. It takes time to lower the temperature of the liquid, and when a healthy part of the heart contacts the surface of the balloon, the healthy part may be cauterized. On the other hand, according to the
Second Embodiment
The ablation catheter of this embodiment is different from the
As shown in FIG. 10, the
Different conducting wires are connected to the
Anatomically, all four pulmonary vein openings are located at the boundary between the left anterior and left atrial walls, so the area around the pulmonary vein opening against which the balloon surface electrode is pressed is It is divided into a region constituting the wall and a region constituting the left atrial posterior wall.
Here, the myocardium in the left anterior wall is relatively thick, and the myocardium in the left atrial rear wall is relatively thin. In addition, an esophagus is located behind the left atrial posterior wall, which is relatively thin.
For this reason, if a suitable electric power is applied to the periphery of the pulmonary vein opening to the myocardial tissue constituting the left atrial posterior wall, the myocardial tissue constituting the left atrial anterior wall cannot be cauterized sufficiently, When a suitable electric power is applied to the constituting myocardial tissue, the myocardial tissue constituting the left atrial posterior wall may be excessively cauterized or the esophagus may be overheated to cause esophageal fistula and the like.
According to the
