Description
ROPの主な病理学的変化である網膜の新生血管形成は、局所虚血及びその後の異常な血管新生の発達に関連する。ROPは、網膜剥離により起こる失明等の大きな続発症に関与することが知られている。しかしながら、ROP発症を調整する因子については良く分かっていない。酸素負荷を受ける組織の制限や、抗酸化剤、特にビタミンEの使用等の予防的処置はこれまでのところ大きな成功を収めていない。
また、血管新生の観点から、ROPの治療又は予防に有効な薬剤が提案されている。このような薬剤としては、特定のシアリルオリゴ糖(例えば、特許文献1)、インスリン様成長因子I(IGF−1)又はその類似体とインスリン様成長因子結合タンパク質又はその類似体とを組み合わせて含む組成物(例えば、特許文献2)、硝酸塩、亜硝酸塩等(例えば、特許文献3)などが挙げられる。
従って、本発明は、ROPの発症機構に即したROPの治療又は予防剤、ROPの検査方法及びROPの治療又は予防物質のスクリーニング方法を提供することを目的とする。
本発明は以下のとおりである。
[1] 肥満細胞に由来するトリプターゼインヒビター及び肥満細胞スタビライザーからなる群より選択された少なくとも1種の化合物を有効成分とする未熟児網膜症の治療又は予防剤。
[2] 未熟児網膜症の治療又は予防を必要とする患者に対して、治療又は予防に有効な量の[1]記載の治療又は予防剤を投与することを含む未熟児網膜症の治療又は予防法。
[3] 被検者に由来する生体試料から、肥満細胞の脱顆粒により放出され得るマーカー物質を検出すること、前記マーカー物質の検出量に基づいて、被検者が未熟児網膜症であるか否かを判定すること、あるいは、未熟児網膜症の治療又は予防を必要とするか否かを判定すること、
を含む、未熟児網膜症の検査方法。
[4] 被検者に由来する生体試料から、肥満細胞の脱顆粒により放出され得るマーカー物質を検出すること、前記マーカー物質の検出量に基づいて、被検者が未熟児網膜症の治療又は予防が必要な患者か否かを判定すること、未熟児網膜症の治療又は予防が必要な患者に対して、肥満細胞に由来するトリプターゼインヒビター及び肥満細胞スタビライザーからなる群より選択された少なくとも1種の化合物を有効成分とする未熟児網膜症の治療又は予防剤をその治療又は予防に有効な量で投与することを含む未熟児網膜症の治療又は予防法。
[5] 肥満細胞を、肥満細胞の脱顆粒により放出され得るマーカー物質の放出誘導処理に供すること、前記マーカー物質放出誘導処理に供された肥満細胞に、候補物質を接触させること、接触後の肥満細胞から放出された前期マーカー物質の検出を行うこと、並びに、前記マーカー物質の検出量の測定結果に基づいて、前記候補物質が前記マーカー物質の放出阻害活性を有するか否かを判定し、前記マーカー物質の放出阻害活性を有すると判定された候補物質を、未熟児網膜症の治療又は予防効果を発揮しうる目的物質としてスクリーニングすること、
を含む、未熟児網膜症の治療又は予防物質のスクリーニング方法。
本発明は、以下の点で画期的である。
(1)数多くの研究が、肥満細胞の役割として眼アレルギーの仲介を強調してきたが、本発明のように、肥満細胞が虚血性網膜症を原因とする眼疾患に関与することを明らかにした先例はない。
(2)高酸素濃度状態から通常酸素状態への酸素濃度の急激な変化が、肥満細胞の血管新生効果の引き金になることを見出したが、かかる機能変化を報告した先例はない。
(3)肥満細胞トリプターゼが、ROPにおける網膜新生血管形成の主要なプロモーターであることを見出したが、かかる知見を報告した例はない。
本発明にかかるROPの治療又は予防剤は、肥満細胞に由来するトリプターゼインヒビター及び/又は肥満細胞スタビライザーを有効成分とするので、高酸素濃度条件下で肥満細胞から放出される因子によって誘導される血管新生を、抑制することができる。これにより、ROPの発症における予防効果又は治療効果が期待できる。
即ち、本発明は、このような肥満細胞が、正常な血管新生や腫瘍関連血管新生ではなく、ROPの発症機構に深く関与しているという新しい知見に基づくものである。
以下、本発明について説明する。
また本明細書において「〜」は、その前後に記載される数値をそれぞれ最小値および最大値として含む範囲を示すものとする。
さらに本明細書において組成物中の各成分の量は、組成物中に各成分に該当する物質が複数存在する場合、特に断らない限り、組成物中に存在する当該複数の物質の合計量を意味する。
本発明におけるROPの治療又は予防剤は、肥満細胞に由来するトリプターゼインヒビター及び肥満細胞スタビライザーからなる群より選択された少なくとも1種の化合物を含む。
