Description
[0001]本出願は、参照によりあたかもその全体があらゆる目的のために記載されているかのようにその開示全体が本明細書に組み込まれている、2013年3月15日に出願された「Thermochemical Reaction Ablation Catheter」と題される米国仮特許出願第61/790133号の優先権および利益を主張する。
Claims (13)
- 患者の部位にエネルギーを送達する、または前記患者の部位からエネルギーを除去するためのデバイスであって、
前記患者の体内に挿入されて治療部位に到達するように構成されるカテーテルシャフト(104)と、
前記カテーテルシャフトの中で前記カテーテルシャフトの遠位領域(128)に配置される第1の試薬(112)と、
前記カテーテルシャフト内に位置し、前記カテーテルシャフトの前記遠位領域に所定量の限定試薬(124)を送達するように構成される第1の管腔(116)であって、前記限定試薬および前記第1の試薬が少なくとも部分的に熱化学反応で互いに反応して反応生成物(172)を生成する、第1の管腔(116)と、
前記カテーテルシャフト内に位置し、前記カテーテルシャフトの前記遠位領域から前記反応生成物を抽出するように構成される第2の管腔(136)と、
前記カテーテルシャフト内に位置し、熱伝達流体(140)を搬送するように構成される第3の管腔(156)であって、前記熱伝達流体が前記治療部位と前記反応生成物との間でエネルギーを伝達するように構成される第3の管腔(156)と、
1または複数のセンサ(181)と、
前記カテーテルシャフトの近位端(158)に取り付けられるハンドル(200)であって、前記カテーテルシャフト内への前記限定試薬の流れ、前記カテーテルシャフト内への前記熱伝達流体の流れ、および前記反応生成物の流量のうち少なくとも1つを制御するように構成されるハンドル(200)と
を備え、
前記熱化学反応が、加熱、冷却またはその両方をもたらし、
前記遠位領域に提供される前記限定試薬の量が、提供される加熱または冷却の量を決定し、
前記1または複数のセンサが所定量の加熱または冷却を検証し、ユーザにフィードバックを提供するように構成される、デバイス。 - 前記限定試薬が前記カテーテルシャフトの遠位端で前記第2の管腔内に保持される、請求項1に記載のデバイス。
- 前記熱化学反応が、以下のリスト、すなわち過飽和塩溶液の結晶化、金属の酸化、酸性反応、テルミット反応、酸/塩基反応、酸化反応、および水和反応、から選択される、請求項1に記載のデバイス。
- 前記カテーテルシャフトの前記遠位領域から流出させられた前記熱伝達流体の少なくとも一部を抽出するように構成される抽出管腔(160)をさらに備える、請求項2に記載のデバイス。
- 前記熱伝達流体が、前記カテーテルシャフトから患者の身体へと流出せずに前記カテーテルシャフト内を循環するように、前記カテーテルシャフトから前記熱伝達流体を抽出するように構成される第4の管腔(176)をさらに備える、請求項1に記載のデバイス。
- 前記1または複数のセンサが前記熱伝達流体の入口温度および出口温度を監視するように構成され、
前記1または複数のセンサが遠位端またはその近傍に位置し、
前記1または複数のセンサがプリントされた導電性電極を介して温度監視モジュールと通信する、請求項5に記載のデバイス。 - 前記1または複数のセンサが、前記カテーテルシャフトの前記遠位領域で前記反応生成物の温度を検出するように構成される、請求項1に記載のデバイス。
- 前記1または複数のセンサが周辺組織の温度を検出するように構成される、請求項1に記載のデバイス。
- 1または複数の監視特性に応じて前記反応生成物と前記治療部位の組織との間で発生するエネルギー伝達の量を調節するように構成されるフィードバック機構(170)をさらに備える、請求項1に記載のデバイス。
- 前記1または複数の監視特性が、組織温度、反応生成物温度、前記カテーテルシャフトの前記遠位領域から出る前記熱伝達流体の流れ、および時間からなる群から選択される、請求項9に記載のデバイス。
- 前記フィードバック機構が、所定の限界値を超える前記1または複数の監視特性のレベルに応じて、前記限定試薬の流れを停止させるか、前記熱伝達流体の流れを停止させるか、またはその両方を行うように構成される、請求項9に記載のデバイス。
- 患者の身体の治療部位と熱伝達流体との間でのエネルギーの伝達のためのシステムであって、前記システムが、請求項1〜11のいずれか一項に記載のデバイスと、
前記第1の管腔に接続される限定試薬流体供給源(191)と、
前記第3の管腔に接続される熱伝達流体供給源(190)と、
内視鏡およびガイドシースからなる群から選択される、患者の体内に挿入可能な導管であって、前記導管の中で前記カテーテルシャフトが挿入可能かつ移動可能である導管と
を備えるシステム。 - 前記システムの1または複数の監視特性を監視するように構成される1または複数のセンサ(181)をさらに備え、前記1または複数の監視特性が、前記カテーテルシャフトの1または複数の位置の温度と、前記限定試薬、前記反応生成物、および前記熱伝達流体のうち1または複数の流体流量とからなる群から選択される、請求項12に記載のシステム。
