【発明の属する技術分野】
本発明は、抗血栓剤及び造影剤を含む医療用チューブである血管処置用、血管診断用並びに血管留置用カテーテルチューブの改良に関する。

【従来の技術】
カテーテルチューブは、患者の血管内に挿入、留置して血管内部との連通を保持し、輸液や薬液の注入、輸血、透析等の診断や疾病の治療に使用される医療用チューブである。

カテーテルチューブとして要求される諸性能のうち、当然のことながら、血液と接触した場合、血液凝固を惹起しその表面に血栓を生成することのない性質(抗血栓性)の付与が重要な課題の一つであり、また加えて、当該血管処置用、血管診断用並びに血管留置用カテーテルチューブを血管内に挿入して、診断や治療等を行う際には、チューブが生体内のいずれの場所に位置しているかを体外からX線等で容易に確認できる性質(造影性)をも有することが好ましい。

すなわち、当該血管処置用、血管診断用並びに血管留置用カテーテルチューブ(以後、単に「カテーテルチューブ」と略記することがある。)21は、単層構造により形成されており、そのカテーテルチューブ21のチューブの断面が、例えば90度に四等分して区画され、それぞれの対向する区画が合成樹脂に抗血栓剤を添加した材料22Aと、合成樹脂に造影剤を添加した材料22Bから構成されている。

しかしながら、かかるカテーテルチューブにおいては、造影剤を含有する材料22Bの部分が血管内の血液に直接触れるので、血栓は、造影剤を含む材料22Bの表面部分に主として発生することが懸念される。また、抗血栓剤を添加した材料22Aは、チューブ全体の約半分を占めているため、高価な抗血栓剤の使用量が多くなり、製造コストを高くする要因にもなっていた。

【発明が解決しようとする課題】
本発明の目的は、かくして、抗血栓剤及び造影剤を含むカテーテルチューブにおいて、高価な抗血栓剤の使用量をできるだけ少なくして当該カテーテルチューブの外側表面及び内側表面に血栓を発生することを効果的に防止し、かつ、カテーテルチューブの造影性を十分確保することである。

本発明に従えば、以下の発明が提供される。

〔1〕坑血栓剤及び造影剤を含むカテーテルチューブ(1)において、当該カテーテルチューブは、三層以上からなり、少なくとも合成樹脂に前記抗血栓剤を添加して構成される合成樹脂組成物からなる外層(2A)及び内層(2C)を有し、かつ、少なくとも合成樹脂に造影剤を添加して構成される合成樹脂組成物からなる中間層(2B)を前記外層(2A)と内層(2C)の間に配置したカテーテルチューブ(1)であり、
前記外層(2A)及び内層(2C)は、少なくとも抗血栓性を発揮できる厚さに形成し、かつ、前記中間層(2B)は、少なくとも造影性を発揮できる厚さに形成し、
前記外層(2A)及び内層(2C)の肉厚は、20〜80μmであり、
前記中間層(2B)の肉厚は、100〜200μmであり、

以下、図1に示したカテーテルチューブを例に挙げて、本発明を詳細に説明するが、本発明のカテーテルチューブは、上記したごとく基本的には、抗血栓剤及び造影剤を含む血管処置用、血管診断用並びに血管留置用カテーテルチューブであって、当該カテーテルチューブの外側表面及び内側表面が、合成樹脂に当該抗血栓剤を添加して構成される合成樹脂組成物で被覆されており、かつ、当該カテーテルチューブ内部に、合成樹脂に当該造影剤を添加して構成される合成樹脂組成物を含有するものであれば、もちろん図1に記載したカテーテルチューブの形態のみに限定されるのものではない。

しかして、当該中間層2Bは、カテーテルチューブの主たる構造部分であり、当該構造部分の両面に外層2A及び内層2Cを支持しているものであるから、その肉厚を適度に厚く形成することにより、造影性を確保することができると同時に、当該厚い構造部分によりチューブに十分な強度(剛性)を付与することができるのである。従って、外層2A及び内層2Cは、中間層に比較してずっと薄く形成することが可能であり、これに添加する高価な抗血栓剤の配合量を大幅に少なくすることができる。なお、抗血栓性の発現は、基本的に、血液と接触する外層2Aや内層2Cの表面部分において行われるものであるから、当該表面部分における抗血栓剤の濃度(表面濃度又は表面密度)が、抗血栓性を発揮出来る程度に十分に高ければよく、外層や内層をあまり内部まで厚く形成することは、必要ではない。

外層2A及び内層2Cの肉厚は、特に限定するものではないが、例えば、5〜100μm、好ましくは20〜80μm、より好ましくは例えば50μm程度に形成することが望ましい。また、外層2Aと内層2Cの厚さは、同じであっても、異なっていてもいずれであってもかまわない。