トリプターゼインヒビター及び肥満細胞スタビライザーは、いずれも、網膜において肥満細胞から放出されて血管の新生に寄与する物質が実際に血管新生を開始することを抑制することができる。前記ROPの治療又は予防剤では、これらの化合物を有効成分とするので、ROPの発症時における網膜中での肥満細胞による新生血管形成の促進を抑制し、ROPの発症又はその進行を抑制する。
前記ROPの治療又は予防剤は、トリプターゼインヒビターと肥満細胞スタビライザーを、いずれか1種単独で含むものであってよく、2種以上を組み合わせて含むものであってもよい。
前記抗トリプターゼ活性を有する抗トリプターゼ抗体(以下、単に抗トリプターゼ中和抗体という)としては、特に制限なく、公知のポリクローナル抗体又はモノクローナル抗体のいずれも使用することができる。これらの抗体は対象とする種のトリプターゼを中和する抗体であることが好ましい。抗トリプターゼ中和抗体としては、対象とする種のトリプターゼで対象とする種以外の動物を免疫して得たポリクローナル抗体、または、当該動物の脾細胞を不死化細胞と融合して作成されたハイブリドーマから得られるモノクローナル抗体であってよい。このような中和抗体は当業者の通常の知識に基づいて一般的な方法により作成可能である。ヒトのトリプターゼを中和する市販の抗体としては、例えば、Human Tryptase beta-2 MAb (Clone 349414), Mouse IgG1 (R&D systems, Cat. No. MAB37961) 等を挙げることができる。
肥満細胞スタビライザーは、ROPの治療又は予防に有効な量で使用されればよく、このような治療又は予防に有効な量としては、例えば、出生後の個体の体重(kg)あたり、0.0001mg/日〜500mg/日とすることができる。好ましい投与量は用いる肥満細胞スタビライザーによっても異なり、その量は当業者が非臨床試験または臨床試験の結果等に基づいて決定することができる。肥満細胞スタビライザーとしてクロモグリク酸ナトリウムを用いる場合の好ましい投与量は例えば出生後の個体の体重(kg)あたり、0.01mg/日〜100mg/日とすることができる。
非経口的な投与形態としては、点眼、硝子体内投与、結膜下投与、テノン嚢投与、前房内投与、鼻腔内投与、経気道投与、経皮投与、静脈内投与、筋肉内投与、皮下投与、皮内投与又は腹腔内投与等としてもよい。
製剤調製物の形態としては、溶液製剤、分散製剤、半固形製剤、粉粒体製剤、成型製剤、浸出製剤などが挙げられ、例えば、錠剤、被覆錠剤、糖衣を施した剤、丸剤、トローチ剤、硬カプセル剤、軟カプセル剤、マイクロカプセル剤、埋込剤、粉末剤、散剤、顆粒剤、細粒剤、注射剤、液剤、エリキシル剤、エマルジョン剤、灌注剤、シロップ剤、水剤、乳剤、懸濁剤、リニメント剤、ローション剤、エアゾール剤、スプレー剤、吸入剤、噴霧剤、軟膏製剤、硬膏製剤、貼付剤、パスタ剤、パップ剤、クリーム剤、油剤、点眼剤、点鼻剤、点耳剤、塗布剤、輸液剤、注射用液剤などのための粉末剤、凍結乾燥製剤、ゲル調製品等が挙げられる。
注射用の油性液としては、ゴマ油、大豆油等が挙げられ、溶解補助剤として安息香酸ベンジル、ベンジルアルコール等を更に含有してもよい。緩衝剤(リン酸塩緩衝液、酢酸ナトリウム緩衝液等)、又は浸透圧調節のための試薬、無痛化剤(塩化ベンザルコニウム、塩酸プロカイン等)、安定剤(ヒト血清アルブミン、ポリエチレングリコール等)、保存剤(ベンジルアルコール、フェノール等)、アスコルビン酸などの酸化防止剤、吸収促進剤などと配合してもよい。調製された注射液は通常、適当なアンプルに充填される。
ヒト以外の哺乳動物を対象とする場合、出生直後から出生後7日目までとすることができる。
ヒトを対象とする場合、特に、ヒトの低出生体重児等、出生直後の一定期間に高濃度酸素環境下に置かれることがある場合には、高濃度酸素環境に移行した時期又はその少し前(例えば1日前)から前記ROPの治療又は予防剤を投与するか、あるいは、高濃度酸素環境から酸素濃度を低下させ始めた時期、もしくは、通常酸素環境下に移行した時期又はこれらの少し前(例えば1日前)から前記ROPの治療又は予防剤を投与することが好ましい。
また、出生直後の一定期間に高濃度酸素環境下に置かれることがある場合において、動脈血酸素飽和度のモニタリングが行われている場合には、動脈血酸素飽和度を指標として本発明の治療剤又は予防剤を投与することができる。この場合において、動脈血酸素飽和度のモニタリングにはパルスオキシメーター等の血中酸素飽和度を非観血的かつ経時的に計測する装置を用いることができる。
