Description

本開示は、創傷に閉鎖を提供するインプラントに関し、特に、腹腔鏡ポート部位等の組織の穿孔を修復および密閉するための創傷閉鎖デバイスに関する。
様々な外科手技(例えば、腹腔鏡手技)は、アクセスポートを介して行われ、その間、アクセスデバイスが組織を穿刺して、手術部位にアクセスを提供する。
本開示は、創傷閉鎖デバイス、同デバイスを作製する方法、および同デバイスを使用する方法を提供する。実施形態において、本開示の創傷閉鎖デバイスは、近位端および遠位端を有する細長い本体と、組織対向面を有する栓部材と、栓部材の組織対向面を細長い本体に接続する蝶番とを含んでもよく、栓部材は、少なくとも1つの反応基を含む。実施形態において、栓部材の組織対向面はまた、少なくとも1つの反応基を含んでもよい。実施形態において、栓部材は、共通の枢軸に沿って蝶番で連結された2つの実質的に同一のセクションを含む。

近位端および遠位端を有する細長い本体と、
組織対向面を有する栓部材と、
該細長い本体と該栓部材とを接続する蝶番とを含み、
該創傷閉鎖デバイスは、挿入のための第1の折り畳み位置および組織密閉のための第2の拡張位置を含む、創傷閉鎖デバイス。
本発明の創傷閉鎖デバイスは、創傷の閉鎖を促進するものであり、また治癒を向上させ、瘢痕、疼痛、および感染を減少させるための生物製剤および/または治療剤を送達するために、ならびに創傷部位に機械的な安定性を提供してポート部位のヘルニア形成を防止するために使用されてもよい。創傷閉鎖デバイスは、創傷の穿孔組織内に挿入し、組織を充填して一緒に保持するための細長い本体と、組織の内部表面に対して位置付けて創傷を塞ぐまたは閉じるための実質的に平坦な組織対向面を有する、細長い本体の遠位端部に装着された栓部材とを含む。実施形態において、創傷閉鎖デバイスは、外套針等の挿入デバイスを通して挿入され、針が除去されると、残された創傷閉鎖デバイスが創傷を閉鎖する。
予め形成されたヒドロゲルは、デバイスがインサイチュで形成するのを待つ時間が存在しないため、創傷閉鎖デバイスの迅速な挿入および送達を可能にする。さらに、予め形成された構成要素は、標的の創傷以外の組織と構成要素が反応する可能性を回避し、体腔または皮膚表面等の他の場所に材料が流出するのを回避する。予め形成されたヒドロゲルを作製するための方法は、所望の形状の型の中に、第1の前駆体および第2の前駆体を同時に噴霧することを含む。
好適な生物活性剤は、製造プロセスの前または最中のいずれかに、創傷栓と組み合わせてもよい。生物活性剤は、ポリマーと混合するかまたは組み合わせて、生物活性特性を有する栓をもたらすことができる。他の実施形態において、生物活性剤は、栓が成形された後に、例えば、コーティングの形態で、本開示と組み合わせてもよい。生物活性剤は、フィルム、粉末、液体、ゲル等の、任意の好適な形態で本開示に適用されてもよいことが想定される。

Claims (14)
- 創傷閉鎖デバイスであって、
長手方向軸を画定し、かつ、近位端および遠位端を有する細長い本体と、
組織対向面を有し、かつ、第1の成形されたセクションおよび第2の成形されたセクションを含む栓部材であって、該第1の成形されたセクションおよび該第2の成形されたセクションの各々は、当接面を有する、栓部材と、
該栓部材の該組織対向面を該細長い本体に接続する蝶番であって、該第1の成形されたセクションおよび該第2の成形されたセクションは、折り畳み位置と展開位置との間で該蝶番の周りを移動可能であり、
該折り畳み位置では、該第1の成形されたセクションおよび該第2の成形されたセクションの該当接面が、該細長い本体の該長手方向軸から離れて延在しており、
該展開位置では、該第1の成形されたセクションおよび該第2の成形されたセクションの該当接面が、該細長い本体の該長手方向軸と当接し、かつ、実質的に整列されており、該当接面が、該細長い本体に対する該組織対向面の展開角度を制御する、蝶番とを含み、
該栓部材は、少なくとも1つの反応基を有する、創傷閉鎖デバイス。 - 前記少なくとも1つの反応基は、イソシアネート、N−ヒドロキシスクシンイミド、シアノアクリレート、アルデヒド、ゲニピン、トリリジン、テトラリジン、ポリリジン、ジイミド、ジイソシアネート、シアナミド、カルボジイミド、ジメチルアジプイミデート、デンプン、およびそれらの組み合わせからなる群より選択される、請求項1に記載の創傷閉鎖デバイス。
- 前記栓部材の組織対向面が、少なくとも1つの反応基をさらに含む、請求項1に記載の創傷閉鎖デバイス。
- 前記組織対向面の前記少なくとも1つの反応基は、イソシアネート、N−ヒドロキシスクシンイミド、シアノアクリレート、アルデヒド、ゲニピン、トリリジン、テトラリジン、ポリリジン、ジイミド、ジイソシアネート、シアナミド、カルボジイミド、ジメチルアジプイミデート、デンプン、およびそれらの組み合わせからなる群より選択される、請求項3に記載の創傷閉鎖デバイス。
- 前記デバイスは、求核性ポリマー、求電子性ポリマーおよびそれらの組み合わせよりなる群より選択されるポリマーを含む、請求項1に記載の創傷閉鎖デバイス。
- 前記細長い本体、前記栓部材、またはその両方は、ヒドロゲルを含む、請求項1に記載の創傷閉鎖デバイス。
- 前記栓部材は、前記組織対向面上にメッシュを含む、請求項1に記載の創傷閉鎖デバイス。
- 前記細長い本体は、メッシュを含む、請求項1に記載の創傷閉鎖デバイス。
- 前記メッシュは、生分解性である、請求項8に記載の創傷閉鎖デバイス。
- 前記細長い本体は、溝付きの外表面を含む、請求項1に記載の創傷閉鎖デバイス。
- 前記細長い本体は、前記近位端から前記遠位端まで長手方向に延在するチャネルを含む、請求項1に記載の創傷閉鎖デバイス。
- 前記栓部材は、抗癒着性コーティングを有する遠位端を含む、請求項1に記載の創傷閉鎖デバイス。
- 前記第1の成形されたセクションおよび前記第2の成形されたセクションは、共通の枢軸に沿って蝶番で連結された2つの実質的に同一のセクションである、請求項1に記載の創傷閉鎖デバイス。
- 前記栓部材は、前記折り畳み位置において前記細長い本体に対して実質的に長手方向に整列され、前記展開位置において該細長い本体に対して実質的に垂直に整列される、請求項1に記載の創傷閉鎖デバイス。
