第2のカッティングバー20の挿入時も、第1のカッティングバー10と同様に、ガイド部26を根管600の内面に沿うようにゆっくりと挿入すれば、根管壁を傷つけることなく第2のカッティングバー20を根管600に挿入することができる。ガイド部26が第1のカッティングバー10が切削停止した部分に到達したところで、カッティングバー装着器具200に備えられている電源等を用いて、第2のカッティングバー20を作動させる。第2のカッティングバー20は、第1のカッティングバー10より短いガイド部26を有し、前方方向に向いた刃24を有しているため、ガイド部26が削られていない根管に嵌り、第1のカッティングバー10が広げることができなかった根管壁を削り、さらに根管を広げながら前進する。そしてガイド部26が根管に残されたリーマやファイル等の破折片に到達したところで、第2のカッティングバー20が先に進めなくなるため、再び根管壁を削るのを停止する。なお、この作業中、破折片が削られることもあるが、削った根管壁及び破折片の粉は、吸引等の器具を用いて根管外へ除去することが好ましい。

この過程で、ガイド部26の長さの異なる複数の第2のカッティングバー20を用いることが好ましい。その場合、ガイド部26の長さは次第に短くするのが好ましい。例えば、まず、ガイド部26の長さの長いカッティングバー(図6参照;例えば、0.3mm)で根管壁を削っておき、次にガイド部26の長さの短いカッティングバー(図7参照;例えば、0.1mm)で根管壁を削ることにより、ガイド部による第2のカッティングバー20の指向が安定するため、余分な部分を削ることなく、さらに安定して根管壁を削ることができる。ここで、第2のカッティングバーとして、2種類に限らず、ガイド部26の長さが異なる3種類以上のものを使用してもよい。さらに、最後に用いる第2のカッティングバー20は、ガイド部26が短い方が好ましく、ガイド部26の無いものを用いても良い。これによって、第2のカッティングバー20による切削工程が終わった段階で、根管壁を過剰に、又は不必要に削ることなく、破折片のごく近傍まで、あるいは破折片が露出するところまで根管壁を広げることができる。