動脈血酸素飽和度を指標に本発明の治療剤または予防剤の投与を行うには、動脈血酸素飽和度を高く維持する場合、動脈血酸素飽和度を維持されている飽和度よりも臨時に高くする必要がある場合、若しくは、何らかの原因で動脈血酸素飽和度が維持目標よりも高くなってしまった場合等に本発明の治療剤または予防剤を投与することができる。これらの場合において、動脈血酸素飽和度が85%超100%以下、好ましくは90%超100%以下、より好ましくは93%超100%以下、さらに好ましくは95%超100%以下に達する場合には、本発明の治療剤または予防剤を投与することが好ましい。投与は、動脈血酸素飽和度を高い状態にする時期又はその少し前(例えば1日前)から前記ROPの治療又は予防剤を投与するか、あるいは、動脈血酸素飽和度を低下させ始めた時期又はその少し前(例えば1日前)から前記ROPの治療又は予防剤を投与することが好ましい。
本発明には、ROPの治療又は予防が必要な対象(患者ともいう)に、前記ROPの治療又は予防剤を投与することを含むROP治療又は予防方法も包含される。
本治療又は予防方法は、発症機構に基づいたROPの治療又は予防剤を当該治療又は予防剤の投与が必要な対象に投与するので、高い治療又は予防効果が期待される。
前記治療又は予防方法における対象の範囲、投与時期、投与量等については、前記の記述をそのまま適用する。
本発明の治療又は予防方法は、生命維持管理の目的で持続的または臨時に与えられている高酸素濃度の環境下、特に酸素含有比率が30〜100体積%である高濃度酸素環境下におかれる低出生体重児に適用するのが好ましく、その場合、投与期間は、高濃度酸素環境に移行した時期又はその少し前(例えば1日前)から、高濃度酸素環境から酸素濃度を低下させ始めた時期から、通常酸素環境下に移行した時期又はこれらの少し前(例えば1日前)から、高濃度酸素環境から通常酸素環境下に移行した時期から、高濃度酸素環境から酸素濃度を低下させ始めた時期から、又は動脈血酸素飽和度を低下させた時期から、5〜10日間、好ましくは7〜8日間とすることができる。
本発明にかかるROPの検査方法は、被検者に由来する生体試料から、肥満細胞の脱顆粒により放出され得るマーカー物質を検出すること(以下、検出工程という)、前記マーカー物質の検出量に基づいて、被検者がROPであるか否かを判定し、または、ROPの予防または治療が必要であるか否かを判定すること(以下、判定工程という)、を含む、ROPの検査方法である。
前記ROPの検査方法では、肥満細胞から放出される肥満細胞の脱顆粒により放出され得るマーカー物質の検出量に基づいて、ROPであるか否か、あるいはROPの予防または治療が必要であるか否かを判定するので、発症機構に基づいて的確に且つ早期にROPであること、あるいはROPの予防または治療が必要であることを判定することができる。
肥満細胞の脱顆粒により放出され得るマーカー物質としては、トリプターゼ、キマーゼ、β−ヘキソサミニダーゼ、VEGF、FGF−2、IL−8、NGF、ヘパリン、TGF−β、TNF−α、ヒスタミン等が挙げられる。
トリプターゼの検出には、タンパク質の検出に通常用いられる定量又は半定量可能な検出方法を適用することができる。
前記検出方法としては、検出感度、特異性及び簡便性の観点から、トリプターゼを認識する抗体をトリプターゼ検出試薬として用いる免疫化学的手法による検出方法であることが好ましい。また、標識されたトリプターゼ基質を用い、酵素活性に基づき遊離された標識物質を検出することにより、酵素活性に基づく検出を行うことも可能である。
前記酵素としては、物理的、化学的方法で定量可能な酵素であれば特に制限はない。例えば、アルカリフォスファターゼ、西洋ワサビペルオキシダーゼ(HRP)、ルシフェラーゼ等の酵素を挙げることができる。
マーカー物質としてβ−ヘキソサミニダーゼを用いる場合にも、トリプターゼの場合と同様に、タンパク質の検出に通常用いられる定量又は半定量可能な検出方法を適用することができ、好ましくはβ−ヘキソサミニダーゼに特異的な抗体を用いた検出方法で検出する。また、本願実施例5に記載したような酵素活性に基づく検出方法を取ることもできる。
判定は、例えば、健常体に由来する生体試料における前記マーカー物質の検出量との対比によって行うことができる。健常体では、肥満細胞からのマーカー物質の放出量は、ROPを発症している患者のマーカー物質の放出量よりも少ないため、健常体に由来する生体試料中の前記マーカー物質の検出量と対比することによって、容易にROPであるか否か、あるいはROPの予防又は治療が必要な状態であるか否かを判定できる。ここで、「健常体」とは、ROPを発症していないことが予め判明している個体を意味する。