一方、中間層2Bの肉厚は、同様に限定するものではないが、例えば、20〜250μm、好ましくは100〜200μm、より好ましくは例えば150μm程度に形成することができる。

本発明において、前記外層2A、内層2C、中間層2Bを構成する基材となる合成樹脂としては、柔軟性、可撓性、弾力性、機械的強度、可滅菌性を有し、かつ添加混合する抗血栓剤が熱分解するような化合物の場合は、当該熱分解温度以下で熱成形可能な熱可塑性の合成樹脂が好ましい。例えば、エーテル系ポリウレタン、ポリエステル系ポリウレタン、ポリカーボネート系ポリウレタン等のポリウレタン、エチレン−ビニルアセテート共重合体、ポリアミド系エラストマー、スチレン−ブタジエン共重合体、ポリエチレン、ポリプロピレン、ポリ塩化ビニル、ポリ塩化ビニリデン、ポリフッ化ビニリデン、ポリアミド、ポリエステル系エラストマー等を用いることができる。

上記したように、抗血栓剤が熱分解するような化合物であるときは、当該熱分解温度以下で熱成形可能な合成樹脂が好ましく、例えば添加混合する抗血栓剤としてアルガトロバン(熱分解温度が220から230℃)を使用する場合は、220から230℃以下の温度で成形可能な合成樹脂が使用される。

本発明において、前記外層2A、内層2Cに添加する抗血栓剤としては、特に限定するものではなく、十分な抗血栓性を有し、かつ、外層や内層を構成する基材となる上記合成樹脂と相溶性があり、当該合成樹脂層(または当該樹脂層表面)に安定的に混合されるものであれば従来公知のいずれの抗血栓剤を使用することもできる。

また、その他、ワルファリンカリウム(warfarin potassium)、塩酸チクロピジン(ticlopidine HCl)、ベラプロストナトリウム(beraprost sodium)、アスピリン(aspirin)等の公知の抗血栓剤を使用することもできるし、さらにこれ以外のものであってもよい。なお、本発明において抗血栓剤とは、血栓の形成を抑制するものだけでなく、形成された血栓を溶解するメカニズムに基づくものであってもよい。

当該抗血栓剤の合成樹脂への添加量は、得られる合成樹脂組成物が十分な抗血栓性を発揮でき、かつチューブの特性を劣化させない範囲の量であることが好ましく、合成樹脂と前記抗血栓剤の合計総質量に対して、1〜50質量%、好ましくは5〜30質量%、更に好ましくは5〜20質量%の範囲である。

本発明において、中間層2Bに添加する造影剤としては、例えばX線に対する不透過性があって、造影性が大きく、また、基材となる上記合成樹脂に安定的に配合しうるものであれば、特に限定するものではなく、従来公知の造影剤を好適に使用することができる。

例えば、硫酸バリウム、次炭酸ビスマス、タングステン、酸化バリウム、炭酸バリウム、リン酸バリウム、リン酸カルシウム、酸化チタン、酸化ビスマス、硫化ビスマス、チタン酸ビスマス、酸化モリブデン、三酸化タングステン、クエン酸鉄アンモニウム、硫酸鉄、ピロリン酸第2鉄等が使用可能であり、特に硫酸バリウム、次炭酸ビスマス、タングステンが好ましい。

当該造影剤の添加量は、得られる合成樹脂組成物が十分な造影性を発揮でき、またチューブの可撓性や機械的強度等を低下させず、また成形性を劣化させない範囲の量であることが好ましく、前記抗血栓剤の添加量と同様に、合成樹脂と造影剤の合計総質量に対して10〜60質量%、好ましくは20〜50質量%である。なお、造影剤の粒子を合成樹脂組成物中でより均一に分散させるために、当該造影剤粒子の表面をシランカップリング剤、アルミカップリング剤、チタンカップリング剤等のカップリング剤で処理することも好ましい。

本発明の抗血栓剤や造影剤を含むカテーテルチューブの成形方法としては、外層、中間層、内層のチューブを押出機により別々に形成し、これらのチューブを積層して熱融着や接着剤により接合して三層構造等のチューブとすることもできるが、外層、中間層及び内層をなす合成樹脂組成物を別々の押出機から多層ダイにより溶融状態で共押出成形し、これらを熱融着(溶融接着)して、一段で三層構造等のカテーテルチューブを形成する共押出成形によることが最も好ましい。

一般的には、外層や内層を構成する抗血栓剤を含有する合成樹脂組成物や中間層を構成する造影剤を含有する合成樹脂組成物を、予めペレット化しておくことが望ましい。すなわち、押出成形機などを用いて基材となる合成樹脂を加熱溶融し、これに抗血栓剤や造影剤を添加して十分に混練、分散せしめ、この混練した樹脂組成物を、ペレットに成形加工する。