この際、健常体とROP発症個体とを区別するために、正常検出量を設定し、正常検出量を超えるか否かに基づいて判定してもよい。
マーカー物質が肥満細胞特異的である場合、すなわち、健康体からのサンプル中には通常当該マーカー物質が検出されない場合には、当該マーカー物質が検出されることによりROPの予防又は治療が必要な状態であると判定することができる
前記肥満細胞は、生体由来の肥満細胞であってもよく、正常細胞の性状を反映した培養肥満細胞株であってもよい。
また、不活性化状態の肥満細胞と、活性化状態の肥満細胞とを区別するために、活性時検出量を設定し、活性時検出量を超えるか否かに基づいて判定してもよい。
本発明のROPの検査方法は、前述のROPの治療又は予防方法と組み合わせることができる。すなわち、本発明は、被検者に由来する生体試料から、肥満細胞の脱顆粒により放出され得るマーカー物質を検出すること、前記マーカー物質の検出量に基づいて、被検者がROPの治療又は予防が必要な患者か否かを判定すること、ROPの治療又は予防が必要な患者に対して、肥満細胞に由来するトリプターゼインヒビター及び肥満細胞スタビライザーからなる群より選択された少なくとも1種の化合物を有効成分とするROPの治療又は予防剤をその治療又は予防に有効な量で投与することを含むROPの治療又は予防法である。
この場合、環境中の酸素濃度又は動脈血酸素飽和度を変化させた後、または、動脈血酸素飽和度が変化した後に、本発明のROPの検査方法によって、ROPの予防又は治療が必要な状態であることを確認し、ROPの予防又は治療が必要と判断された場合に本発明の治療又は予防剤を有効な量で投与してもよい。
本発明にかかるROPの検査キットは、肥満細胞の脱顆粒により放出され得るマーカー物質の検出試薬、例えばトリプターゼ検出試薬を含む検査キットである。
前記検査キットによれば、高濃度酸素環境における肥満細胞から放出されるマーカー物質を検出可能なマーカー物質検出試薬を含むので、発症病理に基づいて的確に且つ早期にROPであるか否かを検査することができる。
前記検査キットに含まれるマーカー物質検出試薬としては、前述したマーカー物質検出試薬をそのまま適用することができる。
本発明のスクリーニング方法は、肥満細胞を、肥満細胞の脱顆粒により放出され得るマーカー物質の放出誘導処理に供すること(以下、マーカー物質放出誘導工程ということがある)、前記マーカー物質の放出誘導処理に供された肥満細胞に、候補物質を接触させること(以下、接触工程ということがある)、接触後の肥満細胞から放出されたマーカー物質の検出を行うこと(以下、マーカー物質検出工程ということがある)、並びに、前記マーカー物質の検出量に基づいて、前記候補物質が前記マーカー物質の放出阻害活性を有するか否かを判定し、前記マーカー物質の放出阻害活性を有すると判定された候補物質を、ROPの治療又は予防効果を発揮しうる目的物質としてスクリーニングすること(以下、スクリーニング工程ということがある)、を含む、ROPの治療又は予防物質のスクリーニング方法である。
前記スクリーニング方法では、前記マーカー物質放出誘導処理に供された肥満細胞から放出されたマーカー物質の検出を行い、得られたマーカー物質の検出量に基づき、マーカー物質の放出阻害活性を有する物質を目的物質としてスクリーニングするので、肥満細胞によるマーカー物質の放出が発症に深く関与していることが判明したROPにおいて治療又は予防効果を発揮しうる目的化合物を、効率よくスクリーニングすることができる。
マーカー物質放出誘導工程に用いられる肥満細胞は、天然の肥満細胞または天然の肥満細胞前駆細胞からin vitroにおいて誘導された肥満細胞であってもよく、あるいは培養肥満細胞株であってもよい。天然の肥満細胞の場合には、生体から抽出した肥満細胞であってもよく、個体内部に存在する状態の肥満細胞であってもよい。即ち、マーカー物質放出誘導処理は、個体そのものを用いたin vivoレベルの処理であってもよく、肥満細胞を用いたin vitroレベルの処理であってもよい。
肥満細胞の通常酸素環境での保持は、個体又は培養細胞の維持に通常適用される条件であればよい。個体の場合には、例えば、20.0〜26.0℃相対湿度30.0〜70.0%の通常酸素環境下で保持することができる。培養肥満細胞の場合には、必要に応じ血清成分を補ったDMEM培地、RPMI1640培地、α−MEM培地等の培養液中で、37℃5v/v%CO2条件下で保持することができる。
in vivoレベルで肥満細胞と候補物質の接触を行う場合、生体外から投与される候補物質が放出誘導処理のタイミングで速やかに生体内に存在する肥満細胞に接触することが好ましい。投与方法や生体内での候補物質の動態により最適な投与開始時期は異なる場合があるが、一般的には放出誘導処理の1日程度前に候補物質を投与開始することが好ましい。試験期間を通じて肥満細胞と候補物質の接触を行うことも、肥満細胞と候補物質の確実な接触の観点から好ましい。In vitroレベルで肥満細胞と候補物質の接触を行う場合、放出誘導処理のタイミングで肥満細胞と候補物質が接触していることが好ましく、肥満細胞と候補物質の確実な接触の観点からは、放出誘導処理の前、例えば試験期間を通じて、肥満細胞と候補物質を接触させておくことも好ましい。
即ち、特定の候補物質が、ROPの治療又は予防活性を有しない他の物質と比較して、マーカー物質の検出量が減少した場合、ROPの発症において肥満細胞からのマーカー物質の放出を阻害可能と考えられるため、このような特定の候補物質であれば、ROPの治療又は予防物質となる可能性がある。
(1) 肥満細胞欠損マウスにおける新生血管形成
(a)モデルマウスの調製
肥満細胞欠損マウスC57BL/6−Wsh/Wshマウス(以下、「Wsh/Wshマウス」)、肥満細胞欠損でないマウスC57BL/6−+/+マウス(以下、「+/+マウス」)、及びそのヘテロ接合体マウスC57BL/6−+/Wshマウス(以下、「+/Wshマウス」)は、それぞれ理研バイオリソースセンターから入手した。全ての動物実験は東京農工大学動物実験指針に従って行われた。Wsh/Wshマウスは、肥満細胞を欠損するマウスとして公知の遺伝子改変マウスである(Blood 80, 1448-1453 (1992))。
以前に報告されているように(Invest. Ophthalmol. Vis. Sci., 35, 101-111 (1994)等)、+/+マウス、+/Wshマウス、及びWsh/Wshマウスのそれぞれについて、P7に、新生仔マウスを、授乳のための雌親と共に75v/v%O2を含む密閉チャンバーに入れ、5日間(P7〜P12)保持した。その後、通常酸素環境(21v/v%O2)下で、更に5日間(P12〜P17)置き、実験的ROPモデルとして酸素誘導網膜症(OIR)を誘導させた。対照マウスは、実験期間全体を通して通常酸素環境(室内環境、25℃、21v/v%O2。以下、実施例の項において同じ)で飼育した。なお、本明細書において日付を表す「P」は出生後であることを意味し、例えばP7は、出生後7日目であることを示す。
P7、P12、P17、又はP25にマウスを屠殺した。以前に報告されているようにホールマウント分析を行った。簡潔に述べると、眼を摘出し、4%パラホルムアルデヒドで1時間固定した。網膜を切開してAlexa Fluor 488結合G.シンプリシフォリア(G. simplicifolia)イソレクチンB4(Molecular Probes社製)で一晩染色した。網膜のフラットマウントを作製し、拡大率4倍で蛍光写真を撮り、BIOREVOシステム(株式会社キーエンス製)を用いて重ねた。
P17に、ROPを発症したマウスから眼を摘出し、ダビッドソン固定液で一晩固定した後、パラフィン包埋した。軸方向の6μmパラフィン包埋連続切片を得、HEで染色し、拡大率20倍で画像を撮り、新生血管細胞の数として、内境界膜の硝子体側の内皮細胞の核の数を計数した。免疫組織化学的検査では、1:100希釈(容量比)した抗PECAM−1抗体(Santa Cruz Biotechnology社製)と一緒に切片を4℃で一晩インキュベートした。
切片毎の内皮細胞の核の数の差を、一元配置分散分析、続いてチューキー法又はフィッシャー直接検定により比較した。P値0.05未満を有意とみなした。全てのデータは平均値±SEMで表す。
高濃度酸素環境から通常酸素環境へのシフトにより、ROPは新生仔マウスで容易に誘発されるため、肥満細胞欠損Wsh/Wshマウス、肥満細胞欠損でない+/+マウス、及びそのヘテロ接合体(+/Wsh)マウスにおける網膜の新生血管形成の重症度を調べた(図1)。5日間(P7〜P12)高濃度酸素環境に暴露した後、ホールマウント分析によりP12で全マウスにおいて網膜中心での血管退縮が観察された。高濃度酸素環境による影響は、以前に報告されているように(Invest Ophthalmol Vis Sci 35, 101-111 (1994)等)、網膜の中心の動脈に隣接する毛細血管に主として現れる。血管染色により、P17の+/+マウスの網膜に、新たに形成中の血管発芽が確認できた。血管発芽は、毛細血管網の再生に失敗し、ヒトの病態におけるROPの特徴である新生血管房(neovascular tuft)を、硝子体に向けて形成していた(図1A)。