なお、本発明においては、前記カテーテルチューブ1を三層以上で形成する場合、外層2Aと中間層2Bの間に、または中間層2Bと内層2Cの間に、さらに一層以上の他の層を配置することができる。また、当該中間層2Bの内部に一層以上の他の層を挿入することもできる。また、これらの他の層は、ホットメルト接着層等の接着剤層であってもよい。

すでに述べたように、本発明のカテーテルチューブにおいては、多層構造を有し、造影剤を添加した合成樹脂組成物は、チューブ内部の中間層2Bとして配置されているので、当該造影剤は血管内の血液に直接触れることはなく、血栓発生の問題が回避される。また当該中間層2Bは、抗血栓剤を含有する外層2Aや内層2Cより肉厚に形成することにより、十分な造影性を確保することができると共に、強度的に外層2Aや内層2Cは、ごく薄く形成しうるため、外層等に添加される抗血栓剤の使用量を大幅に少なくすることができる。

【実施例】
以下、実施例により本発明を説明する。なお、特に断り無き限り、以下%とあるのは質量%を示す。

〔実施例1〕
(1)図1に示す三層構造の血管留置用カテーテルチューブについて試験を行った。すなわち、外層2A、内層2C、中間層2Bを構成する基材となる合成樹脂にエーテル系ポリウレタン、抗血栓剤にアルガトロバン、造影剤に硫酸バリウムをそれぞれ用いた。

(2)ラボ用小型混練機を用いて、エーテル系ポリウレタンに抗血栓剤であるアルガトロバンを10%添加し、十分溶融混練して成形加工し、アルガトロバン/ポリウレタンペレットを作製した。また同様にして、エーテル系ポリウレタンに造影剤である硫酸バリウムを40%添加して溶融混練し、硫酸バリウム/ポリウレタンペレットを作製した。

(3)それぞれ内層及び外層用のφ15mm押出機に前記アルガトロバン/ポリウレタンペレットを供給して溶融し、また、中間層用のφ25mm押出機に硫酸バリウム/ポリウレタンペレットを供給して溶融した。

当該溶融した樹脂組成物を、押出機から同時に三層円形ダイに供給してチューブ状に三層共押出し、アルガトロバンを添加したエーテル系ポリウレタンよりなる外層2A、硫酸バリウムを添加したエーテル系ポリウレタンよりなる中間層2B、及びアルガトロバンを添加したエーテル系ポリウレタンよりなる内層2Cから形成される三層チューブを形成した。当該チューブは、16G(外径1.5mm×内径1.0mm)規格のカテーテルチューブであった。なお、当該チューブの外層2A及び内層2Cの厚さは、それぞれ約50μm、中間層2Bの厚さは150μmであった。

(4)当該カテーテルチューブ10cmを、ウサギの大腿静脈より挿入し、腹部静脈に24時間留置した後、表面の血栓の粘着状態を評価した。その結果、24時間後ウサギより抜去したカテーテルチューブ表面には、血栓の付着は確認できなかった。また、当該カテーテルチューブ内の血液は、凝固することなく、容易に洗い出す事ができた。

以上のごとく、このカテーテルチューブは十分な抗血栓性を有するものであることがわかった。

(5)また、当該カテーテルチューブを、X線透視装置(KTM−2009、東芝メデカル社製)を用いて、感電圧60kV、強度4mA・秒の条件で、1m離れた位置より撮影したところ、撮像はきわめて明瞭であり、十分な造影性を有することが確認された。

〔比較例1〕
(1)外層2Aと内層2Cを形成する樹脂組成物として、アルガトロバンを添加しないポリエーテルウレタンを用いる他は、実施例1と同様にして、三層構造のカテーテルチューブを形成した。

(2)このカテーテルチューブについて、実施例1と同様な試験を行った。X線に対する造影性については、実施例1と同様に十分な造影性を有していたが、抗血栓性については、当該カテーテルチューブ表面に赤色血栓の付着(表面の約40%)が明確に確認され、また、チューブ内の血液は凝固しており、容易に洗い出すことはできなかった。

【発明の効果】
本発明のカテーテルチューブは、その外側表面及び内側表面が、抗血栓剤を添加して構成される合成樹脂組成物で被覆されており、また当該カテーテルチューブの内部に、造影剤を添加して構成される合成樹脂組成物を含有するという構成のものであるから、チューブの内側表面及び外側表面において、血栓を発生することを効果的に防止することができる。

また、当該カテーテル内部の造影剤を含有する合成樹脂組成物からなる中間層を、十分厚く形成することにより、造影性を確保することができるとともに、強度的に、抗血栓剤を添加した合成樹脂組成物の外層や内層を薄く形成することができるので、高価な抗血栓剤の使用量を大幅に少なくすることができる。