以下のようにして、肥満細胞の網膜新生血管形成への関与を調べた。
骨髄由来培養肥満細胞(BMCMC)を、+/+マウスから公知の方法に従って単離した(J Exp Med 174, 7-14 (1991))。5週間以上、常法に従って培養し、1×106/20μLの細胞数に調整した細胞懸濁液を調製後、P1又はP2に、Wsh/Wsh新生仔マウスに20μLの量で細胞懸濁液を腹腔内注射した(n=4〜6)。また、対照群の新生仔マウスには、20μLの生理食塩水を腹腔内注射した。その後、参考例1と同様に、P7、P12、P17、又はP25にマウスを屠殺し、更に、BMCMC注射Wsh/Wshマウスから、硝子体内に伸展した新しい血管中のPECAM−1陽性内皮細胞を回収した。網膜新生血管形成の評価、組織学的検査及び免疫組織学的検査を参考例1と同様に行った。結果を図2に示す。
なお、図2B中、*はP<0.05を示す(生理食塩水注射Wsh/Wsh新生仔マウスでの結果との比較)。統計的有意性は、一元配置分散分析及びチューキー法により決定した。エラーバーはSEMを表す。図2C及び図2Dにおけるスケールバーは、100μmを示す。
各マウスを一晩暗順応させた後、麻酔した。眼の上にコンタクトレンズ電極(Mayo Corporation社, N1530NNC)を置き、参照電極及び接地電極をそれぞれ口の中及び尾の上に配置した。一般的な試験及び電気生理学的装置(UTAS E-3000, LKC Technologies, Inc.社製)を用いてERGを記録した。結果を図3に示す。
図3に示されるように、ROPのモデルに供した+/+及びBMCMC注射Wsh/Wshマウスでは、OP波の振幅が減少し、b波が完全に消失した。一方、生理食塩水のみを注射されたWsh/Wshマウスでは振幅が正常であることが明らかになった。
BMCMCの細胞懸濁液における細胞数を、さらに、1×105細胞/20μL、又は1×104細胞/20μLにそれぞれ調整した以外は、上記参考例2と同様にして、各マウスの腹腔内にBMCMCを投与し、P17眼切片における新生血管核をHE染色を用いて網膜新生血管形成の評価を行った。結果を図4に示す。なお、図4において、*:P<0.05、**:P<0.01(それぞれ+/+の眼における核の数と比較)。統計的有意性は一元配置分散分析及びフィッシャー直接検定により決定した。エラーバーはSEMを表す。
図4に示されるように、肥満細胞がROPモデルの眼における網膜の新生血管形成を用量依存的に促進することが明らかとなった
肥満細胞スタビライザーであるクロモリンの網膜血管新生に対する影響を以下のようにして確認した。
各濃度の酸素状態における影響を確認するために、実験に供するマウスを、P0からP17までの期間において、P0〜P7までの通常酸素環境下(21v/v%酸素)、P7〜P12までの高濃度酸素環境下(75v/v%酸素)及びP12〜P17の通常酸素環境下となるスケジュールで飼育した。なお、P7及びP12に、酸素濃度環境の入れ替えを行った。
+/+マウス及びWsh/Wshマウスの各新生仔マウスを3つの群に分けて、リン酸緩衝生理食塩水(PBS)に溶解させたクロモリン(Sigma Aldrich社製)を、50mg/kg/20μLの濃度で毎日腹腔内注射した。対照群には、同容量のPBSを投与した。各群における薬剤投与方法は、図5に示すとおりである。
即ち、第1群では、新生仔マウスにP6からP16までクロモリンを投与した。第2群では、新生仔マウスにP6からP11にかけてクロモリンを注射した。第3群の新生仔マウスには、P11からP16にかけてクロモリンを投与した。
結果を図6A〜図6Cに示す。なお、図6A〜図6Cにおいて、*:P<0.01(対照マウスにおける核の数との比較)。
また、ROPモデルにおいて、クロモリンの投与によって肥満細胞の脱顆粒を高濃度酸素環境に暴露後5日間(P11〜P16)抑制した場合、高濃度酸素環境の期間中(P6〜P11)のクロモリンの抑制効果がなかったにも関わらず、硝子体スペース内に伸展した内皮細胞の核の数が有意に減少することを示している(図6B及び図6C)。
このことは、肥満細胞に由来する血管新生因子が、高濃度酸素環境への暴露後の酸素濃度の劇的変化によって網膜の新生血管形成を促進している可能性を示唆している。
また、クロモリンの投与は、高濃度酸素環境への暴露後における血管新生に対して抑制効果を有することがわかる。
トリプターゼの特異的阻害剤であるメシル酸ナファモスタット(NM)のROPモデルに対する影響を以下のようにして確認した。
ROPモデルとして、参考例2で作製したBMCMC投与Wsh/Wshマウス(1×106個/20μL投与)と、対照群として生理食塩水を同容量投与したWsh/Wshマウスを用いた。
実施例1と同様のスケジュールでP0〜P17までの期間の酸素濃度を変化させた(図7参照)。
生理食塩水に溶解した1mg/kg/20μlのNM又は生理食塩水を、P12からP17にかけて、各マウスに毎日腹腔内注射した。P17にマウスを屠殺して眼を摘出し、実施例1と同様にして、新生血管形成の評価を行った。結果を図8に示す。図8において、*:P<0.05(対照マウスにおける血管内皮細胞の核の数との比較。)。
トリプターゼがROPのマーカーとして使用できることを以下のようにして確認した。
肥満細胞存在群として、参考例2で作製したBMCMC投与Wsh/Wshマウス(1×106個/20μL投与)と、肥満細胞非存在群として生理食塩水を同容量投与したWsh/Wshマウスを用いた。また、対照群として+/+マウスを使用した。これらの各群は1群あたり4〜5匹の例数を設定した。
各群の全ての動物について実施例1と同様のスケジュールでP0〜P17までの期間酸素濃度環境を変化させた。
P17にそれぞれのマウスより採血を行い、定法に従って血清を得た。この血清を10倍希釈してトリプターゼの定量を行った。血清中トリプターゼの定量には、市販のELISAキット(TPS結合免疫吸着測定キット(mouse),USCN Life Science社,Cat.No. E91070Mu)を使用し、キットの取扱説明書に準じて操作を行った。尚、呈色反応時間は、反応が進みすぎて測定不能となることがないよう適宜調整した。吸光度の測定にはマイクロタイタープレートリーダー(NalgeNunc社,ImmunoMini NJ−2300)を用いた。統計的有意性は一元配置分散分析及びフィッシャー直接検定により決定した。エラーバーはSEMを表す。**:P<0.01(生理食塩水投与マウスの血清トリプターゼ濃度との比較)。
図9に示されるように、P17において肥満細胞が存在する+/+マウス及びBMCMC投与Wsh/WshマウスにROPを誘導した場合、いずれの群においても血清トリプターゼ濃度は高値を示した。一方、肥満細胞が存在しないWsh/Wshマウスにおいては血清トリプターゼ濃度は極めて低い値であった。
参考例2にて記載したように、+/+マウス及びBMCMC投与Wsh/Wshマウスはいずれも酸素濃度変化によってP17において網膜血管新生を示しROPが誘導されるが、肥満細胞が存在しないWsh/Wshマウスは網膜血管新生を生じずROPが誘導されない。本実施例の結果は、ROPが誘導される群では血清トリプターゼ濃度が高いことを示している。このことは、血中のトリプターゼがROPの判定指標として有用であることを示している。
ROPモデルにおける網膜新生血管形成に及ぼすトリプターゼ中和の影響を下記のようにして評価した。
<材料>

骨髄由来培養肥満細胞(BMCMC)
<手法>
1.P1又はP2において、生理食塩水中のBMCMC(1×106)20μlをWsh/Wsh新生仔マウスに腹腔内注射した。
2.P7とP12の間、新生仔マウスを高濃度酸素環境に暴露した。
3.P12で通常酸素環境(室内空気)に戻した。
4.P12とP17の間、1日1回、Mcpt6抗体又はアイソタイプコントロール0.2μg/20μlを注射した。
5.P17で、眼を摘出し、ダビッドソン固定液で固定した。
6.HE染色し、新生血管細胞の数として、内皮細胞の核の数を計数した。
<結果>
計測結果を図10及び表1に示した。
図10及び表1から分かるように、ROPを誘導した+/+及びBMCMC注射Wsh/Wshマウスにおいて、抗トリプターゼ中和抗体は、網膜の新生血管形成を抑制した。このことは、抗トリプターゼ中和抗体を投与することで、ROPを誘導した+/+マウス及びBMCMC投与Wsh/Wshマウスにおける硝子体内の新生血管核をP17において減少させることができたことを示している。
実施例1において、酸素濃度変化により誘発される網膜血管新生を抑制することが確認されているクロモリンと、候補物質である7種の化合物について、肥満細胞の脱顆粒に与える影響をβ−ヘキソサミニダーゼをマーカー物質として以下の方法により試験した。
(1)肥満細胞懸濁液の調製
ヒト肥満細胞株HMC−1はヒト白血病由来の細胞株であり、肥満細胞としての性質を備え、顆粒内にβ−ヘキソサミニダーゼを含むことが知られている。既報(European Journal of Immunology, Vol. 29, 2645-2649 (1999))を参考に、HMC−1細胞を10%(v/v) ウシ胎仔血清及び常用量の抗生物質を添加したα-MEM培地(Medium)に懸濁して細胞濃度5×105セル/mlの肥満細胞懸濁液を調製した。尚、高酸素濃度環境下を含め、実験期間中、酸素濃度以外の培養条件として細胞は全て5%(v/v)CO2、37℃環境下に置かれた。
(2)クロモリン及び候補物質の添加
上記の肥満細胞懸濁液を96−ウェルプレートに4×105セル/ウェルとなるように分注した。クロモリンと7種の候補物質のそれぞれを下記の濃度になるように添加した。薬剤非添加のコントロール(Medium)、及び、マーカー物質放出誘導処理を行わない無処置例として、クロモリン、候補物質のいずれも加えないウェルを設定した。

(3)マーカー物質放出誘導処理工程及びクロモリンまたは候補物質との接触工程
マーカー物質(β−ヘキソサミニダーゼ)の放出誘導処理工程及びマーカー物質放出誘導処理した肥満細胞HMC−1との接触工程は、96−ウェルプレートを高濃度酸素環境(75v/v%酸素)で24時間保持した後、通常酸素環境(20v/v%酸素)で12時間保持することで実施した。無処置例については、96−ウェルプレートを通常酸素環境(20v/v%酸素)で24時間保持した後、さらに通常酸素環境で12時間保持した。
(4)マーカー物質検出工程
上記の接触工程において肥満細胞の脱顆粒により放出されたマーカー物質(β−ヘキソサミニダーゼ)の検出・測定を以前に報告されている方法で行なった(Eur. J. Immunol. 19(1989)2251-2256)。簡潔に述べれば、以下の通りである。接触工程後の各ウェルから上清を採取した後、ペレット(細胞)を同一容量の0.5%トリトンX−100含有α−最小必須培地(α−MEM:ギブコ社製)中に溶解した。96−ウェルプレートの各ウェルにおいて、5μlの上清又は細胞溶解物を50μlの基質溶液(100mMクエン酸ナトリウム中に1.3mg/mlのp−ニトロフェニル−N−アセチル−β−D−グルコサミン;pH4.5)と混合した。37℃で60分インキュベートした後、200mMグリシン溶液(pH10.7)150μlを加えて反応をとめた。414nmで吸光度を測定した。放出されたマーカー物質(β−ヘキソサミニダーゼ)の比率(%)を次式:
OD(上清)/(OD(上清)+OD(ペレット))×100
で求めた。放出されたマーカー物質(β−ヘキソサミニダーゼ)の比率(%)は、脱顆粒の程度を示し、これが低下することは脱顆粒の抑制を意味する。
試験は、3検体試験で行い、測定結果は、平均及び標準誤差を求め、ANOVA+Tukey検定により統計解析した。
(5)結果
測定結果を図11に示す。図11によれば、マーカー物質放出誘導処理を行ったコントロール(Medium)では、マーカー物質放出誘導処理を行わなかった無処置例(Medium (20%O2))よりも脱顆粒が亢進していた。すなわち、HMC−1細胞を用いたin vitroの実験系においても、in vivo ROPモデル(実施例3)と同様に、高濃度酸素環境から通常酸素環境への酸素濃度の低下によって肥満細胞の顆粒内容物の細胞外への放出が生じることが示された。この試験系において、クロモリンと同様に、7種の候補物質(スプラタスト、トラニラスト、アンレキサクノス、レボカバスチン、イブジラスト、エピナスチン、ペミロラス)はいずれも、コントロール(Medium)に比べ、酸素濃度の変化により誘発した脱顆粒を抑制し、その抑制効果はいずれも統計学的に有意(p<0.05)であった。クロモリンは、肥満細胞スタビライザーであることが知られ、且つ、実施例1により酸素濃度の変化により誘発される網膜血管新生を抑制することが確認されている。したがって、これらの7つの候補物質は、実施例1で試験したクロモリンと同様に、網膜血管新生に対して抑制効果を有し、ROPを治療・予防できると考えられる。

- 被検者に由来する生体試料から、肥満細胞の脱顆粒により放出され得るマーカー物質を検出することを含む、
前記マーカー物質の検出量に基づいて、被験者が未熟児網膜症であるか否かを判定することのために、あるいは、未熟児網膜症の治療又は予防が必要か否かを判定することのためにデータを提供する方法であって、
生体試料は血液であり、そしてマーカー物質はトリプターゼである、
方法。 - 請求項1記載の方法により未熟児網膜症の治療又は予防が必要であると判断された患者に投与するための、抗トリプターゼ抗体を有効成分とする未熟児網膜症の治療又は予防剤。
